Abstract: st-effectiveness study of oral hypoglycemic agents in the treatment of outpatients with type 2 diabetes attending a public primary care clinic in Mexico City Original Research (2782) Total Article Views Authors: Díaz de León-Casta eda C, Altagracia-Martínez M, Kravzov-Jinich J, Cárdenas-Elizalde MR, Moreno-Bonett C, Martínez-Nú ez JM Published Date March 2012 Volume 2012:4 Pages 57 - 65 DOI: http://dx.doi.org/10.2147/CEOR.S27826 Received: 01 November 2011 Accepted: 29 November 2011 Published: 07 March 2012 Christian Díaz de León-Casta eda, Marina Altagracia-Martínez, Jaime Kravzov-Jinich, Ma del Rosario Cárdenas-Elizalde, Consuelo Moreno-Bonett, Juan Manuel Martínez-Nú ez Department of Biological Systems and Health Care, Biological and Health Sciences Division, Universidad Autónoma Metropolitana-Xochimilco, Mexico DF, Mexico Introduction: Worldwide, diabetes mellitus presents a high burden for individuals and society. In Latin America, many people with diabetes have limited access to health care, which means that indirect costs may exceed direct health care cost. Diabetes is Mexico's leading cause of death. Purpose: To evaluate the cost-effectiveness ratios of the most used oral hypoglycemic agents (OHA) in the treatment of outpatients with type 2 diabetes attending a public primary care clinic in Mexico City. Design: A cross-sectional and analytic study was conducted in Mexico City. Methodology: Twenty-seven adult outpatients with type 2 diabetes who were treated either with metformin or glibenclamide were included. Acarbose was used as an alternative strategy. The study was carried out from the perspective of Mexican society. Direct medical and nonmedical costs as well as indirect costs were evaluated using a structured questionnaire. Efficacies of all drug treatments were evaluated retrospectively. A systematic search was conducted to select published randomized clinical trials based on predetermined inclusion criteria, and treatment success was defined as glycosylated hemoglobin factor ≤ 7%. Efficacy data of each drug and/or combination were analyzed using meta-analysis. The Monte Carlo Markov model was used. Quality-adjusted life-years (QALY) were used as the unit of effectiveness; incremental and sensitive analyses were performed and a 5% discount rate was calculated. A hypothetical cohort of 10,000 patients was modeled. Results: The odds ratios of the success of each drug treatment were obtained from the meta-analyses, and were the following: 5.82 (glibenclamide), 3.86 (metformin), 3.5 (acarbose), and 6.76 (metformin–glibenclamide). The cost-effectiveness ratios found were US$272.63/QALY (glibenclamide), US$296.48/QALY (metformin), and US$409.86/QALY (acarbose). Sensitivity analysis did not show changes for the most cost-effective therapy when the effectiveness probabilities or treatment costs were modified. Conclusion: Glibenclamide is the most cost-effective treatment for the present study outpatient population diagnosed with type 2 diab

Análisis de la literatura teórica sobre neutralidad de red y sugerencias de política
Castaeda Sabido, Alejandro;
EconoQuantum , 2009,
Abstract: we analyze several definitions of net neutrality; we find that a common element is that all definitions implicitly assume an open architecture network that favors the entry of new content providers. however, the definitions differ in the possibility for the last mile operator to discriminate in prices. we discuss vertical integration when there is competition among platforms and where there is not. we also touch on the literature of two sided markets. we discuss a model of two sided markets to analyze the problem of net neutrality. we find out that, in general, monopolies or oligopolies do not have enough incentives to generate the right price structure that internalizes all the network externalities. a policy of net neutrality is socially superior to a policy that allows the monopolies or oligopolies to fixe their charges freely. the paper argues in favor of a policy of net neutrality that allows for price discrimination based upon quality service to final users. we also argue against a policy that allows for last mile operators that give service to final users, to overcharge to content providers an extra charge for delivering the information to the final users. we propose a bill and keep interconnection agreement that reduces the market power that network operators exert. this has to be accompanied with a policy that favors the entry of various platforms by internet provision: cable, telephone, bpl and wimax.
Determinantes de las tarifas de interconexión: Un estudio a nivel internacional
Castaeda Sabido, Alejandro;
EconoQuantum , 2011,
Abstract: good interconnection conditions between telecommunications networks are a necessary condition to achieve a high level of competition among networks. thus, it is important to study what the determinants of interconnection tariffs are, because the inferences that can be achieved through this study can be used to design policies that aim at an improvement in the interconnection tariffs (and in the conditions of interconnection in general). this paper uses regression analysis and a sample of fifteen countries to study the determinants of interconnection tariffs. we find results that indicate that those countries with autonomy in its regulatory entities, unbundled network schemes and the use of lric methodologies to calculate access prices have on average lower interconnection rates. also, those countries with higher gdp per capita have lower interconnection rates. there is evidence of economies of scale in fixed services. we conclude with policy recommendations that emerge from the results.

Abstract: background: around the world, there are several models in field epidemiology training programs whose trainees and graduates each year have the opportunity to present their abstracts in the scientific meeting of regional or global character tephinet. the aim is to describe the experience of the process of registry, evaluation and selection for the oral presentation or poster abstracts at the 5th scientific regional meeting tephinet for the americas, in addition to evaluating the quadlogic ? software as a tool for registry, classification and evaluation of scientific abstracts. method: we applied the descriptive observational model study, based on 20 field epidemiologist evaluators from the 13 participating countries and 154 abstracts to evaluate. the process was divided into registry, evaluation and selection of abstracts in oral presentation or poster, classified by subject of presentation: outbreaks (ob), hospital acquired infections (hi), surveillance and methods (s/m), vaccine preventable diseases (vpd), emerging infections (ei), sti/hiv, other topics (ot). results: initial registry of 169 abstracts, out of 12 of the 14 fetp belonging to the region of the americas and the epiet of spain, 15(8.9%) eliminated as duplicates. assigned by evaluator =24.3 (range=3:34). score =3.0 (range=0:4). registration evaluator’s comments: at least one, 69(40.9%); two, 42(27.3%), three, 10(6.5%) and without comments 39(25.3%). 125(81.2%) abstracts were selected, 44(18.6%) oral presentation, 81(52.6%) poster and 29(18.8%) rejected. as subject in oral presentation: ob 15(34.1%), s/m 12(27.3%), ot 8(18.2%), ei 5 (11.4%), vpd 2(4.2%) and hi 2(4.2%). as poster: s/m 31(38.3%), ob 24(29.6%), ot 18(22.2%), vpd 5(6.2%), hi 2(2.5%) and sti/hiv 1(1.2%).. conclusions: the emphasis of training in the study of outbreaks and surveillance and methods was reflected, as well as the implementation of field epidemiology in other topics of public health interest. using the system ql allowed randomly as
El parlache: resultados de una investigación lexicográfica
CASTAEDA,LUZ STELLA;
Forma y Función , 2005,
Abstract: the idea of carrying out an investigation on the lexicographical characterization of the parlache in order to elaborate a dictionary, was born by the interest in searching thoroughly on a dialectal variety of diastratic origin from the colombian spanish, called parlache, which is widely used by our society, but specifically by youth of popular and outskirt neighbourhoods of medellin and its metropolitan area. in all the society linguistic changes are occurring which show the transformation of their reality. as a matter of fact, the occurrence of linguistic change in the popular and outskirt districts of medellin advanced so fast that it surpassed the commonl limits of this phenomenon, due to the worsening of the social crisis, the upspring of new kinds of “work”, marked by legal trespass and spread of drug communities which mainly affected the youth as a whole sector of society. this is the reason why the parlache is mainly and most properly used by youth between 15 and 26, although it is a widespread dialectal variety.
