Abstract: Se presenta un paciente del sexo masculino de 51 a os de edad que fue hospitalizado por odinofagia, fiebre elevada, poliartritis, además de leucocitosis con neutrofilia y eritrosedimentación acelerada. Durante el ingreso se le constata poliserositis, neumonitis y miocarditis con insuficiencia cardiaca izquierda, hipertransaminasemia, hiperferritinemia y factor reumatoideo con anti DNA de doble cadena negativos. Se inició tratamiento con corticoesteroides e inmunosupresores (azatioprina). La respuesta no fue la esperada y hubo que cambiar el inmunosupresor por metotrexate. La poliartritis febril de evolución policíclica es la forma de presentación más frecuente de la enfermedad de Still del adulto. La hiperferritinemia mayor de 1 000 ng/mL es una herramienta útil para el diagnóstico y su normalización es indicador de éxito terapéutico. No existen manifestaciones clínicas, ni pruebas de laboratorio patognomónicas, por lo que el diagnóstico continúa siendo por exclusión. This is the case of man aged 51 admitted due to odynophagia, high fever, polyarthritis, as well as leukocytes with neutrophilia and accelerated erythrosedimentation. During admission it was noted polyserositis, pneumonitis and myocarditis with left cardiac insufficiency, hyper-transaminasemia, hyper-ferritinemia and rheumatoid factor with anti-AND of negative double-chain. Treatment was started with corticosteroids and immunosuppressive agents (azathioprine). Response was not the expected one and it was necessary to change the immunosuppressive agent by methotrexate. Febrile polyarthritis of polycyclic evolution is the more frequent presentation of the Still's disease in adult. Hyper-ferritinemia over 1 000 ng/mL is an useful tool for diagnosis and its normalization is an indicator of therapeutical success. There were neither clinical manifestations nor pathognomonic laboratory tests thus, diagnosis remains by exclusion.

Abstract: Se estudiaron 224 fallecidos con autopsias, en Unidad de Cuidados Intensivos del Hospital Dr. Carlos J. Finlay, en el período del 2000 al 2002, con el propósito de precisar las principales causas básicas y directas de muertes, así como determinar el grado de correlación clínico-patológica; con este fin, se utilizó la clasificación empleada en el Sistema Automatizado de Registro y Control de Anatomía Patológica. La ateroesclerosis coronaria constituyó la principal causa básica de muerte, tanto por diagnóstico clínico como autópsico y el choque fue la causa directa más frecuente. La discrepancia diagnóstica fue de 26.1%, en la causa básica de muerte, y de 29.9%, en la causa directa de muerte. In the intensive care unit at Carlos J. New Approaches to the Computer Simulation of Amorphous Alloys: A Review

Abstract: In this work we review our new methods to computer generate amorphous atomic topologies of several binary alloys: SiH, SiN, CN; binary systems based on group IV elements like SiC; the GeSe 2 chalcogenide; aluminum-based systems: AlN and AlSi, and the CuZr amorphous alloy. We use an ab initio approach based on density functionals and computationally thermally-randomized periodically-continued cells with at least 108 atoms. The computational thermal process to generate the amorphous alloys is the undermelt-quench approach, or one of its variants, that consists in linearly heating the samples to just below their melting (or liquidus) temperatures, and then linearly cooling them afterwards. These processes are carried out from initial crystalline conditions using short and long time steps. We find that a step four-times the default time step is adequate for most of the simulations. Radial distribution functions (partial and total) are calculated and compared whenever possible with experimental results, and the agreement is very good. For some materials we report studies of the effect of the topological disorder on their electronic and vibrational densities of states and on their optical properties.

Abstract: agricultural scientists are influenced by several factors when defining the approach to their research. the aim of this paper was to identify demographic, professional and institutional aspects and internal-external influences related to the sustainable, environmental or productivist approach of scientist in aguascalientes, mexico in 2008. the approaches underwent a correlation analysis with 44 influence factors. the sustainable approach had a positive correlation with the specialty, problem-solving with institutional interest, studies on farmers' fields and investigation on farming systems; and a negative correlation with basic research and biotechnology. the environmental approach had a positive correlation with the opinion of the interdisciplinary group in the definition of the problem under investigation. the productivist approach had a positive correlation with specialty, the opinions of users and colleagues in defining the problem, studies on farmers' fields and in the experimental stations, work by technological components and with the researcher's objectivity and a negative correlation with the environmental discipline. the logistic regression model for research with a sustainable approach can be explained by the origin of the researchers, research leaves, type of institution, and source of funding, research on demand of the user and institutional priority, interdisciplinary work, studies on farming systems and by technological components. the environmental model can be explained with research leaves, approval of projects by a technical committee, studies on farmers' fields, interdisciplinary work and studies on farming systems. the productivist model had explicatory variables of the types of institution and work on farming systems.

Abstract: Los científicos(as) agropecuarios(as) están influenciados(as) por diversos factores para definir su enfoque de investigación. El objetivo de este trabajo fue identificar aspectos demográficos y profesionales, institucionales e influencias internas-externas asociados al enfoque sostenible, ecologista o productivista de los científicos(as) en Aguascalientes, México en 2008. Los enfoques fueron sometidos a un análisis de correlación con 44 factores de influencia. El enfoque sostenible correlacionó positivamente con la especialidad, la solución de problemas de interés institucional, investigación en terrenos de productores e investigación en sistemas de producción y negativamente con investigación básica y biotecnología. El enfoque ecologista correlacionó positivamente con la opinión del grupo interdisciplinario en la definición del problema a investigar. El enfoque productivista correlacionó positivamente con la especialidad, opinión del usuario y colegas en la definición del problema, estudios en terrenos de productores y en la estación experimental, trabajos por componente tecnológico y con la objetividad del investigador(a), y negativamente con la disciplina ecología. El modelo de regresión logística para la investigación con enfoque sostenible se explica por el origen de los investigadores(as), estancias de investigación, tipo de institución, fuente de financiamiento, investigación por demanda del usuario y prioridad institucional, trabajo interdisciplinario, estudios tanto en sistemas de producción como por componente tecnológico. El modelo ecologista se explica por las estancias de investigación, aprobación del proyecto por un comité técnico, investigación en terrenos de productores, trabajo interdisciplinario y estudios en sistemas de producción. El modelo productivista tuvo las variables explicatorias tipo de institución y trabajo en sistemas de producción. Microscopy and molecular biology for the diagnosis and evaluation of malaria in a hospital in a rural area of Ethiopia

Abstract: A cross-sectional survey of 471 individuals was carried out in a hospital in the rural area of Gambo (Ethiopia). Blood samples were prepared for microscopic observation and collected in filter paper for Seminested-Multiplex PCR (SnM-PCR) and real time PCR (qPCR) testing. The SnM-PCR was considered as the gold standard technique and compared with the rest. Thus, agreement between SnM-PCR and LM was determined by calculating Kappa Statistics and correlation between LM and qPCR quantification was calculated by pair-wise correlation co-efficient.Samples analysed by LM and SnM-PCR were positive for Plasmodium sp. 5.5% and 10.5%, respectively. Sensitivity was 52.2% by LM and 70% by qPCR. Correlation co-efficient between microscopy counts and qPCR densities for Plasmodium vivax was R2?=?0.586. Prevalence was estimated at 7% (95% CI: 4.7–9.3). Plasmodium vivax was the dominant species detected and the difference was statistically significant (χ2?=?5.121 p?<?0.05). The highest prevalence of the parasite (10.9%) was observed in age groups under 15?years old.Accurate malaria diagnostic methods have a great effect in the reduction of the number of malaria-infected individuals. SnM-PCR detection of malaria parasites may be a very useful complement to microscopic examination in order to obtain the real prevalence of each Plasmodium species. Although SnM-PCR shows that it is a good tool for the determination of Plasmodium species, today light microscopy remains the only viabletool for malaria diagnosis in developing countries. Therefore, re-inforcement in the training of microscopists is essential for making the correct diagnosis of malaria. Plasmodium vivax was the predominant species in Gambo, a meso-endemic area for this species.
