Incidencia de bacteriemia en pacientes portadores de catéter permanente tunelizado para hemodiálisis

Abstract: catheter-related bacteriaemia is one of the main complications increasing the risk of loss of the catheter or even death of the patient. the use of sealing of the catheter lumen with antibiotics or the use of topical mupirocin on the exit orifice of the catheter have been proven to reduce the incident of bacteriaemia; however, the most aseptic possible handling of the catheter is the main tool for reducing this incidence. goal: to assess the rate of incident of bacteriaeima in patients with permanent tunnelled haemodialysis catheters without the use of sealing with antibiotics or the use of mupirocin, and placing particular emphasis on asepsis during handling. all patients with tunnelled catheters between 1st january and 31st december 2007 were included. in total there were 17 patients, 4 men and 13 women with an average age of 71.3 (11.3) years. the protocol consisted of using as aseptic a field as possible, the use of gloves each time the catheter was handled, use of masks both by the handler and the patient and the disinfection of the tunnel exit orifice with chlorhexidine, and of both catheter connections at the start and finish of the session. at 31st december the prevalence of patients with tunnelled catheters was 38.5%. during the period studied, a total of 8 bacteriaemias occurred in a total of 4462 days of monitoring (incidence rate of 1.8 bacteriaemias/1000 catheter-days). four blood cultures were positive for staphylococcus epidermidis, 1 for corynebacterium, 1 for staphylococcus auricularis and 2 were negative. no other staphylococcus aureus bacteriaemia occurred, nor any other sign of infection of the exit orifice. conclusion: an aseptic as possible handling of the catheter reduces the risk of bacteriaemia related to the catheter without the need to use sealing with antibiotics or topical mupirocin.
Color Doppler sonography with contrast in the differentiation of ovarian tumors

Abstract: The objective of this study was to differentiate benign ovarian tumors from malignant ones before surgery using color and pulsed Doppler sonography, and to compare results obtained before and after use of contrast medium, thereby verifying whether contrast results in an improvement in the diagnostic sensitivity. METHODS: Sixty two women (mean age 49.9 years) with ovarian tumors were studied, 45 with benign and 17 with malignant tumors. All women underwent a transvaginal color Doppler ultrasonographic exam. A study of the arterial vascular flow was made in all tumor areas, as well as an impedance evaluation of arterial vascular flow using the resistance index. RESULT: Localization of the vessels in the tumor revealed a greater proportion of malignant tumors with detectable internal vascular flows (64%) than benign tumors with such flows (22%). There was a considerable overlap of these findings. The use of contrast identified a greater number of vessels with confirmation in the totality of tumors, but did not improve the Doppler capacity in tumoral differentiation. Malignant tumors presented lower values of resistance index than the benign ones, whether or not contrast was used. The cutoff value for resistance index that better maximized the Doppler sensitivity and specificity was 0.55. Through this value, an increase of the sensitivity after contrast use was obtained, varying from 47% to 82%, while specificity remained statistically unchanged. CONCLUSION: Although the injection of a microbubble agent improved the sensitivity of the method detecting vascularization of tumors, a positive finding for vascularization by this method was not clinically useful in the differentiation of benign and malignant ovarian tumors.

Rebooting the human mitochondrial phylogeny: an automated and scalable methodology with expert knowledge

Abstract: We develop a systematic, automated methodology to overcome these difficulties, building from readily available, public sequence databases to high-quality alignments and phylogenetic trees. Within each stage in an autonomous workflow, outputs are carefully evaluated and outlier detection rules defined to integrate expert knowledge and automated curation, hence avoiding the manual bottleneck found in past approaches to the problem. Using these techniques, we have performed exhaustive updates to the human mitochondrial phylogeny, illustrating the power and computational scalability of our approach, and we have conducted some initial analyses on the resulting phylogenies.The problem at hand demands careful definition of inputs and adequate algorithmic treatment for its solutions to be realistic and useful. It is possible to define formal rules to address the former requirement by refining inputs directly and through their combination as outputs, and the latter are also of help to ascertain the performance of chosen algorithms. Rules can exploit known or inferred properties of datasets to simplify inputs through partitioning, therefore cutting computational costs and affording work on rapidly growing, otherwise intractable datasets. Although expert guidance may be necessary to assist the learning process, low-risk results can be fully automated and have proved themselves convenient and valuable.Mitochondria are remarkable among eukaryotic organelles for possessing their own genome, which is inherited independently from the nucleus. Even though mitochondrial DNA (mtDNA) constitutes a very small fraction of the whole genome in higher organisms, the few genes it encodes are essential to cellular metabolism, and therefore main actors in the development of genetic diseases. Abstract: the acute toxicity and the genotoxic activity of the fluid extract of leaves of ocimum gratissisum l. were evaluated by two short-term assay systems, one in vitro with the strain d-30 from aspergillus nidulans, and another in vivo, the micronucleus test in mouse bone marrow. the acute toxicity of the extract was proved by the mortality observed that rose to 50 % in the 3g/kg dose, which was dose dependant. it was found a little accentuated genotoxic response of the fluid extract of o. gratissimum 30 %. the results allowed to conclude that this extract showed acute toxic effects and histologic signs of hepatotoxicity and nephrotoxicity. it was not genotoxic neither for the strain d-30 from aspergillus nidulans in the mitotic segregation test, nor in the micronucleus test in mouse bone marrow.

Abstract: the acute toxicity of fluid extract from piper auritum h.b.k. as well as its genotoxic activity were evaluated through two short term assay systems, one in vitro using the d-30 strain of aspergillus nidulans and, another in vivo, the micronuclei assay in mouse bone marrow. the results allow to conclude that the fluid extract from p.auritum h.b.k. showed a very low acute toxicity and that it proved to be genotoxic in vitro but not in vivo .

Abstract: Se evaluó la toxicidad aguda y la actividad genotóxica del extracto fluido de hojas de Ocimum gratissimum L. a través de 2 sistemas de ensayo a corto plazo, uno in vitro con la cepa D-30 de Aspergillus nidulans y otro in vivo, el ensayo de micronúcleos en médula ósea de ratón. La toxicidad aguda del extracto quedó demostrada por la mortalidad observada que alcanzó el 50 % de los animales en la dosis 3g/kg, que fue dosis dependiente. Se encontró una respuesta genotóxica poco acentuada del extracto fluido al 30 % de O. gratissimum. Los resultados permitieron concluir que este extracto mostró efectos tóxicos agudos y signos histológicos de hepatotoxicidad y nefrotoxicidad y no resultó genotóxico sobre la cepa D-30 de Aspergillus nidulans en el ensayo de segregación mitótica y tampoco en el ensayo de micronúcleos en médula ósea de ratón. Abstract: this study describes three collections of texts published in the years following the 1755 earthquake in lisbon. housed at archives and libraries in portugal, the texts represent a variety of literary genres and strategies of argumentation. here they have been analyzed according to pre-established categories. they were written during a time of transition characterized by ideological hesitations and by an eclecticism of conservative bent, a time when the latest experimentalist currents coexisted alongside attempts to reconcile scholastic aristotelianism and experimentalism. the study focuses on a narrower set of texts, of special interest to the history of science; these can be classified into one of two topics: (i) physical explanations for the earth tremors and (ii) hygienist concerns about this type of natural disaster.

Abstract: although kadiwéu presents the same typological facts as the languages analyzed by baker (1995), this work shows that baker's polysynthesis parameter , according to which polysynthetic languages are pronominal argument languages, cannot be applied to this language. this paper offers, then, an alternative analysis to the pronominal argument theory for kadiwéu by arguing that nominal phrases are the verbal arguments in this polysynthetic language, like in any other better known language. on this view, one of the main properties of the polysynthetic languages, the so-called condition c violation (e.g. <>i wants john i to love mary, <> i broke john i's knife), follows from syntactic movement due to the nature of the kadiwéu v-system. that is, this paper questions the existence of a polysynthesis parameter and develops fukui & speas (1996) insight that the syntax of a given language follows from the functional categories present in this language's lexicon.
