The Effect of an Educating versus Normalizing Approach on Treatment Motivation in Patients Presenting with Delusions: An Experimental Investigation with Analogue Patients

Abstract: Until recently a widespread recommendation for clinicians was not to respond to the content of patients' delusions but to stress at an early time point that the patient has a mental illness (educating approach). An opposed recommendation is to validate the patients’ symptoms and normalize them (normalizing approach). This study used an experimental design to compare the impact of these two approaches on treatment motivation (TM). A cover story about a person who develops persecutory delusions was used to guide a sample of 81 healthy participants who served as analogue patients into imagining experiencing delusions. This was followed by a random assignment to either an educating or a normalizing consultation with a fictive clinician. Consultations only differed in content. Finally, we assessed the participants' motivation to accept medication (Medication TM), psychological treatment (Psychological TM), and treatment offered by this particular clinician independent of the kind of treatment (Clinician-related TM). Participants in the normalizing condition showed higher Clinician-related and Psychological TM than those in the educating condition. Medication TM was unaffected by condition. Following our results using a normalizing approach seems to be advisable in a first-contact situation with patients with delusions and favourable to a simple educating approach. 1. Introduction Communication with the patient is a central feature of mental health treatment. In treating delusions, the question of what constitutes a “good communication style” is controversial. There seems to be a considerable gap between patients’ and clinicians’ perspectives of good communication in the consultation. Many patients actively attempt to talk about their delusional beliefs [1] and expect the clinician to listen and respond to their problems [2]. This expectation stands in contrast to clinical practice. Through analysing conversations in routine psychiatrist-patient consultations, McCabe et al. [1] found that psychiatrists avoid responding to the patients’ concerns and rather evade their questions. Van Meer [3] confirmed that many psychiatrists were traditionally trained not to respond to delusional beliefs. Although today the idea of discussing the content of patients’ beliefs is somewhat more widespread, many clinicians still fear that responding to delusional beliefs in an empathic manner or discussing them will make them worse [4]. Low Delay Noise Reduction and Dereverberation for Hearing Aids

Abstract: A new system for single-channel speech enhancement is proposed which achieves a joint suppression of late reverberant speech and background noise with a low signal delay and low computational complexity. It is based on a generalized spectral subtraction rule which depends on the variances of the late reverberant speech and background noise. The calculation of the spectral variances of the late reverberant speech requires an estimate of the reverberation time (RT) which is accomplished by a maximum likelihood (ML) approach. The enhancement with this blind RT estimation achieves almost the same speech quality as by using the actual RT. In comparison to commonly used post-filters in hearing aids which only perform a noise reduction, a significantly better objective and subjective speech quality is achieved. The proposed system performs time-domain filtering with coefficients adapted in the non-uniform (Bark-scaled) frequency-domain. This allows to achieve a high speech quality with low signal delay which is important for speech enhancement in hearing aids or related applications such as hands-free communication systems.
Synthesis and Characterization of a [Li0+xMg2-2xAl1+x(OH)6][Cl·mH2O] Solid Solution with X = 0 - 1 at Different Temperatures  [PDF]

Abstract: The synthesis of a novel Li+ /Mg2+ /Al3+ containing layered double hydroxide (LDH) by using a hydrothermal synthesis route is represented in this work. The autoclaves were heated up to 100oC, 120oC, 140oC and 160oC for 10 h and 48 h with a water to solid ratio (W/S) of 15:1. The physicochemical properties of the synthesized LDHs were investigated by X-ray powder diffraction (PXRD), fourier transform infrared spectroscopy (FTIR), thermo gravimetric and differential thermal analysis (TG-DTA), inductively coupled plasma optical emission spectroscopy (ICP-OES) and scanning electron microscopy (SEM). The formation of a solid solution phase depends strongly on the composition of the reactants and the synthesis temperature. Using an exact stoichiometric ratio of Li+/Mg2+/Al3+ resulted in the synthesis of amorphous phases without producing plenty of crystalline amounts of the expected solid solutions while using higher temperatures than 140oC resulted in a formation of AlO(OH). To avoid the formation of an Al containing amorphous phase or an AlO(OH) crystalline phase, the stoichiometric ratio of Li+ was changed. The results show solid solutions with the formula [Li0+xMg2-2xAl1+x(OH)6][Cl.mH2O] with X ≥ 0.9. The lattice parameters and chemical compositions for solid solutions with different compositions were determined and the pure solid solution with the highest amount of Mg (x = 0.9) is [Li0.9Mg0.2Al1.9(OH)6] [Cl.0.50H2O] with the lattice parameters a = 5.1004(4) Å, c = 15.3512(1) Å, V = 345.844(9) Å3. For X < 0.9 two separate phases, a Mg2+ and a Li+ dominated solid solution, are coexistent.
RESULTS OF SURGICAL TREATMENT IN CERVICAL HERNIATED DISC ANALYSIS OF 275 CASES

Abstract: Background: Problems of surgical treatment of cervical disc hernia are still far from being resolved. It shows variety of surgical procedures it. The operations for cervical level with radiculopatie were quite limited. Limits method were dictated by the absence of accurate diagnosis, microsurgical methods, extensions, sometimes unjustified, conservative treatment. Material and method Complex issue of cervical spine degenerative pathologies, particularly cervical disc hernia with radicular syndrome was studied on a group of 275 patients. Results: From the 275 patients with herniated disc cervical admitted in the study, 182 (66.18% of cases) had surgical intervention to a single level disc, 72 patients (26.18% of cases) in two levels, 20 patients (7.27% cases) at three levels and one case (0.36%) at four levels.

In her book essay that surveys three centuries of cultural history, the cultural studies and literary scholar Hannelore Schlaffer uses rhetorical fireworks to coherently show that the “schooling” of women in the west is determined by binary gender regulations that always remain constant. This happens irrespective of fashionable and clearly defined gender differences such as those in existence at the beginning of the bourgeois era, in the “masculinization” of the woman as “Kn bin” or literally “boyish woman” since the 20s, or in the integration of femininity in youth subcultures such as punk. Hannelore Schlaffer is able to prove that even the ideals of female education follow fashionable developments.

Abstract: se presenta de manera sinóptica y selectiva, el reflejo de la conquista-colonia y reconquista espa?ola en un corpus de narrativa histórica chilena publicado entre 1843 y 1879. teóricamente se apunta en forma breve a la discusión sobre la legitimidad de unir una historia y ficción y se establecen los criterios para seleccionar la narrativa histórica de ficción. la presentación y análisis de algunos relatos históricos muestra el predominio del conflicto con espa?a, ya sea en época colonial o durante la guerra de la independencia, ya sea contra indígenas o chilenos. en los relatos que toman como tema la colonia, la imagen del indígena se presenta con una mirada doble según la perspectiva del emisor y refleja dos estereotipos: el guerrero valiente y el bárbaro. en los relatos que transcurren durante la reconquista espa?ola, el episodio más recurrente es el "desastre" en rancagua. episodio que crea sus propios héroes nacionales y virtudes cívicas. prácticamente todas las narraciones tratadas, se impregnan de un discurso valorativo patriótico y anticolonial

Abstract: La represión franquista provocó una fuerte movilización internacional y el Proceso de Burgos en diciembre de 1970 fue uno de los episodios que más llamó la atención de la opinión pública mundial. Este corto artículo pretende presentar el impacto del proceso de Burgos en la región Lemosín, y en los dos periódicos más vendidos en la zona. Así, a este nivel, evocaré algunas pistas para entender por qué los activistas de ETA se beneficiaron de tanto apoyo. Los trabajos sobre negociación colectiva publicados en revistas espa olas antes de 1995 e incluidos en la base de datos ISOC. Acercamiento biliométrico

Abstract: Legal literature is one of the areas most neglected by bibliometric studies, both in Spa in and abroad. Within this context, the general aim of this note is to check the utility of national databases in the retrieval and overview of the legal literature published in Spanish journals. 447 papers on collective bargaining and allied subjects published before 1995 and included in ISOC database have been retrieved. Using a computer file of these papers, the distributions by years, number of authors per paper, institutions of origin, journals and specific subjects was analysed from a bibliometric approach. As a case study, distributions of the publications which have been used in a research paper on a specific subject of collective bargaining are also included. The results obtained indicate the utility of national databases for two reasons: 1) the systematic retrieval of legal literature which is different from, and at very least complementary to, the information retrieved using traditional methods, 2) the bibliometric approach provides an overview of questions of relevance when researching specific legal subjects. La literatura jurídica es una de las áreas más descuidadas en los estudios bibliométricos tanto espa oles como internacionales. En este contexto, el objetivo general de la presente nota es contribuir a la comprobación de la utilidad de las bases de datos nacionales para recuperar y obtener una visión de conjunto de la bibliografía jurídica publicada en revistas espa olas. Se han recuperado 447 trabajos sobre negociación colectiva y cuestiones afines publicados antes de 1995 e incluidos en la base de datos ISOC. Con un fichero informático de dichos trabajos, se ha realizado un acercamiento bibliométrico relativo a su distribución por a os, número de firmas por trabajo, instituciones de procedencia, revistas y temas específicos. Se incluyen también, como caso concreto, distribuciones de las publicaciones utilizadas en un trabajo de investigación sobre un aspecto específico de la negociación colectiva. Los resultados permiten concluir la utilidad del recurso a las bases de dato s nacionales por dos razones: 1) la recuperación sistemática de la bibliografía jurídica, de carácter distinto y, como mínimo, complementaria a la obtenida mediante los procedimientos tradicionales; 2) el acercamiento bibliométrico proporciona una visión de conjunto sobre aspectos de interés para desarrollar la investigación sobre un tema jurídico determinado.
