Abstract: purpose: to verify the prevalence of breastfeeding in preterm infants weighting over 1500 g at hospital discharge and until six months of life. methods: this is a quantitative descriptive observational retrospective study, based on the analysis of records of infants born between january 2006 and december 2007, which were assisted by the speech-language pathology department in the pediatric unit and the speech-language pathology outpatient clinic of a hospital. the inclusion criteria were: low birth weight preterm infants, with no congenital heart or pulmonary diseases, no genetic syndromes, and apgar score higher than three at one minute and five at five minutes. results: the results from the 156 records studied showed that the prevalence of exclusive breastfeeding at hospital discharge was 58.3%. at six months it was observed a prevalence of 22.2% of exclusive breastfeeding. conclusion: the prevalence of exclusive breastfeeding was well below the world health organization recommendation, which is exclusive breastfeeding until the sixth month of life.

Abstract: currently, breast milk is the main nourishment to be offered to preterm infants, according to the recommendation of national and international organizations for breastfeeding protection, support and incentive. the use of bottles and pacifiers is inadvisable for causing nipple confusion and increasing early weaning. literature indicates "emptied breast" sucking as an alternative method of non-nutritive sucking stimulation in preterm infants. the aim of this case report was to describe "emptied breast" sucking in premature infants according to duration of gastric tube feeding, length of hospital stay, weight gain and prevalence of exclusive breastfeeding at discharge. the sample consisted of 19 infants with mean gestational age of 32 weeks, assisted in the neonatal intensive care unit of the hospital where the study took place. the infants received "emptied breast" sucking stimulation. when the mother was absent, stimulation was performed with a gloved finger. data analysis used descriptive statistics. the premature infants remained, in average, for 22 days with gastric tube feeding. the preterm infants were, in average, for 30 days in the hospital. mean weight gain was 23 grams/day. prevalence of breastfeeding at hospital discharge was 100%. it was observed that non-nutritive sucking on the "emptied breast" can be an effective method to increase the prevalence of breastfeeding at hospital discharge, and as an alternative technique for stimulation of non-nutritive sucking.

Abstract: backgrond: the transition from gastric to oral feeding of preterm infants is one of the greatest concerns of health professionals and therefore needs an objective criterion to support the beginning of this process. aim: to test the reliability of an instrument that assesses the readiness of preterm infants for the transition from gastric to oral feeding. method: the instrument is composed by the following items: corrected gestational age; behavioral state; global posture and tonus; lips and tongue posture; rooting, suck, bite and gag reflexes; tongue and jaw movements; tongue cupping; sucking strain; sucking and pause; maintenance sucking/pause; maintenance alert state and stress signs. the study was conducted at the intermediate care unit of the hospital de clínicas da faculdade de medicina de ribeir?o preto - university of s?o paulo. the research sample consisted of 30 preterm infants who attended the following inclusion criteria: corrected gestational age <36 weeks and 6 days; clinically stable; absence of facial deformities; respiratory, cardiovascular, gastrointestinal and neurological disorders or syndromes that prevent or make oral feeding difficult; and not having received oral feeding of milk. the kappa test was used to verify interjudge reliability. results: the items that presented high reliability levels were: behavioral state, global posture and tonus, lips and tongue posture, gag reflex and maintenance of alert state. the items that presented satisfactory reliability levels were: rooting reflex, sucking and biting and jaw movement, sucking strain and sucking and pause. only the items of tongue cupping, maintenance of sucking/pause and stress signs presented unsatisfactory reliability levels. conclusion: in general, the items investigated by the assessment instrument presented adequate interjudge reliability.

Abstract: the world health organization recommends breastfeeding exclusively to all newborns until the sixth month and on a complementary basis, until the second year of life. thus, some techniques are performed in order to facilitate the breastfeeding, including the use of a cup and recently using the "probe-finger" technique. this practice is very controversial and there are few studies addressing the description of the technique, and indications and usage. this study aims at reporting the clinical experience in order to show and use the "finger-probe" technique. the "probe-finger" technique is offering milk, preferably human milk, using a gastric tube connected to a syringe plunger and fixed on a gloved finger with a tape. the probe is positioned in the oral cavity of the newborn and should serve as a technical aid for sucking adequacy. thus, it is suggested that his statement should be done just in cases where an oral dysfunction is characterized, either in newborns at term or preterm. given the specific evaluation performed by the speech therapist, indicating the "probe-finger" technique in order to adjust the changes obtained in the evaluation of non-nutritive sucking or breastfeeding. it is believed that the "probe-finger" technique is recommended as a supplement to breastfeeding, and should be further investigated in order to clarify the impact of the "probe-finger" technique on the prevalence of breast-feeding and oral motor development of born infants.

Abstract: purpose: the aim this present study is test the discriminate validation of preterm infant oral feeding readiness assessment scale propose by fujinaga. methods: the study was observational descriptive. participated 19 prematures. the instrument was applied with groove finger and was analyzed items: corrected gestational age; behavioral state; global posture and tonus; lips and tongue posture; rooting, suck, bite and gag reflexes; tongue and jaw movements; tongue cupping; sucking strain; sucking and pause; maintenance sucking/pause; maintenance alert state and stress signs. the test discriminate validation was analyzed the compare the performance of premature in the first evaluation (total gavage feeding) and in the second evaluation (total oral feeding), beyond verifying if significant statistics difference exists between first and second evaluation. the scores was compared with t test, with significance level 0,01. results: the results indicate that have a comportamental change in itens behavioral state, rooting and sucking reflex, tongue movement and cupping, jaw movement, sucking strain, sucking and pause, maintenance of the rhythm of sucking and pause, maintenance of alert state and stress signs. the comportment of itens global posture and tonus, lips and tongue posture, biting and gag reflex no varied. ocurred statstics diference between the first an second evaluation scores. conclusions: preterm infant oral feeding readiness assessment scale propose by fujinaga has discriminate validation.

Abstract: this is a bibliographic study on non-nutritive sucking in pre-term infants. objectives: to characterize and analyze the scientific production on non-nutritive sucking in pre-term infants and identify how the transition from gastric to oral feeding takes place as well as how it begins and develops by considering the stimulation of non-nutritive sucking. methodology: review of secondary sources (medline and lilacs). results: pre-term infants presented a stable clinical condition, without cardio-respiratory, neurological or gastro-intestinal alterations. gestational age varied from 30 to 35 weeks and the weight from 1,400 to 1,800 g. the stimulus for non-nutritive sucking used was a pacifier or bottle nipple. the time and frequency of stimulus application varied. as a result of the stimulation programs, there were early hospital discharges and greater weight gain. literature on this subject, particularly that of national authorship, is scarce and the articles do not address a detailed description of the transition from gastric to oral feeding, thus leaving a gap that deserves to be further studied.

Abstract: A recomenda o da Organiza o Mundial da Saúde é que todo recém nascido deva ser alimentado exclusivamente no seio materno até o sexto mês e, de forma complementar, até o segundo ano de vida. Assim, algumas técnicas s o realizadas para facilitar a alimenta o ao seio, dentre elas o uso do copo e, recentemente, a utiliza o da técnica "sonda-dedo". Tal prática é bastante controversa e há escassez de estudos na literatura sobre a descri o da técnica, sua indica o e uso. O objetivo do presente trabalho é relatar a experiência clínica para indica o e uso da técnica "sonda-dedo". A técnica "sonda-dedo" consiste no oferecimento do leite, de preferência humano, utilizando sonda gástrica conectada a uma seringa com êmbolo e fixada em dedo mínimo enluvado com fita adesiva. A sonda é posicionada na cavidade oral do recém nascido e deve servir como uma técnica de auxílio para adequa o do padr o de suc o. Desta forma, sugere-se que sua indica o deve ser apenas nos casos em que seja caracterizada uma disfun o oral, seja em recém nascidos a termo ou pré-termo. Diante da avalia o específica, realizada pelo fonoaudiólogo, indica-se a técnica "sonda-dedo" com objetivo de adequar as altera es obtidas na avalia o da suc o n o nutritiva ou em seio materno. Acredita-se que, para que a técnica "sonda-dedo" seja indicada como complemento do aleitamento materno, devam ser realizados novos estudos para esclarecer quais as repercuss es da técnica "sonda-dedo" na prevalência do aleitamento materno e no desenvolvimento motor oral de recém nascidos. Abstract: As a es de saneamento ambiental devem prover sanidade aos locais onde s o praticadas as atividades humanas, a fim de promover a saúde da popula o, interrompendo o ciclo de transmiss o de doen as. A saúde pública participa como ferramenta para as a es coletivas de saúde. Após constatar problemas de saúde em um grupo de famílias, moradoras de uma zona rural do Paraná, na Esta o Ecológica de Fernandes Pinheiro, o presente trabalho dedicou-se a identificar a ocorrência de doen as infecciosas parasitológicas e alguns aspectos de saneamento básico presentes na área estudada. Foram verificados os fatores físicos, abastecimento de água e esgotamento sanitário de 4 famílias dessa popula o. O estudo realizado foi descritivo-observacional e procurou verificar a precariedade do saneamento básico com a prevalência de diarreia nos sujeitos envolvidos. Para o desenvolvimento do presente estudo foram realizadas entrevistas, visitas de campo, análises de laboratório e revis o bibliográfica. Estes procedimentos possibilitaram verificar inter-rela o entre o indicador de saúde diarreia e a contamina o da água dos po os e rio. Os resultados obtidos revelaram a ocorrência de 27% de prevalência de diarreia, sendo a maioria entre crian as, além da contamina o da água por excretas, devido à falta de fatores de prote o dos locais de capta o e à ausência da prática de desinfec o da água. A conclus o desta pesquisa deve incentivar a realiza o, por parte dos atores envolvidos no problema, quer seja o órg o Municipal Competente e/ou a popula o, que sejam implementadas a es preventivas e de infraestrutura que resultem em melhorias na saúde pública.AbstractThe actions of environmental sanitation must provide sanity to the places where human activities are practiced in order to promote the health care for population, breaking the cycle of disease transmission. Abstract: Estudo bibliográfico sobre suc o n o-nutritiva em bebês prematuros. Objetivos: caracterizar e analisar a produ o científica sobre suc o n o-nutritiva em bebês prematuros e identificar como se processa a transi o da alimenta o gástrica para a oral, considerando a estimula o da suc o n o-nutritiva, seu início e evolu o. Metodologia: revis o de fonte secundária (MEDLINE e LILACS). Resultado: condi o clínica estável dos bebês pré-termo, sem altera es cardiorespiratórias, neurológicas e gastrointestinais. A idade gestacional variou de 30 a 35 semanas, e o peso, de 1400 a 1800g. O estímulo utilizado para suc o n o-nutritiva foi chupeta ou bico de mamadeira, havendo varia o quanto ao tempo e freqüência de aplica o do estímulo. Como impacto dos programas de estimula o, houve alta hospitalar precoce e maior ganho de peso. Os autores ressaltam a escassa literatura sobre o assunto, principalmente a nacional, registrando que os artigos encontrados n o descrevem, detalhadamente, a transi o da alimenta o gástrica para oral, deixando uma lacuna que exige estudos posteriores.

Abstract: objective: to characterize the transition from oral gastric feeding related to: maturity and weight of premature babies, methods and techniques of administration and, duration of the transition to only oral feeding. methods: this is a retrospective study of data gathered in records of 116 infants attended in intensive care units and neonatal intermediate in a university hospital in ribeirao preto-sp. results: the mean-corrected of gestational age was 36 weeks at the start of oral feeding and 37 weeks when the supply came entirely by the oral route. the average weight was 1,743 grams at the start of oral feeding and 1,934 grams when the food came entirely by the oral route. during the transition period were used one or more means of milk administration. the use of forced feeding in conjunction with other techniques (89.5%) predominated, this was complemented by breast feeding and milk bottle (56.9%); the duration of feeding transition ranged from less than 1 day to 47 days. conclusion: the maturity and birth weight, in addition to the clinical conditions from these variables, can interfere in the process of milk feeding transition in preterm babies. considering the advantages of breastfeeding, the practice should be initiated as early as possible in this risk population segment.
