Two Cases of Vulvovaginitis Caused by Shigella flexneri and Shigella sonnei: a Case Report

Abstract: Vulvovaginitis caused by Shigella species (Shigella spp.) has rarely been reported. This paper describes two cases of prepubertal vulvovaginitis, presenting with a bloody and purulent vaginal discharge, separately caused by ampicillin-resistant Shigella flexneri and trimethoprim-sulfomethoxazole-resistant Shigella sonnei. Our conclusions are that Shigella spp. is the potential cause of vulvovaginitis in prepubertal girls in developing countries where these pathogens are endemic, and identification of the bacteria and making antibiotic susceptibility testing in these cases should not be overlooked.

Abstract: The usage of computer technologies with telecommunication, the recent developments in the field of microelectronics caused significant changes in communication in today’s groving competition arena. The Information Technologies (IT) providing the production of data, its storage, verification and transmission are also very important for the tourism industry. The establishments operating in tourism industry need to use IT intensively to collect information about organizations, consumers, competitors and other environmental factors to transmit these data to their employees, to provide the daily operations more effectively and productively and to understand the global marketing applications. The purpose of this study is to identify how many of the information technologies were used by city and resort hotels, their usage areas, and the attitudes of hotels towards IT. The results of the study showed that the 5-star resort hotels in Antalya had more differences compared to the hotels in stanbul with respect to the usage of IT. These differences appeared either non existence for most of the IT or rare applications of it.
Intrathoracic kidney with a right-sided Bochdalek hernia: a case report

Abstract: An intrathoracic ectopic kidney as a result of a congenital diaphragmatic hernia is a rare congenital anomaly. This anomaly is usually asymptomatic and discovered incidentally on routine chest radiography. We report the case of a 16-year-old girl who had a right intrathoracic kidney with a right Bochdalek hernia.
Giant Ureterohydronephrosis Due To A Ureteral Stone

Abstract: A 61-year-old man presented with left renal colic pain within 5 days. He had no history of ureteral colic and no family history of renal stone disease. Physical examination revealed left costovertebral angle tenderness and a palpable mass in the left flank region. Roentgenelogic examination including direct urinary system graphy showed a left pelvic radiopaque nodule (Figure 1). The serum creatinine level was 1.8 mg/dl. The rest of the laboratory parameters were within normal limits. Computed tomography of the abdomen revealed a left lower ureteral stone (2.5cm) and giant ureterohydronephrosis (Figure 2 A, B, C). He received ureteroscopic lithotripsy and placement of a double-J ureteral stent. The most common cause of the giant hydronephrosis is ureteropelvic obstruction, stones, tumors and trauma. Urolithiasis could present asymptomatically and result in a rare condition termed giant hydronephrosis, defined as the presence of more than 1 L of fluid in the collecting system [1]. As in our case the reason of the hydronephrosis and hydroureter is the obstruction caused by a distal ureteral stone. Percutaneous nephrostomy followed by subsequent lithotripsy and even nephrectomy in the case of non-functioning kidney may be the other treatment options.
Six minute walking distance in kyphoscoliosis patients with chronic respiratory failure

Abstract: This prospective cross-sectional study was carried out in a tertiary training and research hospital in Turkey. Kyphoscoliosis patients with CHRF on home mechanical ventilation (HMV) followed in a respiratory intensive care unit (RICU) out-patient clinic were enrolled. Patients' demographics were recorded as well as transthoracic echocardiography (ECHO), 6MWD, spirometry, arterial blood gas (ABG) values and high resolution chest computed tomography. 6MWT results were compared with other parameters.Thirty four patients with kyphoscoliosis and chronic respiratory insufficiency admitted to our outpatient clinic were included in the study but 25 (17 M) patients underwent 6MWT (8 patients walked with oxygen supplement due to PaO2 < 60 mm Hg). The mean 6MWD was 274.4 ± 76.2 (median 270) m and median 6MWD predicted rate was 43.7% (inter quartile ratio, IQR, 37.6% to 47.7%). Median HMV use was 3 years (IQR 2-4). 6MWD predicted rate, body mass index (BMI), HMV duration were similar in male and female patients. 6MWD correlated well with age, BMI, dyspnea score for baseline 6MWT (r: - 0.59, p < 0.002, r: - 0.58, p < 0.003, r: - 0.55, p < 0.005 respectively) but modestly with forced expiratory volume in one second, pulse rate for baseline 6MWT, pulse saturation rate, fatigue and dyspnea score at end of 6MWT (r: - 0.44, p < 0.048; r: 0.44, p < 0.027; r: - 0.43, p < 0.031; r: - 0.42, p < 0.036; r: - 0.42, p < 0.034 respectively). 6MWD predicted rate was only correlated with dyspnea score at baseline (r: - 0.46, p < 0.022). The systolic pulmonary arterial pressure (PAPs) in 6 (24%) cases was more than 40 mmHg, in whom mean PaO2/FiO2 was 301.4 ± 55.4 compared to 280.9 ± 50.2 in those with normal PAPs (p > 0.40).The 6MWT is an easy way to evaluate physical performance limitation in kyphoscoliosis patients with chronic hypercapnic respiratory failure using home mechanical ventilation. Abstract: The aim of this study is investigating the Levels of Strain, From the Point of Various Variables (according to gender, branch, working at a job which provides financial gain, the reason to prefer the teaching profession, the number of digests of related field which have been followed, the frequency of weekly using internet, the aim of using internet, the frequency of weekly utilization from a library at studies related with branch, and the number of read books monthly) , at Their Efforts of Obtain Information of Preservice Teachers’ of Secondary Education. Study carried out by general screening model and data has been collected by content validity and reliability has been through a survey. The opinions of 115 teachers were included in the study. The scale developed by the researcher used in the research. Cronbach Alpha reliability coefficient of the scale has been determined as .968. The value of the normal distribution of the data set as 6.57. Preservice teachers’ in their efforts to obtain information, the information needed to identify, access to information, assessment, use and in this process to consider the ethical and legal values of stages have been forced from time to time. Preservice teachers’ views on the strain level did not vary according to personal characteristics. Preservice teachers in the social field to learn to work less hard than Preservice teachers in the fields of science and mathematics. Study skills and habits of the prospective teachers to learn to be scraped in pre-service education and in-service training of vocational qualifications to support this have been proposed.
Tourism as a paradox creative sector from socio-cultural perspective: A sociological analysis on Dalyan destination

Abstract: While the local people and tourists being in a confrontation is a desirable situation during the tourism activities’ which has economic income and employment creation impact, taking places on the destinations of which the starting point to be their natural and historical resources, it could emerge as a situation that creates paradoxes as well as, on the regions where the necessary socio-cultural awareness is no generated on the local people. In this context, the purpose of this study is to evaluate perceptions of locals in the scope of socio-cultural and economic in Dalyan destination towards tourists coming to this destination. Based on this aim, depth interview technique which is a qualitative research method based on the idea that it could provide more concrete data is preferred. Data, obtained from interviewed participants in the selected sample were subjected to sociological analysis. As a result of the sociological analysis the concerns of the local people especially about the socio-cultural degeneration on behaviors and movements of youths created by tourism which contributes economically in the region was observed.

Abstract: This study highlights populism which is as important as estatism and its changing functionality with regard to periods. Keynesyen socio-economic policy that give priority to improvement of job, employment, wage and societal wellfare generally after WW II, completely abondened with neo-liberalizm and therefore populism that is fairly effective in discourse but action has begun to appear. However, in 1950’s, populism mentioned above has begun put forward its capital tendenced characteristics. This means that neo-liberalizm will be able to be appreciated as a functional characteristcs that make the transfer of capital of neo-libralism especially to the developing countries easy within following years. On the other hand, in this progress, a social context that will put the probability and possibility of a socio-economic policy which gives priority to labour especially after 1970’s on agenda has occured. Nonetheless, after 1980’s, neo-liberalism emerged and intensified global popülizm as an ascendent and perfect social instrument that highly effects whole everyday life and give priority to the individual freedom and therefore to the consumption in order to increase the power of capital despite all of the functions of labour. In fact, post-modernism that emphasizes individualism, freedom and therefore unlimited consumption with regard to modernism that is too difficult to be applied and that give priority to centralism and universalism has notably increased the extent and effectiveness of global populism. So, belief in globalistaion of Turkey’s societal structure and becoming compatible with this era as other developing societies put clearly forth the extention of global populism. Abstract: Comparative literature is a discipline which bases on comparison between literary works and authors. A comparison is necessary in order to fasten down the influences of a work or author on another work or author. Although this discipline rather handles the works written in different languages, also examines the interaction among literary products of same language. Postmodernism raised the importance of comparative literature. Through intertextuality, which is used by postmodernism, the effects among works became more clear. Some parallelisms draw the attention between the novels of Umberto Eco’s Gülün Ad and Orhan Pamuk’s Benim Ad m K rm z in context of comparative literature. The similarities between the two works are being put into words on various occasions but the nature of this resemblance is not being mentioned too much. The events passing through on closer centuries, having contents based on knowledge and art, the quarrel between reformers and conservatives, the methods of approaching to murders and solving them evoke between two novels. In this study, we tried to examine the probable effects of Gülün Ad on Benim Ad m K rm z . Mukayeseli edebiyat, edeb eserler, yazarlar aras nda kar la t rmay esas alan bir disiplindir. Rheumatoid Arthritis and Disease Modifying Antirheumatic Drugs

Abstract: It has now been well established that in Rheumatoid Arthritis early treatment with disease modifying antirheumatic drugs (DMARDs) should be initiated to control the inflammatory activity, structural damage and function. Toxicity of the drugs should be monitored carefully. In cases with insufficient response aggressive treatment options can create a difference so additional strategies should be taken into consideration in time. This review mentions about the more commonly prescribed DMARDs such as methotrexate, sulphasalazine and leflunomide. Practical tips on special situations like pregnancy, lactation and perioperative period are given, first drug choice and combination therapies are summarized.
