Carcinoma Sarcomatoide de la glándula tiroides: a propósito de un caso

Abstract: El carcinoma sarcomatoide de la glándula tiroides es un tumor raro, de curso rápidamente fatal y que no responde en forma favorable a ningún tratamiento conocido. Se presenta el caso de una paciente femenina de 64 a os de edad quien consultó por disnea, disfagia y disfonía progresiva, con aumento de volumen en cuello de 4 meses de evolución posterior a tiroidectomía total por tumor tiroideo cuya biopsia reportó: leiomioma de tiroides. La revisión de las láminas histológicas del espécimen quirúrgico reportó carcinoma sarcomatoide. Se realizó radiología y tomografía de cuello y tórax, fibrobroncoscopia, gammagrafía tiroidea, endoscopia digestiva superior, ecografía abdominal y biopsia por trucut. Después de considerar al tumor como irresecable se inició tratamiento con radioterapia, sin obtener una respuesta favorable. La paciente falleció a los 38 días posterior a su ingreso. We present the case of a 64 year old female patient with a 4 month history of dyspnea, dysphagia, and progressive dysphonia, with a neck mass posterior a total thyroidectomy. She had a past history of total thyroidectomy for a thyroid tumor reported as a leiomyoma four months before admission. The histologic slides from the thyroidectomy specimen were reviewed at our institution and findings were consistent with sarcomatoid carcinoma. Bronchoscopy, abdominal ultrasound, computed tomography and tru-cut core biopsy of the neck mass were performed. The patient was considered inoperable and external radiotherapy is initiated without favorable response. Patient dies 38 days after admission.

Abstract: the hand is a vital organ very important for any life activity, and it can be affected by any nerve entrapment in some place of the upper limb, which give to the suffering patients disability, and disturb their normal life. we present the mayority of the posibilities of treatment of nerve compression in the upper limb, and our experience to resolve the disablity by conservative, orthopaedic and surgical treatment.

Abstract: La mano es un órgano vital muy importante para cualquier actividad de la vida, y puede afectarse por alguna compresión nerviosa en ciertos sitios de la extremidad superior, la cual produce incapacidad y molestias en la vida normal de los pacientes que la sufren. Experimental study on the rheological behaviour of debris flow

Abstract: A model able to describe all the processes involved in a debris flow can be very complex owing to the sudden changing of the material that turns from solid into liquid state. The two phases of the phenomenon are analysed separately referring to soil mechanics procedures with regard to the trigger phase, and to an equivalent fluid for the post-failure phase. The present paper is devoted to show the experimental results carried out to evaluate the behaviour assumed by a pyroclastic-derived soil during the flow. A traditional fluid tool has been utilized: a standard rotational rheometer equipped with two different geometries. The soils tested belong to deposits that cover the slopes of the Campania region, Italy, often affected by debris flows. The influence of solid concentration Cv and grain size distribution was tested: the soils were destructurated, sieved and mixed with water starting from the in situ porosity. All material mixtures showed a non-Newtonian fluid behaviour with a yield stress τy that increases with a solid volumetric concentration and decreases for an increase of sand fraction. The experimental data were fitted with standard model for fluids. A simple relation between Cv and τy was obtained. The yield stress seems to be a key parameter for describing and predicting the post-failure behaviour of debris flows. These results suggest that in the field a small change in solid fraction, due to rainfall, will cause a slight decrease of the static yield stress, readily inducing a rapid flow which will stop only when the dynamic yield stress is reached, namely on a much smoother slope. This can explain the in situ observed post-failure behaviour of debris flows, which are able to flow over very long distances even on smooth slopes.
