Withanolides: Phytoconstituents with significant pharmacological activities

Abstract: Withanolides are a group of naturally occurring oxygenated ergostane type steroids, having lactone in side chain and 2-en-1-one system in the ring. Withanolides are present in medicinal plants of Solanaceae family. Formulations based on these medicinal plants are widely used in Ayurveda and traditional Chinese medicine. Withanolides have shown a wide range of pharmacological activities including hypno sedative, immunomodulatory, anti-inflammatory, antiarthritic, angiogenesis inhibitor, anticholinesterase, antioxidant, antibacterial and above all, antitumour. Withaferin A is the best studied withanolide as far as pharmacological investigations are concerned. The present review summarises the investigative work carried out on bioactive withanolides.

Abstract: The effort invested in a software project is probably one of the most important and most analyzed variables in recent years in the process of project management. The determination of the value of this variable when initiating software projects allows us to plan adequately any forthcoming activities. As far as estimation and prediction is concerned there is still a number of unsolved problems and errors. To obtain good results it is essential to take into consideration any previous projects. Estimating the effort with a high grade of reliability is a problem which has not yet been solved and even the project manager has to deal with it since the beginning. In this study, performance of M5-Rules Algorithm, single conjunctive rule learner and decision table majority classifier are experimented for modeling of Effort Estimation of Software Projects and performance of developed models is compared with the existing algorithms namely Halstead, Walston-Felix, Bailey-Basili, Doty in terms of MAE and RMSE. The proposed techniques are run in the WEKA environment for building the model structure for software effort and the formulae of existing models are calculated in the MATLAB environment. The performance evaluation criteria are based on MAE and RMSE. The result shows that the M5-Rules have the best performance and can be used for the effort estimation of all types of software projects.

Abstract:

Many variations can be applied to traditional mammoplasty techniques to improve outcomes in certain situations. The purpose of this report was to demonstrate the indications and benefits of mammoplasty autoaugmentation using an extended superomedial pedicle. All patients who underwent transfer of an extended superopedicleto other parts of the breast for autoaugmentation were included. Indications were determined and outcomes were assessed. Forty-eight patients were included in the series. The average follow-up was 2.1 years. Indications were categorized into reconstruction of a partial mastectomy defect (oncoplastic group, n = 18), upper pole volume in the contralateral mastopexy (implant reconstruction group, n = 9), and volume improvement and suspension (massive weight loss group, n = 21). The overall complication rate was 15% (n = 7/48), with a revision rate of 10% (5/48). The extended superomedial pedicle is a reliable and versatile adjunct to regular mastopexy techniques for various indications. It gives us the ability to transfer vascularized tissue from the lower pole to areas that require autoaugmentation.

Balloon Mitral Valvuloplasty in Rheumatic Double Orifice Mitral Valve  [PDF]

Abstract: We are reporting four cases of acquired double orifice mitral valve (DOMV) of rheumatic etiology (rare) presenting as significant mitral stenosis (MS) treated successfully by percutaneous intervention. All four patients are young (3 males, 1 female) who presented with dyspnea of Newyork heart association (NYHA) class II with mean duration of 1.3 years. Typical clinical findings of MS were present in all. Echocardiography confirmed the diagnosis of moderate to severe MS of rheumatic etiology with double orifice of mitral valve of Trowitzsch incomplete bridge variety [1], giving appearance of two equal (like a pair of spectacles in one patient) or unequal size orifices (in three patients) in parasternal short axis view. Color Doppler examination revealed separate jets originating from each orifice, determined severity of the lesion and evaluated the eligibility for balloon mitral valvuloplasty (BMV). BMV was carried out under transeosophagial echo (TEE) guidance (for perfect septal puncture and crossing the separate orifices) using Inoue balloon of appropriate size (in accordance with patient’s height) with intention to break central fibrous strand converting into a single orifice. All four patients underwent successful BMV (clinical & ECHO criteria) after dilating the separate orifices (except case 4) but the fibrous strand could not be broken. Hence, this study shows that good results can be obtained in acquired double orifice mitral valve with significant MS with BMV even without breaking the fibrous strand. Considering rarity of isolated DOMV cases and rheumatic etiology being very rare, this report of four rheumatic acquired DOMV cases with successful BMV done with good result is significant.

Abstract: Let ℬ(H) denote the algebra of operators on a Hilbert space H into itself. Let d=δ or Δ, where δAB:ℬ(H)→ℬ(H) is the generalized derivation δAB(S)=AS−SB and ΔAB:ℬ(H)→ℬ(H) is the elementary operator ΔAB(S)=ASB−S. Given A,B,S∈ℬ(H), we say that the pair (A,B) has the property PF(d(S)) if dAB(S)=0 implies dA∗B∗(S)=0. This paper characterizes operators A,B, and S for which the pair (A,B) has property PF(d(S)), and establishes a relationship between the PF(d(S))-property of the pair (A,B) and the range-kernel orthogonality of the operator dAB.

Abstract: A range-kernal orthogonality property is established for the elementary operators ℰ(X)=∑i=1nAiXBi and ℰ*(X)=∑i=1nAi*XBi*, where A=(A1,A2,…,An) and B=(B1,B2,…,Bn) are n-tuples of mutually commuting scalar operators (in the sense of Dunford) in the algebra B(H) of operators on a Hilbert space H. It is proved that the operator ℰ satisfies Weyl's theorem in the case in which A and B are n-tuples of mutually commuting generalized scalar operators.

Abstract: In this second edition, the 25 chapters are arranged in five general sections. The text is well illustrated and competently organized. The authors come from varied clinical backgrounds and include anesthesiologists, surgeons, and intensivists. Each chapter is well researched and appropriately referenced and deals with all aspects of care of bariatric surgical patients.The authors start with a discussion of specific challenges of the pathophysiology in the bariatric population and then move on to preparation and pre-operative management, intra-operative management, and post-operative care of bariatric surgery patients. The text has minimal repetition and flows very well from one section to the next. New features in this edition include chapters on the pathophysiology of pneumoperitoneum, post-operative rhabdomyolysis, informed consent, and bariatric surgery in adolescents.The chapters on positioning, monitoring, airway management, drug dosing, ventilatory strategies, comorbidities, and post-operative care all include information that would be extremely relevant to the practice of both anesthetists and intensivists. Some chapters (for example, the ones dealing with informed consent, renal dysfunction, and cortical electrical activity monitoring) were informative but did not seem to provide obesity-specific information. Although intensivists and anesthetists are not responsible for operative decisions, the authors could have provided greater detail about the selection of bariatric procedures and their associated outcomes and benefits.The book is an excellent resource for anesthetists, surgeons, and intensivists who wish to develop skills to recognize potential complications and provide quality peri-operative care to obese patients. This book will also be valuable to nursing staff, dieticians, and managers involved in the care of morbidly obese patients and will be a useful addition to the reference collection of institutions planning to develop or improve their bariatr
Audiologic monitoring of multi-drug resistant tuberculosis patients on aminoglycoside treatment with long term follow-up

Abstract: Sixty four patients were put on second line aminoglycoside anti-TB drugs. These were divided into three groups: group I, 34 patients using amikacin, group II, 26 patients using kanamycin and group III, 4 patients using capreomycin.Of these, 18.75% of the patients developed sensorineural hearing loss involving higher frequencies while 6.25% had involvement of speech frequencies also. All patients were seen again approximately one year after aminoglycoside discontinuation and all hearing losses were permanent with no threshold improvement.Aminoglycosides used in MDR-TB patients may result in irreversible hearing loss involving higher frequencies and can become a hearing handicap as speech frequencies are also involved in some of the patients thus underlining the need for regular audiologic evaluation in patients of MDR-TB during the treatment.Tuberculosis is one of the leading infectious diseases in the world and is responsible for more than two million deaths and nine million new cases annually [1]. Emergence of resistance to drugs used to treat tuberculosis and particularly multi-drug resistant (MDR-TB) has become an obstacle to effective global TB control [2]. Incomplete and inadequate treatment is the most important factor leading to its development, suggesting that it is often a man-made problem [3]. Inappropriate treatment results in unacceptably low cure rates and the continued spread of tuberculosis in the community because of selection of M. tuberculosis isolates that are resistant to anti- tubercular drugs [4]. Taking into consideration the high success rate of TB treatment under DOT policy (Directly Observed Treatment), the principal cause for the generation of drug resistant TB generally appears to lie in the low degree of patient compliance with treatment [5].
