Toracoplastia: olvidada o poco conocida? Experiencia en dos hospitales de Camagüey

Abstract: a descriptive cross-sectional study was performed on 16 patients operated on by thoracoplasty technique from october 1995 to november 2005. surgeries were performed at “amalia simoni” and “martín chang puga” university hospitals located in camaguey city and nuevitas respectively. thoracoplasty was indicated because of the presence of bronchopleural fistulae in roughly two thirds of the patients (68,75 %) and of the attempt to reduce the thoracic cavity as a complement to a previous operation. most of thoracoplasties were performed in the right hemithorax after pneumonectomy and in a second surgery. resection of two or three costal segments predominated. despite the high mortality rate, the healing index was good in the rest of cases. supplementary thoracoplasty is the most indicated at present and we agree that wide and deforming thoracoplasties are not longer so much practiced in spite of the fact that some patients still need them

Abstract: liposarcoma is the most frequent malignant soft tissue tumor. this article presented a case of well-differentiated liposarcoma located in the mediastinum, which is a rare location. the patient was a 48 years-old man that was admitted to the pneumothysiology service because he was short of breath and had a mediastinal mass located near left hemithorax. he was studied using anteroposterior and lateral thoracic radiography in addition to computerized tomography. it was necessary to urgently perform thorachotomy due to his acute clinical picture with mediastinal compression. the patient evolved positively during surgery and afterwards; at present, he has finished his adjuvant treatment based on radiotherapy and chemotherapy and he feels good

Abstract: Se realizó un estudio descriptivo transversal de 11 pacientes operados por tumor de Pancoast, en el transcurso de dos decenios. Se hizo una revisión bibliográfica amplia del tema haciendo énfasis en el diagnóstico, estadificación y tratamiento. El mayor porcentaje de pacientes era del sexo masculino, fumadores y portadores de masas radiopacas tenues en el vértice, las cuales se acompa aban de dolor en pared costal irradiado a hombro, brazo y cuello. No siempre la resección quirúrgica fue completa y radical y los pacientes que presentaron mejores índices de supervivencia fueron los tratados mediante cirugía excerética amplia y terapia neoadyuvante. La vía de abordaje más utilizada fue la posterolateral y en un sólo caso la anterior-transcervical, que mucho nos satisfizo. Abstract: El liposarcoma es, entre los sarcomas, el tumor maligno de los tejidos blandos más frecuente en el adulto. Se presenta un caso de liposarcoma situado en el mediastino, localización infrecuente, y que resultó ser un liposarcoma bien diferenciado. El paciente fue un hombre de 48 a os de edad que ingresa en la Sala de Neumotisiología con disnea y una masa mediastínica situada hacia el hemitórax izquierdo. Se estudió con radiografía de tórax anteroposterior y lateral y, además, con tomografía axial computadorizada. Fue necesaria una toracotomía con urgencia relativa por la agudización del cuadro clínico mediastínico compresivo. La evolución fue buena durante la intervención quirúrgica y después de ella y actualmente ha concluido su tratamiento adyuvante (radioterapia y quimioterapia) y se siente bien Liposarcoma is the most frequent malignant soft tissue tumor. Abstract: Se realizó un estudio descriptivo transversal con 16 pacientes operados mediante toracoplastia entre octubre de 1995 y noviembre del 2005 (un decenio). Las intervenciones se practicaron en los Hospitales Universitarios Amalia Simoni , en Camagüey, y Martín Chang Puga , en Nuevitas. Las indicaciones de toracoplastia se debían a la presencia de fístulas broncopleurales en aproximadamente dos tercios del total de pacientes (68,75 %) y en el resto (31,25 %) se realizaron con la intensión de reducir la cavidad a modo de complemento de otra intervención. La mayoría de las toracoplastias se realizaron en el hemitórax derecho, después de una neumonectomía y en un segundo acto quirúrgico. Predominó la resección de dos a tres segmentos costales y, a pesar de una mortalidad alta, el índice de curación fue bueno en los casos restantes. La toracoplastia complementaria es la que mayor indicación tiene en la actualidad y estamos de acuerdo en que las amplias y deformantes han visto pasar su mejor momento, a pesar de que algunos pacientes aún las necesitan A descriptive cross-sectional study was performed on 16 patients operated on by thoracoplasty technique from October 1995 to November 2005. Statistics of vertical backscatter profiles of cirrus clouds

Abstract: A nearly global statistical analysis of vertical backscatter and extinction profiles of cirrus clouds collected by the CALIOP lidar, on-board of the Cloud-Aerosol Lidar and Infrared Pathfinder Satellite Observation, is presented. Statistics on frequency of occurrence and distribution of bulk properties of cirrus clouds in general and, for the first time, of horizontally homogeneous (on a 5-km field of view) cirrus clouds only are provided. Annual and seasonal backscatter profiles (BSP) are computed for the horizontally homogeneous cirri. Differences found in the day/night cases and for midlatitudes and tropics are studied in terms of the mean physical parameters of the clouds from which they are derived. The relationship between cloud physical parameters (optical depth, geometrical thickness and temperature) and the shape of the BSP is investigated. It is found that cloud geometrical thickness is the main parameter affecting the shape of the mean CALIOP BSP. Specifically, cirrus clouds with small geometrical thicknesses show a maximum in mean BSP curve located near cloud top. As the cloud geometrical thickness increases the BSP maximum shifts towards cloud base. Cloud optical depth and temperature have smaller effects on the shape of the CALIOP BSPs. In general a slight increase in the BSP maximum is observed as cloud temperature and optical depth increase. In order to fit mean BSPs, as functions of geometrical thickness and position within the cloud layer, polynomial functions are provided. The impact on satellite radiative transfer simulations in the infrared spectrum when using either a constant ice-content (IWC) along the cloud vertical dimension or an IWC profile derived from the BSP fitting functions is evaluated. It is, in fact, demonstrated that, under realistic hypotheses, the mean BSP is linearly proportional to the IWC profile.
Molecular-level understanding of the WGS and reverse WGS reactions on Rh through hierarchical multiscale approach

Abstract: Hierarchically combining semi-empirical methods and first-principles calculations we gain a novel and noteworthy picture of the molecular-level mechanisms that govern the water-gas-shift (WGS) and reverse water-gas-shift (r-WGS) reactions on Rh catalysts. Central to this picture is that the WGS and r-WGS follow two different dominant reaction mechanisms: WGS proceeds according to a carboxyl (COOH) mechanism, whereas r-WGS proceeds according to a redox (CO2 {\to} CO + O) mechanism. The obtained results furthermore underscore the danger of common first-principles analyses that focus on a priori selected dominant paths. Not restricted to such bias, our herein proposed hierarchical approach thus constitutes a promising avenue to properly transport and incorporate the ab initio predictive-quality to a new level of system complexity.

Abstract:

We report the isolation of AtL1, a 249 bp non-LTR retrotransposon fragment from Arabidopsis thaliana by fingerprinting mRNAs extracted from A. thaliana plants, ecotype Columbia, in different heat stress conditions. Southern blot and PCR analysis suggested that AtL1 occurs as a single- or low-copy insert in the genome of A. thaliana ecotype Columbia. The presence of AtL1 in the genome of different Arabidopsis ecotypes was confirmed by PCR amplification and sequencing thus excluding all possible contamination. A preliminary scan of the AtL1 nucleotide sequence against the EMBL and NCBI databases revealed a high degree of similarity to a group of LINE type L1 retrotransposons of mammals and with a cDNA sequence of Artemisia annua. A phylogenetic analysis of LINE elements from animals and plants placed AtL1 and A. annua sequences in close proximity to some mammalian sequences but distant from the other plants LINE elements including those from
