Oral chromoblastomycosis: a case report

Abstract: A chronic fungal infection in tropical regions, chromoblastomycosis is caused by dematiaceous fungi, the form-family ofFungi Imperfecti,usually affecting one lower limb at the site of a trauma but sometimes involving other areas of the bodyincluding head & neck.In this article,we report the case of a rare primary chromoblastomycosis of the palate and chest in a27-year-old man who was successfully treated with surgical resection and combined drug therapy,and eventually free tissuetransfer reconstructive surgical procedure to cure the palatine defect.
Conventional Vs. reverse nerve grafting for peripheral nerve repair in lower extremity of rats

Abstract: Background: Autograft is the best option in nerve defects when end-to-end repair can not sufficiently preserve nerve continuity. Theoretically, if the severed nerve is reversely grafted, it may prevent axonal growth into nerve branches, and larger amounts of axons will reach the target organ and more satisfactory results will be obtained. In this study we aimed to compare conventional versus reverse nerve grafting.Methods: This study was performed in Animal laboratory of Hazrat Fatemeh Hospital from April till August 2011. We randomly divided 40 Wistar rats into two groups. We excised 1.5 cm of the right sciatic nerve and anastomosed it conventionally between the proximal and distal ends of the nerve in rats in group A and in a reverse manner in rats in group B. The rats’ footprints were recorded in the first and 16th weeks after surgery. In week 16, the grafted nerves were removed under anesthesia for pathological examination and axon count. Subsequently, the results were compared clinically by sciatic functional index (SFI) through footprint analysis and paraclinically by axon count. A p-value smaller than 0.05 was considered statistically significant.Results: Conventional and reverse nerve grafting no had statistically significant differences in clinical assessment in the first and 16th weeks (P=0.87) post-surgically and also no difference in paraclinical assessment in week 16 (P=0.68). Conclusion: We had no significant clinically or para clinically differences between two approaches. It should be considered that the diameter and length of nerves and muscles in human is larger than rats, so the results of nerve repair may differ in human. We suggest a study in animal model which is anatomically more similar to human.
Comparing the effects of aspirin, clopidogrel and their combination on random skin flap survival in rats

Abstract: Background: Random pattern flap is a common reconstructive surgery procedure but its necrosis is a challenging problem. A lot of pharmacological agents and surgical procedures have been examined for the prevention of this complication to maximize the length to width ratio of these surgical flaps. Therefore, we designed an experimental study to evaluate the effects of aspirin, clopidogrel bisulfate (Plavix) and their combination on random skin flap survival in rats.Methods: Forty male rats were randomly assigned to four equal groups. Surgery was done under general anesthesia. A random, rectangular 3×11 cm dorsal skin flap was designed, elevated and sutured back into its primary site. In group one, 100 mg/kg Aspirin and in group two, 25 mg/kg Plavix were administered orally for 7 days postoperatively. Aspirin and Plavix were co-administered in the third group for the same period of time while the control group received no medication. After 7 days, the total surface of flaps, the viable and also the necrotic parts were measured by Image J software. Mean standard deviation and analysis of variance were calculated to compare the results.Results: The mean area of flap survival was 62.49% in the control, 64.04% in Aspirin, 65.09% in Plavix and 64.06% in combination groups. No statistically significant differences were found between treatment groups and control rats.Conclusion: In this study, we found no significant differences between Aspirin, Plavix or their combination on the survival of random skin flaps.
Combination of Spirituality and Cognitive-Behavioral Family Therapy on Treatment of Generalized Anxiety Disorders  [PDF]

Abstract:

Therapeutic interventions based on specific religious and spiritual teachings have proven to be effective in dealing with psychological disorders especially general anxiety disorders. Notwithstanding the signs of effectiveness within the integral approaches consisting of religious, spiritual and family therapy’s interventions, there are few studies that indicate the exploration of the integrative implications of the aforementioned interventions. Objective: The present research was to examine the effectiveness of integrative therapeutic interventions consisting of spirituality and Cognitive Behavioral Family Therapy on treatment of generalized anxiety disorders. Method: Case study. Participants: Two clients (one male aged 43 and one female, 37 years old). Procedure: Data was obtained based on the clinical interview, DSM-IV-TR criteria, psychological tests and psychiatrist’s diagnosis. In the course of 55 weekly therapeutic sessions for the male client and 39 weekly sessions for the female client, a series of techniques with a focus on cognitive restructuring were applied. The participants were also given coaching skills, relaxation skills, problem solving techniques, relationship management skills, and positive psychology techniques. The techniques were: a focus on the integrative implications of spiritual therapy and its emphasis on shared love for all human beings, love for others regardless of their ethnicity, belief in monotheism and belief in the day of judgment and hereafter. Findings: The results of pre-tests and post-tests along with a one year follow-up indicated the efficiency of the treatment while highlighting the vital role of integrative interventions based on spirituality and family cognitive behaviour therapy. The results also supported the role of families in contributing to the treatment of the anxiety disorder. Discussion for Further Research:

Abstract: In this study entropy, cross-entropy and discrimination information measures of stochastic intuitionistic fuzzy sets which result from hesitancy and randomness are proposed. It is an extension of De Luca and Termini’s formula for SIFSs. It is also intended to extend the measures proposed by Vlachos and Sergiadis. We will prove that in special cases these measures lead to the same results as the measures proposed by others. We also suggest the application of these measures in image processing in future researches.

Abstract: We present the first inclusive jet longitudinal double spin asymmetry A_LL and differential cross-section measurement from STAR. This analysis reflects ~1 pb^-1 of polarized pp data taken at sqrt{s}=200 GeV with beam polarizations of 30-45% in years 2003 and 2004. Agreement between the measured differential cross-section and next-to-leading order (NLO) calculations validate the interpretation of the asymmetry results within the framework of perturbative Quantum Chromodynamics (pQCD). A_LL is compared to NLO pQCD calculations, which incorporate several possible gluon helicity distributions, and is shown to disfavor the maximum gluon scenario.

Abstract: In this paper, a new method for Principal Component Analysis in intuitionistic fuzzy situations has been proposed. This approach is based on cross entropy as an information index. This new method is a useful method for data reduction for situations in which data are not exact. The inexactness in the situations assumed here is due to fuzziness and missing data information, so that we have two functions (membership and non-membership). Thus, method proposed here is suitable for Atanasov’s Intuitionistic Fuzzy Sets (A-IFSs) in which we have an uncertainty due to a mixture of fuzziness and missing data information. For the demonstration of the application of the method, we have used an example and have presented a conclusion.

Abstract: In this case report, a mature third mandibular molar was transplanted in the socket of a hopeless first mandibular molar of the same quadrant. A 32-year-old woman was diagnosed with strip perforation of her left first lower molar. Orthograde and retrograde treatments were unsuccessful. The tooth was extracted and replaced by the third molar of the same quadrant. Following transplantation, the tooth was splinted and the soft tissue was sutured. Removing sutures and pulp extirpation were carried out one week later simultaneously while the splint was left for one month. Prior to root canal therapy completion, calcium hydroxide therapy was considered for the tooth. Root canal treatment was completed after 3 months and the tooth was restored. Despite presence of granulation tissue in the socket of the first molar and closed apex of the wisdom tooth, the 9-months follow up revealed that the treatment was successful and no signs and symptoms were detected. In clinical examination, probing depth was normal and the radiograph indicated no pathological changes. The tooth was not tender to percussion and the absence of metal sound was indicative of no replacement resorption.
Assessment of Air Way Resistance Indexes and Exercise-Induced Asthma after a Single Session of Submaximal Incremental Aerobic Exercise

Abstract: The present study aimed at assessing air way resistance indexes that include FEV1 (Force expiration Volume in one second), FVC (Forced vital capacity) and FEV1/FVC and exercise-induced asthma (EIA) after one session of sub maximal incremental aerobic exercise. Fifty healthy male subjects (age 19-26) from the faculty of Physical Education, University of Shahid Chamran served as the participants of the study. They were randomly assigned to either exercise or control groups. Body height, body mass and pulmonary factors were measured in the pre-test conditions. The study protocol included a sub maximal incremental Astrand - Rhyming test on an ergocycle. After performing this test by the exercise group, FEV1, FVC and FEV1/FVC, were measured again for both groups and compared with pre test evaluations. The data were analyzed through descriptive and inferential statistics (dependent and independent t test). Results showed that there was a significant difference in FEV1 between the two groups after the exercise protocol (p ≤ 0.05). There was no significant difference in FVC between the two groups after exercise, and a significant difference was registered in FEV1 and FEV1/FVC between pre-test and post-test results in the group that performed the aerobic test protocol (p ≤ 0.05). Our results indicate that one sub maximal incremental aerobic exercise session causes a significant change in FEV1 and FEV1/FVC, and causes exercise-induced asthma.
