Dor de origem dental como motivo de consulta odontológica em uma popula o adulta

Abstract: OBJETIVO: A ocorrência de dor orofacial e dor cr nica é tema freqüente nos estudos da atualidade; porém, a dor de origem dental é pouco estudada no Brasil. O estudo tem como objetivo conhecer a prevalência de dor de dente como motivo de consulta odontológica e os fatores associados em indivíduos adultos. MéTODOS: Realizou-se um estudo transversal com 860 funcionários de uma cooperativa localizada no Estado de Santa Catarina, com idade entre 18 e 58 anos, em 1999. Exames clínicos e entrevistas foram realizados por uma cirurgi -dentista previamente treinada. Analisou-se a queixa de dor de origem dental como motivo da última consulta odontológica como variável dependente em rela o às condi es socioecon micas, demográficas, acesso ao servi o odontológico, turno de trabalho e ataque de cárie por meio do índice CPO-D como variáveis independentes. Foi utilizada a análise de regress o logística múltipla n o-condicional. RESULTADOS: A prevalência de dor de origem dental foi de 18,7% (IC 95%[15,9-20,1]) e o CPO-D médio 20,2 dentes (IC 95%[19,7-20,7]), com 54% representados pelo componente perdido. Foram associados independentes para a presen a de dor de origem dental a escolaridade menor ou igual a oito anos de estudo (OR=1,9[1,1-3,1], a perda por cárie de quatro a 15 dentes (OR=2,6[1,4-4,9]) e de 16 a 32 dentes (OR=2,5[1,1-5,8]) e n o ter freqüentado o servi o odontológico da empresa (OR=2,8[1,6-5,1]). CONCLUS ES: A dor de origem dental reflete a gravidade da cárie dentária, expressa pelo componente perdido do CPO-D e o n o uso de servi os odontológicos da empresa. Esses fatores s o determinados pelas condi es sociais, representadas pela escolaridade.

Abstract: objective: the occurrence of orofacial pain and chronic pain are frequent subjects for study today, but few studies have been made on dental pain in brazil. the objective of the study was to assess the prevalence of dental pain and the associated factors as the reason for visiting a dentist among adults. methods: a cross-sectional study was carried out among 860 workers aged 18-58 years at a cooperative located in the state of santa catarina, in 1999. the clinical examinations and interviews were carried out by dentists who had received prior guidance. complaints of dental pain as the reason for the last visit to a dentist were analyzed as the dependent variable, in relation to the socioeconomic and demographic conditions, access to dental services, shift pattern and caries (via the dmft index), as the independent variables. non-conditional multiple logistic regression analysis was utilized. results: the prevalence of dental pain as the reason for the last visit to a dentist was 18.7% (ci 95%: 15.9-20.1) and the mean dmft index (decayed, missing and filled teeth) was 20.2 (ci 95%: 19.7-20.7), with 54% represented by the 'missing' component. the following were independently associated with the presence of dental pain: schooling of less than or equal to eight years (or=1.9; ci 95%: 1.1-3.1); four to fifteen teeth lost due to caries (or=2.6; ci 95%: 1.4-4.9); 16 to 32 teeth lost due to caries (or=2.5; ci 95%: 1.1-5.8); and not having visited the company's dental service (or=2.8; ci 95%: 1.6-5.1). conclusions: dental pain reflects the severity of the dental caries, expressed by the 'missing' component of the dmft and non-usage of the company's dental services. these factors are determined by social conditions and represented by the schooling level.

Abstract: Objective: to identify the profile of nursing professionals providing care to pregnant women in maternity Alfenas-MG. Method: this is about a descriptive cross sectional study from quantitative approach. Participated in this study, all nursing professionals (24) who worked in the care of patients. Data collection was performed during the period from June to September 2008 using a structured interview. This study was conducted according to ethical principles of research and approved by the Ethics in Research of Universidade Federal de Alfenas/MG, under protocol number3087.001016/2008-98. Results: there was a lack of midwife in the team, keeping workload of 49.17 hours/week, 91.6% being female. As the professional category, the prevailing mid-level professionals with informal training and monitoring at work those with greater experience in the area. Conclusion it is understood that these professionals are not qualified for the obstetric care according to criteria of World Health Organization (WHO), highlighting the need for qualification of this team that assists women in maternity Alfenas, Minas Gerais.

Abstract: the article examines the conditions in chile that fostered development of a social medicine doctrine defended by the left. this doctrine influenced the legal, institutional, and technical construction of the servi?o nacional de saúde (sns) in 1952. central to the workings of the democratic regime (up until 1973) and to the free and open public discussion of national projects of a socializing nature was a party system that encompassed the entire ideological spectrum. the pioneering creation of workers social security in 1924 is underscored. likewise of import was the emergence of transformational proposals regarding the health-assistance model that it consolidated. ensuing conflicts were finally overcome following world war ii, under the influence of england?s national health service. chilean political culture yielded the consolidation of lasting concepts in social medicine but created barriers to the project to build a universalized sanitary-medical system, intended to provide integral care to all citizens.

Abstract: this article discusses: first, the importance of including the political dimension in the institutional diagnose, since the rationale of governmental decisions and actins is that of the conflictant interest e struggle for spaces of power in side the state bureaucracy rather than the logics of rational and neutral technical-administrative plans. second reviews the concept o f bureaucracy from different theoretical approaches so as to show new analytical possibilities for public administration studies. third, focuses "sanitary bureaucracy" as an analytical unity to be investigated and understood at the lights of the conceptual framework already discussed. finally, points out the relevance of the approach not only for public health administration research and teaching but also for designing tactical-operational plans to viabilize the brazilian sanitary reform.

Abstract: this article begins by discussing the concept of social capital and its applications and limitations. second, it examines these theoretical contributions as approaches actually used by scholars to analyze the disenchantment with the fragile political institutions and citizens' apathy that emerged together with re-democratization processes in latin america. third, it focuses on the role of the brazilian health councils, seeking insights from this vast and rich experience of popular mobilization and participation to shed light on the councils' contributions in fostering behaviors inherent to social capital, such as civic values and cooperation.
La reinvención neoliberal de la inequidad en Chile: el caso de la salud

Abstract: this paper examines the main changes in the chilean health system from the 1920s to the present, identifying contradictions, limits, and successes in terms of policy outcomes. taking equity as the central theme, the analysis focuses on decisions made from the mid-1920s until 1973 in moving toward socialized medicine, with the sudden interruption of this process by the military coup in 1973. the author then discusses the set of regressive measures taken by authoritarian neoliberalism, with the help of pinochet, to insert health into the market economy. finally, the article analyzes efforts made by democratic administrations since 1990 to redress the legacy of socioeconomic inequalities, focusing on the commitment to rebuild the health care system with a new basis in equity, solidarity, and people's participation.

Abstract: this work analyses the main political aspects of transformations in the health systems of chile and brazil in the xx century, emphasizing the chilean neo-liberal "modernizations" in the 80s under the pinochet dictatorship, and, in the same decade, the brazilian reforms during the democratic transition initiated after almost 20 years of military rule. political development is pointed to as a relevant dimension in the explanation of the differences in policy-making and, thus, in the configuration of each health system. last but not least, it is postulated that the historical legacy of each health institutional construct challenges changes both under democratic and authoritarian regimes.

Abstract: based on policy-making models that blend the pluralist, neocorporatist, and neopluralist approaches, the article examines decision-making patterns in the health policy arena in 20th century chile. during this period, chile?s political development saw three characteristic phases: liberal democracy, military dictatorship, and "neo-democracy". accompanying these phases, the article analyzes how health policies have been shaped and what forms of interest mediation physicians have employed since 1924, when laborers social insurance was implemented. lastly, it offers some hypotheses regarding the decision-making and organizational formats prevalent in the health care sector during the 1990s.
