How to Reduce Unnecessary Invasive Angiograms When Patients Are Initially Evaluated by Coronary Computed Tomography?  [PDF]

Abstract: Purpose:When Coronary Computed Tomography (CCT) detects coronary obstruction, patients are regularly referred to invasive angiogram. With higher sensitivity than specificity, CCT might induce unnecessary angiograms (“false positive CCT”). We sought to determine the patients and CCT findings associated with false positive CCT. Methods: Patients were prospectively referred to CCT with a 64-slice CT scan for suspected CAD. Inclusion criteria were: 1) clinically suspected angina pectoris; or 2) suspected silent ischemia on resting EKG. Exclusion criteria were acute coronary syndrome and non sinus rhythm. Invasive coronary angiogram (ICA) was performed on the basis of CCT findings (stenosis >50%). Analysis was performed on a per patient basis. Results: Out of 702 patients, 228 had suspected significant stenosis by CCT and ICA was performed in 176 patients. Coronary stenosis >50% was not confirmed by ICA in 44 (25%). In multivariate analysis, we observed that atypical angina (OR 3.63,CI 1.43-9.66), silent ischemia (OR 5.11, CI 1.89-14.6) and number of suspected stenosed arteries by CCT (OR 1.81, CI 1.15-2.94) were independently predictive of false positive CCT (p < 0.05). Lesion location and coronary plaque characteristics did not impact on CCT accuracy. Conclusions:Performing CCT for atypical angina or silent ischemia is associated with higher rate of unnecessary invasive coronary angiograms. We failed to identify lesions characteristics prone to be “false positive” of CCT.
Optimization of Critical Systems for Robustness in a Multistate World  [PDF]

Abstract:

Critical systems are typically complex systems that are required to perform reliably over a wide range of scenarios, or multistate world. Seldom does a single system exist that performs best for all plausible scenarios. A robust solution, one that performs relatively well over a wide range of scenarios, is often the preferred choice for reduced risk at an acceptable cost. The alternative with the maximum expected utility may possess vulnerabilities that could be exploited. The best strategy is likely to be a hybrid solution. The von Neumann-Morgenstern Expected Utility Theory (EUT) would never select such a solution because, given its linear functional form, the expected utility of a hybrid solution cannot be greater than that of every constituent alternative. The continuity axiom and the independence axiom are assessed to be unrealistic for the problem of interest. Several well-known decision models are analyzed and demonstrated to be potentially misleading. The linear disappointment model modifies EUT by adding a term proportional to downside risk; however, it does not provide a mathematical basis for determining preferred hybrid solutions. The paper proposes a portfolio allocation model with stochastic optimization as a flexible and transparent method for defining choice problems and determining hybrid solutions for critical systems with desirable properties such as diversification and robustness.

Abstract: The pervasive presence of the internet and the commonality of mobile devices for communication technology have changed modalities for information exchange. Recent developments in information and communication technology (ICT) have specific implications regarding dissemination of information among youth, as exemplified by the Arab spring. The opportunity of those emerging technologies should be seized upon. ICT platforms should be used to scale-up policies and programmes that promote the sexual and reproductive health of youth due to their low cost, increased access to remote populations, better efficiency and improved flexibility for programming. Successful models should be identified through programme evaluation. L’omniprésence de l’internet et la popularité des appareils mobiles pour la technologie de communication ont changé les modalités pour l’échange d’informations. Les développements récents dans la technologie de l’information (TIC) ont des implications spécifiques en matière de diffusion de l’information chez les jeunes, comme en témoigne le printemps arabe. Ces technologies émergentes devraient être utilisées bien plus souvent en vue des possibilités qu’elles offrent. Les plates-formes TIC devraient être utilisées pour intensifier le développement des et des programmes qui favorisent la santé sexuelle et de reproduction des jeunes en raison de leur faible co t, un accès accru aux populations les plus isolées, une meilleure efficacité et une flexibilité améliorée pour la programmation. Les modèles réussis devraient être identifiés à travers l’évaluation des programmes.
Mass Spectrometric Identification of Propionibacterium Isolates Requires Database Enrichment  [PDF]

Abstract: Propionibacterium species are mostly environmental bacteria, some being commensal of mammals including humans, and sometimes pathogenic. These bacteria are poorly identified using routine laboratory methods. Recently, Matrix Assisted Laser Desorption Ionization Time-of-Flight Mass Spectrometry (MALDI-TOF-MS) has emerged as a rapid and efficient method to identify bacterial species. We evaluated the use of MALDI-TOF-MS for identification of all validated Propionibacterium species. Only four of the 15 tested reference strains (26.7%) were correctly identified at the species level, and P. acnes, the most common human pathogenic species was not identified. When applying MALDI-TOF-MS to 48 P. acnes strains, only 18.7% were correctly identified, suggesting an intraspecific variability of proteic profiles among Propionibacterium strains. However, by enriching the Bruker database with spectra from five of these strains and re-testing the other 43 strains against this new database, 93.0% were correctly identified. Our study demonstrates that MALDI-TOF-MS may be used for the identification of Propionibacterium isolates but requires a database enrichment in spectra from additional isolates.
Antisepsis with Argyrol, acrimony and advocacy for African art

Abstract: Despite lack of evidence regarding its antiseptic superiority over silver nitrate for preventing ophthalmia neonatorum, Argyrol was promoted so savvily by Albert Coombs Barnes that the revenues enabled him to amass a unique art collection reflecting his early appreciation of the African influence on European painters. He addressed social disparities specially through access for disadvantaged individuals to his iconoclastic foundation and collaboration with a local African-American university. Legal wrangling over complex management issues and distinctive display arrangements, led to fiscal anguish, cultural torment and local affliction over that trove which is currently relocating to downtown Philadelphia. Albert Coombs Barnes a développé et commercialisé l'Argyrol avec succès malgré l'absence de preuves sur sa supériorité antiseptique, par rapport au nitrate d'argent, pour la prévention de l'ophtalmie du nouveau-né. Les bénéfices de cette commercialisation intelligente de l'Argyrol lui ont permis d'acquérir une collection d'art, unique en son genre, qui démontre de l'influence africaine sur les peintres européens. Il s'est attaqué aux disparités sociales en donnant un accès préferentiel aux personnes défavorisées à sa fondation iconoclaste et en collaborant avec une université afro-américaine locale. Une querelle juridique, à propos de questions complexes de gestion et de la disposition des oeuvres d'art, a conduit à des problèmes financiers, à une polémique dans le milieu culturel et à une dispute de voisinage concernant ce trésor qui est actuellement en cours de déménagement au centre-ville de Philadelphie.

Abstract: This article aims at analyzing the symbolic form which is technology in its relation with myth so as to examine its anthropological and political consequences. Technology, as opening of the possible, overrunning the imitation of nature and “emancipation with respect to organic barriers” turns the descriptive question “what is this” in the normative one, “what could this be?”. Man builds his horizon of objects, creates his world and the intuition of his own essence, by living in the field of possibility opened by technology. Thus, obeying the laws of nature has to be always “re-formed” and “reasserted”: it is changing. Passing from forma formata to forma formans, from having become to the principle of becoming, Cassirer invites us to establish a criticism of technology neither for the purpose of analyzing his works, nor for searching a historical essence. The purpose is to update the aim for the set up, change of sense and aspect of the world in the process of technological exteriorization.
