Medición ecográfica del espesor medio-intimal carotídeo en pacientes pediátricos con obesidad, hipercolesterolemia familiar y diabetes tipo 1

Abstract: to evalúate by ultrasonographic images carotid intima-media thickness (imt) as an early marker of cardiovascular risk (cvr) in patients with clinical and laboratory diagnosis of familial hypercholesterolemia (fhc), type 1 diabetes (dbt-1) and obesity (ob), compared to a healthy control group. materials and methods. a prospective, descriptive, and cross-sectional study. we evaluated the imt of common carotid (ce) arteries and included in a single dichotomic variable other structural modifications of the wall (atheromatous plaques and intima irregularities). us was blinded to the blood tests results (as recommended by the 2007 mannheim consensus) from 121 patients (aged 6 to 18 years): 24 fhc, 40 dbt-1, 43 ob, and 14 controls. patients with other diseases that could alter the arterial wall were excluded. variables were analyzed using the statistix 8 program. results. the three groups had a higher imt than the control group, with these differences being statistically significant. mean imt (mm) were 0.59 (0.31-2.15), p: 0.006, ci 0.06 to 0.36 in fhc; 0.48 (0.3-0.85), p: 0.001, ci 0.06-0.14 in obesity; and 0.46 (0.25-0.65), p: 0.0004, ci 0.03-0.13 in dbt-1. control group: 0.37 (0.30-0.45). the major difference was observed in fhc patients. no association was found between ldl-c valué and imt upon the examination, 62.5% were receiving pharmacotherapy. in dbt-1 no association was found between imt and the levéis of hbalc and lipids. in the ob group there was no association between imt and bmi z-score. atheromatous plaques and intima irregularities were found in 31% of fhc patients, 8% of dbt-1 patients, and 6% of ob patients. conclusión. ultrasonographic imt measurements demonstrated that patients with chronic diseases and increased cvr at adulthood presented early changes in the carotid intima-media at childhood. this allows clinical prevention strategies and therapeutic guidance.

Abstract: Propósito. Evaluar con ultrasonografía (US) el Espesor Medio Intimal (EMI) como marcador de riesgo cardiovascular (RCV) temprano en pacientes con diagnóstico clínico y de laboratorio de Hipercolesterolemia Familiar (HCF), Diabetes Tipo 1 (DBT-1) y Obesidad (OB), comparando con grupo control. Materiales y Métodos. Estudio prospectivo, descriptivo y de corte transversal. Evaluamos el EMI de las arterias carótidas comunes (CC) e incluimos en una sola variable dicotómica otras alteraciones estructurales de la pared (placas ateromatosas e irregularidades de la íntima). La US fue realizada en forma cegada al resultado de los exámenes de sangre, según las recomendaciones del consenso de Mannheim 2007, en 121 pacientes (de 6 a 18 a os): 24 HCF, 40 DBT-1 y 43 OB; y 14 controles. Se excluyeron pacientes con otras patologías que pudieran alterar la pared arterial. Analizamos las variables con el programa Statistix 8. Resultados. Comparados con los controles, los 3 grupos tuvieron un mayor EMI, siendo las diferencias estadísticamente significativas. Los EMI Medio (mm) fueron en HCF: 0,59 (0,31-2,15), p: 0,006; IC: 0,06-0,36. Obesos: 0,48 (0,3-0,85), p: 0,001; IC: 0,06-0,14. DBT-1: 0,46 (0,25-0,65), p: 0,0004; IC: 0,03-0,13. Grupo control: 0,37 (0,30-0,45). Los pacientes con HCF presentaron la mayor diferencia. No se encontró asociación entre el valor de LDL-C y el EMI. El 62,5% recibían tratamiento farmacológico en el momento de la evaluación. En DBT-1 no se encontró asociación entre el EMI y los niveles de HbA1c y lípidos; y en el grupo OB no se encontró asociación entre el EMI y el Z score IMC. El 31% de HCF, el 8% de DBT-1 y el 6% de OB presentaron placas ateromatosas e irregularidades de la íntima. Conclusión. La US del EMI permitió demostrar que los pacientes con enfermedades crónicas, con RCV aumentado en edad adulta, presentaron alteraciones precoces de la íntima media carotídea en la etapa pediátrica. Esto permite aplicaciones clínicas preventivas y de orientación terapéutica. Morfometria de amendoim forrageiro em diferentes estágios de desenvolvimento

Abstract: O amendoim forrageiro é uma planta que pode ser utilizada como fonte alimentar para animaisno sistema lavoura-pecuária. Desse modo, o objetivo desse trabalho foi avaliar a morfometriado amendoim forrageiro Arachis pintoi cv Belmonte (BRA 031828) em diferentes estágios dedesenvolvimento (alturas de dossel de 5, 10, 20, 30 e 40 cm) e mensurar os parametros referentes àmatéria seca. Os parametros avaliados foram a oferta de matéria seca, o comprimento total dashastes, o comprimento médio dos entrenós, o número de entrenós e folhas por haste e a rela ofolha/haste. Os resultados foram semelhantes para o número de folhas por haste entre as alturasdo dossel, contudo a rela o folha/haste diferiu entre as alturas de 5 e 30 cm e 5 e 40 cm (P<0,05).As médias de matéria seca aumentaram de forma exponencial com a altura do dossel (P<0,05).Os comprimentos das hastes e dos entrenós aumentaram de forma linear (P<0,05). N o houvediferen a para o número de entrenós por haste entre as alturas de dossel acima de 10 cm. Emconclus o, os dados desse experimento indicaram que o aumento na altura do dossel após teratingido 10 cm se dá pelo crescimento do ramo devido ao alongamento dos entrenós e, apesar daprodu o de matéria seca ter alterado conforme a altura de corte, n o se evidenciou diferen asnos números de folhas/ramo e na rela o de folha/haste conforme a altura do dossel.

Abstract: This paper describes a calculation strategy that allows determining the optimal number and placement of sectionalizing switches in MV radial distribution networks, in correspondence to technical, regulatory and economical aspects. A formulation that takes into account the investment, maintenance and power interruption costs has been developed, seeking for a reduction in total costs while taking care of the regulatory and technical aspects. A multicriteria optimization procedure allows incorporating in the calculating process various quality indicators which can be either global or individual indexes. This way of formulation makes the proposal flexible as well as applicable to allow including aspects that were not considered in previous papers. The solution methodology is mainly based on dynamic programming, fuzzy logic, heuristics and economic analysis techniques. Given its flexibility, the proposed tool is easily adapted to real distribution systems, by considering the individual requirements of each network. The solutions obtained in simulations are oriented to help decision-making for the operator.
Hypertrophic osteoarthropathy as the cause of situs inversus totalis: A case report  [PDF]

Abstract: Situs inversus totalis is a very rare anomaly. Even more, cases of situs inversus totalis plus hypertrophic osteoarthropathy. A woman presented with cyanosis, clubbing of fingers and distal edema. Chest examination revealed heart sounds at the right side and a heart murmur. Abdominal examination showed liver dullness in the left side. The X-rays showed dextrocardia and liver in the left side. The Upper digestive barium test showed the stomach in the right side, the barium enema showed caecum on the left. Treatment for heart failure improved the patient’s condition and she remained well. When she became pregnant, developed heart failure again and required admission. She was compensated and continued her pregnancy. 3 years later she developed heart and respiratory refractory failure, was admitted again and after few days she died. We report this case of situs inversus totalis in association with hypertrophic osteoarthropathy because of the rarity of this anomaly.

Abstract: The paper aims to know the idioms that the elderly use to describe and make sense of their experience of aging. It is based on ethnographic data gathered through the in-depth interview of 30 normal and community dwelling elderly in Marikina City, Metro Manila, Philippines in 2012-2013. The participants were chosen through compulsive sampling from the registry of 1385 senior citizens screened free from dementia. Their age ranged from 60 to 90 years, regardless of social class, gender, ethnicity, religion and other indices of difference. The interviews were transcribed, verbatim, and coded for the idioms, their meanings and contexts in which the elderly used them. The elderly communicate in idioms, which maybe basic to denote identity that obtains from aging or secondary in that they develop from the basic ones and help define identity further. The idioms relate to either continuity or discontinuity theory of aging or help rationalize what it means to be human and elderly.

Abstract:

The application of the Moroccan Family Code by Spanish legal operators is not simple, to the extent that it is not only a question of knowing its contents, but also, behind each of its articles and, relating to the institutions contained in the Code, there is a specific understanding of the world and, more specifically, family relations. The Spanish system of Private International Law is to serve as a “communication channel” between legislations which are beginning to coincide due to the new private international situations taking place in Spain, as a new foreign population begins to settle, in particular when it comes to female Moroccan immigrants. In these cases, Moroccan family law may be applied or considered by the Spanish authorities, favouring thus the recognition of decisions taken in Morocco.

Abstract: The recognition of cultural diversity as a value of the European Union Law is undoubtedly present in the current process of integration, which, in turn, is related to the maintenance of peace. Without prejudice to the treatment given by national regulations (Private International Law systems) of each Member State to the issue of cultural diversity existing in the current European society, it may be mentioned the positioning that has also been taken by the EU legislature of PIL on the treatment of said diversity. Tools used by the Private International Law system both for European Union Member States and for the European Private International Law system itself to give answers to this multiculturalism located in the territory that comprises the European Union Member States are discussed below. In particular, art. 10 of the Council Regulation (EU) no. 1259/2010, of 20 December 2010, implementing enhanced cooperation in the area of the law applicable to divorce and legal separation. This exception (public policy) will be practically operational in general when it comes to the implementation of the national system of an Islamic countries, because it may be considered that there is a difference between men and women in everything related to family relationships. The aim of this paper is to analyze the possibility of the “recognition method” as a clause for the interpretation of Private International Law to permit the application of foreign Muslim Law by European authorities in family cases and to avoid limping relationships.

Abstract: Some cellular editing functions can restrict the replication of some viruses but contribute to completion of the life cycle of others. A recent study has identified an isoform of the adenosine deaminase acting on RNA type 1 (ADAR 1) as required for embryogenesis, and as a restriction factor for a number of important RNA virus pathogens [1]. The dual implication of key cellular functions in the innate immunity against viruses, or, paradoxically, as mediators of virus replication is interpreted in the light of the concept of virus-host coevolution and tinkering proposed for general evolution by Fran?ois Jacob decades ago.
