Infecciones de transmisión sexual en Colombia: análisis basado en la Encuesta Nacional de Salud - a?o 2007

Abstract: the aim of this study was to analyze the diagnosis of sexually transmitted diseases (std) according to self-reporting by a sample of colombian adult population. a secondary source analysis was carried out using data of the national health survey-2007. the sample consisted of 29,760 persons between ages 18 and 69. at national level, a self-reported diagnosis of std of 0.6% was found; the highest percentages occurred in the pacific and atlantic regions, and the lowest in bogotá. findings showed that women (or=1.7), adults in the 18 24 age range (or=11.9), and population affiliated to the subsidized regime (or=2.1) were more likely to report std diagnosis. social determinants linked to the socio-economic position, poverty, access to health services and the roles of gender must be taken into consideration when analyzing stds in colombia.
Evaluación de las conductas de salud en jóvenes universitarios

Abstract: objective constructing a questionnaire for assessing university students' behaviour regarding health forming part of their life-styles and evaluating its psychometric properties. methods a two-phases validation study was conducted; the first covered 1,485 students (63.2 % female) from four colombian universities having a mean age of 20.65 years (sd=2.5) and the second covered 1,811 students (55.8 % female) from six universities having a mean age of 19.65 years (sd=2.0). a content validity analysis, an item discrimination analysis, a factorial analysis and reliability assessment were done. results the final version of the university students' life-style questionnaire [cevju-r, by its spanish's abbreviations] finally consisted of 40 items related to health and life-style regarding eight factors: physical activity, leisure time, eating habits, alcohol, cigarette and drug use, sleep, interpersonal abilities, coping and perceived emotional state. it also included 28 items evaluating motivation (whether to engage in certain practices regarding health and whether to change them) and the availability of resources facilitating or interfering with such behaviour in seven out of the eight dimensions. factor reliability level was acceptable (ranging from 0.6 to 0.7). conclusions the cevju-r had suitable psychometric properties and constituted an advance in measuring university students' life-styles and could help identify health need and focus health promotion and prevention strategies in this population.

Abstract: adherence to treatment is currently of main concern for hiv/aids control, being associated with treatment success or failure. this systematic review highlights that adherence to treatment is a concept that traditionally refers only to antiretroviral medication intake and attendance at medical appointments. however, these aspects do not reflect the complexity of this topic. there is evidence about the relation between adherence to treatment and other factors that could facilitate or hinder its effectiveness but usually this evidence is not conclusive. in this review, an integral concept of adherence to treatment for hiv/aids infection is proposed, considering several simple and complex competences necessary to carry out the therapeutic regime. additionally, factors with stronger empirical support associated with adherence are identified, including those related to the patient, the health system, the illness and the treatment, as well as social-interpersonal and cultural factors. the aim of this review is to propose new guidelines to evaluate adherence to treatment for hiv/aids infection and factors that are associated to it, in order to allow the design of more effective interventions.

Eficacia de un programa cognitivo conductual para la disminución de los niveles de presión arterial

Abstract: Este artículo presenta la eficacia de un programa de intervención cognitivo conductual para la disminución de los niveles de presión arterial, a partir de un estudio cuasiexperimental con dos grupos intactos -experimental y control-, medidas pre y post intervención y análisis intra e intergrupo. La muestra estuvo conformada por 94 pacientes que asisten a control médico por hipertensión arterial en una institución de salud de la ciudad de Cali, Colombia. Se utilizaron como medidas los niveles de presión arterial sistólica y presión arterial diastólica, la Escala de estrés percibido y el Cuestionario de adherencia al tratamiento para casos de hipertensión arterial. La intervención logró disminuir significativamente los niveles de presión arterial sistólica, mantuvo controlados los niveles de presión arterial diastólica, disminuyó el nivel de estrés percibido e incrementó significativamente la adherencia al tratamiento. Los resultados del estudio respaldan la eficacia de las intervenciones cognitivo conductuales para el manejo de la hipertensión arterial.
Prevalencia de prácticas sexuales de riesgo en población adulta de Colombia Prevalence of risky sexual behaviours in the adult population of Colombia

Abstract: Objetivos Establecer la prevalencia de prácticas sexuales de riesgo en Colombia y su relación con características sociodemográficas en población adulta. Métodos Estudio de fuente secundaria basada en los datos de la Encuesta Nacional de Salud 2007. La muestra estuvo compuesta por 29 760 personas entre 18 y 69 a os, seleccionada con un muestreo probabilístico, por conglomerados, estratificado y polietápico. Resultados Se encontró una alta prevalencia de prácticas sexuales de riesgo en la población colombiana, en cuanto al número de parejas sexuales, uso de preservativo en las relaciones sexuales y consumo de bebidas alcohólicas o drogas durante estas, principalmente en hombres, en solteros, en rango de edad entre 18 y 24 a os, y en personas sin afiliación a seguridad social. Conclusiones El estudio se ala como grupo de mayor riesgo a los hombres jóvenes y resalta la importancia de tener en cuenta en los programas de promoción de la salud sexual y reproductiva aspectos como la influencia de determinantes culturales que privilegian el "machismo" y la promiscuidad, así como la baja percepción de riesgo hacia las ITS/VIH en parejas estables. Conclusions The study revealed that the young men was the most risky group and highlighted the importance of taking cultural determinants such as male chauvinism and promiscuity and the low risk perception for STD/HIV in stable couples into account by the sexual and reproductive promotion programs.

Abstract: this paper presents the results of a study which purpose was to determine the impact of a lifestyle modification program to reduce blood pressure levels and to improve the quality of life in hypertension patients. the method was a quasi-experimental, with transversal design, intragroup analysis and repeated measures (pre-post intervention). the sample was composed of 44 hypertension patients attended at a health care service from cali, colombia. systolic (sbp) and diastolic (dbp) blood pressure levels were registered, and quality of life was assessed through the sf-36 health survey v2. a reduction of sbp was found with statistical significance, while dbp remained the same in controlled levels. with regard to quality of life, the lifestyle modification program had beneficial effects on the mental summary and the role emotional scale. in the physical summary, the physical function and role physical improved significantly. the quality of life results showed some differences between patients with controlled and uncontrolled sbp and dbp levels.

Abstract: The main purpose of this article is to use a critical and ecological approach to read Horacio Quiroga′s text "Los cuentos de la selva" (1918) so to think about, what I have intended to name, ecological discourse. In order to achieve this objective, I will analyze how character’s speech reveals a “jungle communication”. Animals talk but not like human beings do. They use words to talk about native life but they do not talk about important human topics such as soul, thought, and so on. Words are used as an ecological system. Tigers and boas dialogues and toucan expressions have a particular interest: jungle as an event. We can affirm that Quiroga imagines jungle creating characters able to “talk the green” and to “dialogue the habitat”.
El reto de evaluar la adherencia al tratamiento en la hipertensión arterial

Abstract: A pesar de los avances en cuanto a la detección, tratamiento y control de la hipertensión arterial (HTA), ésta sigue constituyendo un importante problema de salud pública a nivel mundial. Su alta prevalencia, la morbimortalidad asociada y el costo de sus complicaciones se alan una urgente necesidad de controlarla. Si bien los tratamientos farmacológicos han demostrado su eficacia, se ha reportado una baja adherencia a éstos en los pacientes hipertensos, lo que impide que tengan el efecto esperado. Para el control de la HTA es necesario, además, que los pacientes se adhieran a las recomendaciones sobre la modificación de sus estilos de vida, lo cual resulta aún más problemático dada la dificultad inherente a los cambios de comportamiento. Conocer las razones por las cuales un paciente no se adhiere al tratamiento resulta esencial, de manera que se puedan dise ar y reorientar las intervenciones que buscan mejorar la adherencia a los tratamientos farmacológicos y nofarmacológicos. Para esto, es necesario medir la adherencia adecuadamente, sin embargo, los instrumentos de medición disponibles actualmente presentan grandes limitaciones. Mejorar las formas de evaluar la adherencia constituye un reto, en salud pública, urgente de atender, en el que la Psicología tiene mucho por aportar. Este artículo presenta una revisión crítica del problema de la adherencia al tratamiento para la HTA y propone lineamientos y alternativas para su adecuada evaluación en Colombia.
