Evidence-Based Assessment of Voice Disorders: A Theoretical Overview and Model

Abstract: This article deals with the current paradigm of evidence-based practices of the Speech Therapy (Speech language pathology), especially diagnosing based on evidences of voice disorders. One of the main goals of this article is to define voice disorders according to the World Health Organization's ICF multidimensional concept. Using a comparative method, this study attempts to prove that traditionally, the assessment outcomes of voice disorders in the Speech Therapy have been largely based on the speech therapist's point of view and never on the client's position. The research insists on establishing and adopting definitive gold standards, with respect to voice assessment and therapy in Bulgaria.

Abstract: It is a fact that voice pathology has an expressed frequent appearance in the last decades and the problem of voice disorders and their speech ther-apy assessment is not elaborated enough in the logopedical literature in Bulgaria and Macedonia. One of the basic factors on which treatment effec-tiveness depends is the correct diagnosis from logopedics point of view on which treatment pro-gram could be established.The purpose of the present article is to present a new instrument for a secondary voice assessment-an acoustic analysis of the voice as a part of Com-puterized Speech Lab (CSL) named Multi-Dimen-sional Voice Program (MDVP).There is no developed PhD on voice pathology the last 3 decades in Republic of Bulgaria. In compari-son with language, fluency and articulation disor-ders, voice disorders are considerably ignored and neglected. The speech-language pathologists do not feel very well prepared to work with this kind of pathology.

Abstract: We describe our perception of the link between teaching and research at South West University (SWU) in Blagoevgrad, Bulgaria. This analysis is based on a reflection of the existing curricula and research infrastructure at the Faculty of Public Health and Sports (FPHS) and a literature review of currently explored concepts and definitions connected to linking teaching and research. The research was conducted from April to December 2011 and was financed by the university. On the basis of our review we have proposed a framework for integrating teaching, research, and practice for the FPHS. We describe the key prerequisites for linking research and teaching and its clinical representation in Bachelor and Master's degree programs with the aim of encouraging critical thinking and clinical problem-solving skills in students and teachers. O texto descreve nossa percep o do vínculo entre ensino e pesquisa na South West University (SWU) em Blagoevgrad, Bulgaria. Essa análise é baseada em uma reflex o sobre o currículo e infraestrutura de pesquisa existentes na Faculdade de Saúde Pública e Esportes (Faculty of Public Health and Sports - FPHS) e em uma revis o da literatura referente aos conceitos e defini es atualmente explorados na conex o entre ensino e pesquisa. A pesquisa foi conduzida entre abril e dezembro de 2011, e foi financiada pela universidade. Com base na revis o, foi feita umaa proposta para integra o de ensino, pesquisa e prática na FPHS. S o descritos os pré-requisitos essenciais para vincular ensino e pesquisa e sua representa o clínica em programas de gradua o e pós-gradua o no nível de Mestrado, com o objetivo de incentivar o pensamento crítico e as habilidades de resolu o de problemas clínicos em estudantes e docentes.

Abstract: background: people typically regard stutterers as shy, nervous, introverted, and fearful, a so-called "stuttering stereotype". many stutterers are also subjected to teasing and bullying or to illegal discrimination. aim: currently, there are no widely-accepted, standardized instruments used to measure public attitudes toward stuttering around the world. accordingly, an international project on attitudes toward stuttering (ipats) initiative was launched in 1999 to develop such an instrument, named the public opinion survey of human attributes (posha). method: this study compares selected results from three groups of adults from countries in south america (brazil), eastern europe (bulgaria), and the middle east (turkey). respondents were pooled into three groups according to place-of-residence variable by country and a survey language variable. all of the respondents completed the questionnaire in the primary language of their countries. results and conclusion: this pilot study suggests that some attitude differences among respondents may be explained by an interaction of national, cultural, ethnic, religious, or language differences, although the relative contribution of each component cannot be determined. in a subsequent tier of survey analyses, ratings by these respondents will be compared with the ratings of respondents from other countries completing the survey in english.
Trust and solidarity in Bulgarian cooperatives

Abstract: The paper analyzes the strategies of people in agriculture, especially in the mountain regions where the fragmentation of land is significant. The quantity of land there is also limited. This leads to important consequences for the agrarian development. The culture of keeping the lands of the predecessors burdens the market of agricultural lands. The renting of lands depends on the types of plants to be cultivated as the law for renting lands allows contracts for the max of five years and for the farmers it is insecure to cultivate perennial plants on such lands. In this situation the survival of the small-scale farmers depends on their ability to coop and support each other. The article describes the emergence of a coop for bioproduction of roses, mint and milk in the "Valley of Roses", Karlovo and Gabrovo regions, the solidarity between the farmers, the stimuli and barriers to their activities. It reveals the culture of mutual support based not so much on economic but rather on other incentives such as love to nature and environment, strong desire to prove that bioproduction could develop in the country and that bioproducts are the future for Bulgarian agriculture. It is a different coop in comparison with the socialist collectives. The now established coop unites the farmers for education, qualification, market and mutual support for objectives, but each farmer produces his/her products alone. The farmers sell the products together and each one depends on the culture of others' conscious cultivation of bioproducts. Until now, the experience has been positive and the coop has sustainable development. This is also due to the creation of three professional organizations - the coop as a producing unit, organization for distributing knowledge on organic production and a certifying organization of bioproducts. The survival of the coop depends on farmers’ hard work and solidarity, the strong will to survive, the hope for future significant agricultural support and subsidies when the European Union enlargement involves Bulgaria and the improvement of the economic situation in the country that could bring better buying abilities of the population.
Treatment of Patients with Peripheral Arterial Disease

Abstract: Many risk which factors lead to development of arteriosclerotic arterial disease , on one hand and significant possibilities for development of collateral circulation, on the other hand, confirm the possibilities of prevention and need for timely treatments of patients with peripheral arterial disease (PAD). PAD prevention should start as early as possible, through education o population about healthy nutrition and appropriate physical activities, which is programmed and supervised. In treatment of asymptomatic and mild symptoms cases as well as post re-vascularisation cases, along with drug therapy, physical therapy has an important role: it leads to the subjective improvement, it enhances development of collateral circulation, increases claudication distance. By all these, physical therapy contributes to the limb preservation and to the quality of life. Hyperbaric oxygenotherapy (HBOT ) is important in patients with arterial occlusion followed by critical ischemia when oxygen enters the tissue in only way possible – dissolved in plasma.
Natural Connections on Riemannian Product Manifolds

Abstract: A Riemannian almost product manifold with integrable almost product structure is called a Riemannian product manifold. In the present paper the natural connections on such manifolds are studied, i.e. the linear connections preserving the almost product structure and the Riemannian metric.

Abstract: A Riemannian manifold M with an integrable almost product structure P is called a Riemannian product manifold. Our investigations are on the manifolds (M; P; g) of the largest class of Riemannian product manifolds, which is closed with respect to the group of conformal transformations of the metric g. This class is an analogue of the class of locally conformal Kahler manifolds in almost Hermitian geometry. In the present paper we study a natural connection D on (M; P; g) (i.e. DP = Dg = 0). We find necessary and suffcient conditions the curvature tensor of D to have properties similar to the Kahler tensor in Hermitian geometry. We pay attention to the case when D has a parallel torsion.We establish that the Weyl tensors for the connection D and the Levi-Civita connection coincide as well as the invariance of the curvature tensor of D with respect to the usual conformal transformation. We consider the case when D is a at connection. We construct an example of the considered manifold by a Lie group where D is a at connection with non-parallel torsion.
