Abstract: Language is the most important part of culture. Because of that language is made up of writing and oral expressions, it implies that these expressions should be analyze in detail. In general, the people express theirselves orally in their daily lives. That’s why, oral expression should be made more effective. The goal of the study is to develop a measuring tool that can measure oral ability reliable and valid. The working group of this research has 342 students who get teaching formation education at Selcuk University of Konya City in Turkey. There are 230 female studens and 112 male students in this group. The students were wanted to evaluate the speak of the academic staff who did in previous lecture. Scale items were prepared by doing literatüre survey and getting comments of consultants. 111 items of the scale were reduced to 24 items after doing reliability and validity analyses and getting advices of consultants. In the scale, there are 20 items that can show positive situaitons and 4 items that show negative situations. The negative items of the scale are being pointed reversibly.To prove the validity of the scale, factor analyses that anatomize and confirm. The validity of scale was made by applying Cronbach alpha, Spearman Brown, Gutmann Split-Half Techniques, corrected item correlations and calculating the t-test values which are related with the 27 percent of up-down difference. After these, it was concluded that this scale is safe and valid. Dil, bir toplumun kültürünün en nemli unsurudur. Drug Resistance Trends and Patterns of Mycobacterium Tuberculosis Isolates from Pulmonary Tuberculosis Patients at a Tertiary Care Hospital in Turkey  [PDF]

Abstract: Background: We aim to determine the proportions and patterns of resistance to first-line drugs: isoniazid (H), rifampicin (R), ethambutol (E) and streptomycin (S) among pulmonary tuberculosis patients. Methods: Strains were obtained from 1584 culture positive pulmonary tuberculosis patients. All specimens were inoculated into Löwenstein-Jensen media (LJ) and TK selective; drug susceptibility tests (DST) were performed for first-line drugs. Results: Multidrug resistant (MDR) were detected in 146 (9.2%) isolates. Three hundred (18.9%) isolates were resistant to H; 220 (13.9%) to R; 168 (10.6%) to S; 137 (8.6%) to E. Any drug resistance was detected in 442 (27.9%) isolates. MDR rate was higher in male patients than females (P = 0.006). MDR rates were different according to the age groups (P = 0.02). The highest rate was in 35 - 44 years and the lowest rate was in 15 - 24 years. Conclusions: We found an association between middle age and male gender and MDR tuberculosis.
Can Serum-Specific IgE/Total IgE Ratio Predict Clinical Response to Allergen-Specific Immunotherapy in Children Monosensitized to House Dust Mite?

Abstract: Background. Allergen-specific immunotherapy (SIT) is one of the important regimens for the treatment of allergic diseases. Predictive tests for the clinical response to SIT are limited. In this study we aimed to evaluate whether specific IgE/total IgE levels can predict clinical improvement in monosensitized patients to house dust mite treated with immunotherapy. Patients and Methods. We analyzed 32 patients who had undergone 2 years of SIT. Serum t-IgE and s-IgE levels, and serum s-IgE/t-IgE ratios were calculated and tested for correlation with clinical response to SIT. Asthma symptom score (ASS), rhinitis symptom score (RSS), pulmonary functions and visual analogue scales (VAS) were evaluated at the beginning and after 2 years. Results. There were 17 boys and 15 girls with the mean age of years. The mean serum house dust mite s-IgE level was .61?kU/L, t-IgE .98?IU/mL, and s-IgE/t-IgE ratio . Before immunotherapy, ASS was , RSS; , VAS; , FEV1 (%); , PEF (%); and after 2 years, these values were determined as , , , , and respectively. s-IgE/t-IgE ratio was correlated with change in RSS ( , ) and VAS ( , ). Conclusion. Although SIT is very effective treatment, all patients do not benefit from treatment. We assumed that s-IgE/t-IgE ratio would be useful to predict the clinical response to SIT. 1. Introduction Allergen-specific immunotherapy (SIT) has been used to treat IgE-mediated rhinoconjunctivitis, asthma, and venom hypersensitivity for almost a century. The clinical efficacy of SIT has been shown in several randomized, prospective, double blind placebo-controlled studies [1–5]. A number of studies have shown that SIT decreases clinical symptoms and improves lung function and quality of life in allergic diseases. SIT is only curative and specific method of treatment and may modify the natural course of the allergic disease [3–5]. Many complex immunologic changes occur during the course of SIT. Immunological changes induced by SIT are encompassing modulation of allergen-specific antibody responses, reduction in recruitment and activation of proinflammatory cells, and changes in the pattern of allergen-specific T-cell responses [6–9]. It has been postulated that regulatory T (Treg) cells can control an established allergic response via distinct mechanisms. IL-10 and TGF-β decrease IgE production and enhance IgG4 and IgA production, respectively [1, 10–12]. Allergen-specific immunotherapy frequently induced a transient initial increase in serum-allergen specific IgE, and a subsequent decrease is observed in the following months or years of SIT [13, 14].

Abstract: Foreign body aspiration is a common problem in childhood which can result in complications with high morbidity and mortality. It may be misdiagnosed because of symptoms such as cough, wheezing or stridor which may also be seen in other disorders. Although it is the most helpful tool in diagnosis, absence of typical history and inadequate clinical and radiologic findings may lead to delayed diagnosis in some cases. These cases show high risk for complications and therefore early diagnosis is important. Prolonged or recurrent respiratory symptoms should alert the physician for foreign body aspiration and it should be particularly interrogated. We present a case of foreign body aspiration in a 10-year-old boy who had been misdiagnosed as asthma and developed cushingoid appearance due to high dose corticosterioids. An Evaluation of the Theses on Social Studies Education in Turkey

Abstract: The aim of the study is to show how the theses prepared in the field of social studies education in Turkey have been distributed according to different criteria from 1990 to the present. Examining the prepared theses in the field of social studies in terms of various aspects, this research is thought to be beneficial in view the overall framework of the theses prepared on this subject. Descriptive method was used in this study. In this context, during the document analysis process, five major steps were followed as (1) access to documents, (2) checking originality, (3) understanding the documents, (4) analyzing the data, and (5) using the data. The research statement has been determined as “How the theses on the social studies education field in Turkey show distribution according to differentcriteria like the dates, the universities, and the research topics.” Accordingly, in the theses prepared on the field of social studies , the sub-problems have been designed according to a) the dates, b) the universities, c) their status of being open to the reader in National Theses Database d) the research topics and how these topics are distributed according to the years.In the studies made, it has been observed that the first thesis prepared in the field of social studies was in 1990, and it has been found out that the number of the theses in the field has reached to 550 master theses and 62 doctoral dissertations to date. In the process of 20-year research, the number of theses increased rapidly by 2006, and both master and doctoral dissertations were mostly prepared in 2007. In the examination made according to the universities, both master theses and doctoral dissertations were mostprepared at the Gazi University. When the total number of theses is examined, the number of master theses was observed to be approximately 9 times more than doctoral dissertation. In a study made according to theses’ status of being open to the reader or not, the majority of doctoral dissertations and master theses are seen to be open to the reader, a great many of them are found to belong to 2006 and after. When the distribution of theses’ topics is examined, it is found that, in the master theses and doctoral dissertationsrespectively curriculum and instructional methods were studied mostly.

Abstract: I examine convertible bond arbitrageurs’ long-run impact on convertible bond issuers’ stock prices. I find a negative relation between arbitrage activity around convertible bond issues and convertible bond issuers’ long-run stock returns. Average three-year holding period return of convertible bond issuers with no-arbitrage activity around their convertible bond issues is two times larger than that of convertible bond issuers with arbitrage activity around their convertible bond issues. Overall, I show that convertible bond arbitrageurs’ price impact is not limited to short-term [1], but it also has a long-term component.

Abstract: This paper discusses the validity of the urban interface dichotomy in the urban planning and scientific literature by means of a new approach and proposes a new model. Although the concept of “Urban Interface” (UI) is discussed widely in the context of urban-rural and/or natural-artificial environment, UI has been used in the computer and communication disciplines, as well as in economy, geography, regional planning, city planning and architecture. UI has also been used in relation to electronic traffic control systems in urban transportation. UI between buildings and city is a relatively new issue in the literature. UI is defined with variant paradigms since the phenomenon is uncertain. This article reports on the use of fuzzy logic to create a kind of expert system for evaluating architectural elements in the context of UI. Fuzzy is the mathematical models of verbal expression on the area of specialization. This study is useful because this study shows a way to transform non-intuitive and precise concepts to the measurable and accurate results into a model. In order to create a model, the definition of UI in literature is evaluated and according to this evaluation a definition in terms of architecture is determined. The model of this new definition is created with using fuzzy logic using the Fuzzy Tec software. The predictions and experiences of a designer are represented in the model, which has been designed to behave like an expert person. Therefore, it has been shown that the definition of “expert system” can be added to architectural software. Lastly, a model that can give a fuzzy logic system for determining the quality of Urban Interface with sixteen parameters has been presented to the scientific field.
The validity and reliability of Kogan’s attitude towards old people scale in the Turkish society  [PDF]

Abstract: This study was conducted methodologically in order to determine the validity and reliability of Kogan’s Attitude towards Old People Scale after being adapted to the Turkish Society. 145 female students, attending the 4th and 5th year of the faculty of Medical Sciences at Atatürk University, and 118 nurses, working at Ataturk University Aziziye-Yakutiye Research Hospitals, were in-cluded in the study. The sample group was chosen from the population using a non-probability convenience sampling method. A form containing demographic characteristics of nurses and nursing students, and the Turkish version of Kogan’s Attitude towards Old People Scale were used to gather the required data. Data was analysed using the t-test, Cronbach’s alpha coefficient, correlation, and factor analyses. In the study, Cronbach’s Alpha Reliability Coefficient for Kogan’s Attitude towards Old People Scale was 0.81, and the scale comprised of one factor. The attitude of working nurses towards old people was more positive in comparison to the attitude of student nurses. After analysis, the scale was deemed highly valid and reliable, and suitable to be used for Turkish society. The scale may be used in studies to evaluative attitude toward old people and quality of the care offered to older people.
The Adomian Decomposition Method and the Differential Transform Method for Numerical Solution of Multi-Pantograph Delay Differential Equations  [PDF]

Abstract: In this paper, the Adomian Decomposition Method (ADM) and the Differential Transform Method (DTM) are applied to solve the multi-pantograph delay equations. The sufficient conditions are given to assure the convergence of these methods. Several examples are presented to demonstrate the efficiency and reliability of the ADM and the DTM; numerical results are discussed, compared with exact solution. The results of the ADM and the DTM show its better performance than others. These methods give the desired accurate results only in a few terms and in a series form of the solution. The approach is simple and effective. These methods are used to solve many linear and nonlinear problems and reduce the size of computational work.
The Value of Erythrocyte Indices and Red Cell Volume Distribution Width in Differential Diagnosis of Iron Deficiency Anemia and Anemia of Chronic Disease

Abstract: Iron deficiency anemia (IDA) and anemia of chronic disease are the most commonly confused anemias. We investigate the diagnostic value of erythrocyte indices, red cell volume distribution width, and serum ferritin levels to make differential diagnosis of anemia in controls and anemic patients.Iron deficiency anemia (44 patients), anemia of chronic disease (41), IDA with anemia of chronic disease (17) and control (50) groups were compared. We performed serum ferritin, CBC, and sedimentation rate in all patientsand bone marrow aspiration in patients with anemia of chronic disease.Although mean cell volume (MCV) and mean corpuscular hemoglobin (MCH) are low in IDA and anemia of chronic disease, it is much striking in the former one (p<0.001, p<0.001). Only 7.3% of patients with anemia of chronic disease had a MCV<70 fL and MCH<24 pg, 90.0% of patients with IDA were below that cut-off point. Serum ferritin means were in patients with IDA and anemia of chronic disease were 4.6±3.3 and 489.6 ±519.9 ng/ml, respectively.The probability of IDA is low when RDW is normal in microcytic anemias. RDW is high in half of patients with anemia of chronic disease. Reference values for ferritin must be changed in patients with anemia of chronic disease and IDA. If serum ferritin is 57.6-146.4 ng/ml anemia of chronic disease and iron deficiency must ruled out by other diagnostic tests.
