Indicadores de gestión de servicios de salud públicos y asignación de recursos desde el Ministerio de Salud de Chile

Abstract: background: chile has a national health services system, formed by 29 health services. an efficient resource distribution among this services is crucial for an efficient health care delivery. aim: to obtain indices from the chilean public health services, that could improve allocation of resources. material and methods: information from the chilean public health services, corresponding to activities during 2001 budgetary period, was collected. this is the latest complete and official information for the totality of health services in the country. seventeen variables generated or monitored by the instituto nacional de estadísticas (ine), the ministerio de salud (minsal), the ministerio de hacienda, the ministerio de planificación y cooperación (mideplan) and the fondo nacional de salud (fonasa) were studied. the main components analysis (acp) was used, obtained from the r correlation matrix. results: the first two main components were selected, with an accumulated percentage of explained variability of 63.05%. the first component is related to the population assigned to each health service. this corresponds to the number of people needed to treat in the hospitals of these services and their answer to this demand, justified by the expenses in which each health service incurs. there is an inverse relation of the first component with health indicators, measured by burden of disease and death. the second main component would represent the social and economic characteristics of the population, poor and ery poor populations and public health insurance beneficiaries, to take care of in each health service. conclusions: health indicators in each health service are not considered a priority for resource distribution among health services in the country. the transference is done considering the indices contained in the two main components defined (rev méd chile 2004; 132: 1532-42).
Role of a Genetic Polymorphism in the Corticotropin-Releasing Factor Receptor 1 Gene in Alcohol Drinking and Seeking Behaviors of Marchigian Sardinian Alcohol-Preferring Rats

Abstract: Marchigian Sardinian alcohol-preferring (msP) rats exhibit innate preference for alcohol, are highly sensitive to stress and stress-induced alcohol seeking. Genetic analysis showed that over-expression of the corticotropin-releasing factor (CRF) system of msP rats is correlated with the presence of two single nucleotide polymorphisms (SNPs) occurring in the promoter region (position ?1836 and ?2097) of the CRF1 receptor (CRF1-R) gene. Here we examined whether these point mutations were associated to the innate alcohol preference, stress-induced drinking, and seeking. We have recently re-derived the msP rats to obtain two distinct lines carrying the wild type (GG) and the point mutations (AA), respectively. The phenotypic characteristics of these two lines were compared with those of unselected Wistar rats. Both AA and GG rats showed similar patterns of voluntary alcohol intake and preference. Similarly, the pharmacological stressor yohimbine (0.0, 0.625, 1.25, and 2.5 mg/kg) elicited increased operant alcohol self-administration under fixed and progressive ratio reinforcement schedules in all three lines. Following extinction, yohimbine (0.0, 0.625, 1.25, and 2.5 mg/kg) significantly reinstated alcohol seeking in the three groups. However, at the highest dose this effect was no longer evident in AA rats. Treatment with the CRF1-R antagonist antalarmin (0, 5, 10, and 20 mg/kg) significantly reduced alcohol-reinforced lever pressing in the AA line (10 and 20 mg/kg) while a weaker or no effect was observed in the Wistar and the GG group, respectively. Finally, antalarmin significantly reduced yohimbine-induced increase in alcohol drinking in all three groups. In conclusion, these specific SNPs in the CRF1-R gene do not seem to play a primary role in the expression of the msP excessive drinking phenotype or stress-induced drinking but may be associated with a decreased threshold for stress-induced alcohol seeking and an increased sensitivity to the effects of pharmacological blockade of CRF1-R on alcohol drinking.

Abstract: lhe following essay tries to analyze and contextualize the exhibition of the play el húsar de la muerte, carried out by the artistic collective la patogallina at the government house of chile, la moneda, in the framework of the inauguration of the vii international festival lheater (filam) in 2001. in order to carry out the analysis, we propose the categories of liminality, performativity and politicity which allow conceiving this episode not from conventional theatrical structures, but from a transdisciplinary point of view. lhese categories plus the ideological, aesthetic and organizational backgrounds of the collective will allow us to understand the bursting of la patogallina in la moneda as an event which was marked by the conflicts with the augusto pinochet trial. lhis can be read as a fracture in the festivity/cultural space and the normative consensus that articulated the ?politics of aperture? from la moneda to all citizens.

Abstract: this article takes as starting point the play prat, of the company teatro de chile, whose first performance in the year 2002 appeared so disjointed and impertinent that it unleashed a polemic involving ultra conservative sectors, the ministerial bureaucracy and the artistic world, revealing the fissured zones of a truncated democratic transition. thus, this text begins with an aesthetic-artistic analysis of the play, to then look over the speeches - collected from the press and from personal interviews - of the different po-litical and artistic agents, who engaged in the controversy about prat’s first performance. finally, we outline some reflective notes about the role of cultural institutionalism in the theatrical field, and a critical review of bourdieu’s category of relative autonomy to understand the artistic fields.
