Abstract: it is showed the case of a patient with complete genital prolapse with a heavy uterine component; besides, the woman shows a secondary sexual dysfunction but she wants to preserve the uterus because of the incomplete parity. a cervicosacropexia is carried out with a prolene mesh. because of repairing a paravaginal imperfection a good uterine suspension is achieved. six months after the operation, the patient does not have prolapse. she does not show any sign of her previous dysfunction. the woman is in good sexual condition and plans a future pregnancy

Abstract: Se presenta el caso de una paciente con diagnóstico de prolapso genital completo con marcado componente uterino y disfunción sexual secundaria, quien desea conservación de este órgano por paridad incompleta. Se realiza cervicosacropexia con malla de prolene y reparación de defecto paravaginal logrando buena suspensión uterina. La paciente a los seis meses de operada se encuentra sin prolapso. Abstract: Introduction: Endoscopic retrograde cholangiopan-creatography (ERCP) with endoscopic sphincterotomy (ES) is the most widely used technique for treating choledocholithiasis. In some cases, due to anomalies of the papilla or the presence of large or multiple calculi, additional maneuvers are needed to remove the stones. The present study investigates the efficacy and safety of ES with sphincteroplasty (SP) in the management of choledocholithiasis with extraction difficulties. Patients and Methods: A prospective study was made of 153 patients with choledocholithiasis subjected to ERCP. Fifty-two patients underwent ES with SP, while 101 were subjected only to ES. The two groups were compared in terms of age, gender, percentage cannulation, presence of papilla alterations, large or multiple stones, success in stone removal and complications (acute pancreatitis, bleeding and perforation). In the ES with SP group, we moreover recorded the diameters of the balloons employed (10-18 mm). Results: There were no significant differences between the groups in the stone extraction success rate (94.23% in the ES with SP group versus 97.03% in the ES group) or in the appearance of complications (3.8% in the ES with SP group versus 2.7% in the ES group). The presence of difficult papillae, and of multiple or large stones was significantly greater in the ES with SP group. The diameter of the balloon was not associated with the appearance of complications. Conclusion: Endoscopic sphincterotomy with sphincteroplasty is effective and safe in the treatment of choledocholithiasis with extraction difficulties.
Understanding the Mechanism of the Intramolecular Stetter Reaction. A DFT Study

Abstract: The mechanism of the N-heterocyclic carbene (NHC)-catalyzed intramolecular Stetter reaction of salicylaldehyde 1 to yield chromanone 3 has been theoretically studied at the B3LYP/6-31G** level. This NHC-catalyzed reaction takes place through six elementary steps, which involve: (i) formation of the Breslow intermediate IN2; (ii) an intramolecular Michael-Type addition in IN2 to form the new C-C s bond; and (iii) extrusion of the NHC catalyst from the Michael adduct to yield chromanone 3. Analysis of the relative free energies in toluene indicates that while formation of Breslow intermediate IN2 involves the rate-determining step of the catalytic process, the intramolecular Michael-type addition is the stereoselectivity determining step responsible for the configuration of the stereogenic carbon a to the carbonyl of chromanone 3. An ELF analysis at TSs and intermediates involved in the Michael-type addition allows for the characterization of the electronic changes along the C-C bond-formation.
Microarray-based identification of antigenic variants of foot-and-mouth disease virus: a bioinformatics quality assessment

Abstract: A FMDV microarray was designed and optimized to detect SNPs at a major antigenic site of the virus. A screening of point mutants of the genomic region encoding antigenic site A of FMDV C-S8c1 was achieved. The hybridization pattern of a mutant includes specific positive and negative signals as well as crosshybridization signals, which are of different intensity depending on the thermodynamic stability of each probe-target pair. Moreover, an array bioinformatic classification method was developed to evaluate the hybridization signals. This statistical analysis shows that the procedure allows a very accurate classification per variant genome.A specific approach based on a microarray platform aimed at distinguishing point mutants within an important determinant of antigenicity and host cell tropism, namely the G-H loop of capsid protein VP1, was developed. The procedure is of general applicability as a test for specificity and discriminatory power of microarray-based diagnostic procedures using multiple oligonucleotide probes.The control of diseases associated with highly variable RNA viruses requires close monitoring of the variant virus types that periodically dominate in viral populations. This is due to high mutation rates, quasispecies dynamics and population bottlenecks that often accompany virus transmission [reviewed in [1]]. Indeed, RNA viruses replicate with mutation rates in the range of 10-3 to 10-5 substitutions per nucleotide copied [2,3]. As a consequence, RNA virus populations consist of complex and dynamic distributions of related genomes termed viral quasispecies [4,5]. Viral quasispecies can influence viral pathogenesis [6-8], and the response to antiviral treatments [9]. Abstract: Objective: to determine epidemiological, biochemical, virological, and histological factors associated with liver steatosis in chronic hepatitis C. Subjects: the medical histories of 53 patients biopsied for chronic hepatitis C diagnosis between June 2000 and December 2002 were retrospectively studied. Epidemiological, biochemical, and virological data were collected. Patients with hepatitis B virus or human immunodeficiency virus coinfection were excluded. Liver biopsy specimens were reviewed and scored by one pathologist. Weight and height were measured at liver biopsy time. The statistic association between qualitative and quantitative variables and the presence of liver steatosis was studied. Results: steatosis was identified in 52% of biopsies. There was no statistic association with age, sex, method of transmission, duration of infection, alcohol consumption, other diseases, body mass index, glucose, triglycerides, cholesterol, AST, ALT, GGT, alkaline phosphatase, bilirubin, or viral load. Liver steatosis was associated with serum iron, transferrin saturation, and ferritin. Genotype 3 was also associated with steatosis. Piecemeal necrosis, hepatocellular injury, Kupffer cell hyperplasia, liver iron, and portal fibrosis were also associated with steatosis. A multivariate analysis showed that genotype 3, Kupffer cell hyperplasia, and liver iron were associated with the presence of steatosis. Conclusions: liver steatosis in chronic hepatitis C associates with genotype 3, Kupffer cell hyperplasia, and iron overload. Hepatic steatosis also associates with greater inflammation and fibrosis, and must be considered to contribute to disease progression. Objetivo: determinar los factores epidemiológicos, analíticos, virológicos e histológicos a los que se asocia la esteatosis en la hepatitis C. Pacientes: se revisaron de forma retrospectiva 53 historias clínicas de pacientes biopsiados consecutivamente desde junio de 2000 a dicembre de 2002. Se excluyeron pacientes con otros virus. Se revisaron las biopsias hepáticas de forma protocolizada. Se obtuvieron los datos epidemiológicos, analíticos y virológicos. La talla y el peso de los pacientes se recogieron en el momento de la biopsia hepática. Se estudió la asociación estadística de las variables cualitativas y cuantitativas con la presencia de esteatosis y se realizó un análisis multivariante. Resultados: se identificó esteatosis en el 52% de las biopsias. Abstract: within the teaching and learning context that demands an integral education and knowledge integration, this article presents the results of a pedagogic experience developed with secondary education students from 9th grade in the course of venezuelan geography in maracay, aragua state. from geohistory as a central perspective for analysis, the main objective is to conduct a research on maracay spatial metropolization (1940-2007). an aptitude to set and analyze community problems related to the city, the region and the country is recognized through the educational (language and previous knowledge) and socioeconomic (social reality and responsibility towards it) diagnosis and fieldwork. reality-thinking confrontation, collection and time-spatial interpretation, as well as the use of statements derived from the students? context, needs, problems and interests, are the framework of a pedagogical praxis based on knowing-doing-living. these three aspects guide strategies and procedures in lesson planning to identify geohistoric relations that explain its presence in the specific spatial context. the geographic chart synthesis is an approximation to maracay city behaviour in a specific historic period. its constituting elements are the fabric structure for the diagnosis and the syncronicdiacronic process where interdisciplinary views prevail in the teaching of venezuelan geography through the framework of a social dimension that clearly provides the reality to think, to feel, to do and to be, as stated in article 3 of the education law (1980). the results can be applied outside the school context to organize forums, lectures and workshops in order to democratize knowledge and consolidate the school as the center of community work from a regional and national vision.

Abstract: The main objective of this study was analyze the impact of different interventions programs in the declarative and procedimiental knowledge of student at the Secondary Education measured with a CONTEF questionnaire. Students were divided into two groups, one of them called "experimental" (without any experience in practicing with a reflective methodology) and the other one called "control" (with 3 years of experience in the use of a cooperative methodology). This study was carry out during the school year 2002-2003 and results showed that the "experimental" group obtained better results in the learning of conceptual and procedimiental contents in relation to the "control" group, mainly for the affective and social aspects of these learning. PALABRAS CLAVE: Significant learning, reflective methodology, awareness, knowledge, physical education. Se presenta un estudio preliminar llevado a cabo con 50 escolares de tercero de la ESO divididos en dos grupos: uno "experimental" (sin experiencia en la utilización de una metodología reflexiva donde predomina el proceso de toma de conciencia) y otro "control" (con tres a os de experiencia en el uso de una metodología cooperativa) donde se pretende evaluar el conocimiento adquirido en educación física después de un curso escolar. El objetivo del trabajo se ha centrado en analizar y comparar los tipos de contenidos adquiridos en educación física por ambos grupos al recibir diferentes tipos de intervención. El estudio se llevó a cabo en el curso escolar 2002-2003. Los resultados obtenidos a través del cuestionario CONTEF manifestaron el interés que puede tener el empleo de la metodología reflexiva frente a la metodología cooperativa en el aprendizaje de educación física. El grupo experimental obtuvo mejores puntuaciones en los contenidos conceptuales y procedimentales frente al grupo control, no hallándose diferencias significativas en la vertiente socioafectiva. Abstract: A native strain of Trametes versicolor (Coriolaceae) was isolated and cultivated under laboratory conditions. Mycelial colonies were off-white, showing high density, velvety texture, and abundant aerial hyphae. Substrates studied had good mycelial growth and colonization. Higher mushroom yield of 173.8 g was recorded on supplemented oak sawdust, reaching a biological efficiency of 20.3%. Leathery, dark brown fruit bodies were obtained having normal morphology. A lower biological efficiency of 3.2% was reached on not supplemented oak sawdust.
