Evaluación de un programa de atención sanitaria con enfermera tutora en cirugía de traumatologia en un hospital de Granada, 2004

Abstract: background: the current trend in patient care is towards a more humanized and higher-quality healthcare. the objective of the present study was to evaluate the results of implementing a programme of healthcare with a reference nurse who welcomes the patient at hospital admission, visits regularly during hospital stay, and resolves doubts and problems. methods: quasi-experimental study (149 cases and 454 non-tutored controls) in patients admitted for scheduled trauma surgery at the virgen de las nieves university hospital, granada. sociodemographic and clinical data were gathered at admission. at 15 days after discharge, patients were administered with an interviewer-administered zung score and anad hoc questionnaire on satisfaction with different healthcare aspects during hospital stay and understanding of information received. bivariate analyses and linear and multivariate logistic regression analyses were performed with a study of confunding variables. results: for the outcome variable anxiety level non-tutored patiens had a b coeficient of 2.64 (p<0,01) in the multivariate linear regression analisys controlling for the other variables in the final model: sex, presence of informal carer, professional activitiy, days of hospital stay and understanding of health information. for the outcome variable inadequate understanding information non-tutored patients showed an odds ratio of 3.48 in the multivariate analysis controlling for educational level and presence of informal carer. satisfaction with he friendliness of health care personnel and with the hospital setting did not significantly diference, although the percentage of dissatisfied patiens was higher in the non-tutored group: 15% vs 11% (p= 0.34) and 18% vs 12 % (p= 0.11) respectively. conclusions: the implemented continued care programme proved effective for these patients, increasing their understanding of information received and reducing anxiety levels.

Abstract: Introducción: La hospitalización afecta a la dinámica de las relaciones familiares y obliga a cambios en la representación de los roles habituales. El papel de los cuidadores familiares adquiere toda su relevancia en la medida que satisfacen las necesidades del enfermo. Esta actividad en la mayoría de los casos es realizada por mujeres. Objetivos: Conocer el perfil y tipo de cuidados que prestan los cuidadores familiares en las unidades de Maxilofacial, Neurología, Neurocirugía y Traumatología del Hospital de Traumatología de Granada. Analizar las necesidades y problemas con los que se encuentran en el hospital. Conocer la opinión del personal de enfermería sobre el cuidador familiar. Dise o: Cualitativo, mediante Observación sistemática, grupos focales (uno con enfermeras y otro con auxiliares), encuestas y entrevistas en profundidad. El análisis de contenido ha sido realizado mediante el soporte informático Atlas/ti, 2.4. Resultados: correspondientes a la categoría de hábitat hospitalario, en relación con las actividades, nivel de información, demandas, etc, de los cuidadores familiares. Conclusiones: El ámbito hospitalario es hostil para el cuidador familiar; es necesario establecer un nuevo marco relacional entre los profesionales y los cuidadores y reconocer su presencia y actividad dentro de la institución sanitaria. Profile and social features concerning family caregivers in a traumatology and rehabilitation hospital

Abstract: Studies on family care giving within hospitals are not frequent. This situation contributes to make these people underconsidered by the health system. The aim of this study is to analyze the profile of family caregivers, the needs they attend and how they are considered by the health professionals in a traumatology and rehabilitation hospital.Methodology: Descriptive and cross-sectional study, which included one hundred interviews performed between September and October 2002 in traumatology, maxillofacial surgery, neurology, and neurosurgery wards. Results: The family caregiver profile corresponds to a female with a median age of 48 years who spends a mean of 20 hours per day in the hospital. Surveillance and company were the main contributions to the patient care according to caregivers’ answers. Conclusions: Wives or mothers become the main family caregivers. Their contribution implies monetary and time costs as well as physic, psychological and social impact. The care offered by the family caregivers is scarcely supported by professional help. Increasing the professionals’ recognition of the family caregivers is a social requirement.

Abstract: More and more information resources exist in electronic format, such as online databases, journals, books, etc., instead of the traditional print format. Advances in information technology have introduced both new capabilities and interesting challenges in the access to medical literature. This has influenced dramatically medical libraries which have changed and will continue to do so. Studies of the impact of the internet on information retrieval behaviour of users in medical environments could provide very valuable information for medical libraries trying to adapt to this rapid evolution. This study aims to explore the real information needs of physicians at the hospital and their medical library use. A review was carried out of literature relating to health libraries in Pubmed, LISA, CINAHL, and Google Scholar. An ad hoc questionnaire was sent to the different hospital services. A systematic sample was made. Results of 118 completed questionnaires collected from 34 different services were analysed. Items regarding: medical information needs; change of information seeking behaviour; use of library services and services offered by the hospital librarian such as quality filtering and users training. Most users are familiar with information technologies such as Internet, online journals, webs, etc., but at the same time traditional systems are used. The assistance of professional medical librarians retrieving new and relevant information from databases and the Internet and teaching skills to retrieve and select information is considered necessary. En los últimos a os se han producido grandes cambios en la forma de acceder a la información en medicina y esto ha incidido profundamente en las bibliotecas médicas y en el modo de organizar sus servicios. Los bibliotecarios médicos han dedicado un especial interés en tratar de adaptarse al cambiante entorno de la información y satisfacer las demandas de sus usuarios. El presente estudio, aparte de realizar una revisión del estado de la cuestión, pretende averiguar las necesidades reales de información de los médicos de un hospital de más de 1.400 camas. Para ello se realizó una amplia revisión de la literatura a través de las bases de datos Pubmed, LISA, CINAHL y Google Scholar con el fin de conocer la actualidad de los sistemas de acceso a la información en Biomedicina. También se confeccionó un cuestionario ad hoc, con el objetivo de recabar datos concretos sobre necesidades y hábitos actuales de información de los médicos del Hospital. Abstract: Introducción: La innovación en la metodología docente universitaria es una exigencia del nuevo Espacio Europeo de Educación Superior. La utilización de las tecnologías de la información y la comunicación es una realidad de la sociedad actual, y especialmente del alumnado universitario. Materiales y métodos: Este artículo muestra la experiencia en el uso de la plataforma virtual de apoyo a la docencia, en la asignatura de Salud y Medio Ambiente en la Universidad de Granada. Resultados y conclusiones: La opinión de los alumnos es muy favorable hacia su uso. Su nivel de satisfacción es muy alto, aunque les supone una dedicación y esfuerzo mayores a los esperados. La plataforma virtual ofrece unas posibilidades académicas que permiten el acercamiento y adaptación a los nuevos perfiles de alumnado. Para el profesorado supone un cambio en sus principales actividades: la preparación y dise o de contenidos, las tutorías virtuales y la gestión de foros. Prevalencia de hipertensión arterial en una comunidad aborigen del norte argentino

Abstract: the prevalence of hypertension among adults of both sexes was assessed as part of a primary medical attention program in an indian wichi-chorote community in santa victoria este, province of salta, argentina. arterial blood pressure was measured after a five minutes rest in a sitting position in adults (over 18 years old) of both sexes with a calibrated sphygmomanometer. a total of 522 adults (318 women and 204 men) were evaluated with median age of 43.5 ± 16.3. the mean systolic blood pressure (sbp) was 125.2 ± 22.7 mm hg, and the mean dyastolic blood pressure (dbp) was 76.3 ± 12.8 mm hg. thirty five percent of the people evaluated had normal pressure values (sbp<80 and dbp<120), 37% prehypertensive (sbp 120-139 or dbp 80-89) and 28% hypertensive (sbp>140 or dbp>90). thirty one percent of the men (mean age: 43.93 ± 17.11, mean sbp: 126.81 ± 22.61 and mean dbp: 77.80 ± 13.33) and 27% of the women studied (mean age: 42.93 ± 16.3, mean sbp: 124.92 ± 24.02 and mean dbp: 75.28 ± 12.57) were hypertense. fifty nine percent of the people over 65 years of age (n:58) (mean age: 73.15 ± 6.36, mean sbp:144.81 ± 28.72 and mean dbp:79.68 ± 13.17) had blood pressure values in the hypertense range. the prevalence of hypertension in the evaluated rural population, suffering extreme demographic and sanitary conditions, is similar to that reported for urbanized societies in the usa but lower than the one reported for other rural areas of our country.

Abstract: diabetes mellitus is of the outmost importance to be one of the most frequent non communicable disease and for the severity of their chronic complications. we carried out a descriptive study in families of twelve medical clinics of the havana city, with objectives of to describe the behavior of the risk factors and consequent illnesses of the atherosclerosis and to determine the relationship among the diabetes with the risk factors and the consequent illnesses. the principal results were: prevalence prevails of patients' passive smoking, sedentary diabetics, with circumference of the increased waist, with family diabetes antecedents and hypertension. the most frequent illness was the cardiovascular one. the circumference of the altered waist, the family diabetes antecedent, the arterial hypertension, heart diseases, the cerebrovascular diseases and the outlying arterial illness presented a high associate risk.

Abstract: La Diabetes Mellitus es de primera importancia en todo el mundo, por ser una de las patologías no transmisibles más frecuentes y por la severidad de sus complicaciones crónicas. Se realizó un estudio transversal en familias de doce consultorios médicos de la Ciudad de La Habana, con los objetivos de describir el comportamiento de los factores de riesgo y las enfermedades consecuentes de la aterosclerosis y determinar la relación entre la diabetes con ambos. Se encontrópredominio de pacientes diabéticos que, a su vez, eran fumadores pasivos, sedentarios, con circunferencia de la cintura aumentada, y con antecedentes de hipertensión y familiares de diabetes. La enfermedad más frecuente fue la cardiovascular. La circunferencia de la cintura alterada, el antecedente familiar de diabetes, la hipertensión arterial, la enfermedad del corazón, el Accidente Vascular Encefálico y la Enfermedad Arterial Periférica presentaron un alto riesgo asociado.
