Analysis of Autonomic, Respiratory and Motor Function of Infants in Pre- and Post-Liver Transplantation  [PDF]

Abstract:
Purpose: Children with liver impairment are likely to develop changes in autonomic nervous function and delay in motor development. The assessment and identification of these dysfunctions may allow an appropriate physiotherapeutic care. Method: Cross sectional study of 18 infants, 11 controls and 7 infants (post-liver transplantation) with an average age of 10 ± 4.5 months, was evaluated in pre- and post-liver transplant. All infants underwent to assessments of motor skills, body composition, chest and abdominal motion, and cardiac autonomic modulation was measured by heart rate variability. Results: Motor delay and malnutrition were found in all infants. The gravity index (PELD)—pediatric end-stage liver disease—showed a negative correlation with the Alberta Infant Motor Scale (r = 0.83, p = 0.01). In addition, reduced parasympathetic modulation was demonstrated by the rMSSD, SD1 and ApEn, pre- and post-transplant. Conclusion: Infants with liver disease, even after transplantation, have delay in motor development, as well as changes in their nutritional and autonomic dysfunction.

Abstract: Atualmente, expandem-se as áreas verdes nas cidades com a prolifera o dos gramados em diversas áreas (condomínios residenciais, campos de futebol, campos de golfe etc.). Os recursos hídricos s o limitados e nem sempre suficientes para atender a todos os usuários simultaneamente. Portanto, torna-se necessária a utiliza o racional da água para irriga o. Neste trabalho, objetivou-se avaliar o efeito da aplica o das laminas de águacorrespondentes a 40, 60, 80 e 100% da ETo sobre a produ o de matéria seca (MS) da grama-batatais, visando obter a lamina mínima de água que preserve a qualidade visual, com economia de água e energia. A aparência do gramado foi avaliada por meio da aplica o deum questionário a 16 avaliadores. Após cada corte, avaliou-se a produ o de MS, determinando a eficiência do uso da água (EUA). Os modelos foram escolhidos utilizandose o teste t, com nível de até 15% de probabilidade no coeficiente de determina o (R2). Concluiu-se que a produ o de MS e a qualidade visual da grama-batatais n o foramafetadas pelas diferentes laminas de água aplicadas nos meses mais frios. Para a gramabatatais, recomenda-se a lamina de 40% da ETo no mês de julho; nos outros meses, recomenda-se a lamina de 80% da ETo. Green areas have been expanding in cities with the proliferation of green areas at many sites (residential condominium gardens, soccer fields, golf courses etc.). Unfortunately, the water resources are limited and are not enough to attend all users simultaneously. Therefore, the rational use of water becomes necessary, mainly for irrigation purposes. This work had the main objective of evaluating the effect of water depths application, according to different reposition of the grass (Paspalum notatum) reference evapotranspiration (ETo) (40, 60, 80 and 100%), on grass dry matter production, aiming to obtain the minimum water depth necessary to maintain the visual quality of grass with water and energy savings. After each grass cutting, dry matter production was evaluated, allowing the determination of water use efficiency (WUE). Grass visual quality was evaluated by means of applying a questionnaire, using 16 evaluates. The models were chosen using the t-test, with level of probability up to 15% in the determination coefficient(R2). The results indicate that the dry matter production and the visual quality of the grass were not affected by the different water depths applied in the coldest months (July and August). A water depth of 40% ETo was recommended in July. For the other months, the water depth of 80% of ETo was recommended.

Abstract:
The creativity is expressed in all individuals, but it manifests itself in different proportions and it may be carried out at different levels. Development levels of this attribute depend on the teacher’s mediation in the process of learning. Creativity can be a challenge for most teachers, since they are unaware of its importance and unwittingly they inhibit by teaching methods that are not aimed at instigating the potential of students. Thus, this article aims to highlight the importance of teacher training for the development of gifted creativity. In conclusion, the mediation of teachers makes the difference in gifted education. As Vygotsky explains, it is precisely through the mediation that it is possible to develop the creative potential. In this sense, the training of the teaching staff reflects in how the mediation will take place during the process of teaching and learning. Therefore, it is essential that teachers are prepared to create a stimulating environment of potential and talents, as well as in performing a work with creativity of their students.

Abstract: we report a case of a 23-year-old female patient, brazilian, resident of germany, who presented with a bilateral chronic granulomatous conjunctivitis, without lymphoadenopathy and irresponsive to topical treatment. laboratorial work-up confirmed bartonella henselae as the etiologic agent. the case shows that the absence of lymphoadenopathy does not exclude the diagnosis of cat-scratch disease.
Are cutaneous hypersensitivity tests to inhalant allergens a severity marker for vernal keratoconjunctivitis?

Abstract: purpose: the purpose of this study was to analyze the cutaneous sensitivity to a variety of allergens in patients with vernal keratoconjunctivitis (vkc) and to demonstrate the relation between skin response and clinical aspects of the disease. methods: twenty patients with vernal keratoconjunctivitis were randomly chosen from the external disease and cornea sector. they were clinically evaluated, and a score ranging from 0 to 20 was applied based on signs and symptoms on ophthalmic examination. all subjects underwent a skin prick test against standardized allergens, such as house dust mites d. pteronyssinus, d. farinae, and blomia tropicalis, as well as allergens from cat, dog, fungi and feather. results: seventy-five per cent of patients were positive for at least one of the allergens tested. house dust mites were responsible for the majority of the cases (75%). there was a poor correlation between the clinical score and sensitivity to allergens (r= - 0.119 for fungi; r= - 0.174 for dog; r= - 0.243 for house dust mites; r= - 0.090 for feather). a significant correlation was found only for cat allergen extract (r = - 0.510; p=0.024). conclusions: our study demonstrated poor correlation between cutaneous hypersensitivity tests and clinical findings in patients with vernal keratoconjuntivitis. we concluded that skin response to inhalant allergens is not a useful test to identify clinical severity and chronicity of inflammatory process in this disease.

Abstract: Relatamos caso de paciente do sexo feminino, brasileira, 23 anos, residente na Alemanha, que cursou com quadro de conjuntivite granulomatosa bilateral cr nica, sem acometimento ganglionar, n o responsiva a tratamento tópico. A pesquisa laboratorial confirmou diagnóstico de conjuntivite por Bartonella henselae. O caso demonstra que a ausência de acometimento ganglionar n o exclui o diagnóstico de doen a da arranhadura do gato.

Abstract: the sustainable use of natural resources by local populations must be based on an understanding of the relationships between humans and their environment. in this context, the present study analyzed aspects of the interactions between human populations and the biodiversity found in the genipabu environmental protection area (apa-genipabu), coast of the state of rio grande do norte, brazil. a total of 60 open interviews were carried out in three communities established within the apa-genipabu, with questions designed to inquire aspects of their general perception of the area and their knowledge and use of the local biodiversity. approximately 82% of the residents do not fully understand the concepts behind creating a conservation area. the interviewees identified 97 plants species (belonging to 48 families) and 172 animals (131 vertebrates and 41 invertebrates), including three animal species considered endangered in brazil. useful species were most often cited. among the positive perceptions of the apa-genipabu, interviewees cited the importance of preserving the sand dunes, animals, plants and lakes, and maintaining the natural landscape to attract tourism. in order to improve conservation strategies in the protection area it will be important to build on the positive aspects of the reserve that were cited by the local population and provide these people with more information about its conservation goals.

Abstract: objective: to analyze and compare the mutations in two different regions of the katg gene, which is responsible for isoniazid (inh) resistance. methods: we analyzed 97 multidrug-resistant mycobacterium tuberculosis strains isolated in cultures of sputum samples obtained from the professor hélio fraga referral center, in brasília, brazil. another 6 inh-sensitive strains did not present mutations and were included as controls. we used pcr to amplify two regions of the katg gene (genbank accession no. u06258)-region 1, (from codon 1 to codon 119) and region 2 (from codon 267 to codon 504)-which were then sequenced in order to identify mutations. results: seven strains were resistant to inh and did not contain mutations in either region. thirty strains carried mutations in region 1, which was characterized by a high number of deletions, especially at codon 4 (24 strains). region 2 carried 83 point mutations, especially at codon 315, and there was a serine-to-threonine (agc-to-acc) substitution in 73 of those cases. the analysis of region 2 allowed inh resistance to be diagnosed in 81.4% of the strains. nine strains had mutations exclusively in region 1, which allowed the proportion of inh-resistant strains identified to be increased to 90.6%. conclusions: the number of mutations at codon 315 was high, which is consistent with cases described in brazil and in other countries, and the analysis of region 1 resulted in a 9.2% increase in the rate at which mutations were identified.
Evolu??o clínica de um grupo de pacientes com TB multirresistente atendidos em um centro de referência na cidade do Rio de Janeiro

Abstract: objective: to analyze the clinical characteristics and evolution of a group of patients with positive sputum cultures for multidrug-resistant (mdr) mycobacterium tuberculosis and treated at a referral center in the city of rio de janeiro, brazil. methods: based on the positive results in sputum cultures for mdr m. tuberculosis, 50 patients were selected, and their clinical data were obtained from the brazilian ministry of health mdr-tb database. the frequencies of noncompliance, relapses, failures and previous treatments for tb up to diagnosis of mdr-tb were compiled. the radiological patterns were classified as unilateral or bilateral, and with or without cavitation. two years after the end of the standard treatment for mdr-tb, the outcome (cure, failure, noncompliance or death) for each patient was evaluated and reassessed every two years. the post-treatment follow-up period was eight years. results: the mean number of previous treatments was 2.3 ± 0.9. the mean interval between the initial diagnosis and the development of mdr-tb was 2.0 ± 1.7 years. two years after the initial treatment for mdr-tb, 2 patients had abandoned treatment, 8 had died, 18 had been cured, and 22 had presented treatment failure. the bivariate analysis showed that bilateral pulmonary involvement and cavitary pattern markedly reduced the chances for cure, with a relative risk of 1-0.6 (40%) and 1-0.7 (30%), respectively. at the end of the follow-up period, 2 patients had abandoned treatment, 9 had presented treatment failure, 17 had been cured, and 22 had died. conclusions: bilateral pulmonary involvement and cavity pattern greatly reduced the chances for cure of the patients with mdr-tb. most patients who presented treatment failure died within the 8-year follow-up period.
