Arabin Cerclage Pessary as a Treatment of an Acute Urinary Retention in a Pregnant Woman with Uterine Prolapse

Abstract: A 35-year-old gravida 7, para 1, and abortus 5?female with hypogastric pain and inability to void urine after 14?+?3 weeks of amenorrhea was examined in the emergency department. One year before, a uterine prolapse had been diagnosed in another hospital. Examination showed a uterine prolapse grade 2 with palpable bladder. The patient was unable to void urine. After a manual reduction of the uterine prolapse, the patient underwent an emergency catheterization for bladder drainage. A Hodge pessary (size 70) was placed, which led to spontaneous micturitions. Due to the persistence of the symptoms the following day, Hodge pessary was replaced by an Arabin cerclage pessary. Although the pessary could be removed from the beginning of the second trimester, due to the uterine prolapse as a predisposing factor in the patient and the uncomplicated progression of pregnancy, it was decided to maintain it in our patient. Therefore, Arabin cerclage pessary allowed a successful pregnancy outcome and was not associated with threatened preterm delivery or vaginal infection. 1. Introduction Acute urinary retention (AUR) is defined as the inability to void urine, with a retained volume of urine of 200?mL or greater [1]. AUR in early pregnancy is a very rare complication which leads to a real emergency [2]. 2. Case Presentation A 35-year-old gravida 7, para 1, and abortus 5 female with hypogastric pain and inability to void urine after weeks of amenorrhea was examined in the emergency department. One year before, a uterine prolapse had been diagnosed in another hospital. Examination showed an anteverted uterus and uterine prolapse grade 2 with palpable bladder. The patient was unable to void urine. Ultrasound revealed a cervical length of 30?mm and a singleton, cephalic fetus. A manual reduction of the uterine prolapse was made, and the patient underwent an emergency catheterization for bladder drainage (500?mL urine). No urinary tract infection was found. Then, it a Hodge pessary (size 70), and was placed after a spontaneous micturition, the woman was discharged for further ambulatory followup. The following day, the patient was admitted in to the emergency department because of a new AUR. Hodge pessary was replaced by another one of size 75, and due to the persistence of the symptoms, finally an Arabin cerclage pessary was placed (Figure 1). On spacelike surfaces in 4-dimensional Lorentz-Minkowski spacetime through a lightcone

Abstract: On any spacelike surface in a lightcone of four dimensional Lorentz-Minkowski space a distinguished smooth function is considered. It is shown how both extrinsic and intrinsic geometry of such a surface is codified by this function. The existence of a local maximum is assumed to decide when the spacelike surface must be totally umbilical, deriving a Liebmann type result. Two remarkable families of examples of spacelike surfaces in a lightcone are explicitly constructed. Finally, several results which involve the first eigenvalue of the Laplace operator of a compact spacelike surface in a lightcone are obtained.
The European rabbit (Oryctolagus cuniculus L.) as seed disperser of the invasive opium poppy (Papaver somniferum L.) in Robinson Crusoe island, Chile

Abstract: we investigated whether introduced european rabbit on robinson crusoe island, chile, has the potential to spread the seeds of the exotic papaver somniferum (opium poppy) via endozoochory. a total of 1320 rabbit droppings were collected during early autumn in the study area and analyzed in laboratory with binocular magnifying glass. seeds from six different species were found in the droppings: papaver somniferum (papaveraceae), centaurea melitensis (asteraceae), amaranthus sp. (amarantaceae), melilotus indicus (fabaceae), rumex acetosella (polygonaceae), and one unidentified species. poppy seeds were dominant. most seeds (82%) were destroyed during ingestion, but some of the intact seeds found in pellets remained viable. germination rate of ingested seeds was similar to control seeds but germination success was lower in the former. poppy seeds were found in pellets deposited inside poppy patches but also up to 100 m away from patches. in this way, rabbit expands the seed shadow of the plant. our results suggest that, although poppy lacks morphological adaptations to long distance dispersal, seeds reach favorable places for seedling germination and recruitment via consumption by rabbits, an unspecialized seed disperser. rabbit activity, together with the effect of other animals and some abiotic factors-wind and rain water, principally-contributes to the spread of poppy with implications for the management of both invasive species in the island.

Abstract: Se ha investigado si el conejo europeo, introducido en la isla de Robinson Crusoe, Chile, tiene capacidad para dispersar semillas de la amapola del opio (Papaver somniferum) vía digestiva. Se recolectaron 1320 fecas a comienzos de oto o en el área de estudio y fueron analizadas en laboratorio con lupa binocular. Se encontraron semillas de seis especies: Papaver somniferum (Papaveraceae), Centaurea melitensis (Asteraceae), Amaranthus sp. (Amarantaceae), Melilotus indicus (Fabaceae), Rumex acetosella (Polygonaceae) y una no identificada. Las semillas de amapola fueron las más numerosas. La mayoría de las semillas (82%) fueron destruidas durante la ingestión, pero algunas de las que aparecieron intactas en las fecas se mantuvieron viables tras el tránsito por el aparato digestivo. La tasa de germinación de las semillas ingeridas fue similar a la de las semillas control, pero el éxito germinativo fue menor en las primeras. Se encontraron semillas de amapolas en fecas depositadas dentro de las manchas de amapola pero también en fecas depositadas hasta 100 metros de distancia. Por lo tanto, el conejo expande semillas de esta planta. Nuestros resultados sugieren que, a pesar de que la amapola carece de adaptaciones morfológicas específicas para la dispersión a larga distancia, sus semillas pueden alcanzar lugares favorables para la germinación y establecimiento a través del conejo, un dispersor no especializado. La actividad del conejo, junto al efecto de otros animales y de algunos factores abióticos, principalmente el viento y la lluvia, contribuyen a la expansión de la amapola con implicaciones para el manejo de ambas especies invasoras en la isla.

A new method to construct spacetimes with a spacelike circle action

Abstract: A new general procedure to construct realistic spacetimes is introduced. It is based on the null congruence on a time-oriented Lorentzian manifold associated to a certain timelike vector field. As an application, new examples of stably causal Petrov type D spacetimes which obey the timelike convergence condition and which admit an isometric spacelike circle action are obtained.
N-dimensional integrability from two-photon coalgebra symmetry

Abstract: A wide class of Hamiltonian systems with N degrees of freedom and endowed with, at least, (N-2) functionally independent integrals of motion in involution is constructed by making use of the two-photon Lie-Poisson coalgebra. The set of (N-2) constants of the motion is shown to be a universal one for all these Hamiltonians, irrespectively of the dependence of the latter on several arbitrary functions and N free parameters. Within this large class of quasi-integrable N-dimensional Hamiltonians, new families of completely integrable systems are identified by finding explicitly a new independent integral through the analysis of the sub-coalgebra structure of the two-photon algebra. In particular, new completely integrable N-dimensional Hamiltonians describing natural systems, geodesic flows and static electromagnetic Hamiltonians are presented.
A curved Henon-Heiles system and its integrable perturbations

Abstract: The constant curvature analogue on the two-dimensional sphere and the hyperbolic space of the integrable H\'enon-Heiles Hamiltonian $\mathcal{H}$ given by $$ \mathcal{H}=\dfrac{1}{2}(p_{1}^{2}+p_{2}^{2})+ \Omega \left(q_{1}^{2}+ 4 q_{2}^{2}\right) +\alpha \left(q_{1}^{2}q_{2}+2 q_{2}^{3}\right), $$ where $\Omega$ and $\alpha$ are real constants, is revisited. The resulting integrable curved Hamiltonian, $\mathcal{H}_\kappa$, depends on a parameter $\kappa$ which is just the curvature of the underlying space and allows one to recover $\mathcal{H}$ under the smooth flat/Euclidean limit $\kappa\to 0$. This system can be regarded as an integrable cubic perturbation of a specific curved $1:2$ anisotropic oscillator, which was already known in the literature. The Ramani-Dorizzi-Grammaticos (RDG) series of potentials associated to $\mathcal{H}_\kappa$ is fully constructed, and corresponds to the curved integrable analogues of homogeneous polynomial perturbations of $\mathcal{H}$ that are separable in parabolic coordinates. Integrable perturbations of $\mathcal{H}_\kappa$ are also fully presented, and they can be regarded as the curved counterpart of integrable rational perturbations of the Euclidean Hamiltonian $\mathcal{H}$. It will be explicitly shown that the latter perturbations can be understood as the "negative index" counterpart of the curved RDG series of potentials. Furthermore, it is shown that the integrability of the curved H\'enon-Heiles Hamiltonian $\mathcal{H}_\kappa$ is preserved under the simultaneous addition of curved analogues of "positive" and "negative" families of RDG potentials.
