Encuesta nacional online aplicada en pacientes con enfermedad celíaca en Chile A national online survey applied to patients with celiac disease in Chile

Abstract: Background: Celiac disease (CD) is an immune-mediated enteropathy triggered by the ingestion of gluten in genetically susceptible individuals. Its prevalence in Europe and the USA is 0.5 to 1%. Aim: To analyze epidemiological aspects and degree of compliance with gluten-free diet (GFD) among Chilean individuals with CD. Material and Methods: Subjects with confirmed or suspected CD were invited to answer an online survey published on the web at www.fundacionconvivir.cl. The answers were reinforced with a telephone interview. Results: The survey was answered by 1212 subjects (79% females). Median age at diagnosis was 25.8 years (range 1 to 84 years), with a bimodal curve with two peaks at less than 3 years and at 20 to 40 years of age. The diagnosis was made only by serologic markers in 9%, only by intestinal biopsy in 17.5%, and by a combination of both methods in 70%o. Conditions associated with CD were reported by 30%> of subjects and 20%> had relatives with CD. The GFD was strictly adhered to by 70%>, occasionally by 27%> and never by 3%>. Seventy five percent of subjects with a strict adherence to GFD had a favorable clinical response compared with 42%> of those with incomplete or lack of adherence (odds ratio 4.0, 95%> confidence intervals 2.8-5.7p < 0.01). Conclusions: In 30% of respondents, the diagnosis of CD was not confirmed according to international guidelines that require serology and duodenal biopsy. One third of subjects recognized a poor compliance with GFD. Those with a strict adherence to it had a more favorable clinical course. However, 25%> did not experience a clinical improvement despite a strict GFD, a finding which requires further study.

Abstract: the purpose of this work is to demonstrate that the aortic enlargement procedures, first described for patients with small aortic annulus, are reproducible with excellent results. a retrospective revision of 72 cases in which an enlargement procedure was done, with consideration regarding the relationship between valve size and patient body surface area, in order to avoid prosthesis-patient mismatch, was done. we consider the immediate mortality of the group, differentiating the cardiac mortality, concluding with other authors that body surface areas between 1.3 and 1.7 m2 should receive aortic prosthesis larger than # 23, which will eventually improve the late outcome of this population. we demonstrate that the aortic annular enlargement procedures can be safely done in our hospital, with a slight increase in mortality during the learning phase, but great benefit afterwards.

Abstract: in this paper the conception of argumentation theory as applied epistemology is discussed. the point of departure is the description of four perspectives that are considered as founders of the modern theory of argumentation, in order to observe whether there was a similar concept in those theories or if they provided the patterns to go into that direction. further on the reasons why contemporary scholars have given this emphasis to the notion of argumentation theory is discussed.

Abstract: this paper suggests a way to understand and analyze the working of metaphors from the point of view of argumentative practice. for this purpose, it discusses not only the main contributions made by cognitive linguistics to the analysis of metaphors, but also stephen toulmin's (1958) perspective for argumentative reconstruction, known as factual logic. the parallel between the metaphorical descriptive design and working logic model shows that the conceptual metaphors work as 'backing' when they are manifested as social assumptions, experience or general evidence, as well as 'warrants' when one of the correspondences is manifested as a passage-law between the premises and the conclusion of the argument.

Abstract: El artículo sugiere una forma de entender y analizar el funcionamiento de las metáforas conceptuales desde la perspectiva de la práctica argumentativa. Para el efecto, se discuten tanto algunos de los principales aportes de la lingüística cognitiva en el análisis de las metáforas, como la perspectiva de Stephen Toulmin (1958) para la reconstrucción de argumentos conocida como lógica factual. El paralelo entre el dise o de descripción metafórica y el modelo de la lógica factual, arroja que las metáforas conceptuales operan como 'apoyos' argumentativos cuando se manifiestan como asunciones sociales, campo de experiencia o evidencia general, y como 'garantías' cuando una de las correspondencias se manifiesta como ley de paso entre las premisas y la conclusión del argumento. Abstract: El modelo mixto de ense anza-aprendizaje pretende utilizar las tecnologías de la información y de la comunicación (TIC) para garantizar una formación más ajustada al Espacio Europeo de Educación Superior (EEES). Se formularon los siguientes objetivos de investigación: Averiguar la valoración que hacen los alumnos de Magisterio del aula virtual WebCT como apoyo a la docencia presencial, y conocer las ventajas del uso de la WebCT y de las TIC por los alumnos en el estudio de caso: Valores y contravalores transmitidos por series televisivas visionadas por ni os y adolescentes . La investigación se realizó con una muestra de 205 alumnos de la Universidad de La Rioja que cursaban la asignatura de Tecnologías aplicadas a la Educación . Para la descripción objetiva, sistemática y cuantitativa del contenido manifiesto de los documentos se ha utilizado la técnica de análisis de contenido cualitativa y cuantitativa. Los resultados obtenidos demuestran que las herramientas de comunicación, contenidos y evaluación son valoradas favorablemente por los alumnos. Se llega a la conclusión de que la WebCT y las TIC constituyen un apoyo a la innovación metodológica del EEES basada en el aprendizaje centrado en el alumno. Los alumnos evidencian su competencia audiovisual en los ámbitos de análisis de valores y de expresión a través de documentos audiovisuales en formatos multimedia. Dichos alumnos aportan un nuevo sentido innovador y creativo al uso docente de series televisivas. The mixed model of Teaching-Learning intends to use Information and Communication Technologies (ICTs) to guarantee an education more adjusted to European Space for Higher Education (ESHE). The following research objectives were formulated: 1) To find out the assessment made by teacher-training college students of the virtual classroom WebCT as an aid to face-to-face teaching. 2) To know the advantages of the use of WebCT and ICTs by students in the case study: Values and counter-values transmitted by television series for children and teenagers . The research has been carried out using a sample of 205 students of the University of La Rioja who attended a course in Technologies Applied to Education. The technique of qualitative and quantitative content analysis has been used to provide an objective, systematic and quantitative description of the different documents content. The results obtained show that the communication, content and assessment tools of WebCT are favourably assessed by the students. Abstract: in this paper the problem of presumption is analysed as much from a pragmatic perspective, as from the angle of contemporary argumentation theory. the main goal is to characterise and discuss the role of this type of speech act in controversial exchanges. in this effort, cognitive, pragmatic and argumentative aspects will be considered. the analysis will conclude that although presumption has a conservative social strength, it is characterised mainly by being a complex speech act that is manifested in sequence and, for this reason, is at the same time a compromising speech act as well as a defeasible and fallible reasoning.

Abstract: the concepts of 'paradigm', 'incommensurability' and 'rule' are still widely used. thomas kuhn applied them to refer to the way in which science could be organized. the problem is that the structure of scientific revolutions is quoted as a case of epistemological discussion, rather than a case of an historical perspective on science. in this paper kuhn's use of these concepts is commented upon to show that this position has no other backing than a particular visión of science as exposed in the context ofthe 1950's and 1960's in the united states.
