Survival of patients with colon and rectal cancer in central and northern Denmark, 1998–2009

We conducted this population-based study to monitor recent trends in colon and rectal cancer survival in the central and northern regions of Denmark.Material and methods: Using the Danish National Registry of Patients, we identified 9412 patients with an incident diagnosis of colon cancer and 5685 patients diagnosed with rectal cancer between 1998 and 2009. We determined survival, and used Cox proportional hazard regression analysis to compare mortality over time, adjusting for age and gender. Among surgically treated patients, we computed 30-day mortality and corresponding mortality rate ratios (MRRs).Results: The annual numbers of colon and rectal cancer increased from 1998 through 2009. For colon cancer, 1-year survival improved from 65% to 70%, and 5-year survival improved from 37% to 43%. For rectal cancer, 1-year survival improved from 73% to 78%, and 5-year survival improved from 39% to 47%. Men aged 80+ showed most pronounced improvements. The 1- and 5-year adjusted MRRs decreased: for colon cancer 0.83 (95% confidence interval CI: 0.76–0.92) and 0.84 (95% CI: 0.78–0.90) respectively; for rectal cancer 0.79 (95% CI: 0.68–0.91) and 0.81 (95% CI: 0.73–0.89) respectively. The 30-day postoperative mortality after resection also declined over the study period. Compared with 1998–2000 the 30-day MRRs in 2007–2009 were 0.68 (95% CI: 0.53–0.87) for colon cancer and 0.59 (95% CI: 0.37–0.96) for rectal cancer.Conclusion: The survival after colon and rectal cancer has improved in central and northern Denmark during the 1998–2009 period, as well as the 30-day postoperative mortality.Keywords: neoplasms, survival, epidemiology, colorectal cancer

We therefore examined the changes in survival in Danish patients with colorectal carcinoma (CRC) or other solid tumors (non-CRC) who had liver metastases at time of diagnosis.Study design and methods: We included patients diagnosed with liver metastases synchronous with a primary cancer (ie, a solid cancer diagnosed at the same date or within 60 days after liver metastasis diagnosis) during the period 1998 to 2009 identified through the Danish National Registry of Patients. We followed those who survived for more than 60 days in a survival analysis (n = 1021). Survival and mortality rate ratio (MRR) at 1, 3, and 5 years stratified by year of diagnosis were estimated using Cox proportional hazards regression analysis.Results: In the total study population of 1021 patients, 541 patients had a primary CRC and 480 patients non-CRC. Overall, the 5-year survival improved from 3% (95% confidence interval [CI]: 1%–6%) in 1998–2000 to 10% (95% CI: 6%–14%) in 2007 to 2009 (predicted value). The 5-year survival for CRC-patients improved from 1% (95% CI: 0%–5%) to 11% (95% CI: 6%–18%) whereas survival for non-CRC patients only increased from 5% (95% CI: 1%–10%) to 8% (95% CI: 4%–14%).Conclusion: We observed improved survival in patients with liver metastases in a time period characterized by introduction of a structured multidisciplinary care and improved treatment options. MRI of the Bladder in Patients Suspected of Bladder Tumors  [PDF]

Abstract: Objective: To prospectively evaluate the use of MRI for the detecting of bladder tumors and the T- stage using T2W, T1W and diffusion-weighted images (DWI). Material and methods: Twenty-eight consecutive patients (21 men, 7 women; age range, 20 - 82 years; mean age, 62.8 years) suspected of bladder tumors underwent MRI, flexible cystoscopy and transurethral resection (TURB). The presence of bladder tumor was confirmed by histopathology in 21 patients; 18 patients had pTa, one pT1 and two pT2. The images were reviewed by two uroradiologists. They assigned the presence of a bladder tumor and whether the tumor was non-muscle invasive (Ta and T1) or muscle- invasive (T2, T3 or T4). Results: Compared to the histopathological results, the accuracy for identifying a bladder tumor was 60.7% and 53.7% for reviewer A and B, respectively. The sensitivity and specificity were 66.7%/61.9% and 57.1%/42.9%. Positive predictive values were 82.6%/ 76.5%. The overall staging was correct in 47.6%/52.5%, but improved on stage-by-stage up to 50%/66.7%. The agreement between the reviewers was moderate in the detecting, staging and location of the tumor (Kappa = 0.47 - 0.57). Conclusion: A simple MRI using no contrast media, but DWI, cannot replace flexible cystoscopy in the detection of new or recurrent bladder tumors.


Hybrid Fuzzy Controller Based Frequency Regulation in Restructured Power System  [PDF]

Abstract: This paper discusses the implementation of Load Frequency Control (LFC) in restructured power system using Hybrid Fuzzy controller. The formulation of LFC in open energy market is much more challenging; hence it needs an intelligent controller to adapt the changes imposed by the dynamics of restructured bilateral contracts. Fuzzy Logic Control deals well with uncertainty and indistinctness while Particle Swarm Optimization (PSO) is a well-known optimization tool. Abovementioned techniques are combined and called as Hybrid Fuzzy to improve the dynamic performance of the system. Frequency control of restructured system has been achieved by automatic Membership Function (MF) tuned fuzzy logic controller. The parameters defining membership function has been tuned and updated from time to time using Particle Swarm Optimization (PSO). The robustness of the proposed hybrid fuzzy controller has been compared with conventional fuzzy logic controller using performance measures like overshoot and settling time following a step load perturbation. The motivation for using membership function tuning using PSO is to show the behavior of the controller for a wide range of system parameters and load changes. Error based analysis with parametric uncertainties and load changes is tested on a two-area restructured power system.

Abstract: Performance index based analysis is made to examine and highlight the effective application of Particle Swarm Optimization (PSO) to optimize the Proportional Integral gains for Load Frequency Control (LFC) in a restructured power system that operates under Bilateral based policy scheme. Various Integral Performance Criteria measures are taken as fitness function in PSO and are compared using overshoot, settling time and frequency and tie-line power deviation following a step load perturbation (SLP). The motivation for using different fitness technique in PSO is to show the behavior of the controller for a wide range of system parameters and load changes. Error based analysis with parametric uncertainties and load changes are tested on a two-area restructured power system. The results of the proposed PSO based controller show the better performance compared to the classical Ziegler-Nichols (Z-N) tuned PI andFuzzy Rule based PI controller.

Abstract:
It is well known that the line of intersection of an ellipsoid and a plane is an ellipse (see for instance [1]). In this note the semi-axes of the ellipse of intersection will be projected from 3d space onto a 2d plane. It is shown that the projected semi-axes agree with results of a method used by Bektas [2] and also with results obtained by Schrantz [3].

Abstract: A study of the spatial and temporal changes in land use and land cover (LULC) was conducted using Remote Sensing and GIS. We analyzed the LULC of Bharathapuzha river basin, south India using multispectral LANDSAT imageries of 1973-2005 time periods. 31% depletion in the natural vegetation cover and 8.7% depletion in wetland agriculture area were seen in the basin during the period. On the other hand the urban spread in the basin increased by 32%. The study highlights the need for a scientific management plan for the sustainability of the river basin, keeping in view the recent climatic anomalies and hydrological conditions of the basin.
Morphometric Analysis of a Tropical Medium River System: A Case from Bharathapuzha River Southern India  [PDF]

Abstract: Baseline morphometric information at a sub basin level is essential to develop appropriate strategy for sustainable, socially acceptable, ecologically benign and economically viable development of a river basin. The present study was carried out in one of the less studied Bharathapuzha river basin (BRB), the second longest river in the state of Kerala, India. The annual discharge of the river is 3.94 km3. Nevertheless the basin, which receives about 1828 mm of annual rainfall, in recent years has been facing dearth of water. We used GIS and RS tools to study the morphometric characteristics of the basin. The seventh order main river is formed by several lower order streams forming a dentritic flow pattern. Basin geology, slope and rainfall pattern in the basin determine the morphometric characteristics of the basin. The linear aspects of the basin including stream length ratio and bifurcation ratio indicate the role of relief in the basin while the areal ratios indicate the elongate nature of the basin.

Abstract:

The electrical characterization of bone is essential for the better understanding of the role of electrical stimulation in bone remodeling. Calcium Hydrogen Phosphate Dihydrate or brushite (CaHPO4 2H2O) has been used in bone substitution owing to their fast resorption under physiological condition. Brushite is a suitable matrix for osteoconductive bone grafts. In this work, Calcium Hydrogen Phosphate single crystals have been grown by single diffusion gel growth technique. The powder XRD studies revealed the monoclinic structure of the grown crystals. The vibrational analysis of the crystals is done with FTIR spectroscopy and the major functional groups and their assigned vibrations are discussed. The frequency dependence of dielectric constant and ac conductivity at different temperatures have been studied in detail. This study shows decrease in the dielectric constant with the increase in frequency and temperature. The variation of ac conductivity is found to be increasing with frequency and decreasing with temperature.
