A comperative clinical study of mucotomy and KTP laser treatment of the inferior turbinate in allergic and non-allergic subjects  [PDF]

Abstract: Mucotomy is one of the most frequently applied surgical techniques for the management of inferior turbinate hyperplasia. Mucotomy guarantees patent airway, however, it might lead to the emergence of sicca syndrome. In contrast, KTP (potassium titanyl phosphate) laser treatment spares the medial part of the inferior turbinate mucosa, contributing to maintenance of physiological nasal function. A retrospective comparative clinical study was performed to reveal the advantages and side-effects of both surgical methods in medium- and long-term in allergic rhinitis and non-allergic patients. Furthermore, we wished to determine the exact indications of the up-to-date laser treatment. Ninety-one of the 117 patients who underwent bilateral turbinate surgery during an 8-year period (2000-2007) responded to our questionnaire focusing on subjective postoperative changes. Patients were separated into 6 groups, based on the type of operation they underwent, the length of the follow-up and whether they suffered from allergies. The major complaint, nasal obstruction, improved in all 6 groups, which reached significance (p ? 0.05) in 4 groups. The most pronounced improvement was observed in the group of non-allergic patients with medium-term follow-up who underwent mucotomy. Nevertheless, a serious side- effect: crusting also increased significantly (p ? 0.05) in the latter group, while it was absent in allergic patients with medium-term follow-up, who underwent mucotomy. These results lead us to propose the following protocol for the treatment of inferior turbinate hyperplasia: 1) after unsuccessful conservative treatment, laser treatment is suggested for non-allergic patients; 2) following unsuccessful conservative and even repeated laser treatment in the allergic group, mucotomy or turbinoplasty should be attempted.
Elementary proof techniques for the maximum number of islands

Abstract: Islands are combinatorial objects that can be intuitively defined on a board consisting of a finite number of cells. Based on the neighbor relation of the cells, it is a fundamental property that two islands are either containing or disjoint. Recently, numerous extremal questions have been answered using different methods. We show elementary techniques unifying these approaches. Our building parts are based on rooted binary trees and discrete geometry. Among other things, we show the maximum cardinality of islands on a toroidal board and in a hypercube. We also strengthen a previous result by rarefying the neighborhood relation.
Restriction Enzyme Body Doubles and PCR Cloning: On the General Use of Type IIS Restriction Enzymes for Cloning

Abstract: The procedure described here allows the cloning of PCR fragments containing a recognition site of the restriction endonuclease (Type IIP) used for cloning in the sequence of the insert. A Type IIS endonuclease - a Body Double of the Type IIP enzyme - is used to generate the same protruding palindrome. Thus, the insert can be cloned to the Type IIP site of the vector without digesting the PCR product with the same Type IIP enzyme. We achieve this by incorporating the recognition site of a Type IIS restriction enzyme that cleaves the DNA outside of its recognition site in the PCR primer in such a way that the cutting positions straddle the desired overhang sequence. Digestion of the PCR product by the Body Double generates the required overhang. Hitherto the use of Type IIS restriction enzymes in cloning reactions has only been used for special applications, the approach presented here makes Type IIS enzymes as useful as Type IIP enzymes for general cloning purposes. To assist in finding Body Double enzymes, we summarised the available Type IIS enzymes which are potentially useful for Body Double cloning and created an online program (http://group.szbk.u-szeged.hu/welkergr/b?ody_double/index.html) for the selection of suitable Body Double enzymes and the design of the appropriate primers.
TGFβ Activated Kinase 1 (TAK1) at the Crossroad of B Cell Receptor and Toll-Like Receptor 9 Signaling Pathways in Human B Cells

Abstract: B cell development and activation are regulated by combined signals mediated by the B cell receptor (BCR), receptors for the B-cell activating factor of the tumor necrosis factor family (BAFF-R) and the innate receptor, Toll-like receptor 9 (TLR9). However, the underlying mechanisms by which these signals cooperate in human B cells remain unclear. Our aim was to elucidate the key signaling molecules at the crossroads of BCR, BAFF-R and TLR9 mediated pathways and to follow the functional consequences of costimulation.Therefore we stimulated purified human B cells by combinations of anti-Ig, B-cell activating factor of the tumor necrosis factor family (BAFF) and the TLR9 agonist, CpG oligodeoxynucleotide. Phosphorylation status of various signaling molecules, B cell proliferation, cytokine secretion, plasma blast generation and the frequency of IgG producing cells were investigated. We have found that BCR induced signals cooperate with BAFF-R- and TLR9-mediated signals at different levels of cell activation. BCR and BAFF- as well as TLR9 and BAFF-mediated signals cooperate at NFκB activation, while BCR and TLR9 synergistically costimulate mitogen activated protein kinases (MAPKs), ERK, JNK and p38. We show here for the first time that the MAP3K7 (TGF beta activated kinase, TAK1) is responsible for the synergistic costimulation of B cells by BCR and TLR9, resulting in an enhanced cell proliferation, plasma blast generation, cytokine and antibody production. Specific inhibitor of TAK1 as well as knocking down TAK1 by siRNA abrogates the synergistic signals. We conclude that TAK1 is a key regulator of receptor crosstalk between BCR and TLR9, thus plays a critical role in B cell development and activation.
Mesenchymal stem cells rescue cardiomyoblasts from cell death in an in vitro ischemia model via direct cell-to-cell connections

Abstract: Our model revealed that adding normal MSCs to the ischemic cell population significantly decreased the ratio of dead H9c2 cells (H9c2 only: 0.85 ± 0.086 vs. H9c2+MSCs: 0.16 ± 0.035). This effect was dependent on direct cell-to-cell contact since co-cultivation with MSCs cultured in cell inserts did not exert the same beneficial effect (ratio of dead H9c2 cells: 0.90 ± 0.055). Confocal microscopy revealed that cardiomyoblasts and MSCs frequently formed 200-500 nm wide intercellular connections and cell fusion rarely occurred between these cells.Based on these results we hypothesize that mesenchymal stem cells may reduce the number of dead cardiomyoblasts after ischemic damage via direct cell-to-cell interactions and intercellular tubular connections may play an important role in these processes.Cardiovascular diseases represent an enormous medical and social burden [1,2] and the pathophysiology of most of these diseases, such as myocardial infarction or heart failure, involves death of cardiac myocytes leading to a loss of functional tissue. Cell based therapies are commonly believed to be the next generation of therapies for replacing such lost tissue [3-5]. Several in vivo animal and human studies have found that implantation of various cell types, typically bone marrow derived stem cells, into damaged myocardium improved cardiac performance. Also where the experimental protocol allowed, surviving grafted cells were detected in the myocardium [6,7], suggesting that grafting is an effective treatment of acute myocardial infarction [8]. However, the extent of the beneficial effect, the optimal cell type and number, the best method of administration, and the mechanism of action need to be further evaluated [9].One important issue is the exact mechanism of action, in other words, the interaction between graft and host. Lattice induced threshold functions and Boolean functions

Abstract: Lattice induced threshold function is a Boolean function determined by a particular linear combination of lattice elements. We prove that every isotone Boolean function is a lattice induced threshold function and vice versa. We also represent lattice valued up-sets on a finite Boolean lattice in the framework of cuts and lattice induced threshold functions. In terms of closure systems we present necessary and sufficient conditions for a representation of lattice valued up-sets on a finite Boolean lattice by linear combinations of elements of the co-domain lattice.
Invariance groups of finite functions and orbit equivalence of permutation groups

Abstract: Which subgroups of the symmetric group S_n arise as invariance groups of n-variable functions defined on a k-element domain? It appears that the higher the difference n-k, the more difficult it is to answer this question. For k>=n, the answer is easy: all subgroups of S_n are invariance groups. We give a complete answer in the cases k=n-1 and k=n-2, and we also give a partial answer in the general case: we describe invariance groups when n is much larger than n-k. The proof utilizes Galois connections and the corresponding closure operators on S_n, which turn out to provide a generalization of orbit equivalence of permutation groups. We also present some computational results, which show that all primitive groups except for the alternating groups arise as invariance groups of functions defined on a three-element domain.
A general framework for island systems

Abstract: The notion of an island defined on a rectangular board is an elementary combinatorial concept that occurred first in [G. Cz\'edli, The number of rectangular islands by means of distributive lattices, European J. Combin. 30 (2009), 208-215]. Results of this paper were starting points for investigations exploring several variations and various aspects of this notion. In this paper we introduce a general framework for islands that subsumes all earlier studied concepts of islands on finite boards, moreover we show that the prime implicants of a Boolean function, the formal concepts of a formal context, convex subgraphs of a simple graph, and some particular subsets of a projective plane also fit into this framework. We axiomatize those cases where islands have the comparable or disjoint property, or they are distant, introducing the notion of a connective island domain and of a proximity domain, respectively. In the general case the maximal systems of islands are characterised by using the concept of an admissible system. We also characterise all possible island systems in the case of island domains and proximity domains.
