Objetivos y metodología de la clase práctica de Propedéutica Clínica Objectives and methodology of the practical lectures of Clinical Propedeutics

Abstract: Introducción: la clase práctica de Propedéutica Clínica y Semiología Médica es la actividad docente fundamental de la disciplina durante las 10 primeras semanas de la estancia dirigida a la vinculación vertical con los contenidos de las ciencias básicas, las clínicas posteriores, la vinculación horizontal con las disciplinas diagnósticas, laboratorio clínico, imagenología, electrocardiografía y otros estudios complementarios. Objetivo: dise ar una guía metodológica sobre la conducción de la clase práctica de Propedéutica Clínica y Semiología Médica. Material y método: se realizó el análisis documental tomando como base los documentos, resoluciones, indicaciones metodológicas y programas que han regido el desarrollo de la asignatura durante los últimos a os. Resultados: se identificaron las principales dificultades y obstáculos para su ejecución entre los que se destacan el dise o de la actividad asistencial en las salas hospitalarias y el hospital en su conjunto, la masividad actual de estudiantes en cada grupo básico de trabajo y fundamentalmente la ausencia de documentos normativos de esta actividad docente. Conclusiones: se propone una metodología que permita dar cumplimiento a estos objetivos, y de esta forma perfeccionar el aprendizaje por parte de los estudiantes de las habilidades que deberá utilizar como premisa para el desarrollo de todas las disciplinas clínicas que abordará en el futuro a lo largo de la carrera. La utilización de dicha metodología ofrece la posibilidad de uniformar el trabajo de los profesores para de esta forma lograr resultados similares en todos los escenarios en que se imparte la disciplina. Introduction: the practical lectures of Clinical Propedeutics and Medical Semiology is the main teaching activity in the subject during the 10 first weeks of the medical practice. This is aimed at achieving key objectives in the teaching-learning process as well as a vertical connection with the contents of the basic sciences, posterior clinical sciences, and horizontal connection with the subjects of diagnosis, clinical laboratory, imaging, electrocardiography and other complementary studies. Abstract: the vaso-occlusion in the sickle cell anemia is only characteristic in the haemolytic anemias. the idea that the falciform erythrocyte induces the vaso-occlusive process has been abolished and without doubt the event is produced by the adhesion of the low density deformed erythrocytes ( stress reticulocytes ) to the active vascular endothelium in post-capillary venule participating in the process molecules of cellular adhesion ( erythrocytic and vascular) as well as a group of plasma factors; the external phosphatidilserine , the thrombine action , the expression of tissue factor associated to the disorders of the cationic transportation mechanism as well as the aggregates (band 3) are crucial elements in the pathophysiological explanation of vaso-occlusion and its relation to different therapeutic options.

Abstract: Se realizó el estudio de la función autonómica a 25 pacientes con drepanocitosis mediante el análisis de la tensión arterial continua durante la maniobra de Valsalva, y las variaciones de la frecuencia cardíaca y la tensión arterial en la posición ortostática con el objetivo de identificar la presencia en ellos de disautonomía. Previamente, se había determinado la existencia de amiloidosis sistémica mediante biopsia por aguja fina de la grasa abdominal para evaluar si su presencia se relaciona con dicha anomalía neurológica. La amiloidosis estuvo presente en el 60 % de los pacientes y la afectación del sistema nervioso autónomo predominante fue la caída de la tensión arterial sistólica durante el ortostatismo así como el índice VS-IIA/IIB mayor de 60 %, lo que constituye un indicador de disfunción simpática. Se concluyó que la presencia de amiloidosis no guardó relación significativa con la disfunción neurológica vegetativa encontrada. We studied autonomic function in 25 patients with sickle cell disease through the analysis of continuous blood pressure during Valsalva maneuver, and changes in heart rate and blood pressure in the standing position in order to identify the presence of dysautonomy in them. The existence of systemic amyloidosis had been previously determined by fine needle biopsy of abdominal fat to determine whether its presence is related to such neurological abnormality. Amyloidosis was present in 60 % of the patients and the predominant autonomic nervous system involvement was the fall in systolic blood pressure during orthostatism, as well as a VS-IIA/IIB index greater than 60 %, which is an indicator of sympathetic dysfunction. It was concluded that the presence of amyloidosis was not significantly related to the vegetative neurological dysfunction found.

Abstract: La vaso-oclusión en la drepanocitosis es una característica única entre las anemias hemolíticas. La idea de que el eritrocito falciforme induce el proceso vaso-oclusivo ha sido desechada y no cabe duda que el fenómeno ocurre debido a la adhesión de los hematíes deformables menos densos (reticulocitos de stress) al endotelio vascular activado en las vénulas post-capilares, proceso en el que participan moléculas de adhesión celular (MAC) eritrocitarias y vasculares así como un conjunto de factores plasmáticos; la externalización de la fosfatidilserina, la acción de la trombina, la expresión de factor tisular asociada a alteraciones del mecanismo de transporte catiónico, conjuntamente con la formación de agregados de banda 3 constituyen un conjunto de elementos cruciales en la explicación fisiopatológica de la vaso-oclusión y su relación con diferentes opciones terapéuticas. Función pulmonar en pacientes con anemia drepanocítica

Abstract: in order to assess the ventilatory capacity in patients with sickle cell anemia were selected and assisted at leon cuervo rubio provincial hospital hematology department, from january - april 2003. the selection of patients was randomized. along with the selection of clinical and hematological parameters obtained from the micro-histories made for the followup of patients; the patients were analyzed as for the lung performance. results from ventilarory study were related to the selected variants using microstat and applying chi ? square test. it was found all patients, exept one, presented ventilatory dysfunction, mostly restrictive. the obstructive component was present in 50 % cases, some of them as unique abnormality. there were no patients with chronic respiratory symptoms and the thoracic radiography was normal in all cases. there was no association between the typeof severity of the ventilatory dysfunction and the behaviour of the clinical and hematological variates evaluated. as a conclusion, the ventilatory repercussion in patients with sickle cell anemia must be taken into account even in the absence of clinical translation, and independently of the clinical and hematological behaviour of the disease.

Abstract: Con el propósito de evaluar la capacidad ventilatoria en pacientes siclémicos, se seleccionaron 50 pacientes atendidos en el servicio de hematología del Hospital Provincial "León Cuervo Rubio" en el período comprendido de enero-abril del 2003. La selección de los pacientes fue al azar. Unido a la selección de los parámetros clínicos y hematológicos obtenidos de las microhistorias confeccionadas para el seguimiento de los pacientes, a los mismos se les realizó un estudio funcional respiratorio. Los resultados del estudio ventilatorio se relacionaron con las variantes seleccionadas utilizándose Microsta y aplicando la prueba chi-cuadrado. Se encontró que todos, excepto uno, presentaban disfunción ventilatoria, en su mayoría de tipo restrictiva. El componente obstructivo estuvo presente en casi la mitad de los casos, en alguno de ellos como anomalía única. No hubo pacientes con síntomas respiratorios crónicos y el Rx de tórax fue normal en todos los casos. No se encontró correlación entre el tipo o severidad de la disfunción ventilatoria y el comportamiento de las variables clínicas y hematológicas evaluadas. Se concluye que la repercusión ventilatoria en los pacientes con siclemia debe ser tenida en cuenta aún en ausencia de traducción clínica, e independientemente del comportamiento clínico-hematológico de la enfermedad. La amiloidosis como complicación de la anemia drepanocítica

Abstract: an ambiopective, analytical and longitudinal study was conducted in 15 patients with drepanocytic anemia between 1995 and 2001 aimed at evaluating the existance of systemic amyloidosis by performing biopsy of the abdominal fat. the violet glass and and congo red techniques were used. the positive results in both stainings, which were observed in 6 patients (40 %), were considered as a diagnosis. the microhistories of follow-up were reviewed to determine the clinical and hematological behavior of the disease. the average figures of hemoglobin and reticulocytes, the levels of fetal hemoglobin and the frequency of vasoocclusive crises, of transfusions, as well as of infectious episodes, were used as variables. the relation between these variables and the existance of amyloidosis was evaluated by calculating the relative risk. no statistical significant relation was found in any case, although an increase of the risk for amyloidosis related to the frequency of infections and to the vasoocclusive crises could be proved.
