Inmunodiagnóstico en Didelphis marsupialis usando un antígeno de Paragonimus de Venezuela Immunodiagnostic in Didelphis marsupialis using an antigen of Venezuelan Paragonimus

Abstract: Los Paragonimus son trematodos que habitualmente viven en los pulmones de mamíferos carnívoros y omnívoros, entre ellos el hombre. En el oriente venezolano se encuentra el único foco de Paragonimus sp. donde Didelphis marsupialis es el único reservorio demostrado hasta ahora. Con el fin de tener herramientas de inmunodiagnóstico que detecten la presencia de Paragonimus sp. en esta especie, se elaboraron varios reactivos para realizar un ensayo inmunoenzimático ELISA. Entre ellos se obtuvo un antígeno crudo soluble de vermes adultos de Paragonimus y una inmunoglobulina de gallina anti-IgG de Didelphis marsupialis. Los mismos se capturaron en la localidad de Aguas Blancas, municipio Montes, estado Sucre, Venezuela, y se obtuvieron muestras sanguíneas; en el caso de estar infectados, los vermes adultos se extrajeron del pulmón. Los parásitos se homogenizaron y ultracentrifugaron para obtener la fracción soluble del parásito (FSPA) como antígeno para el ELISA y Western blot y detectar los anticuerpos en los Didelphis marsupialis. El análisis electroforético mostró 22 moléculas entre 6 y 82 kDa; por Western blot se presentó un reconocimiento antigénico de 8 moléculas siendo las de 112 kDa y 268 kDa las más reconocidas por los sueros positivos. Los sueros negativos no reconocieron ninguna proteína. La producción de IgY en gallinas permitió desarrollar las técnicas de inmunodiagnóstico para la búsqueda de anticuerpos específicos anti-Paragonimus sp. en Didelphis, cuya aplicación permitirá establecer la vigilancia epidemiológica de estos reservorios en áreas endémicas sin sacrificio de los mismos. Abstract: hypertriglyceridemia is a cardiovascular risk factor in type 2 diabetes. however, this abnormality may be caused by several mechanisms. in familial hypertriglyceridemia, a hyperlipidemia associated with type-2 diabetes, plasma accumulation of non atherogenic particles explains the presence of hypertriglyceridemia. our objective was to compare the prevalence of coronary insufficiency and carotid artery stenosis in patients with type-2 diabetes with or without familial hypertriglyceridemia. controls were paired against cases based on age, gender, diabetes duration, treatment and other cardiovascular risk factors. controls had either a normal lipid profile (n=48) or hyperlipidemia (n=15). the intima media thickness of the carotid arteries was significantly lower in cases compared to controls (0.55±0.12 vs 0.63± 0.22 in normolipidemic controls and 0.66 ±0.18 mm in hyperlipidemic subjects (p=0.02)). exercise treadmill testing was abnormal in a similar proportion of cases and controls (4.8 vs 6.2%). incidence of cardiovascular complications was not different between groups. we therefore conclude that severe hypertriglyceridemia due to familial hypertriglyceridemia is not associated with an increased prevalence of symptomatic atherosclerosis in patients with type-2 diabetes.

Diagnóstico de Dirofilaria immitis en el municipio Sucre, estado Sucre, Venezuela

Abstract: heartworm is a parasitic disease, caused by nematodes dirofilaria immitis and d. repens, mainly. while d. immitis presents worldwide distribution, d. repens extends the old world. both species affect canines and felines domestic and wild, as well as human, so it is a global public health problem. in this study we determined the prevalence of d. immitis in dogs in the municipality of sucre, sucre state, venezuela. blood samples were taken for 138 domestic dogs, over six months, regardless of race or sex, prior informed consent by their owners. the parasitological diagnosis of circulating microfilariae was done by direct examination and concentration method modified knott, the morphological characteristics were observed in giemsa-stained smears. we used a commercial kit, for the capture of excretory-secretory antigens of d. immitis. of the 138 dogs studied, 12 cases (8.7%) were detected parasitologically, whereas the commercial kit antigens were detected in 20 dogs (14.5%), demonstrating that there is an occult infections in 5.8% of the cases. one of the dogs with microfilariae in blood, did not submit antigens circulating. the prevalence of heartworm in the municipality of sucre, sucre state was 15.2%. this report of canine heartworm in the municipality of sucre, represents an important contribution in an epidemiologic survey of the disease. future studies will assess the risk of infection for people living with dogs with heartworm disease.

Abstract: paragonimus sp. is a trematode that causes chronic inflammation of the lung in carnivorous mammals and humans, which constitute a public health problem in asian and latin american countries. trematodes have enzymes that facilitate their penetration and migration through different host organs to ensure their life cycle. to evaluate the enzymatic diversity of the soluble fraction (fspa, 100,000 g) of a venezuelan isolate of paragonimus sp. adult worms, several enzyme determinations were conducted at different ph. the activities of enzymes releasing p-nitrophenol or p-nitroanilina from the corresponding dye-related synthetic peptides were assessed by interpolating absorbance (a 405 nm) values in the corresponding calibration curve (a 405 nm vs. nmol); on the other hand, absorbances of 2-naphtylamine and 2-naphtols released from another series of synthetic substrates were read at different wavelengths between 450 nm and 620 nm to assess for the activity of the corresponding hydrolases. phosphohydrolase, glycosidase and peptidase activities were detected in fspa, β-n-acetyl-β-d-glucosaminidase (0.55 μmol/h/mg, ph 5.5) and cystein protease (0.4 μmol/h/mg, ph 5.5) being higher than all the other detected activities. these activities are probably related to the adult worm habitat and its need for glycan and peptide degradation of lung secretions. these results represent the first enzymatic study done with a venezuelan isolate of adult paragonimus sp. worms collected from the common reservoir didelphis marsupialis.

Abstract: neo-vascular glaucoma is one of the most complex variants of glaucoma to be accurately detected and treated; it is a secondary glaucoma of hard management. it is recognized by its clinical findings, associated with underlying diseases and difficult to be treated. until not long ago, the prognosis was uniformly severe, and most of the time loss of eyeball was frequent, having a secondary onset after a retinal ischemia. the proceedings and treatment of a case, secondary to a thrombosis of the central vein of retina, that was diagnosed in pinar del rio province is presented.

Abstract: congenital glaucoma is not frequent, it appears in the first three years after birth and it is an isolated disorder in the development of the angle of the anterior chamber, the majority of patients do not manifest its presence at birth because of the small production of aqueous humor. the early onset has a poor prognosis, these patients present a very high altered drainage, lack of aqueous humor produced in the eye of the newborn that cannot be managed. proceedings and treatment performed in a case that was diagnosed in pinar del rio province is presented. the patient achieved a satisfactory evolution.

Abstract: El glaucoma congénito es poco frecuente, aparece en los primeros tres a os de la vida y se debe a un trastorno aislado en el desarrollo del ángulo de la cámara anterior; la mayoría de los pacientes no se manifiestan al nacimiento a causa de la peque a producción de humor acuoso en los primeros meses. La aparición precoz tiene un mal pronóstico. Estos pacientes tienen un drenaje tan alterado que ni siquiera pueden manejar la escasa cantidad de humor acuoso producida por el ojo del neonato. Se expone el manejo y tratamiento de un caso diagnosticado en la provincia de Pinar del Río. Se obtuvo evolución satisfactoria del paciente. Abstract: El glaucoma neovascular es de las variantes de glaucoma más difíciles de detectar y tratar correctamente; es un glaucoma secundario de difícil manejo y tratamiento. Se reconoce por sus hallazgos clínicos, las enfermedades subyacentes asociadas y la dificultad de su tratamiento, hasta hace poco tiempo, el pronóstico era uniformemente grave y con frecuencia significaba la pérdida del globo ocular Se presenta secundario a una isquemia retiniana. Se expone el manejo y el tratamiento de un caso diagnosticado en la provincia de Pinar del Río, secundario a una trombosis de la vena central de la retina.
