Safety and efficacy of sildenafil in Brazilian men with erectile dysfunction secondary to spinal cord injury

Abstract: Objective: To evaluate the effect of sildenafil in Brazilian patientswith erectile dysfunction secondary to spinal cord injury. Methods:Participants were examined at the time of enrollment and two andsix weeks later. Baseline and follow-up data on sexual functionwere collected. After the second week, the initial dose of sildenafil(50 mg) could be adjusted according to efficacy and tolerability.Efficacy was assessed primarily through the scores on questions 3and 4 of the International Index of Erectile Dysfunction. Secondaryanalyses included questions and domains of the index, globalefficacy evaluation, percentage of success in sexual intercourseattempts, answers to the Quality of Life and Erectile FunctionQuestionnaire, and partner’s satisfaction. Results: Ninety-onepatients were evaluated as to efficacy and 94 as to safety. Medianage of patients was 33 years, and median time between spinal cordinjury and study enrollment was three years. Sildenafil produceda significant increase in the mean scores of questions 3 and 4(p < 0.001 in both comparisons), as well as in other questionsand in all domains of the International Index of Erectile Dysfunction.Improved erections were reported by 89% of patients, and theproportion of successful sexual intercourse attempts went from6 to 74% (p < 0.001). Mean scores on the Quality of Life andErectile Dysfunction Questionnaire increased from 60 to 74%(p < 0.001). Ninety percent of 42 women reported being moderatelysatisfied to very satisfied with their partners’ treatment. The mostcommon adverse events were headache (16%), flushing (11%) andnasal congestion (10%). Conclusions: Sildenafil proved to be safeand effective in treating Brazilian men with erectile dysfunctionsecondary to traumatic spinal injury.

Abstract: Ao atuar como perito do juízo, o contador responsável por um escritório de perícias deverádecidir sobre os honorários a serem cobrados, considerando as especificidades de cadaesfera judiciária, o tempo médio de recebimento e o fator remuneratório das períciasrealizadas. Esses elementos levar o o perito a delimitar seu campo de atua o judicial.Assim, este trabalho procurou demonstrar como a Pesquisa Operacional, por meio daPrograma o Linear, pode ser usada no processo decisório relativo aos honorários e ao mixoperacional de um escritório de perícias. A pesquisa é aplicada, estrutural e exploratória,baseada na proposi o de Abramo (1979), suportada por investiga o documental e bibliográfica,realizada durante o 2o. semestre de 2005. Foi desenvolvido de um modelo dePrograma o Linear, com base na proposta de Corrar et al. (2004). Os resultados obtidosforneceram informa es que podem contribuir para otimizar o mix operacional do escritóriode perícias nas esferas cível e trabalhista, considerando-se tempo médio de recebimento e aremunera o em cada esfera judiciária, e ilustraram como a Pesquisa Operacional podeauxiliar o perito contábil, enquanto tomador de decis es na gest o de um escritório de perícias.

Abstract: introduction: the dentist's liability can be understood as the criminal, civil, ethical and administrative obligation that they have in their professional exercise. thus, when producing a harmful result to the patient, due to imprudence, ineptitude or recklessness, the dentists will be liable to the foreseen penalties on the civil code, where the compensation will be obligatory to satisfy the damage according to the reached consequence. in these types of processes, the involved parts will be able to contract a technical assistant to supply the respective lawyers about biological, technician and professional knowledge. aim: this paper aim to report about the technical assistant's importance, in the performance of each one of the involved parts in a civil process. conclusion: it is necessary a major knowledge, by dentists, about legal and ethical aspects in the professional activity.
Técnica intramuscular na gluteoplastia de aumento

Abstract: background: a harmonious gluteal region is considered an essential element in the composition of body beauty and expression of femininity. therefore, there is growing interest, from both men and women, in the aesthetical improvement of this region. the aim of this study is to present an alternative to the published augmented gluteoplasty techniques, as well as to demonstrate the viability of this alternative as a safe and reproducible technique. based on intramuscular placement of the prosthesis, the technique uses simplified limits of dissection with reference to the fixed anatomical structures (bones) of the gluteal region in order to construct the implant pocket. methods: from 2001 to 2011, 18 patients with ages ranging from 25 years to 50 years underwent augmentation gluteoplasty. specific implants were used for the gluteal area, with implant volume ranging from 250 ml to 800 ml (mean, 367 ml). results: there was a significant increase in the gluteal volume of patients along with achievement of natural and harmonious contours. immediate and long-term satisfaction with the cosmetic results was obtained in 100% of patients. conclusions: the use of intramuscular gluteal prostheses is a safe and easily reproducible method of augmentation gluteoplasty with good results and a low complication rate. determination of the limits of detachment using fixed bone structures simplifies the implementation of this technique, allowing the use of even larger implants than those reported in the literature.

Abstract: the objective of this study was to evaluate the effect of different poultry house heating systems on the thermal comfort and performance of broiler chicks during the winter period, in the southern region of brazil. three buildings were used, each containing 17,700 broiler chickens (cobb), during two complete productive cycles. three heating systems were evaluated: furnace with indirect air heating; infrared heater and radiant experimental "drum" system with an infrared supplemental heating system. in order to evaluate the thermal environment and animal performance, a randomized block experimental design with three heating system was used, in two complete productive cycles. in the first two weeks after birth, the radiant experimental "drum" system is the most efficient in maintaining the air temperature and relative humidity in thermal comfort condition of the broiler chicks and, consequently, result in the best results of weigh gain, dietary conversion and efficient production.

Abstract: introduction: phytotherapy has been drawing interest from the scientific community regarding its potential wound healing properties. there are few studies available that present a correct scientific methodology and, therefore, phytotherapy remains an open field for further research. purpose: to evaluate the effects of jatropha gossypiifolia l. (bellyache bush) on the healing of colonic anastomosis in rats. methods: forty wistar rats were subjected to a complete section of the colon, that was soon sutured with eight separate stitches using monofilament polipropilene 6-0. randomly, the animals were divided into control group (cg), with 20 rats, which were injected with 0.9% sodium chloride solution intraperitoneally (1ml/kg) and jatropha group (jg), also with 20 rats, which were injected with jatropha gossypiifolia l. alcoholic extract intraperitoneally (1ml/kg). each of the groups was subdivided into two subgroups of ten rats, according to the sacrifice date, third and seventh days (cg3/ cg7 and jg3/ jg7). after sacrifice, the segment of the colon containing 1cm proximal and distal to the site of anastomosis was removed and subjected to the pressure test with air insuflation. this segment was then opened and a 1.0 x 0.5cm part was removed and placed under 10% formaldehyde for histological analysis with hematoxylin-eosin and masson's trichrome stainings. results: the evaluation of the pressure rupture test demonstrated statistical significance regarding the sacrifice date. on the third day, the average pressure of the cg (25,4mmhg) and the jg (76,4mhg) revealed p = 0.013, and on the seventh day, the average pressure of the cg (187,3 mmhg.) and the jg (135,1 mmhg.) revealed p = 0.014. when assessing the microscopic variables among the groups of the 3rd day, only the variables edema and polymorphonuclear cells did not show statistical differences. on the 7th day, all of the variables assessed demonstrated significant differences. conclusions: regarding mechanical resist

Abstract: purpose: to assess the evolution of epileptic seizures during pregnancy and the occurrence of malformations in neonates born to epileptic mothers who used anticonvulsant drugs during pregnancy, as well as the perinatal characteristics of the newborns. methods: a total of 126 medical records of epileptic patients seen at the high-risk pregnancy outpatient clinic were analyzed retrospectively in terms of the following variables: age, parity, diagnosis of the type of epileptic seizure, anticonvulsant drug used during the prenatal period, evolution of epileptic seizures during the prenatal period, type of delivery, gestational age at resolution, and perinatal characteristics of the newborns. results: the incidence of pregnant women with epilepsy was 0.2% in relation to prenatal patients, with simple partial epilepsy being the most frequent type (40% of cases). monotherapy was applied to 75% of the patients and carbamazepine was the most frequently used drug. among the 111 patients evaluated in terms of course of the disease during pregnancy, 53% showed no change, 31% became worse and 16% improved. normal delivery was performed in 62.5% of cases, with a satisfactory perinatal result in terms of apgar score, and with a rate of low birth weight neonates above the values for low-risk populations. no fetal malformations were observed. conclusion: epilepsy showed a favorable course during pregnancy and was not aggravated by the latter, with cases of worsening of signs and symptoms being associated with epilepsy of difficult control before pregnancy. evaluation of the perinatal characteristics of the neonates showed satisfactory apgar scores and evolution, indicating that epilepsy and anticonvulsant drugs do not cause severe impairment of intrapartum vitality. no cases of malformations or hemorrhagic complications were detected in the present study.

Abstract: the behavior of passionfruit, cultivated in sites with disease history, as to premature death, was researched at universidade estadual paulista, campus of jaboticabal, sp. the purpose of the research was to evaluate the behavior of acesses and passionfruit species related to this disease, whereas resistant plants are to be used as yellow passionfruit rootstocks, as well as in breeding programs. the species p. edulis sims, p. edulis sims f. flavicarpa degener, p. nitida h.b.k., p. cincinnata, p. giberti, p. laurifolia, p. morifolia, p. foetida, p. capsularis were used. in a site with a known record of the disease, the plants were planted in a variable number and at distinct times. plant conduction was done as recommended for yellow passionfruit. the premature death of the plants ocurred somewhere between two months and two years of field culture. p. giberti, p. nitida were resistant to the disease despite its place of origin. no other promising resistance sources were found among the remaining "acesses". however, new "acesses" and new species should be researched in search of resistance.
Prevalence of HIV, syphilis, hepatitis B and C among adults with mental illness: a multicenter study in Brazil

Abstract: abstract objective: there is evidence that patients with mental illness have increased prevalence of sexually transmitted infections, but data in brazil are scarce. the objective of this study was to determine the prevalence of hiv, hepatitis c and b, and syphilis among patients with mental illness in brazil. method: a multicenter representative sample of adults with mental illness was randomly selected from 26 mental health institutions throughout brazil. sociodemographic, sexual behavior and clinical data were obtained from person-to-person interviews and blood was collected for serology testing. seroprevalence with 95% confidence limits were obtained correcting for sampling scheme. results: of the 2,475 patients interviewed, 2,238 had blood collected. most participants were sexually active ever (88.8%) or in the last 6 months (61.6%), female (51.9%), and single (66.6%). half of the sample had less than 5 years of schooling and the mean monthly individual income was low (us$ 210.00). condom use was very low either during lifetime (8%) or in the last 6 months (16%). overall seroprevalence were 1.12%, 0.80%, 1.64%, 14.7% and 2.63% for, respectively, syphilis, hiv, hbsag, anti-hbc and anti-hcv. conclusions: seroprevalences found were higher than other populations with representative studies in brazil, with high rates of sexual risk behavior. this is of public health concern, and prevention and care strategies for sexually transmitted infections among psychiatric patients should urgently be implemented by health authorities.

Abstract: purpose: to assess the validity of several fetal weight charts, commonly used in portugal, to classify its population. methods: observational retrospective study. singleton birth data was analyzed, from a two- year period (may 2008 to april 2010), from pregnancies with an ultrasound in the same institution, between the 8th and 14th gestational week. upon data validation, percentiles for each completed gestational week were created, smoothed by a quadratic function, analyzed and compared to the tables more commonly utilized, in the institution and country, by using z-scores, percentile comparison, sample 10th percentile detection sensibility and birthweight means comparison. results: a total of 5,378 newborns (nb) were born in the period; 2,195 (42%) nb were included, born from the 24th to 42nd gestational week, allowing statistical analysis from the 34th to the 41st week. there were differences in the mean birthweight for each gestational age, between references and with the sample, as well as between sexes. the 10th percentile from some references has shown differences ranging from -288g at 37 weeks (-11% in lubchenco et al. data), with and +133g at 34 weeks (+7,6% with carrascosa et al. data) compared to the values found with the sample. differences were also found concerning the sensitivity of the identification of a sample birthweight below the 10th percentile, which was between 14.1 and 100%, depending on the reference used. discussion: the limitation of these kinds of reference values must be remembered and minimized, with the adoption of regionally or nationally produced references, contemplating other variables, such as sex, with precisely known gestation duration and with validation of the utilized references in loco.
