Is it possible to block the cancer cells in circulating blood by extracorporeal circulation? Implications of the immune system and other factors (review)  [PDF]

Abstract: The presence of cancer cells in circulating blood and the possibility of colonization in the tissues of the body worsen in a determinant way the prognosis of the disease. Nevertheless, there are factors in the body that can lead to a prognostic improvement, even in such conditions. In addition to the immune system, other favorable factors can act at the level of the microenvironment of the tumor so much that cases of spontaneous total regression, not only of primary tumors but also metastases, have been reported in the literature. Since it was recently reported that patients with renal tumors and in permanent hemodialysis present at post mortem examination, a metastatic spread much less extensive than observed in patients deceased for renal tumors but not on hemodialysis. The authors maintain it likely that the dialytic membrane concurs to block at least in part the cancer cells circulating in the blood. The possibility to block cancer cells can be extended in addition to the dialytic membrane also to other types of filters inserted in the extracorporeal circulation. The block of the cancer cells thus is obtained and that occurs in regional lymph nodes and in the microenvironment of the tumor causing a relative increase in the elements of the immune system compared to the number of cancer cells, which could determine clearly positive therapeutic results also in cases with advanced metastatic spread.

Abstract: Having changed our understanding about issues related to poverty, even in the fight against malaria we must keep in mind a number of issues other than simple lack of economic resources. In this article we tried to discuss the various aspects that make malaria a disease closely related to poverty and the effects of malaria on the same poverty of patients who are affected. If you want the program to "Rool Back Malaria" to succeed, you must program interventions that improve the living conditions of populations in endemic area, individually and as communities. As has become clear that the discovery of an effective vaccine will not eradicate the disease, remains a fundamental understanding of mechanisms related to poverty that cause Malaria remains one of the major killers in the world, to help communities affected and individuals to prevent, cure properly and not being afraid of this ancient disease.
Multiferroicity in V-doped PbTiO$_{3}$

Abstract: We report \emph{ab initio} predictions on the proper multiferroic (ferromagnetic, insulating and ferroelectric) character of PbTiO$_{3}$ doped with vanadium. V impurities coupled ferromagnetically carry a magnetization of 1 $\mu_{\rm B}$ each. The coupling is expected to be strong, since the paramagnetic solution is higher by 150 meV/vanadium, and no stable antiferromagnetic solution was found. The electronic gap in the doped system is about 0.2-0.3 eV in GGA, hence the system is properly multiferroic. V doping increases the spontaneous polarization in PbTiO$_{3}$, with an approximate percentual rate of 0.7 $\mu$C/cm$^{2}$.

Permissive hypofiltration: an alternative view

Abstract: We read with great interest the Viewpoint by Chawla and colleagues [1] addressing the intriguing physiopathological hypothesis that resting a failing organ is potentially helpful to promote recovery. The authors proposed a 'permissive' therapy for acute kidney injury (AKI) by resting the kidneys with early renal replacement therapy (RRT), somewhat similar to treatment of acute respiratory distress syndrome with early intubation and per-missive hypercapnia during 'protective' mechanical ventilation. During mechanical ventilation, however, rather than being at rest, alveoli are exposed to significant stress injuries (ventilator-induced lung injury) even under 'protective' ventilation [2]. Similarly, although early initiation of RRT could be beneficial, it should be emphasized that the advantage of such therapies over a 'conservative strategy' has not been consistently demonstrated to date, and that RRT also carries the risk of significant side effects that should not be overlooked [3], as also clearly pointed out by Chawla and colleagues. This may be particularly relevant when treating very young patients, which are often challenging from the standpoints of vascular accesses and fluid balance [4]. Furthermore, early initiation of extracorporeal ultrafiltration in patients with non-oliguric AKI might cause decreased urine output, a clearly undesired and detrimental effect. On the other side, to date, it has not convincingly been shown whether loop diuretics are harmful in the setting of AKI [5] and 'for the vast majority of clinicians' they are not generally discontinued in early AKI.For acute respiratory distress syndrome therapy, extracorporeal carbon dioxide removal associated with ultraprotective ventilation has recently been proposed [6]. With a similar approach, it is conceivable that early initiation of 'protective' low-dose (10 ml/kg/h) continuous RRT with zero balanced ultrailtration in association with administration of (high dose) diuretics may help to rest
Giant electroresistance and tunable magnetoelectricity in a multiferroic junction

Abstract: First-principles density functional calculations show that the $\textrm{SrRuO}_{3}/\textrm{PbTiO}_{3}/\textrm{SrRuO}_{3}$ multiferroic junction with asymmetric (RuO$_{2}$/PbO and TiO$_{2}$/SrO) interfaces has a large ferroelectric depolarizing field, whose switching changes the interface transmission probabilities for tunneling electrons, leading to electroresistance modulation over several orders of magnitude. The switching further affects the interface spin density, naturally driving magnetoresistance as well as modulated spin-dependent in-plane resistivity, which may be exploited in field-effect devices.
Low In solubility and band offsets in the small-$x$ $β$-Ga$_2$O$_3$/(Ga$_{1-x}$In$_x$)$_2$O$_3$ system

Abstract: Based on first-principles calculations, we show that the maximum reachable concentration $x$ in the (Ga$_{1-x}$In$_x$)$_2$O$_3$ alloy in the low-$x$ regime (i.e. In solubility in $\beta$-Ga$_2$O$_3$) is around 10%. We then calculate the band alignment at the (100) interface between $\beta$-Ga$_2$O$_3$ and (Ga$_{1-x}$In$_x$)$_2$O$_3$ at 12%, the nearest computationally treatable concentration. The alignment is strongly strain-dependent: it is of type-B staggered when the alloy is epitaxial on Ga$_2$O$_3$, and type-A straddling in a free-standing superlattice. Our results suggest a limited range of applicability of low-In-content GaInO alloys.
Properties of (Ga$_{1-x}$In$_x$)$_2$O$_3$ over the whole $x$ range

Abstract: Using density-functional ab initio theoretical techniques, we study (Ga$_{1-x}$In$_x$)$_2$O$_3$ in both its equilibrium structures (monoclinic $\beta$ and bixbyite) and over the whole range of composition. We establish that the alloy exhibits a large and temperature-independent miscibility gap. On the low-$x$ side, the favored phase is isostructural with $\beta$-Ga$_2$O$_3$; on the high-$x$ side, it is isostructural with bixbyite In$_2$O$_3$. The miscibility gap opens between approximately 15\% and 55\% In content for the bixbyite alloy grown epitaxially on In$_2$O$_3$, and 15\% and 85\% In content for the free-standing bixbyite alloy. The gap, volume and band offsets to the parent compound also exhibit anomalies as function of $x$. Specifically, the offsets in epitaxial conditions are predominantly type-B staggered, but have opposite signs in the two end-of-range phases.
Abdominal-Pelvic Actinomycosis Mimicking Malignant Neoplasm

Abstract: Abdominal-pelvic actinomycosis is often mistaken for other conditions, presenting a preoperative diagnostic challenge. In a 46-year-old female, computed tomography showed an abdominal-pelvic retroperitoneal mass extending from the lower pole of the right kidney to the lower pelvis. The patient had a 3-year history of intrauterine device. The mass appeared to involve the ascending colon, cecum, distal ileum, right Fallopian tube and ovary, and ureter anteriorly and the psoas muscle posteriorly. The resection of retroperitoneal mass, distal ileum appendicectomy, right hemicolectomy, and right salpingo-oophorectomy was performed. The postoperative period was uneventful. Penicillin therapy was given for six months without any complication. The retroperitoneal mass measured 4.5 × 3.5 × 3?cm, surrounded adjacent organs and histologically showed inflammatory granulomatous tissue, agglomeration of filaments, and sulfur granules of Actinomyces, with positive reaction with periodic acid Schiff. Right tubo-ovarian abscess was present. Abdominalpelvic actinomycosis should always be considered in patients with a pelvic mass especially in ones using intrauterine device. 1. Introduction In developed countries, actinomycosis is a relatively rare disease that is mainly caused by Actinomyces israelii. Actinomyces israelii is an anaerobic, gram-positive organism that is normally present in oral cavity, throughout the gastrointestinal tract, female genital tract, and the bronchus. Actinomycosis occurs most frequently in the cervical facial (50%–65%), abdominal (20%), and thoracic (15%) regions. The overall incidence of registered cases of actinomycosis is decreasing. Abdominalpelvic actinomycosis, however, are increasing in frequency and is associated with abdominal surgery (such as appendectomy), bowel perforation, or trauma [1]. In addition, the presence of a long-standing intrauterine device (IUD) is a reported risk factor in young women [2]. The abdomen is the most frequent site for actinomycosis and when an abdominal tumor presents as the clinical symptom, the local lesion needs to be differentiated from abdominal tumors of other etiologies, malignancy in particular. In the majority of cases, the indolent clinical course together with the malignant like tumour appearance at imaging investigations make a delay in diagnosis the rule rather than the exception. Preoperative diagnosis is usually difficult with the majority of cases being diagnosed after the histological and bacteriological examination of the resected specimen. Abstract: La gabapentine (Gabapentin) et la quétiapine (Quetiapina) sont deux médicaments appartenant à des catégories pharmaceutiques différentes ayant des indications thérapeutiques distinctes. Ils ont en commun la caractéristique négative d’être liés à une augmentation possible du risque de suicide. En outre, la prescription non conforme de médicaments (off-label use) comme la gabapentine, récemment choisie a fait que les deux médicaments peuvent être utilisés pour le traitement des troubles bipolaires, ouvrant la possibilité de faire prendre les deux aux patients jugés réfractaires à la monothérapie : le risque est que l’effet secondaire de l’un se somme à celui de l’autre.Grace à l’analyse de la littérature scientifique, l’objectif de cette étude est : d’évaluer la relation entre l’assomption de la gabapentine et de la quétiapine et le risque d’idéation suicidaire et/ou de passage à l’acte ; d’identifier un mécanisme d’action pouvant expliquer ce risque ; évaluer l’usage de ces médicaments comme moyen de passage à l’acte dans la crise suicidaire. Le médecin qui prescrit la gabapentine (Gabapentin) ou la quétiapine (Quetiapina) doit être conscient de leurs risques et doit en informer le patient le plus complètement possible afin que ce dernier puisse donner, en toute conscience, un son consentement à la thérapie. En outre, par le biais de consultations régulières, le médecin doit effectuer un monitorage attentif du traitement pour détecter les signaux d’alarme et trouver les échappatoires, comportementales et thérapeutiques, pour réduire ou prévenir le risque suicidaire chez les patients.Tout cela est plus important encore, vu la possibilité de l’association de deux médicaments sur laquelle il n’existe aucune étude spécifique.Gabapentin and Quietiapina are two drugs belonging to two different pharmaceutical classes thus offering different therapeutic indications. They both share the negative feature of being related to a possible implication in suicidal events.Moreover, the latest off-label use of Gabapentin for the bipolar disorders therapy has allowed the use of both these drugs in the treatment of the same pathology, thus opening the possibility of their combination in treating those patients refractory to single-drug therapy. By doing in this way, there is the possibility of joining their separate side effects.
