Abstract: a female patient with a radiographic image compatible with a tumor of the right hemidiaphram that was previously studied and diagnosed as a diaphragm tumor is presented. according to this diagnosis, surgery via thorax was performed and it was found a tumor in the right lobule of the liver that on histic examination proved to be a liver hamartoma, an extremely rare bening liver tumor, which due to its atypical form is considered as a curious case that may enrich the experience of other health professionals

Abstract: the case of a female patient at her sixth decade of life with high digestive symptomatology of 4 years of evolution, characterized by epigastralgia, acidity and vomits, is presented. as the patient did not resolve with medical treatment, she underwent a radiological contrast study of the esophagus, stomach and duodenum, where a mixed hiatal hernia was diagnosed. she was operated on through the abdomen. closing of the right pillar of the esophageal hiatus and nissen i type fundoplication, with which she was definitely cured, were performed. taking into account the magnitude and uncommonness of this variety of hiatal hernia, we consider it is of interest for medical literature

Abstract: introduction. the universalization of the use of bioprostheses and its pioneers lichtenstein, gilbert, rutkow and robbins are being able to overcome what has been the ghost of hernial surgery: recurrences. the techniques based on mesh plug have been evaluated for more than 20 years and are widely accepted by most of the surgeons. methods. 334 patients with inguinal hernia that had undergone rutkow y robbins' hernioplasty from january 2001 to december 2004 were studied. the following variables were analyzed in order to determine the effectiveness of this technique: sex, age, side and variety of hernia, time of hospitalization and anesthesia, prosthetic material used, postoperative complications and recurrences. results. the sample included mostly males with an average age of 56 years old. it was observed a predominance of type 2 and 3 hernias on the right side according to gilbert's classification modified by rutkow y robbins. they were of primary etiology in 82,35 % of the cases. 79.41 % of the patients underwent ambulatory surgery, mostly with spinal anesthesia and the most used prosthetic material was polypropylene. the index of complications was 14.74 % and there have been no recurrences yet. conclusions. the results obtained with rutkow and robbins' technique are encouraging, with low morbidity and without recurrences.

Abstract: INTRODUCCIóN. La universalización del uso de las bioprótesis y sus actuales pioneros Lichtenstein, Gilbert, Rutkow y Robbins, están logrando vencer lo que hasta hoy ha sido el fantasma de la cirugía herniaria: las recurrencias. Las técnicas a base de tapón de malla ya tienen más de 20 a os de evaluación y son ampliamente aceptadas por la mayoría de los cirujanos. MéTODOS. Se estudiaron 34 pacientes afectos de hernia inguinal a los cuales se les realizó hernioplastia de Rutkow y Robbins entre enero del 2001 y diciembre del 2004. Con el objetivo de determinar la efectividad de dicha técnica, se analizaron las variables: edad, sexo, lado y variedad de hernia, tiempo de hospitalización y anestesia, material protésico utilizado, complicaciones posoperatorias y recurrencias. RESULTADOS. La muestra estuvo constituida mayormente por hombres, con un promedio de edad de 56 a os. Predominaron las hernias del lado derecho, de los tipos 2 y 3 según la clasificación de Gilbert modificada por Rutkow y Robbins, de etiología primaria en el 82,35 % de los casos. El 79,41 % de los pacientes se operó de forma ambulatoria, mayormente con anestesia espinal y el material protésico más empleado fue el polipropileno. El índice de complicaciones fue del 14,74 % y hasta la fecha no han existido recurrencias. CONCLUSIONES. Los resultados obtenidos con la técnica de Rutkow y Robbins son alentadores, con baja morbilidad y sin recurrencias. INTRODUCTION. Abstract: Se presenta una paciente en la sexta década de la vida, con sintomatología digestiva alta de 4 a os de evolución, caracterizada por epigastralgia, acidez y vómitos que no resuelve con tratamiento médico y a la cual se le realiza un estudio radiográfico contrastado de esófago, estómago y duodeno, donde se diagnostica una hernia hiatal mixta, la cual se interviene por vía abdominal y se le realiza cierre del pilar derecho del hiato esofágico y funduplicatura tipo Nissen I, con lo cual se le cura definitivamente. Abstract: Se presenta un paciente que acude a consulta por presentar cuadros sincopales con una peque a tumoración en la región lateral izquierda del cuello de más de 6 meses de evolución. Se le realiza ecografía del cuello, arteriografía carotídea y biopsia por aspiración con aguja fina y se llega al diagnóstico preoperatorio de tumor del corpúsculo carotídeo. Abstract: the traumatisms that affect the extremities represent 80% of all the vascular traumatisms approximately. the inadequate handling of these it contributes to fateful consequences as the loss of the life or of the function of the extremity. the vascular lesions for firearm constitute one of the main causes with the appearance of arteriovenous fistula. we present the case of a 36 year old masculine patient with antecedents of having hurt 1 year ago by firearm in the root of the left thigh goes to present increase of volume of the extremity and difficulty to the march with sensation of fatigue and gravity. to the physical exam increase of volume of the limb more marked affection was verified in the root of the thigh, in the area related with the bullet impact thrill is felt and blow holosistólico is auscultated. the echo doppler and the tomography with contrast use evidenced the presence of a arteriovenous fistula. with this diagnosis was carried out surgical exploration and repair by means of veins bond for exclusion and interference of ilio-femoral arterial implant of politetrafluoroethylene (ptfe). the postoperative evolution was satisfactory with regression of all the clinical signs.

Abstract: a case of a patient of the nephrology service is presented that is integrated to the hemodialysis program since five years ago by presenting an irreversible chronic renal failure, not able for transplant, with multiple conventional vascular accesses carried out (direct and prosthetic) that have failed by progressive and generalized decay of the vascular system (arterial and venous), due to the effect of the associated pain to the renal disease and the triggering ones by the therapeutic intervention. because of that was surgically intervened and a vascular access at the level of the right inguinal region was performed with the objective to connect the venous system with the arterial one by means of an artificial prosthesis of polytetrafluoroethylene that was implanted in the form of a loop and in superficial planes to facilitate the access to the same one. the use of prosthetic implants of polytetrafluoroethylene in the surgery of vascular access for hemodialysis is a first election alternative when the conventional methods fail.
