Ataxia aguda em idade pediátrica: revis o retrospectiva de cinco anos = Acute ataxia in childhood: a five year retrospective review

Abstract: Objetivos: Caracterizar os casos de ataxia aguda internados em um Servi o de Pediatria e avaliar a sua abordagem no Departamento de Urgência. Métodos: Análise retrospectiva dos registos clínicos das crian as internadas entre janeiro de 2006 e dezembro de 2010 com quadro clínico de altera o da marcha e/ou dos movimentos motores finos com início dentro das últimas 72 horas. Resultados: Foram incluídas 82 crian as, 44 do sexo feminino. A mediana de idade foi de 4 anos. Trinta crian as apresentaram pródromos e 18 tinham história de infec o prévia. Os sintomas concomitantes mais frequentes foram sonolência (46 casos), v mitos (18 casos) e irritabilidade (14 casos). O exame físico n o mostrou altera es em dois ter os dos casos. Setenta crian as foram sujeitas a pelo menos um exame complementar de diagnóstico. As principais causas identificadas foram intoxica es (53 casos) e ataxia pós-infecciosa (15 casos). Só 16% necessitaram de tratamento específico e a maioria das interna es foi de curta dura o. Foram referenciados para seguimento em consulta hospitalar 32 casos. Registrou-se uma evolu o benigna na maioria dos casos. Conclus es: A atitude face a uma crian a com ataxia aguda deve ser ponderada e individualizada, tornando-se difícil a implementa o de um protocolo de abordagem uniforme. A maioria dos casos tem um curso benigno e auto-limitado, com interna o de curta dura o e apenas tratamento de suporte. No entanto, causas mais graves devem ser devidamente excluídas. História clínica minuciosa, exame físico e neurológico completos e interna o visando a vigilancia da evolu o clínica s o aspectos fundamentais. Aims: To characterize the cases of acute ataxia hospitalized in a Pediatric Department and to evaluate its approach in the Emergency Department. Methods: Retrospective analysis of medical records of children admitted between January 2006 and December 2010 with clinical gait and/or fine motor movements with onset within the last 72 hours. Results: We included 82 children, 44 girls. The median age was 4 years. Thirty children had prodromes and 18 had a history of previous infection. The most frequent accompanying symptoms were somnolence (46 cases), vomiting (18 cases) and irritability (14 cases). Physical examination showed no changes in two thirds of cases. Seventy children have been subjected to at least one further diagnostic examination. The main causes were poisoning (53 cases) and post-infectious ataxia (15 cases). Abstract: OBJETIVOS: descrever as características clínicas de crian as e adolescentes com Síndrome de Klinefelter. MéTODOS: foi realizado um estudo transversal retrospectivo, com base em dados clínicos dos pacientes com diagnóstico de Síndrome de Klinefelter acompanhados entre janeiro de 1992 e dezembro de 2009 (18 anos) na Unidade de Endocrinologia Pediátrica de um hospital de cuidados terciários em Portugal. RESULTADOS: foram identificados 15 pacientes no período do estudo. Sete tiveram diagnóstico positivo pré-natal de Síndrome de Klinefelter e oito tiveram o diagnóstico pós-natal, após investiga o por atraso no desenvolvimento psicomotor, distúrbios comportamentais e / ou fenótipo sugestivo (quatro deles diagnosticados apenas durante a adolescência). Treze pacientes tinham um cariótipo de sangue periférico de 47, XXY e dois apresentaram 47, XXY/46, mosaicismo XY. A idade mediana da primeira visita foi de sete anos. Início da puberdade ocorreu espontaneamente em sete casos. Indu o da puberdade foi realizada em três pacientes, com a idade de 14 anos. Aqueles que precisaram de tratamento hormonal tiveram uma resposta eficaz, sem efeitos colaterais. Inadequa es comportamentais e sociais foram identificadas em dois casos e atraso moderado de desenvolvimento motor global em nove. Hiperatividade e transtorno de déficit de aten o foram diagnosticados em cinco pacientes, atualmente tratados com metilfenidato. CONCLUS ES: atraso global do desenvolvimento, juntamente com características físicas da Síndrome de Klinefelter em uma crian a, justifica a solicita o de cariótipo, dada a elevada prevalência dessa síndrome, especialmente se estiveram presentes problemas de socializa o e dificuldades de aprendizagem. Este estudo destaca a necessidade de uma maior conscientiza o para o diagnóstico, bem como a importancia da abordagem multidisciplinar no cuidado e na educa o desses pacientes. Methods: A cross-sectional retrospective study was conducted, based on clinical data of patients diagnosed with Klinefelter Syndrome and followed from January 1992 to December 2009 (18 years) at a Pediatric Endocrinology Unit of a tertiary care-level hospital in Portugal. Abstract: Introdu o: O infliximab e um anticorpo monoclonal utilizado na doen a inflamatória intestinal (DII) refrataria. Objetivo: Avaliar a resposta clinica e os efeitos adversos da terapêutica com infliximab na DII em doentes pediátricos. Métodos: Estudo descritivo, analítico e transversal dos doentes da consulta de gastrenterologia pediátrica que ingressaram em protocolo terapêutico com infliximab. Resultados: Dos 6 doentes que realizam terapêutica monoclonal com infliximab, 5 apresentam doen a de Crohn moderada a grave e um colite ulcerosa. Verificou-se remiss o clinica em 5 doentes. Observou-se resolu o das altera es analíticas apos 6 meses de terapêutica. Verificaram-se 2 rea es alérgicas ligeiras e uma eleva o transitória das transaminases. Conclus es: Em concordancia com o descrito na literatura, na nossa popula o a terapeutica com infliximab tem sido bem tolerada e eficaz no controlo da doen a. Results: Of six patients treated with infliximab, five have moderate to severe Crohn’s disease and one present Ulcerative Colitis. There was clinical remission in five patients. Resolution in analytical changes was observed after 6 months of therapy. There were two mild allergic reactions and a transient elevation of transaminases. Conclusions: In accordance with the literature, in our population, infliximab therapy has been well tolerated and effective in disease control.

Abstract: autistic spectrum disorders are some of the most severe disorders that mental health professionals have to deal with, given its impact on core areas of children's functioning, like communication and social abilities, academic learning and also because of the uncertainty of prognosis. the developmental, structural model created by s. greenspan and collaborators proposes an innovative explanatory approach for these disorders, assuming that in every child there is an ability to communicate which is dependent on the level of motivation and of affective involvement. d.i.r. (based on development, individual differences and relationship) model is an intervention model stemming from those theoretical assumptions. it encompasses floor-time approach and other types of interventions (sensory integration, augmentative communication). through associa??o de apoio à unidade da primeira infancia an intensive clinical intervention program, based on this model, was created. the paper present a description of the program, of the children and their evolution in terms of developement and social and emotional adaptation.

Abstract: this article deals with two conceptions of communication which cut across research in this field, examining their roots and discussing their ideological origins. the argumentative, civic and universalist conception of communication, formulated by the first two generations of the chicago school of sociology, is shown to be relatively weak when compared to a different conception based on a technological and scientific model which sees the technical means for transmitting signals as being independent of the logical processes of social mediation, and which is suited to current developments in information technology, to modern persuasions and to the culture industry.
