Timpanoplastias: Veinte a?os después

Abstract: 25 patients who underwent surgery of the middle ear and mastoid at the provincial clinical and surgical hospital of sancti spiritus in 1975 were studied. some of the aspects analyzed were age, sex, color, of the skin, place of origin, characteristics of otorrhea and tympanic perforation, state of the mastoid, regional associated affections, audiometry and type of surgical procedure. the results published at 6 months first and then one year after the operation were discussed and compared with a control carried out 20 years later. 84 % of them are under control at present. the best clinical and audiometric results were obtained in type i tympanoplasties (83.3 %). the failures reported by the expulsion of the polyethylene prothesis occurred mostly less than a year after performing type ii and iii tympanoplasties. it was necessary to operate on 24 % of them again. clinical evolution was good but not the audiometric one

Abstract: Se hizo un estudio a 25 pacientes a los que se les realizó operación de oído medio y mastoides en 1975, en el Hospital Clinicoquirúrgico Provincial Docente de Sancti Spíritus, los cuales fueron analizados según edad, sexo, color de la piel, área de procedencia, característica de la otorrea y perforación timpánica, estado de la mastoides, afecciones regionales asociadas, audiometría y tipo de intervención. Los resultados, publicados a los 6 meses y al a o de operados se compararon y discutieron con un control realizado 20 a os después, donde el 84 % de ellos es controlado actualmente en consulta. Los mejores resultados clínicos y audiométricos se obtienen en las timpanoplastias tipo I (83,3 %). Los fracasos reportados, por expulsión de la prótesis de polietileno fueron en su mayoría antes del a o de operados en las timpanoplastias tipo II y III. Abstract: a new method is proposed for separating fiber material from the main components of cartonboard commonly used for storing liquids and for evaluating the advantages and disadvantages of this material as fiber sources for paper manufacture. the mechanical resistance of the cartonboard has been reduced by hydration using naoh to accelerate the process. this process destroys hydrogen bonding that joint fiber-fiber. the process allows recovering 92% of the existing fiber in the carton packaging , which makes this material a potential alternative as fibber source for the manufacture of paper. the recovered material contains a high quantity of stickies, although it is possible to reduce this amount to levels that allows using the recovered fiber in paper manufacture.

Abstract: Se propone un método para separar la fibra de los componentes principales del envase de cartón para líquidos y para evaluar las ventajas y desventajas del empleo de esta materia prima como fuente de fibras secundarias en la fabricación de papel. La resistencia mecánica del envase postconsumo se reduce por hidratación utilizando NaOH. Este proceso destruye los puentes de hidrógeno que unen fibra-fibra. El procedimiento permite rescatar 92% de fibra existente en el envase, por ello este material se ubica como potencial alternativa de fuente de fibra secundaria para la fabricación de papel. El material recuperado registra un alto contenido de material adhesivo (stickies), aunque es factible reducir esta cantidad a niveles que permiten su utilización en la fabricación de papel. Role of SST, CORT and ghrelin and its receptors at the endocrine pancreas

Abstract: Somatostatin (SST), cortistatin (CORT), and its receptors (sst1–5), and ghrelin and its receptors (GHS-R) are two highly interrelated neuropeptide systems with a broad range of overlapping biological actions at central, cardiovascular, and immune levels among others. Besides their potent regulatory role on GH release, its endocrine actions are highlighted by SST/CORT and ghrelin influence on insulin secretion, glucose homeostasis, and insulin resistance. Interestingly, most components of these systems are expressed at the endocrine pancreas and are actively involved in the modulation of pancreatic islet function and, consequently influence glucose homeostasis. In addition, some of them also participate in islet survival and regeneration. Furthermore, under severe metabolic condition as well as in endocrine pathologies, their expression profile is severely deregulated. These findings suggest that SST/CORT and ghrelin systems could play a relevant role in pancreatic function under metabolic and endocrine pathologies. Accordingly, these systems have been therapeutically targeted for the prevention or amelioration of certain metabolic conditions (obesity) as well as for tumor growth inhibition and/or hormonal regulation in endocrine pathologies (neuroendocrine tumors). This review focuses on the interrelationship between SST/CORT and ghrelin systems and their role in severe metabolic conditions and some endocrine disorders.

Abstract: the article seeks to review the justifying arguments proposed for legitimizing the copulative application of sentences and security measures on individuals deemed guilty of a criminal offense and that furthermore have been qualified as dangerous. for this, tendencies are addressed that understand that the mere fact that penal models of continental influence are structured in a dualist manner offers sufficient justification to impose a sentence and a measure; as well as the approach by those who understand that for this an external reason is required (based on variants in the general interest of crime prevention) and, finally, that of those who propose the need to attribute said dangerousness to the individual.

Abstract: The position of Spain in the international migration system has substantially changed over the last decade. The settlement of immigrant communities poses a series of important challenges to Spanish society. The social protection schemes included in the welfare state are considerably affected by the social, political, cultural and demographic transformations derived from this phenomenon. On the one hand these schemes must define the way to grant access to their services and benefits to the new residents (definition of the boundaries of entitlement to access the health care system, the social services, the education system or the housing policies). On the other hand, they need to articulate mechanisms to respond to the differential demands of a changing population (in terms of socio-demographic structure, cultural and linguistic profiles, as well as biological characteristics). The welfare regime is also subject to a modification in the social perception of its legitimacy due to the increase of the internal social heterogeneity. In addition to posing a challenge, this phenomenon constitutes in itself a good opportunity to identify the weaker points of the social protection system, a necessary step to reinforce it and to guarantee its future sustainability.

Abstract: Objective Munc18c is associated with glucose metabolism and could play a relevant role in obesity. However, little is known about the regulation of Munc18c expression. We analyzed Munc18c gene expression in human visceral (VAT) and subcutaneous (SAT) adipose tissue and its relationship with obesity and insulin. Materials and Methods We evaluated 70 subjects distributed in 12 non-obese lean subjects, 23 overweight subjects, 12 obese subjects and 23 nondiabetic morbidly obese patients (11 with low insulin resistance and 12 with high insulin resistance). Results The lean, overweight and obese persons had a greater Munc18c gene expression in adipose tissue than the morbidly obese patients (p<0.001). VAT Munc18c gene expression was predicted by the body mass index (B = ?0.001, p = 0.009). In SAT, no associations were found by different multiple regression analysis models. SAT Munc18c gene expression was the main determinant of the improvement in the HOMA-IR index 15 days after bariatric surgery (B = ?2148.4, p = 0.038). SAT explant cultures showed that insulin produced a significant down-regulation of Munc18c gene expression (p = 0.048). This decrease was also obtained when explants were incubated with liver X receptor alpha (LXRα) agonist, either without (p = 0.038) or with insulin (p = 0.050). However, Munc18c gene expression was not affected when explants were incubated with insulin plus a sterol regulatory element-binding protein-1c (SREBP-1c) inhibitor (p = 0.504). Conclusions Munc18c gene expression in human adipose tissue is down-regulated in morbid obesity. Insulin may have an effect on the Munc18c expression, probably through LXRα and SREBP-1c.
