ADVANCED DIAGNOSTIC TESTING FOR FEMALE PELVIC FLOOR DYSFUNCTIONS

Abstract: The word “pelvic floor dysfunction” has different meanings in different specialties, i.e. radiology, urology, gynaecology, coloproctology. Despite the fact that the concept of perineology is not exactly new, most clinicians have only slowly adapted their practice to this transversal view. The multidisciplinary approach (several specialists dealing with various pelvic floor problems) still prevails over the interdisciplinary one (one specialist explaining what is happening) with several problems in the choice of therapeutic strategies. Obviously, the interdisciplinary approach requires a wide knowledge of the principles and techniques of each specialty. This monographic issue reviews advanced diagnostic testing for female pelvic floor dysfunctions from the perspective of a single specialist, namely a urologist.
Focusing inversion techniques applied to electrical resistance tomography in an experimental tank

Abstract: We present an algorithm for focusing inversion of electrical resistivity tomography (ERT) data. ERT is a typical example of ill-posed problem. Regularization is the most common way to face this kind of problems; it basically consists in using a priori information about targets to reduce the ambiguity and the instability of the solution. By using the minimum gradient support (MGS) stabilizing functional, we introduce the following geometrical prior information in the reconstruction process: anomalies have sharp boundaries. The presented work is embedded in a project (L.A.R.A.) which aims at the estimation of hydrogeological properties from geophysical investigations. L.A.R.A. facilities include a simulation tank (4 m x 8 m x 1.35 m); 160 electrodes are located all around the tank and used for 3-D ERT. Because of the large number of electrodes and their dimensions, it is important to model their effect in order to correctly evaluate the electrical system response. The forward modelling in the presented algorithm is based on the so-called complete electrode model that takes into account the presence of the electrodes and their contact impedances. In this paper, we compare the results obtained with different regularizing functionals applied on a synthetic model.
Focusing inversion technique applied to radar tomographic data

Abstract: Traveltime tomography is a very effective tool to reconstruct acoustic, seismic or electromagnetic wave speed distribution. To infer the velocity image of the medium from the measurements of first arrivals is a typical example of ill-posed problem. In the framework of Tikhonov regularization theory, in order to replace an ill-posed problem by a well-posed one and to get a unique and stable solution, a stabilizing functional (stabilizer) has to be introduced. The stabilizer selects the desired solution from a class of solutions with a specific physical and/or geometrical property; e.g., the existence of sharp boundaries separating media with different petrophysical parameters. Usually stabilizers based on maximum smoothness criteria are used during the inversion process; in these cases the solutions provide smooth images which, in many situations, do not describe the examined objects properly. Recently a new algorithm of direct minimization of the Tikhonov parametric functional with minimum support stabilizer has been introduced; it produces clear and focused images of targets with sharp boundaries. In this research we apply this new technique to real radar tomographic data and we compare the obtained result with the solution generated by the more traditional minimum norm stabilizer.

Abstract: in 1882, a sphere of sociability was created in san miguel de tucumán that brought together students, graduates and teachers from two educational institutes established in the province during the second half of the 19th century within the framework of the emergence of the nascent central state, the teachers' college and the national college. known as the sarmiento society, it constituted a literary sphere that enabled its members to achieve a sort of continuity in their studies in the absence of a university. the society embarked on various major endeavors, including the creation of a library and the edition of two periodic publications, el porvenir and el tucumán literario. these projects shaped an experience of male sociability, with a strong liberal imprint, that developed in constant tensión with the project of including the latest arrivals in the world of education, literature and reading, women. the central concern of the article is to examine the ways through which women were incorporated into this space ancl took up the challenge of beginning to express themselves in their own way.

Abstract: En 1882 se creó en San Miguel de Tucumán un espacio de sociabilidad que reunía a alumnos, egresados y maestros de dos instituciones educativas implantadas en la provincia durante la segunda mitad del siglo XIX en el marco del despliegue del naciente Estado central, la Escuela Normal y el Colegio Nacional. Se trata de la Sociedad Sarmiento que se constituyó en un ámbito literario que permitía a sus miembros una suerte de continuidad de sus estudios ante la ausencia de una universidad. La Sociedad llevó a cabo importantes emprendimientos, entre los que destacan la creación de una biblioteca y la edición de dos publicaciones periódicas, El Porvenir y El Tucumán Literario. A partir de estos proyectos se fue moldeando una experiencia de sociabilidad masculina, con una fuerte impronta liberal, que se desarrolló en constante tensión con las posibilidades de incluir en el proyecto a las recién llegadas al mundo de la educación, de la literatura y de la lectura: las mujeres. La preocupación central del artículo es examinar las vías a través de las cuales estas se incorporaron a ese espacio y asumieron el desafío de comenzar a expresarse con su propia voz. Fertility change in Egypt: From second to third birth

Abstract: This work focuses on Egypt, a country that notwithstanding its advanced stage of socio-demographic transition has shown near stagnation in the reduction of fertility levels in the last decade. The progression from second to third birth is a crucial component in fertility change, since the reduction especially of third and higher-order births maintains the fertility decline. For this reason, the study aims at analyzing the main determinants of the third-birth intensities of Egyptian two-child mothers, applying an event-history analysis to the 2000 Egyptian Demographic and Health Survey. The study's results show that fertility differentials among the country's social groups continue to persist. Moreover, the difficult change in the fertility of women with high educational standards seems to be responsible for the stalling fertility decline during recent years. The analysis, however, has demonstrated that within the framework of the ongoing process of modernization in the country, even the most laggard groups of women showed a decrease in third-birth intensity during the 1990s. The study also reveals that the preference for at least one son in the family on the progression to the third child is weakening among women who have completed secondary education.

Abstract: we studied two ctx-m-2-producing klebsiella pneumoniae clinical strains, k96005 and k13, isolated from hospitalized patients in uruguay, during 1996 and 2003, respectively. the genomic surroundings of blactx-m-2 were characterized by pcr-mapping and dna sequencing. our results show that blactx-m-2 is included in a complex class-1 integron (ink13), associated with an orf513 in both isolates. the genetic array of the integron, aac(6')-ib, blactx-m-2, orfd (gene cassette region), associated with an orf513-blactx-m-2, seems to be widely disseminated over the río de la plata region.

Abstract: for the first time in history, the 2000 round of censuses in brazil, mexico and chile asked citizens a question about the location of their places of work or study. analysis of census data produced new results on how daily commutes in rio de janeiro, sao paulo, santiago and mexico city vary across socioeconomic groups. in santiago and mexico city, where the census question better captures daily commutes, there is an interesting paradox regarding individuals economic status and their geographic location because the likelihood that one's workplace is located in a different municipality from their residence increases as their socioeconomic status rises. however, the poorest municipalities tend to have higher indices of daily commuting this is due to the concentration of employment in the central business districts and wealthy areas, which are usually located far from the poorest municipalities, especially in highly segregated cities. this finding challenges the hypothesis that the latin american cities of the future will be polycentric.
Better in the dark: two Mediterranean amphibians synchronize reproduction with moonlit nights

Abstract: In Amphibians, both positive and negative correlations between activity and full moon phase have been observed. In this study, we present data for two anuran species (Hyla intermedia and Rana dalmatina) studied in a hilly Mediterranean area of central Italy. We analysed, in a two-year survey, the relationships between the number of egg clutches laid each night and the moon phases by means of circular statistics. Moreover, the studied species exhibited clear oviposition site selection behaviour influenced, at least in H. intermedia, by moon phases. We observed the occurrence of an avoidance effect by amphibians for oviposition and specific egg-laying behaviour during moon phases around the full moon. This apparent lunar phobia was evident in both species when yearly data were pooled. On the other hand, while this pattern continued to be also evident in H. intermedia when single years were considered, in R. dalmatina it stood just in one year of study. Nonetheless, during cloudy nights, when moonlight arriving on the ground was low, the frogs' behaviour was similar to that observed in new moon phases. We interpreted the observed pattern as an anti-predatory strategy. Overall, comparisons between our own study and previous research suggest that there was insufficient evidence to establish any unequivocal patterns and that further research in this regard is needed.
La fecundidad alta en el istmo centroamericano: un riesgo en transición

Abstract: Por muchas razones, índices reproductivos elevados suelen considerarse una desventaja. En términos empíricos, el planteamiento se apoya en dos hechos estilizados: (a) la mayor fecundidad prevaleciente en comunidades pobres (por ejemplo en países pobres), lo que sugiere una relación entre ambos aunque sobre los mecanismos a través de los cuales opera tal relación aún haya discusión; (b) las brechas de fecundidad que hay entre grupos socioeconómicos de la población y que obedecen sistemáticamente a una sobrefecundidad de los pobres. Ambas piezas de evidencia tienden a considerar indicadores agregados de la fecundidad, como la tasa global de fecundidad, lo que pierde, en parte, la componente individual de la relación entre trayectoria reproductiva intensa y desventajas sociales. Recientemente se ha propuesto un enfoque de vulnerabilidad para trabajar directamente con las mujeres y los hogares afectados por esta trayectoria reproductiva intensa. Este enfoque que se basa en: (a) la identificación de un riesgo (evento probabilístico que tiende a generar adversidad); (b) la estimación de las probabilidades de materialización del mismo entre grupos sociales o áreas geográficas; (c) la detección de respuestas concretas o potenciales para enfrentar tales adversidades una vez materializado el riesgo; y (d) el análisis de mecanismos de adaptación, para acomodarse a la persistencia del riesgo tiene ventajas en términos de política, pues remite directamente a una población que puede ser considerada "objeto" de las intervenciones en materia de salud sexual y reproductiva. Así, además de cuantificarla y caracterizarla puede localizarla, mas aun si como fuente primaria de información se usan los microdatos censales. En este documento se usará dicho enfoque considerando como fuentes de información las bases de microdatos censales de los países del Istmo Centroamericano disponibles en CELADE. El análisis se concentrará en los dos países que tienen más de una base de microdatos (Costa Rica, 1973, 1984 y 200; Panamá, 1990 y 2000), aunque también está previsto un examen más detenido del caso mexicano (o los estados del Sur de México). El documento dará continuidad y profundizará una línea de trabajo sobre vulnerabilidad sociodemográfica desarrollada por CELADE desde 2001.
