Evaluation of a combination of low-dose ketamine and low-dose midazolam in terminal dyspnea-attenuation of "double-effect"

Abstract: Aim: Of all symptoms in palliative medicine those concerning respiration are most excruciating and difficult to treat. Reticence about the use of morphine for palliation of dyspnea is common, especially in nonmalignant diseases, as there is a fear of causing respiratory depression, particularly where Chronic Obstructive Pulmonary Disease (COPD) exists. This factor is also compounded by the lack of availability of morphine in parts of developing countries. Ketamine has excellent anesthetic and analgesic effects in addition to being easily available. It produces bronchodilatation and does not produce respiratory or cardiovascular depression. The author seeks to evaluate the role of low-dose (0.2 mg/kg) ketamine and midazolam (0.02 mg/kg) in the attenuation of terminal dyspnea. Methods: Sixteen patients with terminal dyspnea, admitted to the Critical Care Unit (CCU) with cancer and other noncancer diagnoses were recruited. The subjective component of dyspnea was assessed using the Graphic Rating Scale (GRS), which has values from 0 - 10, 10 being maximum dyspnea. Each patient received a low-dose of ketamine and midazolam for relief of dyspnea. All the patients received low-flow (2 L/min.) oxygen therapy via nasal cannula. Immediately after admission, all the patients were reassured and nursed in a decubitus position of their choice. The GRS was recorded at the point of admission, 10 minutes after starting oxygen therapy, and ten minutes after administration of low-dose ketamine and midazolam. Hemodynamic parameters were also recorded at these three points. Result: All the patients who enrolled in our study had significant dyspnea at admission, as was evident from the GRS scores of 8.250 (SD 0.91), respiratory rate of 28.56 (SD 5.0), mean arterial blood pressure (MABP) of 102.7 (SD 14.63), pulse rate of 115.62 (SD 23.3), and SpO2 of 92.43 (SD 2.38). All the patients benefited from the combination of ketamine and midazolam, as evidenced by the statistically significant ( P < 0.01) improvement in all monitored parameters. Conclusion: The author concludes that a combination of low-dose ketamine and midazolam can be safely used in the palliation of terminal dyspnea, with excellent results. The duration of action of this combination in providing symptomatic relief is however short-lived, being 36.7 minutes on an average (SD 5.23), thus necessitating intermittent IV top-ups. No adverse reports were obtained in the study.

Abstract: Die m nnliche Osteoporose stellt ein wichtiges, bis jetzt zu wenig beachtetes Gesundheitsproblem beim lteren Mann dar. Wie bei der Frau führt auch beim Mann mit zunehmendem Alter die stetige Abnahme der Knochendichte zu einem Anstieg des Frakturrisikos (Inzidenz Frau zu Mann 2-3 : 1). W hrend die Frauen nach der Fraktur h ufiger pflegebedürftig werden, steigt bei den M nnern das Mortalit tsrisiko. über 50% der M nner mit symptomatischen WS-Frakturen haben eine sekund re Ursache der Osteoporose, wie Hypogonadismus, orale Steroidtherapie oder chronischer Alkoholismus. Sturzneigung durch Parkinson, Demenz, Hemiparese oder Sehst rungen erh hen das Risiko für Schenkelhalsfrakturen wesentlich. Jeder Mann mit Osteoporose sollte kausal abgekl rt und behandelt werden, um das Frakturrisiko zu senken. Eine etablierte Therapie der m nnlichen Osteoporose gibt es bis dato nicht, jedoch ist die Hormonsubstitution bei Hypogonadismus genauso eine therapeutische Option wie die Bisphosphonattherapie bei der idiopathischen Osteoporose und die Kalzium-Vitamin D-Substitution bei lteren M nnern.
Blended e-Health in Cognitive Behavioural Therapy: Usage Intensity, Attitude and Therapeutic Alliance in Clinical Practice  [PDF]

Abstract: Background: Though internet based cognitive behaviour therapy is proven to be effective, e-health is applied only in a minority of treatments in specialised mental health care. The low application rates are associated with therapists’ attitudes towards e-health. One of the major concerns is limitations in the therapeutic relation and communication between patient and therapist. Since therapist involvement is important in an effective e-health treatment, negative attitudes towards e-health can be a risk for effective e-health application. Aims: This study aimed to examine the relationship between patients’ e-health usage as well as patients’ attitudes towards e-health and the therapeutic alliance. The possible influence of therapists’ attitudes on patients’ e-health usage is also examined. Method: In an outpatient mental health setting, patients attended a blended treatment program for major depressive disorder and completed questionnaires on attitudes towards e-health and the therapeutic alliance. Therapists completed an alliance and attitude questionnaire as well. Results: Patients with more positive attitudes used the e-health application more intensively. Higher work alliance rates were related to sharing digital homework assignments and seeking contact with their therapist. Also, when treated by therapists with more positive attitudes towards e-health, patients shared more assignments with their therapist. Patients with high symptom rates at start, had more negative attitude rates and tended to use the e-health application less frequently. Conclusions: In a treatment cohesion in which therapists are positive about blended e-health, patients’ attitudes are positively related to e-health usage and therefore a factor of interest in improvement of effective e-health application. Patients with severe symptoms may need extra attention like active support and iteration to promote adherence to the e-health program.
Indigenous Territories and REDD in Latin America: Opportunity or Threat?

Abstract: An important proportion of Latin America’s forests are located in indigenous territories, and indigenous peoples are the beneficiaries of about 85% of the area for which local rights to land and forest have been recognized in Latin America since the 1980s. Nevertheless, many of these areas, whether or not rights have been recognized, are subject to threats from colonists, illegal loggers, mining and oil interests and others, whose practices endanger not only the forests but also indigenous people’s territory as a whole. In this context, REDD could constitute a new threat or intensify others, particularly in places where indigenous tenure rights have not been recognized, but REDD could also offer new opportunities. This article describes the limitations of thinking only in terms of communities, rather than territories, and examines the extent to which REDD has been conceived considering the characteristics of this new territorial configuration. It also identifies the challenges that REDD may face with this new ‘stakeholder’, such as numerous specific characteristics of territories, given their heterogeneity, in the context of past experiences regarding ‘forest options’. This paper analyses the situation in already-titled indigenous territories in particular, and also discusses problems facing territories in the titling process.

Abstract: Dieser Beitrag besch ftigt sich mit der Bedeutung von Heim(at) für vietnamesische Flüchtlinge, die sich in Hamilton (Ontario, Kanada) niedergelassen haben; eine Situation, die auch die Erstautorin dieses Artikels erfahren hat. Wir beginnen mit einem Literaturüberblick zur Bedeutung von Heim(at) und m glichen Implikationen für die uns interessierende Gruppe. Nachdem wir zun chst eher konventionell eine Fallstudie vorgesehen hatten, wandelten wir das Design sukzessive in Richtung eines autoethnografischen Ansatzes, nachdem die Erstautorin nach Vietnam gereist war und ihre eigenen Erfahrungen ebenso wie die der Untersuchungsteilnehmer/innen reflektiert hatte. Diese Reise der Erstautorin spielte eine zentrale Rolle für die Untersuchung. Ihr eigenes implizites Wissen kreierte zusammen mit den Erz hlungen der zw lf Gespr chspartner/innen ein spezifisches Verst ndnis von Heim(at), für einen Prozess der Entwurzelung, Umsiedelung und (r umlichen) Wiedereinbettung. Im Kern sind das Verst ndnis und die Bedeutung von Heim(at) in Kultur und Familie verankert, und genau diese Faktoren erlauben, so unser Ergebnis, dass Heim(at) für diese Migrant/innen beweglich geblieben ist. We begin with a review of the meaning of place and home and their possible implications for this group. Initially designed as a case study, the research process was influenced by the conventional paradigm assumptions of an emerging design and was later transformed into an autoethnography as the first author journeyed to Vietnam and reflected on her experience, as well as that of her 12 respondents. The first author's own journey to understand home played a critical part in this investigation. Her tacit knowledge together with the stories of these 12 refugees reflect a co-created understanding of home—a definition that is characterized by a process of being uprooted, relocated and embedded into place. Yet at the core of home, its comprehension and value is grounded in culture and family, and as this article suggests, these two factors enable home to remain fluid for these migrants.
