Sleep, Fatigue, and Functional Health in Psychotic Patients

Abstract: This study sought to examine the association between sleep, fatigue, and functional health in psychotic patients. Participants included 93 psychotic inpatients ( with schizophrenia) who completed the Chalder Fatigue Scale (ChFS), the Fatigue Symptom Inventory (FSI), the Pittsburgh Sleep Quality Index (PSQI), and the SF36 Health Survey. Patients were classified on the basis of their performance on sleep and fatigue measures: 60% reported significant levels of fatigue and 67% significant sleep disturbances. 28.4% reported both, suggesting that fatigue and sleep dysfunctions do not necessarily cooccur. A closer examination of patterns showed that fatigue was only related to qualitative aspects of sleep and not quantifiable aspects of sleep disturbances. The results also showed that functional health was the lowest in patients with high levels of fatigue, compared to patients with sleep problems only or patients with neither symptom. A regression analysis further showed that the size of the contribution of fatigue onto functional health was twice as much as that of sleep dysfunctions. In conclusion, the results show that (i) dissatisfaction with sleep—and not sleep itself—is related to fatigue symptoms and that (ii) fatigue is particularly detrimental to functional health, regardless of the presence of sleep dysfunctions. 1. Introduction Fatigue is a condition characterised by persistent weakness or exhaustion and a combination of symptoms that feature self-reported impairments in some of the following: impaired attention and concentration, headaches, unrefreshing sleep, and/or musculoskeletal pain [1]. Fatigue is an experiential state typically diagnosed on the basis of self-reports and is a common complaint in the general population [2, 3]. It is usually associated with middle age, being female, and having lower education and occupation attainment [4]. Fatigue is common in psychiatric conditions such as anxiety and depression (25–36%) [5–7] and in chronic medical conditions such as cancer, Parkinson’s disease, multiple sclerosis, diabetes, and viral infection [8–12]. Fatigue worsens with increasing physical disease severity [10] and is independent of medication suggesting that medication itself is not responsible for fatigue. Studies of fatigue in patients with psychotic disorders such as schizophrenia or bipolar disorder are currently lacking. Abstract: Bionanomedicine and environmental research share need common terms and ontologies. This study applied knowledge systems, data mining, and bibliometrics used in nano-scale ADME research from 1991 to 2011. The prominence of nano-ADME in environmental research began to exceed the publication rate in medical research in 2006. That trend appears to continue as a result of the growing products in commerce using nanotechnology, that is, 5-fold growth in number of countries with nanomaterials research centers. Funding for this research virtually did not exist prior to 2002, whereas today both medical and environmental research is funded globally. Key nanoparticle research began with pharmacology and therapeutic drug-delivery and contrasting agents, but the advances have found utility in the environmental research community. As evidence ultrafine aerosols and aquatic colloids research increased 6-fold, indicating a new emphasis on environmental nanotoxicology. User-directed expert elicitation from the engineering and chemical/ADME domains can be combined with appropriate Boolean logic and queries to define the corpus of nanoparticle interest. The study combined pharmacological expertise and informatics to identify the corpus by building logical conclusions and observations. Publication records informatics can lead to an enhanced understanding the connectivity between fields, as well as overcoming the differences in ontology between the fields.
Season of birth and handedness in Serbian high school students

Abstract: We have investigated the effect of season of birth in an epidemiologically robust sample of randomly selected young people (n = 977), all born in the same year. A Kolmogorov-Smirnov type statistical test was used to determine season of birth.Neither the right-handed nor the non-right-handed groups demonstrated birth asymmetry relative to the normal population birth distribution. There was no between-group difference in the seasonal distribution of birth when comparing the right-handed to the non-right-handed groups.The present study failed to provide support for a season of birth effect on atypical lateralisation of handedness in humans.Functional dominance of the right hand is the norm across different populations, various geographical regions, and diverse cultures, with approximately 90% of humans exhibiting clear dominance of the right side of the body. This behavioural characteristic is considered as uniquely human, as there is no other species that displays such a large behavioural asymmetry at the population level. It is also widely accepted that this behavioural feature emerged at some point during the hominid evolution, and that this feature preceded the evolution of another uniquely human feature – language, and in particular, speech as its central component [1].The transmission of handedness over many generations of humans is widely believed to be under genetic control [2-4], rather than resulting from learning. Converging lines of evidence provide support for the genetic hypothesis, including imaging studies on twins [5], meta-analysis of handedness in twins [6], and molecular genetic studies [7,8]. To achieve a good therapeutic ratio the radiation dose to the tumor should be as high as possible with the lowest possible dose to the surrounding normal tissue. This is especially the case for brain tumors. Technological advancements in diagnostic imaging, dose calculations, and radiation delivery systems, combined with a better understanding of the pathophysiology of brain tumors have led to improvements in the therapeutic results. The widely used technology of delivering 3-D conformal therapy with photon beams (gamma rays) produced by Linear Accelerators has progressed into the use of Intensity modulated radiation therapy (IMRT). Particle beams have been used for several decades for radiotherapy because of their favorable depth dose characteristics. The introduction of clinically dedicated proton beam therapy facilities has improved the access for cancer patients to this treatment. Proton therapy is of particular interest for pediatric malignancies. These technical improvements are further enhanced by the evolution in tumor physiology imaging which allows for improved delineation of the tumor. This in turn opens the potential to adjust the radiation dose to maximize the radiobiological effects. The advances in both imaging and radiation therapy delivery will be discussed.

Generation of recombinant Orf virus using an enhanced green fluorescent protein reporter gene as a selectable marker

Abstract: In this study, we developed a rapid method to generate recombinant Orf viruses (ORFV) based on the enhanced green fluorescent protein (EGFP) reporter gene as a selectable marker. The coding sequence of EGFP gene was amplified from pEGFP-N1 vector and subcloned into the pZIPPY-neo/gus plasmid under the control of the early-late vaccinia virus (VACV) VV7.5 promoter and flanked by two multiple cloning sites (MCS) to generate a novel transfer vector pSPV-EGFP. Using the pSPV-EGFP, two recombination cassettes pSPV-113LF-EGFP-113RF and pSPV-116LF-EGFP-116RF were constructed by cloning the flanking regions of the ORFV113 and ORFV116 and inserted into two MCS flanking the EGFP gene. Using this novel system, two single gene deletion mutants OV-IA82Δ113 and OV-IA82Δ116 were successfully generated.This approach shortens the time needed to generate recombinant ORFVs (rORFVs). Thus, the pSPV-EGFP vector provides a direct, fast, and convenient way to manipulate the recombinant viruses, indicating that it is highly suited for its designed purpose.Orf virus (ORFV), the prototypic member of the Parapoxviridae, is the cause of a papular demertitis in sheep and goats known as contagious ecthyma [1]. ORFV has been used in veterinary medicine as a preventive and therapeutic immunomodulatory agent. Live or inactivated ORFV preparations exhibit dose-dependent immunomodulatory effects when administered to multiple animal species including cattle, horses, cats and dogs. Significant therapeutic efficacy of ORFV preparations in preventing or treating stress-associated and other infectious disease conditions has been well documented [1-4]. Association between value-based purchasing score and hospital characteristics

Abstract: Background Medicare hospital Value-based purchasing (VBP) program that links Medicare payments to quality of care will become effective from 2013. It is unclear whether specific hospital characteristics are associated with a hospital’s VBP score, and consequently incentive payments. The objective of the study was to assess the association of hospital characteristics with (i) the mean VBP score, and (ii) specific percentiles of the VBP score distribution. The secondary objective was to quantify the associations of hospital characteristics with the VBP score components: clinical process of care (CPC) score and patient satisfaction score. Methods Observational analysis that used data from three sources: Medicare Hospital Compare Database, American Hospital Association 2010 Annual Survey and Medicare Impact File. The final study sample included 2,491 U.S. acute care hospitals eligible for the VBP program. The association of hospital characteristics with the mean VBP score and specific VBP score percentiles were assessed by ordinary least square (OLS) regression and quantile regression (QR), respectively. Results VBP score had substantial variations, with mean score of 30 and 60 in the first and fourth quartiles of the VBP score distribution. For-profit status (vs. non-profit), smaller bed size (vs. 100–199 beds), East South Central region (vs. New England region) and the report of specific CPC measures (discharge instructions, timely provision of antibiotics and beta blockers, and serum glucose controls in cardiac surgery patients) were positively associated with mean VBP scores (p<0.01 in all). Total number of CPC measures reported, bed size of 400–499 (vs. 100–199 beds), a few geographic regions (Mid-Atlantic, West North Central, Mountain and Pacific) compared to the New England region were negatively associated with mean VBP score (p<0.01 in all). Disproportionate share index, proportion of Medicare and Medicaid days to total inpatient days had significant (p<0.01) but small effects. QR results indicate evidence of differential effects of some of the hospital characteristics across low-, medium- and high-quality providers. Conclusions Although hospitals serving the poor and the elderly are more likely to score lower under the VBP program, the correlation appears small. Profit status, geographic regions, number and type of CPC measures reported explain the most variation among scores.

Abstract: The nucleon electromagnetic form factors G_E and G_M are complex quantities in the time-like region. The absolute values can be determined by measuring the angular distribution of the nucleons in e^+ e^- --> N N-bar. The complex phase can only be determined by measuring one or more polarizations of the initial or final state. For PEP-N, we can use unpolarized e+ and e- and measure the polarization of one of the outgoing nucleons.
Acquisition of Weak Signals in Multi-Constellation Frequency Domain Receivers  [PDF]

New positioning applications’ availability requirements demand receivers with higher sensitivities and ability to process multiple GNSS signals. Possible applications include acquiring one signal per GNSS constellation in the same frequency band and combining them for increased sensitivity or predicting acquisition of other signals. Frequency domain processing can be used for this purpose, since it benefits from parallel processing capabilities of Fast Fourier Transform (FFT), which can be efficiently implemented in software receivers. On the other hand, long coherent integration times are mainly limited due to large FFT size in receivers using frequency domain techniques. A new method is proposed to address the problems in frequency domain receivers without compromising the resources and execution time. A pre-correlation accumulation (PCA) is proposed to partition the received samples into one-code-period blocks, and to sum them together. As a result, the noise is averaged out and the correlation results will gain more power, provided that the relative phase between the data segments is compensated for. In addition to simplicity, the proposed PCA method enables the use of one-size FFT for all integration times. A post-correlation peak combination is also proposed to remove the need for double buffering. The proposed methods are implemented in a configurable Simulink model, developed for acquiring recorded GNSS signals. For weak signal scenarios, a Spirent GPS simulator is used as a source. Acquisition results for GPS L1 C/A and GLONASS L1OF are shown and the performance of the proposed technique is discussed. The proposed techniques target GNSS receivers using frequency domain processing aiming at accommodating all the GNSS signals, while minimizing resource usage. They also apply to weak signal acquisition in frequency domain to answer the availability demand of today’s GNSS positioning applications.

Analysis of the Genome of the Sexually Transmitted Insect Virus Helicoverpa zea Nudivirus 2

Abstract: The sexually transmitted insect virus Helicoverpa zea nudivirus 2 (HzNV-2) was determined to have a circular double-stranded DNA genome of 231,621 bp coding for an estimated 113 open reading frames (ORFs). HzNV-2 is most closely related to the nudiviruses, a sister group of the insect baculoviruses. Several putative ORFs that share homology with the baculovirus core genes were identified in the viral genome. However, HzNV-2 lacks several key genetic features of baculoviruses including the late transcriptional regulation factor, LEF-1 and the palindromic hrs, which serve as origins of replication. The HzNV-2 genome was found to code for three ORFs that had significant sequence homology to cellular genes which are not generally found in viral genomes. These included a presumed juvenile hormone esterase gene, a gene coding for a putative zinc-dependent matrix metalloprotease, and a major facilitator superfamily protein gene; all of which are believed to play a role in the cellular proliferation and the tissue hypertrophy observed in the malformation of reproductive organs observed in HzNV-2 infected corn earworm moths, Helicoverpa zea.
