Anemia como problema de saúde pública: uma realidade atual

Abstract: in 1990, the united nations held a world summit in new york, in which goals for the upcoming decade were established. one of these goals was a one-third reduction in the prevalence of anemia among women at childbearing age. despite this modest percentage, epidemiological indicators point to the opposite direction, indicating an increasing occurrence of anemia in different regions of the world, including in brazil. these data show that anemia has continued since the antiquity to be one of the most prevalent and widespread diseases in human populations. thus, a number of questions without answers arise regarding the actual prevalence and etiopathogenesis of the problem as well as with respect to the degree of implantation and the effectiveness of control measures.

Abstract: objectives: to describe the frequency of anemia and iron deficiency anemia, using two cutoff points for hemoglobin concentration (hb) as well as serum ferritin levels in women with low-risk pregnancies. methods: a cross-sectional study was carried out based on the database records of 318 pregnant women at the instituto materno infantil professor fernando figueira-imip, in the city of recife, state of pernambuco, brazil, between may 2000 and june 2001. the frequency of anemia and/or iron deficiency anemia were determined using two hb cutoff points (<11.0 and <10.5g/dl) in addition to serum ferritin levels. data were analyzed using the epi-info 6.04b and minitab v.14.2 software programs. the project was approved by the research ethics committee. results: the frequency of anemia (hb<11.0g/dl) and the frequency of iron deficiency anemia (hb<11.0g/dl; ferritin<12ng/ml) were 56.6% and 10.7%, respectively. when the hb cutoff point was changed to 10.5g/dl, the frequency of anemia fell to 37.4% and the frequency of iron deficiency anemia to 7.9%. higher serum ferritin levels were found in anemic patients than in non-anemic patients, regardless of the hb cutoff point used. conclusions: the frequency of anemia was acceptable, but the proportion of iron deficiency anemia was far lower than expected. ferritin levels seemed to behave in a way that contradicts the theoretical literature on the subject.
Accuracy of erythrogram and serum ferritin for the maternal anemia diagnosis (AMA): a phase 3 diagnostic study on prediction of the therapeutic responsiveness to oral iron in pregnancy

Abstract: The aim of this protocol is to verify the accuracy of erythrocyte indices and serum ferritin (studied tests) for the diagnosis of functional iron-deficiency in pregnancy using the iron-therapy responsiveness as the gold-standard. This is an ongoing phase III accuracy study initiated in August 2011 and to be concluded in April 2013. The subjects are anemic pregnant women (haemoglobin concentration < 11.0 g/dL) attended at a low-risk prenatal care center in the Northeast of Brazil. The sample size (n 278) was calculated to estimate sensitivity of 90% and 80% of specificity with relative error of 10% and power of 95%. This study has a prospective design with a before-after intervention of 80 mg of daily oral iron during 90 days and will be analyzed as a delayed-type cross-sectional study. Women at the second trimester of pregnancy are being evaluated with clinical and laboratorial examinations at the enrollment and monthly. The 'responsiveness to therapeutic test with oral iron' (gold-standard) was defined to an increase of at least 0.55 Z-score in haemoglobin after 4 weeks of treatment and a total dose of 1200 mg of iron. At the study conclusion, sensitivities, specificities, predictive values, likelihood ratios and areas under the ROC (Receiver Operating Characteristic) curves of serum ferritin and erythrocyte indices (red blood cell count, haematocrit, haemoglobin concentration, mean corpuscular volume, mean corpuscular haemoglobin, mean corpuscular haemoglobin concentration, red blood cell distribution width, reticulocyte count) will be tested. The compliance and adverse effects are considered confounding variables, since they are the main obstacles for the iron-therapy responsiveness.This study protocol shows a new approach on iron-deficiency anemia in pregnancy from a functional point of view that could bring some insights about the diagnostic misclassifications arising from the dynamic physiologic changes during the gestational cycle.Trial registration: WHO Inte
Adherence and side effects of three ferrous sulfate treatment regimens on anemic pregnant women in clinical trials

Abstract: the objective of this study was to analyze adherence and side effects of three iron supplement regimens (ferrous sulfate) on anemic pregnant women. the clinical trial involved 150 women between the 16th and 20th gestational weeks, at low obstetric risk and with hemoglobin concentration of between 8.0 and 11.0g/dl. treatment was provided by ferrous sulfate with 60mg of elemental iron during 16 (± 1) weeks, in three regimens: single tablet a week (n = 48); single tablet twice a week (n = 53) or single tablet a day (n = 49). the outcomes were adherence, assessed through interviews and by counting tablets, and side effects, according to patient information. the adherence showed a declining trend (92%, 83% and 71%; p = 0.010) and the side effects revealed a growing trend (40%, 45% and 71%; p = 0.002) as the dosage increased. diarrhea and epigastric pain were significantly associated with the dose administered (p = 0.002). these results suggest that in anemic pregnant women, complaints are directly proportional and the compliance is inversely proportional to the amount of medicinal iron.
Erythrocyte indices in the second trimester of pregnancy: are reference values well established?

Abstract: the erythrocyte index values in the pregnancy-puerperal cycle behave in a particular manner, however, the physiological or nosological meaning of such alterations are not well established. this study was aimed at analyzing the distribution of erythrocyte indices in the second trimester of pregnancy. the study was carried out with secondary data from 347 pregnant women in the 2nd trimester of low-risk prenatal care at the instituto materno infantil professor fernando figueira (recife, brazil) between may 2000 and june 2001. measurements were performed using an automatic counter. frequency distribution curves of erythrocyte indices (means and standard deviations) and correlation curves of hematocrit (htc) and hemoglobin (hb) concentrations were constructed. the mean red blood cell number (rbc) was 3.7 x 1012cells/l; mean htc was 32.9% and mean hb was 10.8 g/dl; the values exhibited gaussian distributions. the mean morphological values were 88.4 fl for the mean corpuscular volume; 29.0 pg for the mean corpuscular hemoglobin and 32.8 g/dl for the mean corpuscular hemoglobin concentration; these values were not adjusted to normal distributions. htc values were three times higher than hb values. mean rbc, htc and hb values (used in the diagnosis of anemia) were lower than thresholds considered normal and exhibited gaussian distributions. morphological indices remained within normal ranges, but without delineating a normal curve. as expected htc and hb values behaved following a linear correlation.
Analysis of the physical activity effects and measurement of pro-inflammatory cytokines in irradiated lungs in rats

Abstract: purpose: to study if the pre-radiotherapy physical activity has radio-protective elements, by measuring the radio-induced activation of pro-inflammatory cytokines as interleukin-6 (il-6), transforming growth factor -β (tgf -β), tumor necrosis factor -α (tnf-α) and protein beta kinase β (ikkβ), through western blotting analysis. methods: a randomized study with 28 wistar hannover rats, males, with a mean age of 90 days and weighing about 200 grams. the animals were divided into three groups: (gi, gii and giii). giii group were submitted to swimming for eight weeks (zero load, three times a week, about 30 minutes). then, the groups (except the control group) were submitted to irradiation by cobalt therapy, single dose of 3.5 gray in the whole body. all animals were sacrificed by overdose of pentobarbital, according to the time for analysis of cytokines, and then a fragment of the lower lobe of the right lung went to western blotting analysis. results: the cytokines ikk β, tnf-α and il-6 induced by radiation in the lung were lower in the exercised animals. however, exercise did not alter the radiation-induced increase in tgf-β. conclusion: the results show a lower response in relation to inflammatory cytokines in the group that practiced the exercise pre-radiotherapy, showing that exercise can protect tissues from tissue damage due to irradiation.

Abstract: objectives: this study assessed anemia and hypovitaminosis a in puerperal women seen at the instituto materno infantil prof. fernando figueira - imip. methods: descriptive cross sectional study using a sample of 72 puerperal hospital discharged women. results: age ranged from 12 to 41 years old, median age of 22.5 years old. a good school level was noted, although seven of these women declared three years or less of education. some type of iron supplement intake during pregnancy was informed by 48 women. it was noted that 43.1% showed hemoglobin concentration below 10,0 g/dl and 65.3% below 11,0 g/dl. microcitosis occurred in 14 patients (19.4%) with no microcitosis case. the levels of hypovitaminosis a were high: 25.0% had retinol levels under 20 μg/dl. an association tendency between puerperal non-anemic women and normocitosis (median corpuscular weight x anemia) was present; with a cut-off point for anemia of 11,0 g/dl, a positive association between anemia during pregnancy and the use of iron supplements was noted. conclusions: anemia prevalence was high, although microcitosis frequency was low. prevalence of hypovitaminosis a was high reaching levels considered a public health issue in brazil.

Abstract: the evolution of the educational role of librarians, since the appearance of reference work to the emergence of information literacy and its assimilation by the library profession, is described. the idea of information literacy is shown to be intrinsically embedded related to reference work practices and user education. however, the new concept brought about the widening of the educational role of the librarian. in view of the responsibility assumed currently by librarians -to help people learn how to search and use information-a better understanding of the concept of information literacy is necessary, as well as changes in educational practice.

Abstract: this study aims to analyse the information literacy movement, which started in the united states in the seventies. that movement represents the effort the american library profession is doing in order to get more visibility in the educational community. its appearance occurred in peculiar circumstances, along with changes in the librarian teaching role concerning bibliographic instruction. the discourse of the information literacy movement evolves around the following aspects: the information society, constructivist perspective of education, information technology and the librarian. considering the word is coming into view in the brazilian literature on library and information science this article suggests the establishment of a research program towards our reality in an attempt to integrate information literacy in the education field.

Abstract: Se describe la evolución del papel educativo del bibliotecario desde la aparición del trabajo de referencia hasta el surgimiento del concepto de alfabetización informativa y su asimilación por la biblioteconomía. Y demuestra que la idea de alfabetización informativa estaba ya incrustada en las prácticas relacionadas con el trabajo de referencia y de educación de usuarios. Sin embargo el nuevo concepto trajo consigo la expansión de la función educativa del bibliotecario. Tomando en cuenta la responsabilidad que el bibliotecario asume actualmente, de ayudarles a las personas a que aprendan a buscar y usar la información, se concluye que tendrá no sólo que buscar una mejor comprensión del concepto de alfabetización informativa sino también de modificar su práctica educativa.
