Abstract: we evaluated 43 magnetic resonance scans of the leg or thigh of patients suffering from sports trauma. strains were the most frequent lesions observed. these lesions presented iso- or hypointense signal on t1 and hyperintense signal on t2 images, and were classified according to the intensity of the injury of the fibers into grades 1, 2 and 3. the second most common lesions in these series were contusions that appeared iso- or hypointense on t1 and hyperintense on t2 images. fibrosis was also observed as low signal lesions on t1 and t2 images.

Abstract: objective: to describe epidemiological, clinical and magnetic resonance imaging findings of osteonecrosis in the distal femur and proximal tibia. materials and methods: evaluation of 19 patients (12 women and 7 men), with no previous history of causative factors, with magnetic resonance imaging findings suggestive of osteonecrosis in the tibial plateau or femoral condyle. results: osteochondral abnormalities were observed in 63.1% of the cases; in 73.6% of them, such abnormality was associated with ipsilateral meniscal lesion. also, a significant association with bone marrow edema (grade iii in 16 cases) was observed. conclusion: magnetic resonance imaging has demonstrated to be a noninvasive method with good sensitivity in the diagnosis of knee osteonecrosis as well as of associated lesions which are most frequently found in women (63% of cases).

Abstract: we report a case of spontaneous healing of bucket-handle tear of the medial meniscus in a 32-year-old man. the patient twisted his left knee during a soccer game. at clinical examination, there was pain at mobilization and lock of extension at 45°. magnetic resonance imaging showed a bucket-handle tear of medial meniscus and a detached fragment in the intercondylar notch. after conservative treatment there was spontaneous healing of the injury, seen on magnetic resonance imaging.

Abstract: the purpose of this study was to determine the degree of concordance between radiological and radioisotopic methods and, if positive, to evaluate the usefulness of ictal spect in the localization of the epileptogenic focus. ictal brain spect, magnetic resonance imaging (mri) and magnetic resonance spectroscopy (mrs) were performed on six patients with refractory temporal lobe epilepsy. ictal spect was performed after withdrawal of the anti-epileptogenic drugs during video-eeg monitoring, using 99mtc-ecd, administered to patients at the time of the ictus. mri was performed in t1, t2 and flair sequences and mrs was obtained using the press technique, with a single voxel positioned in both hippocampi. the statistical analysis included the determination of the values of kappa (k), standard error (se) and significance level (p) for the lateralization of the ictal focus. the analysis of all findings was based on eeg localization of the ictal discharge, seizure duration (109-280 s; 152 s average) and time of radiotracer injection (30-262 s; 96 s average). we obtained correlated data in four patients (67%), and values of k = 0.67, se = 0.38, and p = 0.041. we concluded that there is a concordance between ictal spect, mri and mrs data and the usefulness of the radioisotopic procedure is related to a nondiagnostic eeg and when there is a discordant or misleading diagnosis after a comparative analysis of eeg and mrs.

Abstract: Neste trabalho é relatado um caso de les o do menisco medial do tipo em al a de balde, que se resolveu espontaneamente. O paciente torceu o joelho esquerdo durante jogo de futebol, evoluindo com dor e bloqueio articular. A ressonancia magnética realizada após o trauma demonstrou uma les o em al a de balde do menisco medial, com fragmento deslocado na incisura intercondilar. Após tratamento clínico, o exame de controle demonstrou que havia ocorrido resolu o espontanea da referida les o.

Abstract: O objetivo deste trabalho foi determinar a existência de concordancia entre os métodos radioisotópico e radiológico e, em caso positivo, avaliar a utilidade do SPECT ictal na determina o do foco epileptogênico. Foram realizados SPECT ictal, ressonancia magnética (RM) e ressonancia magnética com espectroscopia de prótons (RME) em seis pacientes com epilepsia de lobo temporal refratária. O SPECT ictal foi realizado após a retirada das drogas antiepilépticas durante monitoramento por vídeo-EEG, utilizando-se o 99mTc-ECD, administrado aos pacientes no início da crise. As imagens de RM foram obtidas em T1, T2 e FLAIR, com cortes de 3 e 5 mm de espessura, e a RME foi realizada com técnica PRESS, com voxel único posicionado no hipocampo, bilateralmente. A análise estatística incluiu os valores de Kappa (k), erro-padr o (ep) e o nível de significancia (p) para a lateraliza o do foco. Os achados foram analisados com base na localiza o por EEG da descarga ictal, no tempo de dura o da crise (109-280 s; média: 152 s) e no tempo de administra o do tra ador (30-262 s; média: 96 s). Obtivemos dados correlatos em quatro pacientes (67%), com valores de k = 0,67, ep = 0,38 e p = 0,041. Concluímos que existe concordancia entre SPECT ictal, RM e RME, e a utilidade do procedimento radioisotópico está relacionada aos casos em que o EEG n o é diagnóstico e quando há discordancia ou indefini o diagnóstica na análise comparativa entre EEG, RM e RME.
Internal carotid artery stenosis: comparison of duplex scan and magnetic resonance angiography with digital subtraction angiography

Abstract: OBJECTIVE: We compare duplex scan (DS) and magnetic resonance angiography (MRA) with digital angiography (DGA) in respect to accuracy in measuring internal carotid artery (IC) stenosis in symptomatic patients. METHOD: Ten symptomatic patients with IC stenosis greater than 70% previously diagnosed by DS were submitted to another DS and to both MRA and DGA. Both ICs from each patient (total 20 ICs) were evaluated by physicians blinded for the results of other tests. DS and MRA were compared with DGA, using the intraclass correlation coeficient (r) and its 95% confidence interval (95% ci). For each diagnostic test, the study group (20 Ics) was also divided in surgical patients (IC stenosis between 70 and 99%) and non surgical patients, using kappa concordance coeficient (k) to compare the results. RESULTS: Main comparisons are: DS and DGA, r = 0.71 (0.4 - 0.87); MRA and DGA, r = 0.61 (0.25 - 0.82). After division into surgical vs. non-surgical groups, k = 0.857 (p < 0.0001) between DS and DGA; and k = 0.545 (p = 0.003) between MRA and DGA. Most DS and MRA errors occurred in IC sub-occlusions. CONCLUSION: The results suggest that when they are used together, DS and non-contrast MRA may substitute DGA in the evaluation of patients for IC stenosis surgery, except when there is discordance between their results or when the methods show sub-occlusive stenosis.

Abstract: the combination of positron emission tomography (pet) with magnetic resonance imaging (mri) has been the subject of several studies in recent years. positron emission tomography is the most sensitive and specific imaging modality in the detection of metabolic changes, but presents limited spatial resolution. on the other hand, mri presents a significant spatial resolution, besides evaluating soft tissues signal intensity with excellent contrast resolution. the present iconographic essay is aimed at demonstrating the potential clinical application of pet/mri coregistration. the studies were performed in a dedicated pet unit with 18f-fluorodeoxyglucose (fdg) as radiopharmaceutical and coregistered with 1.5 t or 3 t brain mri. the brain images fusion software presents an already well-established accuracy, so a significant synergy between a functional pet study and an excellent mri anatomical detail is achieved. the most attractive clinical applications of this approach are the following: epileptogenic zone assessment in patients refractory to drug therapy, identification of patients with cognitive impairment at higher risk for progression to dementia and differentiation of dementias and parkinsonian syndromes.
