Substitution of amalgam restorations: participative training to standardize criteria

Abstract: considering that the variations on clinical judgment with respect to replacement of restorations are a problem which affects dentistry, the aim of this study was to elaborate, implement and evaluate a programme of participative training to standardize criteria for the evaluation of amalgam restorations. five professors of integrated clinic of the school of dentistry of the university of vale do rio doce (univale), brazil, visually and radiographically evaluated 28 extracted permanent teeth presenting amalgam restorations. the research was developed in four distinct phases: first - the teeth were evaluated according to individual criteria to determine whether restorations should be replaced or not; second - the examiners participated in a training programme with the objective of standardizing the evaluation criteria; third - soon after the training, the teeth were re-evaluated using the adopted standardized criteria; fourth - five months after the training, the procedures on third phase were repeated. for each restoration the examiners registered the main reason for considering the restoration: satisfactory, requiring total substitution, requiring partial substitution or requiring finishing/polishing. after participating in the training programme the examiners presented a statistically significant reduction (sign test: z = 0.4989, p = 0.0022) in indicating the need to substitute restorations, result which was maintained five months after training. this programme of participative training can be organized and implemented to standardize the criteria to evaluate amalgam restorations achieving satisfactory results with an impact on clinical practice.

Abstract: in order to study the dental service inclusion in the family health program of pompéu city, minas gerais, a group of 120 users were interviewed in volta do brejo district. a questionnary was used. the main problems reported were prompt service and resolute dental care. the health communitaries agents and the short distance between the headquarter and the user's residences were positive factors. the "mouth problems" remained without solution as those observed in the health: inequalities in access, no integrated, insufficient workers and financial resources.

Abstract: the aim of this study was to identify the knowledge users of a teaching clinic have regarding basic rights related to the possibilities of dental care. a structured interview was conducted with 56 users (proportion estimate). upon entering the healthcare service, users are far from what could be described as aware citizens. they obtain care through friendship (32.1%) or cases of urgency (28.6%). when they do not obtain care, the most mentioned alternative is a private clinic (42.8%), which indicates that the constitutional right to healthcare is not incorporated by the population of the present study, as they attempt to solve their problems individually and with their own resources. this line of thinking remains (50%) even in cases of urgency (pain), as only four individuals reported they would seek care in the public sector. this underlines the lack of knowledge of the basic right to healthcare and other alternatives that the organized public clinic offers. the characterization that a service of this nature performs favors is confirmed, as 76.8% of the interviewees reported not knowing who finances their treatment and 42.8% would not file complaints despite being dissatisfied with the service. in spite of the explanatory work carried out by the institution, users still do not have information about their rights and citizenship, a factor that prevents political participation. the interviewed community does not display the characteristics of a group that is aware of their rights regarding dental care.

Background: Adolescents who rely on social aid networks tend to be more socially competent and enjoy higher levels of quality of life than those who interact only with their own limited groups of family and friends. In this light, the present study aimed to measure social capital and analyse its possible relation to adolescents’ quality of life. Methods: This is a cross-sectional analytical study with 363 adolescents from the Brazilian NGO. The instruments used included a short Portuguese version of the Integrated Questionnaire to Measure Social Capital (SC-QI) from the World Bank and its abbreviated Portuguese version from the World Health Organization Quality of Life Bref (WHOQoL-Bref) used to measure the quality of life. To measure social capital, the present study chose to use a segmentation analysis, along with the Two-Step Cluster method, applying Euclidian distance and the criteria of centroid aggregation. The Mann-Whitney test was used to compare the scores of WHOQoL-Bref domains among the clusters, at a significance level of 5%. Results: The segmentation analysis resulted in two clusters, with 160 (44.1%) adolescents classified as Low Social Capital (LSC) and 178 (49%) classified as High Social Capital (HSC). Participation in activities for the good of the community (p < 0.001), opinion regarding the differences between people in the community (p = 0.002), and number of community meetings (p < 0.001) constituted the decisive variables for determining the separation of the clusters. When quality of life among the newly formed clusters was compared, the highest average score in the HSC cluster was found in the Psychological Domain (75.4 ± 11.9), whereas in the LSC cluster, the highest average score was found in the Physical Domain (74.6 ± 12.6). Lower quality of life scores was found in the environmental domain for both the LSC clusters (51.3 ± 14.0) and the HSC clusters (54.7 ± 15.5). Abstract: objective: to analyze the perceptions and participation of female basic health unit users with regard to prevention and health promotion. methodological procedures: qualitative study with 20 users of a family health unit in the municipality of belo horizonte, southeastern brazil, in 2007. the interview script included questions about the health-disease process and prevention and health promotion. the content analysis technique was used to analyze the reports. results analysis: the perception of disease prevention was influenced by leavell & clark's theory, which is expressed as actions that avoid the appearance, progression or worsening of disease. health promotion was regarded as a level of prevention and associated with the individual responsibility and the positive concept of health. prevention and health promotion practices were influenced by the positive health concept, the possibility of causing pleasure/displeasure, the way in which they might interfere in daily life, by the concept of will-power and the value attributed to life. conclusions: the discourse about disease prevention and health promotion is marked by traditional concepts. however, the inclusion of the positive health concept, allied to pleasure and will-power, are the main behavior determinants. strategies based on a more comprehensive approach to the health-disease process are needed, thus reflecting the modern principles of health promotion.

Abstract: the interventions that public health brazilian system propose, denominated unique health system (uhs) depend on the value given to primary health care (phc). therefore, human resources involved must be prepared. the aim of this study was to evaluate dimension concepts related to phc by students of eight and nine periods of dentistry department of a brazilian public university, with the participation of a sample of students. in order to study the dimensions, one researcher applied individual questionnaires. the answers were analyzed with biostat 4.0 program. respondents understand broadly “the process health-disease” and that “adequate technologies” are limited to hard and medium technologies, with little mention to the use of soft technologies. with respect to “humanizing health care”, they belief that ethics represent a compromise with human dignity and that solidarity must be understood politically; furthermore, they establish a link between technical experience and responsibility, reflected in giving priority to the person. on the other hand, they do not understand that it is possible a deep study of ethics and solidarity, understanding them more as “values” which cannot be systematized in curricular content.
Determinant factors of toothache in 8- and 9-year-old schoolchildren, Belo Horizonte, MG, Brazil

Abstract: a cross-sectional study was carried out to determine the prevalence, severity and impact of toothache among schoolchildren associated with socio-demographic variables (gender, degree of maternal schooling, economic group, and oral health status). six hundred and one 8- and 9-year-old children were randomly selected from schools in belo horizonte, mg, brazil. after formal authorization was obtained from their parents, the children were interviewed and clinically examined by a single examiner. the chi squared test was applied and the odds ratio obtained. the prevalence of toothache was 45.9% (276/601), of which 15.6% (94/601) had occurred during the previous month. among the children who had experienced pain, 39.4% (109/276) classified its severity as intense or very intense. nearly 35% (96/276) were awoken by the pain, and 63.8% (176/276) were unable to carry out daily tasks as a result. the prevalence of pain was greater among children from less privileged economic groups, in which the mothers' level of schooling was lower (0-7 years of formal study) and who showed poorer conditions of oral health, determined by the presence of dental and periodontal pathology (p<0.05). gender did not influence either the experience of toothache or its severity and impact. the prevalence of toothache found in the age group between 8 and 9 years is very high and associated to social determinants and poorer conditions of oral health.

Abstract: this was a cross-sectional study on periodontal disease in a black community in brazil. the sample consisted of 104 individuals over 13 years of age (63 females and 41 males). all teeth were examined except the third molars. six sites per tooth were analyzed in order to measure pocket depth and clinical loss of attachment. four sites per tooth were used to measure bleeding on probing and presence of calculus. it was observed that bleeding on probing occurred in 97.9% of the total of individuals with teeth, with probing depth > 4mm in 43.3% and clinical attachment loss in 63.5%. calculus was detected in 81.2% of the sample. in the study, individuals 46 to 60 years of age showed the highest mean clinical attachment loss, 8.3mm (± 4.27), and a probing depth of 4.6mm (± 3.5). prevalence rate for periodontal disease in this community was similar to rates elsewhere in brazil and the world, i.e., 9.6% for the severe form. age, schooling, and tobacco use were identified as risk indicators statistically associated with periodontal disease.
