Abstract: objective: recognition of transexuality as a clinical entity for which medical attention should be available is currently a well-established reality, but institutional care has not been uniformly instituted throughout spain. the aim of the present study was to determine the current situation of healthcare for transexualism in the publicly-funded health service in spain. materials and methods: a descriptive study based on data provided by the spanish society of endocrinology group on identity and sexual differentiation was performed. the resources in the regions that have created specific gender units for these disorders are described. results: nine autonomous regions (55%) have started to provide various procedures, although only four provide genitoplastic procedures. the first region to include all sex reassignment surgeries was andalusia (year 1999). at the same time, madrid and catalonia also began to provide specialized mental health care and endocrinology but did not include surgical procedures until 2007 and institutional recognition until 2008. since 2007 other regions have incorporated healthcare for transsexual patients. overall, 3,303 patients (a male-to female/female-to-male transsexual ratio of 1.9/1) and 864 surgical procedures have been registered in this study. the composition and proportion of working hours of specialists, as well as the kinds of treatments provided, differ widely in each region. conclusions: the geographical distribution of healthcare to transsexual persons and the services provided vary. few regions offer genitoplastic procedures. the number of applicants exceeds the number estimated by the national health system.

Abstract: The main objective was to evaluate the association between SNPs and haplotypes of the FABP1-4 genes and type 2 diabetes, as well as its interaction with fat intake, in one general Spanish population. The association was replicated in a second population in which HOMA index was also evaluated.
ELOVL6 Genetic Variation Is Related to Insulin Sensitivity: A New Candidate Gene in Energy Metabolism

Abstract: Background The elongase of long chain fatty acids family 6 (ELOVL6) is an enzyme that specifically catalyzes the elongation of saturated and monounsaturated fatty acids with 12, 14 and 16 carbons. ELOVL6 is expressed in lipogenic tissues and it is regulated by sterol regulatory element binding protein 1 (SREBP-1). Objective We investigated whether ELOVL6 genetic variation is associated with insulin sensitivity in a population from southern Spain. Design We undertook a prospective, population-based study collecting phenotypic, metabolic, nutritional and genetic information. Measurements were made of weight and height and the body mass index (BMI) was calculated. Insulin resistance was measured by homeostasis model assessment. The type of dietary fat was assessed from samples of cooking oil taken from the participants' kitchens and analyzed by gas chromatography. Five SNPs of the ELOVL6 gene were analyzed by SNPlex. Results Carriers of the minor alleles of the SNPs rs9997926 and rs6824447 had a lower risk of having high HOMA_IR, whereas carriers of the minor allele rs17041272 had a higher risk of being insulin resistant. An interaction was detected between the rs6824447 polymorphism and the intake of oil in relation with insulin resistance, such that carriers of this minor allele who consumed sunflower oil had lower HOMA_IR than those who did not have this allele (P = 0.001). Conclusions Genetic variations in the ELOVL6 gene were associated with insulin sensitivity in this population-based study.
Serum sCD163 Levels Are Associated with Type 2 Diabetes Mellitus and Are Influenced by Coffee and Wine Consumption: Results of the Di@bet.es Study

Abstract: Objective Serum levels of soluble TNF-like weak inducer of apoptosis (sTWEAK) and its scavenger receptor CD163 (sCD163) have been linked to insulin resistance. We analysed the usefulness of these cytokines as biomarkers of type 2 diabetes in a Spanish cohort, together with their relationship to food consumption in the setting of the Di@bet.es study. Research Design and Methods This is a cross-sectional, matched case-control study of 514 type 2 diabetes subjects and 517 controls with a Normal Oral Glucose Tolerance Test (NOGTT), using data from the Di@bet.es study. Study variables included clinical and demographic structured survey, food frequency questionnaire and physical examination. Serum concentrations of sTWEAK and sCD163 were measured by ELISA. Linear regression analysis determined which variables were related to sTWEAK and sCD163 levels. Logistic regression analysis was used to estimate odd ratios of presenting type 2 diabetes. Results sCD163 concentrations and sCD163/sTWEAK ratio were 11.0% and 15.0% higher, respectively, (P<0.001) in type 2 diabetes than in controls. Following adjustment for various confounders, the OR for presenting type 2 diabetes in subjects in the highest vs the lowest tertile of sCD163 was [(OR), 2,01 (95%CI, 1,46–2,97); P for trend <0.001]. Coffee and red wine consumption was negatively associated with serum levels of sCD163 (P = 0.0001 and; P = 0.002 for coffee and red wine intake, respectively). Conclusions High circulating levels of sCD163 are associated with type 2 diabetes in the Spanish population. The association between coffee and red wine intake and these biomarkers deserves further study to confirm its potential role in type 2 diabetes.

A General Purpose Analysis Package  [PDF]

Abstract: This paper presents a general-purpose analysis package able to solve two- and three- dimensional analysis problems. The system can use the following methods of solution: Successive Approximation (SA), Optimal Interpolation (OI), and 3D-Var. Analyses are given for the following parameters: zonal and meridional wind components, temperature, relative humidity, and geopotential height. The analysis package was applied to produce analyses at 6 h time interval for the period 1-11 August 2008. The period was selected for data availability and forty-one analyses were collected. The results show the validity of the different solutions, which can be chosen depending on the physical problem to solve and on the computational resources available. In particular, assuming the observations as the reference, all solutions show a decrease of the RMSE compared to the background. The decrease is consistent with the particular setting of the analysis system used in this paper. The comparison between different solutions shows that the SA converges to OI in few iterations, and that the SA solution with ten iteration is, in practice, equal to OI. Moreover, the 3D-Var method shows its potential to improve the analysis, once the horizontal and vertical length-scales and the background and observational errors are set optimally, because its solution may be sizeably different from two-dimensional methods.

A data assimilation system combines all available information on the atmospheric state in a given time-window to produce an estimate of atmospheric conditions valid at a prescribed analysis time. Nowadays, increased computing power coupled with greater access to real-time asynoptic data is paving the way toward a new generation of high-resolution (i.e. on the order of 10 km) operational mesoscale analyses and forecasting systems. Moreover, better initial conditions are increasingly considered of the utmost importance for Numerical Weather Prediction (NWP) at the short range (0 - 12 h). This paper presents a general-purpose data assimilation system, which is coupled with the Regional Atmospheric Modelling System (RAMS) to give the analyses for: zonal and meridional wind components, temperature, relative humidity, and geopotential height. In order to show its potential, the data assimilation systems applied to produce analyses over Central Europe. For this application the background field is given by a short-range forecast (12 h) of the RAMS and analyses are produced by 2D-Var with 0.25? horizontal resolution. Results show the validity of the analyses because they are closer to the observations, consistently with the settings of the data assimilation system. To quantify the impact of improved initial conditions on the forecast, the analyses are then used as initial conditions of a short-range (6 h) forecast of the RAMS model. The results show that the RMSE is effectively reduced for the one- and two hours forecast, with some improvement for the three-hours forecast.

Mexico is the largest cactus exporter of the world and 40% of the production cost is provided from the harvest operation. Many workers are required to operate daily and suffer wrist pain after two weeks of collecting pears with mechanical pruners. A motor driven tool supplied from a photovoltaic system was employed for harvesting; different blades were tested at different cutting speeds. It was found that toothed blades sliced the pears with efficiencies over 90%. Abrasive blades were finally selected as the current required per cut was minimum, permitting a longer battery lifetime before requiring recharging.
