Association of Mitral Annulus Calcification with High-Sensitivity C-Reactive Protein, Which Is a Marker of Inflammation

Abstract: Objectives. There are limited clinical data revealing the relationship between mitral annular calcification (MAC) and systemic inflammation. The goal of the present study was to compare high-sensitivity C-reactive protein (hs-CRP) levels in patients with and without MAC and investigate the relationship between MAC and hs-CRP. Methods. One hundred patients with MAC who underwent transthoracic echocardiography (TTE) and 100 age-matched controls without MAC who underwent TTE were included in our study. Hs-CRP levels were compared between groups. Results. Prevalence of female gender, hypertension, and coronary artery disease were significantly higher in the MAC group than in the control group (64% versus 45%, =0.007, 42% versus 28%, =0.03 and 37% versus 18%, =0.003, resp.). On multivariate analysis, age, gender, and coronary artery disease were the only independent predictors of MAC. The levels of hs-CRP were higher in the MAC group than in the control group (2.02±0.35 versus 1.43±0.47 mg/dl, <0.001). This increase in hs-CRP levels in the MAC group persisted in patients without hypertension, coronary artery disease, and in male patients when compared to the control group. Conclusions. Our study demonstrated that hs-CRP, which is a sensitive marker of systemic inflammation, increased in patients with MAC.
The Effects of Blended Learning and Online Learning on Academic Achievement and Learner Satisfaction

Abstract: In this study, the effects of blended and online learning environments on students academic achievement and distance education learning satisfaction was investigated by comparison. The study was carried out among 73 students who were from Group-A and Group-B of the “Planning and Evaluation in Instruction” course, a second year course in the Department of Elementary Education in Gazi University K rsehir Faculty of Education in the academic year of 2006. Group-A was selected as the treatment group and Group-B was selected control group randomly. Treatment group studied in the blended learning environment four weeks while control group studied in online learning environment for four weeks. Quasi experimental design with pretest-posttest-retention test with control group was used in the analysis of academic achievement. Distance Learning Satisfaction Survey was used in the analysis of students distance learning satisfaction. According the results, students satisfied in both of the distance learning environments in general. On the other hand, the result of the one-way ANOVA showed that the students of the blended learning environment satisfied more with student-student interaction, instructor support, content, goals, and institutional support than did the students of the online learning environment. Also, the ANCOVA results by controlling pretest scores indicated that students of the blended learning were more succesful in the posttest than were students of online learning. Finally, The ANCOVA results by controlling posttest scores indicated that students of the blended learning showed more success in the retention test than were students of online learning.
Serum Asymmetric Dimethylarginine and Nitric Oxide Levels in Patients with Diabetic Neuropathy

Abstract: Objective: Asymmetric dimethylarginine (ADMA) is a novel marker of endothelial dysfunction. ADMA is an endothelial nitric oxide synthase inhibitor. Due to scarcity of studies on the role of ADMA in pathogenesis of diabetic neuropathy, it remains to be determined whether blood ADMA and nitric oxide (NO) levels are associated with diabetic neuropathy. In this case control study, we aimed to evaluate the relationship between serum NO and ADMA levels among diabetic neuropathy casesMethods: 48 patients with diabetic neuropathy and 34 healthy subjects were included in the study. All type 2 diabetic patients underwent electrophysiological and neurological examination. NO levels were determined using the Griess method. Serum ADMA levels were determined by using the ELISA method.Results: The serum ADMA levels in diabetic neuropathy patients were found to be statistically significantly lower than those of the control group (p=0.005). However, a statistically significant increase was observed in serum NO levels in patients with diabetic neuropathy compared with those in the control group (p = 0.009). A negative correlation was found between NO levels and ADMA levels (p=0.041, r=-0.23).Discussion: Changes in NO and ADMA levels in the blood of diabetic neuropathy patients may indicate oxidative stress and endothelial dysfunction in the pathogenesis of diabetic neuropathy. Abstract: In this experimental study was investigated the effect of reaction temperature in NiFe2O4 nano particles synthesis with hydrothermal method. An appropriate ratio of solutions nickel nitrate and ferric nitrate were dissolved in deionized water and poured into a crucible. Later, polyethylene glycol 600 (PEG 600) was added to this mixture. Samples were adjusted to pH 11 values using NaOH solution . Accordingly, experments were made at 180, 200 and 250 oC, respectively. The other parameters, were fixed as reaction time 24 h and pH value 11. The structural and morphological properties of NiFe2O4 nano particles were determined by X-ray powder diffraction (XRD) and Scanning Electron microscopy (SEM). Results showed that increasing calcination temperature contributed to cyristallinity of NiFe2O4 nano particles. But also average particle size increased. As a result, average particle size was calculated by using Debye-Scherrer Formula as aproximately 30 nm. However, this results was confirmed with SEM andTEM analysis. Serum levels of fibroblast growth factor in patients with intracerebral hemorrhage

Abstract: Objectives: Intracerebral hemorrhage (ICH) is one of the most mortal subtypes of stroke. Due to the angiogenic, neurotropic, and vessel-dilating properties of basic fibroblast growth factor (bFGF) in the brain, role of bFGF has been investigated in a number of neurological disorders. So far, there is only study about serum bFGF levels in patients with ICH. The first aim of the present research is to investigate whether increased serum bFGF in patients with ICH. Also, second aim was to study the association between serum levels of bFGF and clinical status in patients with ICH.Materials and methods: We measured the serum levels of bFGF in 30 patients with ICH during acute period. Age and sex matched healthy subjects (n=30) were included in controls. Serum bFGF levels were measured using an enzyme-linked immunosorbent assay method.Results: The patients with intracerebral hemorrhage had higher serum levels of bFGF when compared with the healthy controls (12.89±3.23 ng/ml, 5.28±1.75 ng/ml; p=0.001). No statistically significant difference was determined between bFGF levels of the patients who died as compared to the patients who lived (13.49±4.13 ng/ml; 12.43±3.43 ng/ml, p>0.05). No statistically significant difference was found between bFGF levels of the patients with intraventricular hemorrhage as compared to the patients without intraventricular hemorrhage (13.54±3.92 ng/ml; 12.24±2.29 ng/ml, p>0.05). There was no correlation between serum bFGF, hematoma volume, Gloskow coma scale, and National Institutes of Health stroke scale (p>0.05).Conclusion: The increased bFGF level may be one of the mechanisms that lead to angiogenesis and neuroprotection after ICH in human. . Silent Cerebral Ischemia and Infarct Prevelance in Chronic Renal Failure Patients

Abstract: Objective: Cerebrovascular diseases are the most common causes of morbidity and mortality. The ischemic type is the most common and most fatal o among the all cerebrovasculer diseases. In patients having silent cerebral ischemia the incidence of ischemic infarction is ten times greater than in the normal population. The treatment modalities are not efficient after the cerebral infarct has occurred. For this reason academic studies are gaining importance for the determination of risk factors, primary and secondary prophylaxis, early diagnosis, treatment and rehabilitation. Cerebrovascular diseases are also the most common major causes of death in hemodialysis patients. The effect of dialysis treatment and chronic renal failure disease on the development of cerebral ischemia and infarct has not been explained, In this study we analyzed the prevalence of silent cerebral ischemia and infarct in the cronic renal failure patients having hemodialysis and peritoneal dialysis treatment, using magnetic resonance imaging. Material and Methods: The study included 30 patients with hemodialysis, 40 patients with peritoneal dialysis and control group of 50 patients admitted to emergency department due to complaints other than neurological ones.Results: Although the case and control groups having similar demographical properties and risk factors, the prevalence of silent cerebral ischemia and infarct is determined as significantly increased in the chronic renal failure patient group.Conclusion: It is determined by MR that chronic renal failure has significant effects on developing cerebral ischemia and infarct. These findings showed us chronic renal failiure is an important risk factor for developing cerebral ischemic stroke. Additional clinical and experimental studies are needed on this subject.
Cd138 Expression in Renal Tumors and Its Usage in the Differential Diagnosis

Abstract: Objective: The differential diagnosis of kidney tumors, especially those with eosinophilic cytoplasms, can be problematic due to overlapping morphologic features. CD138 is primarily a plasma cell marker but is known to be expressed in the proximal renal tubular epithelium as well. This study aims to investigate the possible contribution of CD138 expression in the differential diagnosis of kidney tumors with eosinophilic cytoplasm.Material and Method: The case series consisted of 15 chromophobe (ChRCC), 5 eosinophilic variant (EoRCC), 10 clear cell (CCRCC) and 9 papillary (PRCC) renal cell carcinomas, and 13 oncocytomas. Sections obtained from representative paraffin blocks were stained against CD138 antibody.Results: All CCRCC and PRCC showed membranous CD138 expression. In some of the other eosinophilic renal tumors, cytoplasmic CD138 labeling in varying degrees was detected. In CCRCC cases, CD138 expression was especially observed in low grade areas and areas showing cystic and pseudopapillary growth patterns. A similar pattern of cytoplasmic staining was seen in 3 of the EoRCC and the most of the PRCC cases (6/9).Conclusion: Our findings suggest that CD138 may contribute to the differential diagnosis of renal tumors because of the membranous staining pattern in CCRCC and EoRCC cases and the cytoplasmic staining in CHRCC and oncocytoma cases. Its contributory role may be improved by combined usage with markers like Cytokeratin 7 and RCC marker.
The influence of α-adducin gene polymorphism on response of blood pressure to exercise in patients with hypertension

Abstract: Objective: Clinical studies have indicated that an excessive response of blood pressure (BP) to exercise predicts risk of cardiovascular mortality. Although the mechanism responsible for the excessive BP response to exercise has not been revealed, there are some plausible mechanisms linking with underlying structural abnormalities in the cardiovascular system. Carriers of the Trp460 allele of the α-adducin Gly460Trp polymorphism have an increased risk of hypertension. The aim of the present study was to examine the influence of α-adducin gene polymorphism on response of BP to exercise in patients with hypertension. Methods: The cross-sectional observational study consisted of 49 hypertensive patients (29 women and 20 men; mean age, 53.1±8.8 years). All participants underwent a multistage exercise treadmill test according to the Bruce protocol. Arterial BPs were compared at rest, peak exercise and end of the recovery phase. Patients were classified according to their α-adducin gene polymorphisms; Gly460Gly homozygotes - Group 1 (n=28) and Trp460Trp homozygotes and Gly460Trp heterozygotes - Group 2 (n=21). Statistical analysis was performed using Chi-square, unpaired t, Mann-Whitney U and ANCOVA tests.Results: Mean exercise duration and mean exercise capacity in metabolic equivalents were not different between Group 1 and 2. The major finding of the study was that systolic BP responses at peak exercise and recovery period (3. min) were significantly higher (p=0.036) in hypertensive patients carrying at least one Trp460 allele of the α-adducin gene. Conclusion: Our results suggest that genetic variants that alter renal function and/or vasoreactivity are logical candidates to explain some of the individual variability in the BP response to exercise.
