Abstract: La actitud terapéutica ante la meningitis bacteriana (MEB) es uno de los retos más importantes a los que se enfrenta el pediatra, pues de su oportuno y adecuado manejo depende el pronóstico. Los cambios en la epidemiología de los síndromes neurológicos infecciosos bacterianos observados en Cuba a partir del a o 2000, promovieron la implementación de modificaciones al régimen terapéutico antibiótico convencional. En este artículo, sin olvidar que el tratamiento ideal es el preventivo, se expone una revisión actualizada del manejo hospitalario de las MEB por S.pneumoniae, que incluye: adopción de medidas generales, instauración temprana de acciones dirigidas a atenuar los efectos perjudiciales de la respuesta inflamatoria sistémica, conducta específica contra el agente (antibioticoterapia) y finalmente la identificación precoz e inicio de la rehabilitación de secuelas. Se hace énfasis en las modificaciones al esquema antibiótico adoptadas recientemente por la Comisión Nacional de SNI. Se exponen consideraciones sobre la introducción de la vancomicina en esquema antimicrobiano de las MEB por S.pneumoniae. The therapeutic attitude to face bacterial meningitis (BME) is one of the most important challenges for pediatricians, since the prognosis depends on its timely and adequate management The changes in the epidemiology of the bacterial infectious neurological syndromes ocurred in Cuba from 2000 on, promoted the implementation of modifications in the conventional therapeutical antibiotic regimen. In this article, without forgetting that the ideal treatment is the preventive one, it is exposed an updated review of the hospital management of BME due to S.Pneumoniae that includes: adoption of general measures, early establishment of actions directed to alleviate the harmful effects of the systemic inflammatory reponse, specific conduct against the agent (antibiotic therapy) and, finally, the identification and beginning of the rehabilitation of sequelae. Emphasis is made on the modifications of the antibiotic scheme recently adopted by the National Commission of the National System of Information. Considerations are made on the introduction of vancomycin in the antimicrobial scheme of the BME caused by S. pneumnoniae.

Abstract: it is reported that pneumonia associated with mechanical ventilation is a particular type of nosocomial infection that is frequently found among critical patients and it is related to high morbidity and mortality rates. its origin is polymicrobial and depends on multiple risk factors, such as: age, days of ventilation, little mobilization, gastric alcanilization, trauma, coma and use of drugs as sedatives and blockading agents, among others. the fundamental strategies to prevent this disease are aimed at reducing the impact of the intrinsic or extrinsic risk factors. its prognosis depends on the moment of appearance of the symptoms, so it may be classified as of early onset when it appears before the 4 days of ventilation. it is produced by germs from the oropharynx and is associated with low mortality and its prognosis is generally good. the late onset pneumonia arises after 5 days of ventilatory support. it is caused by multiresistant strains and its prognosis is very poor. the diagnosis is difficult due to the multiple criteria proposed in different studies. those made by the american thorax association, in 1999, are more specific, since they have the clinical, microbiological and non-microbiological conditions, which are non-invasive, they may be established easily and fast and can be applied to any age group, independently of the cause of the disease. the direct microscopic analysis and the knowledge of the epidemiological map of each service, allow to use a more effective empiric treatment. although monotherapy is the present trend, there is not an ideal antibiotic covering the whole spectrum of microorganisms responsible for this entity.

Abstract: Se informa que la neumonía asociada a ventilación mecánica es un tipo particular de infección nosocomial frecuente en los pacientes críticos y se asocia con altas tasas de morbilidad y mortalidad. Su origen es polimicrobiano y depende de múltiples factores de riesgo como: edad, días de ventilación, poca movilización, alcalinización gástrica, trauma, coma y uso de medicamentos como: sedantes y bloqueadores, entre otros. Las estrategias fundamentales para la prevención de esta entidad, tienen como objetivo reducir el impacto de los factores de riesgo ya sean intrínsecos o extrínsecos. Su pronóstico depende del momento de aparición de los síntomas, por lo que se clasifica de inicio temprano, aquella que aparece antes de los 4 días de ventilación, producida por los gérmenes de la orofaringe, se asocia a baja mortalidad y generalmente es de buen pronóstico y la de inicio tardío que aparece después de los 5 días de apoyo ventilatorio, producida por cepas multirresistentes y de muy mal pronóstico. El diagnóstico es difícil por los múltiples criterios propuestos a través de los diferentes estudios realizados. Los emitidos por la Sociedad Americana del Tórax en 1999 son más específicos, pues reúnen las condiciones clínicas, microbiológicas y no microbiológicas, las cuales no son invasivas, resultan fáciles y rápidas de recoger y pueden aplicarse a cualquier grupo de edad, independientemente de la causa que provocó la enfermedad. El análisis microscópico directo y el conocimiento del mapa epidemiológico de cada servicio, permiten el inicio de un tratamiento empírico más efectivo. Aunque la tendencia actual en el tratamiento es la monoterapia, no existe un antibiótico ideal que cubra la totalidad del espectro de los microorganismos responsables de esta entidad. Abstract: This research shall analyse the implementation of a new product and an entrepreneurial business model in Brazil of the ′90s and endeavour to trace cause and effect relations by means of the elaboration of scenarios. To ensure effectiveness, the methodology foresees a combination of inductive and deductive techniques. This choice arises from an understanding that it this is the best way to analyse a complex environment such as that of Brazil which, despite linguistic unity, comprises significant distinctions in each of its five regions. It is herein expected to bring forth an improved understanding of studies, related to the set up of information technology companies in the Brazilian territory, and pinpoint this market′s peculiarities to possible entrants, regardless of their segment. Síndrome de Stevens-Johnson: Presentación de 1 caso

Abstract: stevens-johnson syndrome is an acute inflammatory disease caused by a hypersensibility that includes the skin and the mucous membranes and that in a severe form endangers the patient?s life. it may be induced by many precipitating factors, from drugs to infection. the affection is characterized by a sudden morphologically variable rash, accompanied with stomatitis and ophthalmia. an 8-year-old boy suffering from this clinical entity in a very severe form, stevens-johnson syndrome, with skin , ocular and oral mucosa manifestations and with other serious complications, is presented in this paper. he had to be treated at the icu and a satisfactory evolution was attained.

Abstract: a prospective, descriptive and cross-sectional study of the clinical aspects of patients diagnosed with sepsis and admitted to the pediatric intensive care unit of "orlando pantoja tamayo" general teaching hospital in contramaestre, santiago de cuba province from 1998 to 2000 was carried out. the universe of study was made up of 90 patients and the most frequent age was over one year-old. bacterial sepsis was highly incidental and the patients came from the emergency department. length of stay at hospital increased with the advance of sepsis. the majority of children recovered from the disease.

Abstract: El síndrome de Stevens-Johnson, es una enfermedad inflamatoria aguda, originada por una hipersensibilidad, que incluye la piel y las membranas mucosas; en una forma mayor pone en peligro la vida del paciente. Puede ser inducida por muchos factores precipitantes, desde los medicamentos hasta una infección. La afección se caracteriza por una súbita erupción morfológicamente variable, acompa ada de estomatitis y oftalmia. En este trabajo se presenta un ni o de 8 a os de edad, con la entidad clínica en forma muy severa, el síndrome de Stevens-Johnson, que involucra manifestaciones cutáneas, oculares y de la mucosa oral, acompa ada de otras complicaciones graves, por lo que requirió terapéutica en cuidados intensivos, y se logró una evolución satisfactoria del paciente. Abstract: Se realiza un estudio prospectivo, descriptivo y transversal de aspectos clínicos de los pacientes que ingresaron con el diagnóstico de sepsis, en la Unidad de Cuidados Intensivos Pediátricos del Hospital General Docente "Orlando Pantoja Tamayo" de Contramaestre, Santiago de Cuba, en el período comprendido de 1998 al 2000. El universo estuvo constituido por 90 pacientes, y fue la edad más frecuente la de los ni os mayores de 1 a o. El diagnóstico de mayor incidencia fue el de sepsis bacteriana y los enfermos procedían del Cuerpo de Guardia. La estadía se incrementó a medida que avanzó el estadio de sepsis. En la evolución final la mayoría de los ni os egresó vivo.
