The Knowledge and Participation of the Father in Breast Feeding, Salvador, Brazil  [PDF]

Abstract: The most fundamental influence in the breastfeeding process is the fathers support. The mother feels safer in breastfeeding when she has her partners support and a more active and effective participation. This study aimed at evaluating the fathers knowledge about breastfeeding in a quantitative approach through a transversal cohort study, interviewing 78 fathers in a public maternity in Salvador. The average age of the interviewees was 29.9 years and 53.8% had an incomplete fundamental schooling. Regarding knowledge of the subject, 10.3% believed that breastfeeding can be substituted. All fathers interviewed answered that they support breastfeeding for their children and believe in its benefits; 39.7% reported that their opinion could interfere their partners decision to prolong the process. Moreover, 69.2% of the males attended their partners antenatal meetings, and of these, over a third had no intervention on the subject. Of all the participants, 80.8% confirmed that they had participated in the breastfeeding process and emphasized their gratification during this phase. About the paternal opinion, 39.7% believed that there is a degree of influence over the partners decision to breastfeed, and for that reason 50% told the mother their opinion. Abstract: objective: to describe homicide mortality in the municipality of s?o paulo according to type of weapon, sex, race or skin color, age, and areas of socioeconomic inequalities, between 1996 and 2008. method: for this ecological time-series study, data about deaths in the municipality of s?o paulo were collected from the municipal program for improvement of mortality information, using international classification of diseases, 10th revision (icd-10) codes. homicide mortality rates (hmr) were calculated for the overall population and specifically for each sex, race or skin color, age range, type of weapon, and occurrence in social deprivation/affluence areas. hmr were adjusted for age using the direct method. the percentage age of variation in hmr was calculated for the study period. for areas of socioeconomic inequalities, the relative risk of death from homicide was calculated. results: hmr fell 73.7% between 2001 and 2008. a reduction in hmr was observed in all groups, especially males (-74.5%), young men between 15 and 24 years of age (-78.0%), and residents in areas of extreme socioeconomic deprivation (-79.3%). the reduction occurred mostly in firearm homicide rates (-74.1%). the relative risk of death from homicide in areas of extreme socioeconomic deprivation, as compared to areas with some degree of socioeconomic deprivation, was 2.77 in 1996, 3.9 in 2001, and 2.13 in 2008. in areas of high socioeconomic deprivation, the relative risk was 2.07 in 1996 and 1.96 in 2008. conclusions: to understand the reduction in homicide rates in the municipality of s?o paulo, it is important to take into consideration macrodeterminants that affect the entire municipality and all population subgroups, as well as micro/local determinants that have special impact on homicides committed with firearms and on subgroups such as the young, males, and residents of areas of high socioeconomic deprivation.

Abstract: the role of alcohol ingestion in the incidence of arterial hypertension has not been completely established. in addition, there are few studies addressing this point in relation to populations of workers. the objective of this study was to evaluate the association between alcoholism and arterial hypertension among workers in an oil refinery in mataripe, bahia, brazil, from 1986 to 1993. we designed a retrospective cohort study with a 7-year follow-up in a stratified systematic sample of 335 workers from the refinery. arterial hypertension was diagnosed based on blood pressure measurements done during routine medical examinations. at the beginning of follow-up, three groups were defined using the cage test of alcohol dependency: nondrinkers (n = 121), cage-negative workers (n = 116), and cage-positive workers (n = 98). in comparison with the cage-negative group, the cage-positive group had both greater relative risk and greater attributable risk for developing arterial hypertension (rr = 2.58; ar = 24.95 per 1 000 person-years). the cage-positive group also had greater risks compared to nondrinkers (rr = 2.06; ar = 20.97 per 1 000 person-years). the attributable fractions for the same two comparisons of groups were 61% and 51%, respectively. rate standardization by age or smoking habit did not substantially change the results. alcoholism is an important risk factor for arterial hypertension.

Abstract: in public health, violence has been studied according to the classic risk approach. the analyses developed in such perspective are able to show population tendencies of morbidity and mortality, and also identify risk factors that are part of the causal chain of violence. nevertheless, even though they are important as sources of information and hypotheses, when isolated such analyses are not capable of dealing with the complexity that is involved in violence. this paper aims at starting a reflection on the possibilities of the use of the vulnerability concept in the study of violence, specifically for understanding the situations that make youths be the main homicide victims. it is proposed a new approach to risk that considers the sociocultural processes that are present in the vulnerability of such group to lethal violence, through a perspective that presents the specificities of youth and the challenge imposed by the contemporary social life.

Abstract: the so called "new transcultural psychiatry" proposes a reformulation of the relationship between words and things in the field of medicine. this reformulation consists of a polissemic reading of the illness experience. in this article our aim is to question this proposal and point out its limits, through a foucaultian perspective,. considering the importance of discursivity in the theoretical work of the author, it is possible to radicalize the reformulation of the relationship between words and things; between discourse and its meaning and between speech and the speaking subject. through the notion of discursive practice, discursivity gains materiality and produces forms of subjectivity. it is in the discursive order that the illness experience is initiated and that the illness subject is constructed; that illness emerges as an event and makes itself present as "disgovernance".

Abstract: psychiatric information and practice are closely related with the field of criminal law, questioning classical penal law premises, such as responsibility and freewill. we have analyzed the articles related to mental health in brazilian penal laws, since código criminal do império do brazil (brazilian empire criminal laws) from 1830. our objective is to describe the structuring of a legal status for the mentally ill in brazil, as well as the model of penal intervention in the lives of those considered as 'dangerous' and 'irresponsible'. in order to do so, we have analyzed not only specific articles on penal law, but also texts by specialized analysts. in addition, we have discussed the concepts that keep mentally-ill criminals in a rather ambiguous situation, i.e. legal irresponsibility, potential aggressiveness and safety policies.
Mortalidade por homicídios no Brasil na década de 90: o papel das armas de fogo

Abstract: objective: to describe the growth of homicide death in brazil and to investigate the contribution of firearms to homicide death growth in the 90s. methods: an ecological descriptive time series was carried out for the period 1991 to 2000. external cause mortality data were obtained according to place of residence for the overall population and by gender from the ministry of health mortality information system. homicides were classified as firearm, non-firearm and unknown (weapon) deaths. proportional mortality and mortality rates (by 100,000) were calculated. results: homicides accounted for 33% of all external cause deaths in the 90s. firearms were used in more than 50% of deaths in 1991 and in almost 70% in 2000. this growth was seen for both genders and in all state capitals. homicide mortality rate grew 27.5% in the whole country, and firearm homicide mortality rate grew 72.5%. during the same period there was a drop in deaths classified as unknown weapon homicide, which could partially explain the growth seen in firearm homicide rates. conclusions: data suggest the magnitude of firearm contribution for the growing tendency of homicide deaths in brazil in the 90s. data quality regarding intentionality and type of weapon used has hindered data analysis.

Abstract: A prática e o saber psiquiátricos constroem-se em estreita rela o com o campo da justi a criminal, questionando os pressupostos da doutrina clássica do direito penal, tais como responsabilidade e livre-arbítrio. Pretendemos analisar os artigos que tratam da doen a mental nos códigos penais brasileiros, desde o Código Criminal do Império do Brazil, datado de 1830. Nosso objetivo é descrever a constitui o do estatuto jurídico penal dos doentes mentais no Brasil e do modelo de interven o penal na vida daqueles indivíduos considerados "perigosos" e "irresponsáveis". Mortalidade por homicídios no Brasil na década de 90: o papel das armas de fogo

Abstract: OBJETIVO: Descrever a evolu o da mortalidade por homicídio no Brasil, na década de 1990, buscando analisar a contribui o das armas de fogo. MéTODOS: Estudo ecológico descritivo de série temporal para o período 1991 a 2000. Dados sobre óbitos por causas externas, segundo local de residência, foram obtidos para a popula o total e grupos de sexo. A fonte de dados utilizada foi o Sistema de Informa o sobre Mortalidade do Ministério da Saúde (SIM/Datasus). Os homicídios foram classificados em homicídios por armas de fogo, por outros instrumentos e por meios n o especificados. Foram calculados a mortalidade proporcional e os coeficientes de mortalidade (/100.000) habitantes. RESULTADOS: Os homicídios foram responsáveis por 33% dos óbitos por causas externas na década de 1990. As armas de fogo contribuíram com mais de 50% dos casos já em 1991, e com cerca de 70% no ano 2000. Esse crescimento ocorreu em ambos os grupos de sexo e em todas as capitais. O coeficiente de mortalidade por homicídio cresceu 27,5%, sendo que o incremento nos homicídios cometidos com armas de fogo foi de 72,5%. No período houve uma queda nos casos decorrentes de instrumentos n o especificados, o que pode explicar parte do incremento observado para os homicídios com armas de fogo. CONCLUS ES: Os dados indicam significativa contribui o das armas de fogo para o crescimento dos homicídios na década de 1990. Problemas na qualidade das informa es, tanto no que se refere às mortes com intencionalidade indeterminada como ao tipo de instrumento utilizado, prejudicaram a análise dos dados.
