Reemplazo de la válvula aórtica con homoinjertos valvulares

Abstract: between 1989 and 2005 aortic valve homografts were done in 212 patients. the surgical techniques used were the classical ross or subcoronarian implantation in 36 patients with an average age of 54 years old. thirty one of these patients had stenosis or aortic insufficiency and 5 of them had endocarditis. the aortic root replacement technique was performed in 174 patients with an average age of 62 years old. the majority of them had complex pathologies, such as ascending aortic aneurysm, acute or chronic dissections, native and prosthetic endocarditis, annular destruction and outlet tract abscesses. the initial results were evaluated by transoperatory echocardiography and long term assessments were made by yearly echos. there were no mortality, endocarditis or re-intervention cases in the subcoronarian group during the first 4 years of evolution. in the root replacement group there was a 10.9 % hospital mortality, always related to the complex pathology intervention. there were no additional complications in the survivors during the first 3 years of evolution. the ventricular mass regression was more significant in the second group, with 26 % reduction in the first year, while the ventricular mass reduction was 11% in the first group during the same time. the subcoronorian technique had no mortality cases but it is more difficult and more residual insufficiency occurred. at the contrary, the aortic root replacement technique is more reproducible and is less influential on early insufficiency. however, the mortality is higher due to the complexity of the pathology and the magnitude of the procedure. at the present time indications have been reduced to attend patients with endocarditis, complex aortic reconstruction in older people (aneurysms, abscesses), other biological valve contraindications, small aortic ring and patients with anticoagulant contraindications.

Abstract: Se hicieron homoinjertos de válvulas aórticas en 212 pacientes entre 1989 y 2005. Las técnicas quirúrgicas usadas fueron la clásica de Ross o sea el implante subcoronario en 36 pacientes con edad promedio de 54 a os. De estos 31 pacientes tenían estenosis o insuficiencia aórtica y 5 de ellos endocarditis. La técnica de reemplazo de la raíz aórtica fue hecha en 174 pacientes con edad promedio de 62 a os. La mayoría de ellos tenían patología compleja tal como aneurismas de la aorta ascendente, disecciones agudas o crónicas, endocarditis nativa y de prótesis, destrucción anular y abscesos del tracto de salida. Los resultados iniciales fueron evaluados con ecocardiografía transoperatoria y ecos anuales suministraron evaluación a largo plazo. No hubo mortalidad, endocarditis o reoperaciones en el grupo subcoronario en los primeros 4 a os de su evolución. En el grupo de reemplazo de raíz hubo una mortalidad hospitalaria de 10,9 % siempre relacionada con la patología compleja de la indicación operatoria. No hubo complicaciones adicionales en los sobrevivientes durante los primeros 3 a os de evolución. La regresión de masa ventricular fue más significante en el segundo grupo con una reducción del 26 % durante el primer a o, mientras que la reducción de masa ventricular fue de 11 % en el primer grupo durante el primer a o. La técnica subcoronaria no tuvo mortalidad pero es más difícil y con mayor ocurrencia de insuficiencia residual. Por el contrario, la técnica de reemplazo de la raíz aórtica es más reproducible, tiene menos influencia en insuficiencia temprana, pero la mortalidad es más alta en vista de la patología compleja y la magnitud del procedimiento. Las indicaciones actuales las hemos reducido a pacientes con endocarditis, reconstrucciones complejas de la aorta en pacientes mayores (aneurismas, abscesos), contraindicaciones a otras válvulas biológicas, anillo aórtico peque o, y a pacientes con contraindicación a anticoagulantes. Zn-Al LAYERED DOUBLE HYDROXIDE PILLARED BY DIFFERENT DICARBOXYLATE ANIONS

Abstract: Zn-Al layered double hydroxides (LDHs) intercalated by terephthalate (TPH) and biphenyl-4,4'-dicarboxylate (BPH) anions have been synthesized by direct co-precipitation from aqueous solution. The Zn/Al ratio in the final materials was 1.8. The products were characterized by powder X-ray diffraction, thermogravimetric analysis, FTIR and FT Raman spectroscopy, and MAS NMR spectroscopy. The basal spacing for the TPH-LDH intercalate was 14.62 , indicating that the guest anions stack to form a monolayer with the aromatic rings perpendicular to the host layers. For the LDH intercalate containing BPH anions, a basal spacing of at least 19.2 would be expected if the anions adopted an arrangement similar to that for the TPH anions. The observed spacing was 18.24 , suggesting that the anions are tilted slightly with respect to the host layers.

El presente artículo invita a la reflexión acerca de las consecuencias que se derivan de esta postura, las dificultades que se presentan a la hora de dar cabida a nuevos planteamientos que promuevan la incorporación de valores más positivos del individuo y la necesidad de que se produzca dicho cambio.Although it is well proved that human behaviour is not always selfish, most theories used in the teaching of business management are based on that premise. This article invites the reader to think about the consequences this stance leads to, the obstacles faced when accepting new approaches that promote the consideration of more positive individual's values and how necessary such a new perspective is.

Abstract: The purpose of this paper is to prove that, through exuberant sensuous imagery - the strange tropical setting, the mystery of elements, light and darkness - Joseph Conrad, in his short-story "The Lagoon" describes and transmits Impressionistically, the dramatic story of a men surprised by his own act of cowardice, a man divided between loyalty to his own blood and love. This man, Arsat suffers a blockaded feeling towards himself because he failed to act when he had the occasion, and he missed it: he left his brother to die in the hands of the enemy while he. Arsat, escaped with his love Diamelen, to live in the "shadows" of "The Lagoon" - his own psychological, subconscious guilty conscience. The story starts with a description of nature, as motionless, and undefined as the plot still is: The forests, somber and dull, stood motionless and silent... In the stillness of the air ... minute blossoms seemed do have been bewitched into an immobility perfect and final. Abstract: This paper examines the ambiguity of characterization in Christopher Marlowe’s Edward II, probably written in 1591. Marlowe’s characters belong to the History of England in the fourteenth century and are portrayed as vividly interesting, contradicting, unpredictable human beings who sometimes surprise us, even astonish us with their sudden change of attitude and mind. The appraisal that follows is meant to demonstrate how these features are both a result of the appropriation of History and of its dramaturgical transformation. Abstract: El presente trabajo propone explorar de qué modo el contenido significativo del rumor, en la medida en que genera resonancias en el universo de las significaciones interiorizadas de un colectivo social anónimo, a nivel de los procesos primarios de su subjetividad, es capaz de generar efectos en el orden de los comportamientos o prácticas sociales. Si bien en las sociedades de cultura oral el vehículo de las noticias era el rumor que corría de persona a persona, con la generalización de Internet asumen otras características y otros medios de circulación pero no presentan siderales diferencias en cuanto a los mecanismos de su funcionamiento subjetivo.

Abstract: Proponemos un análisis comparativo de los así llamados gobiernos progresistas de América Latina tomando en cuenta cómo los movimientos que los precedieron e inauguraron las condiciones de posibilidad para esos gobiernos en cada país se han planteado algunos puntos decisivos. Esto es: a) en su capacidad de innovación, b) en la interpretación política que lograron difundir de esa invención y c) en los modos de lograr o no presentarse como fuerza constituyente de nueva institucionalidad y, por tanto, de reformulación de las relaciones con la institucionalidad existente, pero también los modos en que en el encuentro/desencuentro entre potencia multitudinaria y dispositivos de poder la relación es cambiada de signo.
