Prevención del prolapso genital en histerectomía abdominal utilizando los ligamentos redondos: Experiencia personal

Abstract: objective: to prevent vaginal vault prolapse, using a preoperative technique to fix the vaginal vault to the round ligaments in total abdominal hysterectomy, and identify associated pathology in the genital area. methods: after hysterectomy, we proceed to the closure of the vaginal vault with interrupted sutures with non-absorbable material, in number of 6-8, depending of the amplitude of the vagina, the distal ends are used to tie horizontally bilateral round ligaments. the setting of the two upper walls of the vagina, ensures greater stability of the vagina and lower possibility of occurrence of prolapse of the dome and/or the anterior and posterior vaginal walls. setting: servicio de ginecologia, hospital universitario de caracas results: the 124 patients undergoing hysterectomy, checked annually over a period of time between 2 and 27 years, with an average of 7 years, neither had vaginal prolapse. conclusion: histerectomy is the firstone gynecological operation. it requires making the suspension and fixation of the vaginal vault and simultaneously correct the genital tract associated pathology. the procedure we used had very favorable results. we can not compare our results with other techniques, do to scarse national an international references. the quality of life of women surgically intervened did not deteriorated, as usually happens when there is prolapse of the vaginal vault.

Abstract: Objetivo: Prevenir el prolapso de la cúpula vaginal, empleando una técnica peroperatoria que consiste en fijar la cúpula vaginal a los ligamentos redondos, en el momento de realizar la intervención. Métodos: Realizada la histerectomía, se procedió al cierre de la cúpula vaginal con puntos separados en número de 6 a 8 con material no absorbible, dependiendo de la amplitud de la vagina. Los cabos distales son utilizados para anudar horizontalmente los ligamentos redondos. La fijación de las dos paredes del extremo superior de la vagina, garantiza mayor estabilidad de la vagina y menor posibilidad de producir el prolapso de la cúpula y/o de las paredes anterior y posterior de la vagina. Ambiente: Servicio de Ginecología, Hospital Universitario de Caracas Resultados: Las 124 pacientes histerectomizadas y controladas anualmente en un lapso entre 2 y 27 a os, con un promedio de 7 a os, ninguna de ellas presentó prolapso de cúpula vaginal. Las histerectomías se realizaron en el ejercicio privado. En centros asistenciales públicos, es muy difícil hacer seguimiento y evaluar las pacientes, por razones múltiples. Conclusiones: La histerectomía ocupa el primer lugar de las intervenciones ginecológicas, esto obliga a realizar la suspensión y fijación de la cúpula vaginal y simultáneamente corregir la patología asociada del aparato genital. El procedimiento que hemos empleado tuvo resultados muy favorables. La calidad de vida de las mujeres intervenidas quirúrgicamente no se desmejoró, como suele suceder cuando se produce el prolapso de la cúpula vaginal. Abstract: objective: to evaluate the relationship between the vaginal clinical and physiological recuperation, and the increase of estrogen receptors in postmenopausal women, after administration of estrogens alone or combined with progestogens. method: thirty postmenopausal women divided into two groups were studied, 8 hysterectomized patients received 0.625 mg of conjugated equine estrogens continuous orally, and 22 patients intact uterus that additionally received medroxyprogesterone acetate at doses of 0,5 mg orally during 12 days every month. vaginal biopsy before and 4 months after treatment as well as estrogen receptor determinations were performed. setting: hospital universitario de caracas and private practice of the author. results: age ranged between 49 and 71 years. mean level of estrogen receptors before and after treatment were 7,30 ± 6,47 and 45,40 ± 26,87 respectively and this difference was statistically significant. there was a significative difference in the response to the two therapies, been more pronounced with estrogens without opposition that with combine treatment with basal measure of 6,40 ± 3,83 against 61,34 ± 33,35 and 7,68 ± 7,52 against 38,19 ± 21,00 respectively. response of vaginal dryness and dyspareunia was significative, but not the vaginal contraction. conclusion: the study shows an increase of vaginal mucosa estrogen receptors in postmenopausal women that received hormonal therapy during 4 months. the increase was higher in patients that received only estrogens.

Abstract: Objetivo: Evaluar si existe relación entre la recuperación clínica y fisiológica de la vagina, con el incremento de los receptores de estrógenos en posmenopáusicas, después de la administración de estrógenos solos o combinados con progestágenos. Método: Se estudiaron 30 mujeres posmenopáusicas divididas en dos grupos, 8 pacientes histerectomizadas recibieron 0,625 mg diarios de estrogenos equinos conjugados continuos por vía oral, y 22 pacientes con útero intacto recibieron adicionalmente acetato de medroxiprogesterona a dosis de 0,5 mg al día por vía oral durante 12 días cada mes. Se realizó biopsia de vagina antes y a los 4 meses del tratamiento así como determinación de receptores estrogénicos. Ambiente: Hospital Universitario de Caracas y consulta privada del autor. Resultados: Las edades estaban comprendidas entre 49 y 71 a os. El nivel promedio de los receptores estrogénicos antes y al final de la terapia fue de 7,30 ± 6,47 y 45,40 ± 26,87 respectivamente y esta diferencia fue estadísticamente significativa. Igualmente hubo una diferencia significativa en la respuesta a las dos terapias, y fue más significativa con los estrógenos sin oposición que con la terapia combinada comparado con la medición basal 6,40 ± 3,83 contra 61,34 ± 33,35 y 7,68 ± 7,52 contra 38,19 ± 21,00 respectivamente. La respuesta en cuanto a la sequedad vaginal y a la dispareunia fue evidente, no así la estrechez vaginal en la que hubo una respuesta menor. Conclusión: Este estudio permitió comprobar el incremento de los receptores estrogénicos a nivel de la mucosa vaginal en las mujeres posmenopáusicas que recibieron terapia hormonal durante 4 meses. El incremento fue mayor en las que recibieron estrógenos solos que en aquellas que recibieron terapia combinada. Abstract: objectives: to detect the presence of streptococcus mutans and streptococcus sobrinus in dental plaque of children from cartagena and correlate it to dental caries precavity stages, applying a standardized pcr-based technique for epidemiological purposes. methods: descriptive study using a non-probabilistic sample of 50 children between 3 and 5 years of age, preschoolers from a caribbean population in colombia. criteria for selection were that children should exhibit plaque accumulations on the surface of the cervical margins of the rearmost molars, and placed in one of two study groups: carious lesions and sound surfaces. dental plaque samples from both groups were subjected to molecular analysis and statistical analysis was applied to determine the difference between the two groups using the frequencies of presence of s. mutans, s. sobrinus or both in the two groups applying fisher's exact test for association between the presence of microorganisms and the state of the tooth surface from where the dental plaque was taken. results: the frequency of s. mutans in carious lesions was 76% and 24% in healthy surfaces. the frequency of s. sobrinus in carious lesions was 81.9% and 18.1% in caries-free surfaces. there was statistical significance between the presence of s. mutans and the presence of caries (p=0.001) and between the presence of s. sobrinus (p=0.02) and the presence of caries. there was no statistical significance between the presence of caries and the simultaneous presence of both microorganisms (p=0.08). conclusions: the presence of s. mutans and s. sobrinus in dental plaque samples is highly prevalent and associated to non cavitated carious lesions, being the molecular identification of these microorganisms by pcr a sensitive, fast, and easy to use detection method for the mutans group of oral bacteria.

Abstract: background. the internet and computerized bibliographic databases are the most dynamic tools for the best biomedical information that will allow a best clinical practice based on evidence. the use of data as a result of formal investigations and resolved problems and proposals for new scientific and generally for those who want to learn and be updated therefore dominate the management of these tools is a goal which compliance is increasingly desirable among students and health professionals. objectives. to describe the perceptions of students of one of faculty of dentistry from cartagena city in relation to their performance in the search for information in databases. methods. descriptive, cross-sectional quantitative methodological approach, 179 subjects of 433 students registered in the school of dentistry. variables: perceptions of knowledge, traditions, challenges, attitudes and preferences through the application of questionnaires assisted. results. students have access to databases, but have a single search mode. the use is less frequent with increasing age. it is most frequently used databases for the female sex. the reasons for initiating search responding to the fulfillment of tasks assigned. one of the main difficulties during the search is the knowledge of english. conclusions. perceptions of students regarding the use of the databases are related to aspects such as using the tool, guided in a way by a self-taught's attitudes and the other by internal or external stimuli of the subject. it recommended that complement these educational initiatives to achieve structured learning processes, framed more by self-interest and update progress in scientific knowledge than in the accomplishment of tasks assigned during the passage of an academic period.

Abstract: objective: to describe the representations that guide the social construiction of knowledge on oral health and disease in families living in the town of la boquilla cartagena. design: qualitative study, using an ethnographical method. setting: the town of la boquilla cartagena-colombia. participants: 15 families from the town in order to obtain useful information for the purpose of the study. main measurements: representations on the aeiology, prevention and treatment of oral disease, origin and meaning of oral health knowledge. results: we found interpretations on health and oral disease, which were influenced by culture, work, poverty and emotions, scientifically unsound which are able to recognize a social being with certain health attitudes and behaviour different from those of western culture, but trying to find a solution to the problem from different dimensions. conclusions: the social representations on health and oral disease in the town of la boquilla- cartagena, exert an influence on the appropriation of useful knowledge applicable to the family, to the point of challenging scientific knowledge transmitted by health professionals and thus draw conclusions based on an individual reflection from an empirical perspective.

Abstract: pyogenic granuloma is one of the reactive inflammatory oral diseases associated with contraceptive intake. the clinical features are not pathognomonic therefore its diagnosis must be made with histological support. its incidence is higher in women who are in the second and third decade of life. although its aetiology remains controversial there is clear evidence that it is a reaction to the presence of local irritants such as biofilms together with systemic conditions such as hormonal changes.
Factores Familiares asociados a la presencia de Caries Dental en Ni?os Escolares de Cartagena, Colombia

Abstract: objective: to identify sociodemographic and family factors and their association with the prevalence of caries in children attending the john f. kennedy school in cartagena de indias. design: cross sectional study. setting: cartagena de indias, colombia. participants: 243 students randomly selected by stratified sampling. main measurements: sociodemographic and family factors (marital relationship, education and role of parents, socioeconomic status and family structure and function using the family apgar) and prevalence of caries (cop-d index and ceo-d). the data were analyzed based on proportions and odds ratio using the stata 10.0 programme results: the prevalence of caries was 51% (95% ci: 44-57), higher in females (70%; p=0.02). the role of the father as educator of his children was the only significant factor (or: 0.40; 95% ci: 0.18 to 0.86; p <0.01) related to the prevalence of caries. conclusions: in addition to being traditionally an economic provider, the new role of the father as educator of his children can have a positive effect on reducing dental caries. fathers should be actively involved in the promotion and prevention activities in oral health.
