Abstract: Nos últimos 10 anos, os consumidores têm passado por mudan as em seus desejos e atitudes sendo que estas posturas, faz com que as organiza es voltem sua aten o para a perspectiva do consumidor, reconfigurando os seus papéis e responsabilidades na cadeia de valor. A presente pesquisa tem como objetivo analisar as intera es existentes entre os stakeholders do setor da saúde no processo de cocria o de valor. Como referencial teórico utilizou-se a cocria o de valor (MAGRETTA; STONE, 2002, SRIVASTAVA; SHERVANI; FAHEY, 1998, VARGO; LUSCH, 2008). A pesquisa caracteriza-se como exploratória e descritiva, desenvolvida por meio de um estudo de caso múltiplo. Coletaram-se dados secundários (relatórios disponibilizados pelas empresas) e primários (entrevistas semiestruturada com executivos da alta administra o e da gerência operacional). Conclui-se que as intera es ocorrem, principalmente, entre as operadoras, as empresas contratantes, as corretoras e os prestadores de servi os, através de a es que acontecem antes, durante e depois da utiliza o do benefício pelo usuário final.
In-vitro analysis of the microbicidal activity of 6 contact lens care solutions

Abstract: Background Contact lens-related infections are often associated with inadequate contact lens hygiene, and therefore, contact lens care products should be able to sufficiently minimise the amount of pathogens that are responsible for these infections. In 2001, the EN ISO 14729 was introduced to ensure adequate disinfection efficacy of contact lens care solutions, but this norm has recently been criticised. Methods In this study, six frequently used contact lens care solutions were retested according to the Stand Alone Test of the EN ISO 14729 (2001). The Stand Alone Test is a quantitative suspension test. In addition, the products were tested in a modified setting adding an organic load. The load was a mixture of human blood serum, lysozyme, and mucine, which resembles tear fluid. Results The criteria of the Stand Alone Test recommended in EN ISO 14729 were only met by Aosept Plus. This 3% hydrogen-peroxide-based contact lens care solution attained a reduction factor of > 5 log units for bacteria and > 4 for fungi in all cases. Two further contact lens care solutions, Blue Vision and Optifree Replenish, met the criteria of a reduction factor of > 3 log units for bacteria and > 1 log unit for fungi, but only in the presence of artificial tear fluid. The three remaining products did not exhibit adequate disinfecting efficacy, at least against one of the tested microorganisms. Conclusions Through the observation that the artificial tear fluid used in this study influences the disinfecting efficacy of contact lens care solutions, especially that of multi-purpose solutions, in a different way than does albumin, mucine, or even the organic load suggested in EN ISO 14729, it becomes obvious that the test conditions in the EN ISO 14729 should be revised in order to create more realistic conditions, e.g., by using a more realistic artificial tear fluid. Furthermore, we suggest adapting the EN ISO 14729 to the European test hierarchy for chemical disinfectants and antiseptics, which consists of three test phases and also requests meeting stricter criteria in order to pass the test. Unless the test conditions guarantee a sufficient reduction of potential pathogens, the risk of contact lens-related microbial keratitis and other infections will remain for the users.

Abstract:

This paper is based on research carried out with children and adults at early childhood education institutions in Rio de Janeiro, Brazil. The research goals were to identify interactions between adults and children and to rethink pedagogical practice. The theoretical-methodological framework was based on language and culture studies, the sociology of childhood and anthropology. After studying children and adults in different contexts and early childhood education public policies and practice, the results present challenges of changing and creating early education practices in difficult contexts, especially with regard to the creation and use of spaces intended for children. This is the focus of the text, which is structured in three sections. Section one presents the theoretical and methodological issues. The second section analyzes three case studies developed in public pre-schools situated in different cities in the state of Rio de Janeiro. Section three presents conclusions and priorities for intervention and change concerning children, adults and their interactions.

Training Needs of International Medical Graduates [IMGs] in Psychiatry  [PDF]

Abstract: The potential shortage of psychiatrists over the next 5 - 10 years has focused attention on the need to recruit more IMGs to fill the needs rather than use nurse practitioners or physician assistants. IMGs make up about 1/3 of first year psychiatry residents. These individuals have been found to provide services to the poor, the elderly and the psychotic. The quality of their medical work has been found to be satisfactory. The training needs of these physicians require an understanding on the part of their teachers that they come from cultures with different values that we have. The extended families of these primarily Asian residents clash with our strong commitment to individualism. It leads to a We-self rather than our I-Self. This difference coupled with the stress of leaving to come to a new culture is a great stress. Their exposure to psychiatry has been limited. They request and need more interview demonstration and practice, ore feedback and examinations. They should have help in accent reduction. They should be exposed to the working of the hospital by sitting on departmental and hospital committees. The faculty should extend their social opportunities and work as mentors on joint projects. Courses on the history of American culture should be taught. Psychotherapy for them should be encouraged as well as teaching medical ethics. They must become the major educational concern for the department that they are in.

NOD2-C2 - a novel NOD2 isoform activating NF-κB in a muramyl dipeptide-independent manner

Abstract: Here, we report a novel alternative transcript of the NOD2 gene, which codes for a truncated tandem CARD only protein, called NOD2-C2. The transcript isoform is highest expressed in leucocytes, a natural barrier against pathogen invasion, and is strictly linked to promoter usage as well as predominantly to one allele of the single nucleotide polymorphism rs2067085. Contrary to a previously identified truncated single CARD NOD2 isoform, NOD2-S, NOD2-C2 is able to activate NF-κB in a dose dependent manner independently of muramyl dipeptide (MDP). On the other hand NOD2-C2 competes with MDPs ability to activate the NOD2-driven NF-κB signaling cascade.NOD2 transcripts having included an alternative exon downstream of exon 3 (exon 3a) are the endogenous equivalents of a previously described in vitro construct with the putative protein composed of only the two N-terminal CARDs. This protein form (NOD2-C2) activates NF-κB independent of an MDP stimulus and is a potential regulator of NOD2 signaling.The innate immune system uses several molecules that sense pathogen-associated molecular patterns (PAMPs) including Toll-like, RIG-1 (retinoic acid inducible gene protein 1)-like and the NOD (nucleotide-binding and oligomerization domain)-like receptors (NLRs) to trigger a protective response against intracellular danger signals, e.g. cytoinvasive pathogens. The NLRs family consists of more than 20 related members defined by a tripartite structure consisting of: (i) a variable N-terminal protein-protein interaction domain, defined by the caspase recruitment domain (CARD), pyrin domain (PYD), or the baculovirus inhibitor domain (BIR); (ii) a centrally located NOD domain facilitating self-oligomerization during activation [1], and (iii) a C-terminal leucine-rich repeat (LRR) responsible for binding/detecting of PAMPs. Abstract: 164 previous sperm donors completed an online survey regarding health and genetic experiences and views. Results highlight that donors desire to act responsibly with recruiting facilities is not always possible. Objective: Obtaining the views and experiences of sperm donors regarding health and genetic matters. Design: Online survey. Setting: Not applicable. Participants: 164 previous sperm donors. Interventions: Not applicable. Main outcome measures: Views and experiences on health and genetic issues. Results: A variety of approaches are adopted by recruiting facilities in regard to selection and post-donation factors. The vast majority of donors said they had not been contacted by the facility they donated at to update their medical information, while almost one quarter of donors indicated that a health or genetic risk factor had occurred. A great majority of donors felt that they had not received any education or counselling on the potential curiosities of donor conceived people. Donors sought to be honest and open with staff but often found there were difficulties in doing so. Conclusions: Overall, donors indicate that they see donating as involving responsibilities to the offspring and families. The study highlights however that their ability to act responsibly is limited by some of the interactions or lack of them with the facilities where they donated. Implications for recruiting facilities need to be considered.
Interacting electrons in a magnetic field in a center-of-mass free basis

Abstract: We present an extension of the spin-adapted configuration-interaction method for the computation of four electrons in a quasi two-dimensional quantum dot. By a group-theoretical decomposition of the basis set and working with relative and center-of-mass coordinates we obtain an analytical identification of all spurious center-of-mass states of the Coulomb-interacting electrons. We find a substantial reduction in the basis set used for numerical computations. At the same time we increase the accuracy compared to the standard spin-adapted configuration-interaction method (SACI) due to the absence of distortions caused by an unbalanced cut-off of center-of-mass excitations.

Abstract: Exact one-electron eigenstates in finite parts of 1D periodic and Fibonacci chains of attractive and repulsive delta potentials are computed and analyzed. Bloch and bound state boundary conditions are related in terms of transfer matrices. Scenarios of positive and negative energy are distinguished. The dependence on the potential strength parameter is analyzed. The scattering matrix is computed. Implications for the interpretation of band germs in quasiperiodic chains are discussed.
Epigenetic Regulation of Learning and Memory by Drosophila EHMT/G9a

Abstract: The epigenetic modification of chromatin structure and its effect on complex neuronal processes like learning and memory is an emerging field in neuroscience. However, little is known about the “writers” of the neuronal epigenome and how they lay down the basis for proper cognition. Here, we have dissected the neuronal function of the Drosophila euchromatin histone methyltransferase (EHMT), a member of a conserved protein family that methylates histone 3 at lysine 9 (H3K9). EHMT is widely expressed in the nervous system and other tissues, yet EHMT mutant flies are viable. Neurodevelopmental and behavioral analyses identified EHMT as a regulator of peripheral dendrite development, larval locomotor behavior, non-associative learning, and courtship memory. The requirement for EHMT in memory was mapped to 7B-Gal4 positive cells, which are, in adult brains, predominantly mushroom body neurons. Moreover, memory was restored by EHMT re-expression during adulthood, indicating that cognitive defects are reversible in EHMT mutants. To uncover the underlying molecular mechanisms, we generated genome-wide H3K9 dimethylation profiles by ChIP-seq. Loss of H3K9 dimethylation in EHMT mutants occurs at 5% of the euchromatic genome and is enriched at the 5′ and 3′ ends of distinct classes of genes that control neuronal and behavioral processes that are corrupted in EHMT mutants. Our study identifies Drosophila EHMT as a key regulator of cognition that orchestrates an epigenetic program featuring classic learning and memory genes. Our findings are relevant to the pathophysiological mechanisms underlying Kleefstra Syndrome, a severe form of intellectual disability caused by mutations in human EHMT1, and have potential therapeutic implications. Our work thus provides novel insights into the epigenetic control of cognition in health and disease.
