The Reform of The Eastern Anatolia From The Pen of Ahmet A ao lu (1913 – 1914)

Abstract: Ahmet A ao lu (Agayef), a politician and a philosopher, played an active role in the Turkism movement. He defended the rights of Turks in the articles he wrote in various newspapers in Azarbaijan. After he settled down in stanbul, in 1909, he was appointed as Minister of Education Inspectorate. He worked in several different newspapers, then he became the editorial writer of “ Tercüman- Hakikat” newspaper. A ao lu, who is among the founders of “Türk Derne i” (The Association of Turks) and “Türk Oca ” (The Organization of Turks), was selected as deputy from Afyon. During the time he was the editorial writer of Tercüman- Hakikat, he emphasized the necessity of this and compared this notion with the reform of Balkans, in the articles about the reform of the Eastern Anatolia. Furthermore, he tried to define the attitudes and intentions of great powers (states) towards reforming. Referring to the eagerness and determination of Ottoman Goverment, he also pointed out that the expected contribution was not carried out by the Armenians, neither inside nor outside the country.

Abstract: Hüseyin Cahit Yal n has been elected as the Istanbul parliamentarian in 1908 from the “ ttihat ve Terakki” party. He has attracted attentions also with his articles that he wrote in the “Tanin” newspaper. He has been employed in the“Düyun-u Umumiye” administration (1911-1922) and he has been exiled by the British to Malta. He has been electedin 1939 as a parliamentarian from the CHP party and he dealt with journalism and politics until he died (1957). He wrote articles on Eastern Anatolian Reform that came into the agenda after the Balkan War; he emphasized the necessity of thisand apart from this, he has compared this notion with the Balkan reformation. He has criticized the Western countries that tried to give direction to the matter and that were the interveners, where Russia and England were at the forefronts. Yal n, who was also against the approaches of the Armenians who were causing invitation to external intervention on continuous basis, has exhibited care in supporting the decisive approaches of the Ottoman Government.

Abstract: The state of Ottoman administration, after the Balkan war, considering the tension between the Alliance and Entente countries, wanted to have German support for the land forces and England assistance for the navy. But,after the arrival of German army committee at Istanbul and appointment of Liman von Sanders to the commander of the first army, Russia converted it to an international problem because of its sensitivity about the problem of the straits. The Ottoman administration keeping its’ determined attitude and having partial assignment revisions, kept reform improvements in spite of everything. The course of this issue is followed from the newspapers Tercüman- Hakikat, Tanin, kdam, Tasfir-i Efkar, Peyam and Sabah.
Comparison of Five Percent Prilocaine-lidocaine Cream and Intravenous Fentanyl in Reducing the Spinal Puncture Pain

Abstract: One of the major disadvantages of the spinal anesthesia is that the patients feel anxiety and pain during the spinal needle puncture. It is aimed to compare the analgesic effects of 5% prilocaine-lidocaine cream with 1 g kg-1 intravenous fentanyl applied before spinal puncture. Seventy-five male patients of ASA I-II, with the age range 20-65 were included and randomly divided into three groups in the study. Forty-five minutes before the spinal needle puncture 2.5 g 5% prilocaine-lidocaine cream was applied on the intervention area of the Group A patients. Ten minutes before the spinal puncture 1 g kg-1 fentanyl intravenous bolus was applied to the Group B patients; but no analgesics or local anesthetics were applied to the Group C patients. The anxiety level of the patients was measured with APAIS-A anxiety score and the sedation level was measured with Ramsay Sedation Scale. The pain felt by the insertion of the spinal needle was assessed with 0-4 range Verbal Rating Scale. Additionally, patients satisfaction, quality of performance and irritation movement were evaluated. Verbal Rating Scale scores and irritation movements were significantly lower in Group A. Patients satisfaction was significantly higher in Group A. Quality of performance was significantly higher in Group A than the rest of the groups and in Group B than Group C. As a result, it is concluded that by considering the parameters of analgesic activity, patients satisfaction, irritation movements and quality of performance; 2.5 g, 5% prilocaine-lidocaine cream is superior to intravenous 1 g kg-1 fentanyl in reducing the spinal puncture pain.
Reconstruction of the Chest Wall Defects with Mammary Dermoglandular Advancement Flaps in Female Complicated Cases

Abstract: Objectives: There are several myocutaneous flaps used for chest wall reconstruction. However, in some cases, usage of these flaps is not suitable. If the patient is female, we aimed to show that use of the breast tissue while saving its lateral and medial pedicles is an option for the reconstruction.Patients and Methods: Our study included four female patients who presented with recurrence of cancer (50-year-old with malign thymoma, 60-year-old with left lung cancer, 41-year-old with chondrosarcoma, and 49-year-old with a history of intraductal carcinoma). Mammary tissue can be elevated either to its lateral or medial pedicles and used as an advancement flap.Results: Breast tissue was used for the reconstruction as an advancement flap on either its lateral or medial pedicles on all patients. In the follow-up of the patients, there was no circulatory problem of the flaps or necrosis. The first three patients have died due to their primary diseases. The fourth patient is being followed up by the oncology department with no local recurrence yet.Conclusion: In selected female patients, breast tissue is a good choice for reconstruction due to its rich blood supply and easy accessibility.
Pott’s Disease in a 2-Year-Old Child Treated by Decompression and Anterior-Posterior Instrumented Fusion

Abstract: Introduction. Paraplegia and kyphotic deformity are two major disease-related problems of spinal tuberculosis, especially in the early age disease. In this study a 2-year-old boy who underwent surgical decompression, correction, and 360° instrumented fusion via simultaneous anterior-posterior technique for Pott’s disease was reported. Case Report. A 2-year-and-9-month-old boy presented with severe back pain and paraparesis of one-month duration. Thoracic magnetic resonance imaging demonstrated destruction with a large paraspinal abscess involving T5-T6-T7 levels, compressing the spinal cord. The paraspinal abscess drained and three-level corpectomy was performed at T5-6-7 with transthoracic approach. Anterior instrumentation and fusion was performed with structural 1 autogenous fibula and rib graft using screw-rod system. In prone position pedicle screws were inserted at T4 and T8 levels and rods were placed. Six months after surgery, there was no weakness or paraparesis and no correction loss at the end of follow-up period. Discussion. In cases of vertebral osteomyelitis with severe anterior column destruction in the very early child ages the use of anterior structural grafts and instrumentation in combination with posterior instrumentation is safe and effective in maintenance of the correction achieved and allows efficient stabilization and early mobilization. 1. Introduction Spinal tuberculosis, the most common pattern of extra pulmonary tuberculosis, also has increased rapidly around the world in recent years, especially in the undeveloped and developing countries [1]. Paraplegia and kyphotic deformity development are two major disease-related problems. Medical and surgical decompression (antituberculous treatment and debridement) and early reconstruction of spinal stability plays an important role in the surgical management of spinal tuberculosis [2, 3]. The reported techniques of surgical treatment range from anterior debridement and interbody fusion [4, 5] and debridement and internal fixation from the posterior approach [6] to combined single-stage or two-stage posterior instrumentation with anterior debridement and bone grafting [7–14]. Recently decompression, fusion, and instrumentation by simultaneous posterior-anterior-posterior surgery are recommended for effective kyphosis correction and early ambulation [15]. Anti-pancreatic antibody in Turkish patients with inflammatory bowel disease and first-degree relatives

Abstract: AIM: To identify the role of anti-pancreatic antibody (PAB) in the diagnosis of inflammatory bowel diseases (IBD) among Turkish patients, and its frequency in first-degree relatives.METHODS: PAB and anti-Saccharomyces cerevisiae (ASCA) were examined in serum samples of 214 subjects including patients with Crohn’s disease (CD, n = 64), ulcerative colitis (UC, n = 63), first-degree relatives of patients with CD (n = 25), first-degree relatives of patients with UC (n = 28),and a control group with gastrointestinal symptoms other than (IBD) (n = 34) by indirect immunofluorescence Positivity of PAB and ASCA was compared in terms of Vienna classification, disease activity and medications used.RESULTS: In terms of PAB positivity, no difference was found between patients with CD (14.1%) and UC (7.9%) however, significant difference was observed between patients with CD and subjects in the control group (P < 0.05). No difference was found between patients with CD and their relatives in terms of ASCA positivity, whereas a significant difference was found between other groups (P < 0.001). Compared to ASCA, the sensitivity of the PAB was 19% (7/37), its specificity was 93% (25/27), positive predictive value was 77% (7/9) and negative predictive value was 45% (25/55). ASCA was found with significantly higher prevalence in patients with CD activity index > 150 (P < 0.05).CONCLUSION: PAB is valuable in the diagnosis of IBD rather than CD, but cannot be used alone for diagnostic purposes. PAB is not superior to ASCA in CD diagnosis and in detecting CD among relatives of patients with CD.

Abstract: G lgesizler, which is written by Hasan Ali Topta attracts attention at first glance with its extraordinary plot technique. The novel is double stranded. The events take place in parallel to each other.In the first strand the events takes place in city; in the second one the events take place in a village. These two strand and the places are tied to each other with the people who get lost in village appearing in the city or get lost in the city appearing in the village. The narrator sometimes looks into the city sometimes into the village. The novel is plotted between these two places so the two strands mingle with each other turning into the shape of spiral. In this essay this novel will be analysed in terms of its plot technique. Other element which attracts attention is the theme of the novel. Topta handles people who exist on the one hand and do not exist on the other hand. Existence and nonexistence are the vicious circle of the life. Abstract: The Second New is a poetical movement which came out in the second half of 1950s. The leading fig ures of the movement are lhan Berk, Cemal Süreya, Edip Cansever, Turgut Uyar, Ece Ayhan and Sezai Ka rako . These poets have also concerned themselves with music, cinema, painting, drama and other fields of art and literature. In some cases they have benefited from these arts in their poems. Particularly painting has had a great influence in their woks.
