Lability of IgE Levels Early in Life

Abstract: We report a case of a very fast and intriguing decrease in IgE concentrations after exclusion from the diet of any CM lysate in an unusual clinical presentation of cow's milk allergy in an infant. Analysis of IgE kinetics after allergen elimination suggests rapid cessation of IgE biosynthesis and a short IgE half-life. 1. Introduction An unusual clinical presentation of cow's milk allergy (CMA) in an infant gave us the opportunity to observe a very fast and intriguing decrease of IgE concentrations after allergen withdrawal, raising new questions about IgE production and metabolism in newborns and infants. 2. Case Report A 17-day-old neonate, born to atopic parents at full term, was hospitalized in the children hospital of Tours (France) for poor feeding and increasing diarrhoea for 4 days, associated with a severe metabolic acidosis, after a symptom-free interval of 2 weeks. The diarrhoea ceased soon after admission. Erythema and a pustular rash of the face as well as a “gloves and socks-” type skin rash were noted at 18 days of age, leading to the finding of very high concentrations of total IgE (1298?kIU/L) and IgE specific to cow’s milk (CM) (83?kAU/L, Phadia) (Figure 1). First-stage formula milk had been introduced on day 11 in addition to breastfeeding according to the mother, although it is possible that this had been given since birth. Although the clinical presentation was very unusual and severe [1, 2], the diagnosis of CMA was confirmed by a rapid regression of all symptoms after withdrawal of CM proteins. Seven days later, total IgE fell to 121?kIU/L and specific IgE to 1.44?kAU/L, and total IgE returned to within the normal range (10.5?kIU/L) (Figure 1) five days later. Discussion The IgE detected in the newborn had been self-produced since IgE did not cross the placenta and his mother had no CM-specific IgE (not shown). This IgE immune response thus probably reflected a rapid maturation of IgE+ B cells into plasmablasts, as recently evidenced in mice [3]. No less spectacular was the 100-fold decrease in IgE concentrations 12 days later (Figure 1), following exclusion of any CM lysate from the diet. High levels of both specific and total IgE are regularly observed in infant cow’s milk allergy [4]
Infliximab in ankylosing spondylitis: alone or in combination with methotrexate? A pharmacokinetic comparative study

Abstract: Patients with AS patients who had predominantly axial symptoms were randomised to receive infliximab alone (infusions of 5 mg/kg at weeks 0, 2, 6, 12 and 18) or infliximab combined with MTX (10 mg/week). Infliximab concentrations were measured before and 2 hours after each infusion and at 1, 3, 4, 5, 8, 10, 14 and 18 weeks. We estimated individual cumulative area under the concentration versus time curves (AUC) for infliximab concentration between baseline and week 18 (AUC0-18). Clinical and laboratory evaluations were performed at each visit. The Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) score was the primary end point for clinical response.Twenty-six patients were included (infliximab group: n = 12, infliximab + MTX group: n = 14), and 507 serum samples were available for measurement of infliximab concentration. The two groups did not differ with regard to AUC0-18 or evolution of BASDAI scores and biomarkers of inflammation.The combination of MTX and infliximab does not increase the exposure to infliximab over infliximab alone in patients with AS.ClinicalTrials.gov: NCT00507403Infliximab, a chimeric monoclonal antibody to TNF-α, showed efficacy for ankylosing spondylitis (AS) in a randomised, placebo-controlled trial in which 61.2% of the patients were responders at 24 weeks [1]. Although methotrexate (MTX) is often used for patients with predominantly peripheral AS and those with psoriatic arthritis, the few attempts to treat predominantly axial disease were disappointing. Haibel et al. [2] studied 20 patients with AS who received MTX 15 to 20 mg/week subcutaneously and found no difference in Assessment in Ankylosing Spondylitis 20% improvement criteria (ASAS 20) scores before and 16 weeks after treatment. Until now, MTX has been evaluated in only three small, randomised, controlled trials [3-5], and a Cochrane review [6] concluded that there was insufficient evidence to support the use of MTX for AS with predominantly axial symptoms.Data compar

Abstract: All patients with rheumatoid arthritis (RA) or spondyloarthritis (SpA) receiving infliximab beginning in December 2005 were retrospectively followed until January 2009 or until infliximab discontinuation. Trough serum infliximab and ATI concentrations were measured at each visit. The patients were separated into two groups: ATIpos if ATI were detected at least once during the follow-up period and ATIneg otherwise. Repeated measures analysis of variance was used to study the association of infliximab concentration at treatment initiation and the development of ATI. Maintenance of infliximab in the two groups was studied by using Kaplan-Meier curves.We included 108 patients: 17 with RA and 91 with SpA. ATI were detected in 21 patients (19%). The median time to ATI detection after initiation of infliximab was 3.7 months (1.7 to 26.0 months). For both RA and SpA patients, trough infliximab concentration during the initiation period was significantly lower for ATIpos than ATIneg patients. RA patients showed maintenance of infliximab at a median of 19.5 months (5.0 to 31.0 months) and 12.0 months (2.0 to 24.0 months) for ATIneg and ATIpos groups, respectively (P = 0.08). SpA patients showed infliximab maintenance at a median of 16.0 months (3.0 to 34.0 months) and 9.5 months (3.0 to 39.0 months) for ATIneg and ATIpos groups, respectively (P = 0.20). Among SpA patients, those who were being treated concomitantly with methotrexate had a lower risk of developing ATI than patients not taking methotrexate (0 of 14 patients (0%) vs. 25 of 77 patients (32%); P = 0.03).High concentrations of infliximab during treatment initiation reduce the development of ATI, and the absence of ATI may be associated with prolonged maintenance of infliximab. Abstract: The most general dynamic tendencies of the energy redistribution in the high-pressure region are considered. Their influence on the possible deviations from the kinetic conceptions, which is now generally accepted, is examined. In this way, the structural elements of an organic molecule that promote internal energy mobilization in the high-pressure region and, conversely, hamper it, are defined. The first of these elements reduces both the Arrhenius parameters of the unimolecular reactions while the second leads to the opposite results. Some well-known exceptions to existing kinetic theories, which find an explanation in the framework of these proposed concepts, is considered. The proposed concept is very general as distinct from the existing dynamic studies, which investigate more particular details of the separate bond behaviors. The proposed general concept can broaden the study of chemical kinetics.

Abstract: This paper reports on a pilot study that investigates the widely reported issue of underachievement of students from Culturally Linguistically and Economically Diverse (CLED) backgrounds. It involves 15 university education faculty student volunteers and over 40 students and their families in primary (elementary) schools situated in disadvantaged communities of Victoria whose students come from 40 different nationalities, speaking 36 languages and with 75 per cent of its student cohort coming from Non English Speaking Backgrounds. A partnership was formed to focus on the problem of CLED children’s disengagement from their academic learning. We focus on how a productive partnership between schools and a university impact on inclusive teaching and learning practices both at the school and the university level. We investigate whether such an intervention can have an impact on engagement levels and the learning and social outcomes of students from refugee, migrant and working class families. Privileging participant voice we analyze data from interviews, surveys and focus groups with students, teachers and parents to argue that such a program has the capacity to re-engage underachieving students at a minimal cost to the community as an alternative model to other expensive and unsuccessful intervention programs. We conclude that at the core of this successful program is the need for both participants to feel they are empowered in the process. We know that student outcomes can be enhanced when the students feel connected to and involved in their community. Through this project, the students have the opportunity to experience greater community engagement leading to improved school attendance and retention, as well as better academic outcomes.

Abstract: The variety of definitions of Fourier transforms can create confusion for practical applications. This paper examines the choice of formulas for Fourier transforms and determines the appropriate choices for geoscience applications. One set of Discrete Fourier Transforms can be defined that approximate Fourier integrals and provide transforms between sampled continuous functions in both domains. For applications involving transforms between a continuous function and a discrete function a second set of Discrete Fourier Transforms is needed with different scaling factors. Two classes of application are presented: those where either form of transforms can be used and those where it is necessary to use a particular transform to obtain the correct results.

Abstract: In the literature on price regulation, the price-cap mechanism is seen as a very powerful incentive mechanism towards efficiency improvements. What about quality investments? The empirical literature is not univocal: Some studies suggest a deterioration of quality, while others do not find any statistically significant impact. We analyze the incentive provided by price-cap regulation in a setting in which the investment decisions of the regulated firm suffer from hold-up, and contacts are incomplete. We show that the incentives to invest in cost-saving innovations can be fostered by a price-cap contract with a “sufficient” regulatory lag, while for other types of investments, such as quality enhancement, the same contract does not help. Furthermore, we show that if the firm faces a binding resource constraint the price-cap contract generates a crowding-out effect between the two types of investment. This might explain the non univocal empirical evidence.

IMPEX: An Approach to Analyze Source Code Changes on Software Run Behavior  [PDF]

The development of software nowadays is getting more complex due to the need to use software programs to accomplish more elaborated tasks. Developers may have a hard time knowing what could happen to the software when making changes. To support the developer in reducing the uncertainty of the impact on the software run behavior due to changes in the source code, this paper presents a tool called IMPEX which analyzes the differences in the source code and differences on the run behavior of two subsequent software versions, in the entire repository, demonstrating to the developer the impact that a change in the source code has had on the software run, over the whole software history. This impact helps the developers in knowing what is affected during execution due to their changes in the source code. This study verifies that the software runs that are most impacted by a given change in the source code, have higher chances in being impacted in the future whenever this part of the code is changed again. The approach taken in this paper was able to precisely predict what would be impacted on the software execution when a change in the source code was made in 70% of the cases.
