Abstract: closed loop obstruction of the bowel is an extremely grave condition that may lead to serious consequences and even be fatal. the authors describe a case of a patient with intestinal obstruction secondary to cancer of the sigmoid and highlight the importance of computed tomography in the analysis of the pathological changes in routine management of patients with acute abdomen.

Os autores descrevem um caso de obstru o intestinal secundária a cancer no sigmóide, destacando a importancia da tomografia computadorizada na análise das altera es patológicas na rotina de abdome agudo.

Abstract: O pseudomixoma peritoneal é um tumor incomum, de curso indolente, que se caracteriza pela presen a de ascite mucinosa ou implantes na cavidade peritoneal. Origina-se geralmente de les es no apêndice ou no ovário. O diagnóstico pode ser feito por meio da citologia de aspira o por agulha fina, ultra-sonografia, tomografia computadorizada ou ressonancia magnética. Os autores relatam três casos de pseudomixoma peritoneal cujo sítio primário era o ovário, e que foram submetidos a tomografia computadorizada e a ressonancia magnética do abdome. Este trabalho enfatiza a importancia destes métodos em fun o de sua capacidade de resolu o espacial, imagens multiplanares e diferentes seqüências (na ressonancia magnética), permitindo melhor avalia o das les es. Os exames tomográficos demonstraram massas lobuladas, hipodensas, com limites bem definidos, determinando "lobula es" nas margens hepática e esplênica por compress o extrínseca secundária a implantes peritoneais, sem invas o dos órg os. A ressonancia magnética revelou les es expansivas com baixo sinal nas imagens ponderadas em T1 e alto sinal em T2, de localiza o peritoneal, junto às margens do fígado e ba o.

Abstract: pseudomyxoma peritonei is rare tumor that presents insidious evolution and is characterized by mucinous ascites or implants in the peritoneal cavity. the origin of this tumor is frequently the appendix or the ovaries. diagnosis can be established by fine needle aspiration cytology, ultrasonography, computed tomography or magnetic resonance imaging . the authors report three cases of patients with pseudomyxoma peritonei arising from the ovary that were submitted to computed tomography and magnetic resonance imaging examinations of the abdomen. this paper emphasizes the importance of these imaging methods that permit greater spatial resolution, multiplanar images and different types of sequences (magnetic resonance imaging), and allow better evaluation of these lesions. computed tomography examinations showed lobulated low-density masses with well defined limits causing "scalloping" of the hepatic and splenic borders due to extrinsic compression by adjacent peritoneal implants. invasion of the liver and spleen was not observed. magnetic resonance imaging examinations revealed expansive peritoneal lesions with low-intensity signal on t1-weighted images and high-intensity signal on t2-weighted images, adjacent to the hepatic and splenic borders.

Abstract: we report the computed tomography findings of five patients with pathology proven pulmonary amyloidosis. tracheobronchial amyloidosis with calcified nodules and plaques in the tracheal wall were seen in two patients. two other patients had diffuse parenchymal disease with calcified lesions, one had reticular and nodular subpleural opacities whereas the other had nodular interlobular septal thickening and a parenchymal consolidation. the latter presented the nodular type of the disease with multiple sharp nodules scattered throughout the lungs and interspersed calcifications. the computed tomography findings observed were not specific but strongly suggestive of amyloidosis.

Análise de custo-efetividade da trombólise com alteplase no Acidente Vascular Cerebral

Abstract: background: the cerebrovascular accident (cva) or stroke is the main cause of death in brazil and little information is available on the cost of treatment. objective: to carry out a cost-effectiveness analysis of thrombolysis in stroke, up to three hours after symptom onset, comparing the treatment with alteplase versus the conservative treatment, under the perspective of the brazilian public health system (sus). methods: a decision analysis model was developed to compare the two types of treatment. cycles were considered, during which the patients would go through five stages of disability post-stroke, based on the modified rankin scale. the probability to present intracranial hemorrhage in the first year was obtained from the ninds trial. for the subsequent years, one-year cycles were considered to calculate patients' mortality. the outcome was expressed as quality-adjusted life years (qaly). both direct and indirect costs were considered in the analysis. costs and outcomes were discounted at 5% a year. results: in the first year, the qaly gained was 0.06 for both sexes, with an incremental cost of r$ 2,558 for men and r$ 2,312 for women. the incremental cost-effectiveness ratio (icer) in one year was r$ 40,539 / qaly (usd 28,956) for men and r$ 36,640 / qaly (usd 26,171) for women. after the second year, the treatment with alteplase reduced the cost of treatment (purchasing power parity index: us$ 1 = r$ 1.4). conclusion: the thrombolytic therapy with alteplase within the first three hours following a stroke is cost-effective in the brazilian public health system scenario.
Castleman disease: an uncommon diagnosis in pediatrics

Abstract: First described by Dr. Benjamin Castleman in 1956, Castleman disease is an uncommon disease of an etiology that is not yet thoroughly known. Three distinct histological subtypes have already been described: hyaline-vascular-, plasma cell-, and human herpes virus 8-associated variant, clinically distinguished in multi or unicentric types. Castleman disease is occasionally diagnosed in children, but more often in young adults, with no gender predominance. The symptoms are rather heterogeneous, varying from an asymptomatic mass in the unicentric Castleman disease type, to life-threatening systemic inflammatory state with systemic symptoms in the multicentric Castleman disease type. The authors report a case of a 15-year-old boy who sought medical attention due to a cervical tumor mass, without systemic symptoms. Pathology exam of the excised mass diagnosed a very typical example of the hyaline-vascular unicentric type of Castleman disease.

Abstract:

Language can be a powerful tool to convince others and make them cooperative. Cialdini (2007) has worked out several principles along which it is possible to analyze discourses in terms of their persuasiveness. Others also have contributed with tools to analyzing discourses such as Fairclough (2003). These tools are used to analyze the “Evil Empire Speech” of the US President Ronald Reagan that he held at the National Association of Evangelicals, 1983, in Orlando Florida. His historical speech was aimed at convincing the nation about the righteousness of his nuclear policy. He partly rewrote the already prepared script and included the “evil empire” part. The analysis supports that his speech was an exceptionally effective one. Reagan made his speech an example of the following principles of persuasiveness such as reciprocity, authority, commitment, liking, scarcity and social proof. He wanted support for belligerent intentions from a faithful community, which was already problematic, but he got the audience on his side through emphasizing his similarities with them, his own faithfulness, the presentation of strong examples and balancing humor and seriousness. Additionally, he introduced the striking metaphor “evil empire”, which stuck to the peoples’ minds and had an impact on them. He also appealed to the people through implicitly distinguishing the evil from the ones who were not evil—the US citizens. Thus, he made the American people feel better, to ensure them that they do the right thing when following him. He ranked religious people above him when he was joking about clergy men and politicians. To reinforce his authority, he borrowed the authority of various respected men through citing them. He improved his position and the power of his arguments using the philosophical wisdom of others.

Renin-Angiotensin System Blockers Protect Pancreatic Islets against Diet-Induced Obesity and Insulin Resistance in Mice

Abstract: Background The associations between obesity, hypertension and diabetes are well established, and the renin-angiotensin system (RAS) may provide a link among them. The effect of RAS inhibition on type 2 diabetes is still unclear; however, RAS seems to play an important role in the regulation of the pancreas and glucose intolerance of mice fed high-fat (HF) diet. Methods C57BL/6 mice fed a HF diet (8 weeks) were treated with aliskiren (50 mg/kg/day), enalapril (30 mg/kg/day) or losartan (10 mg/kg/day) for 6 weeks, and the protective effects were extensively compared among groups by morphometry, stereological tools, immunostaining, Western blotting and hormonal analysis. Results All RAS inhibitors significantly attenuated the increased blood pressure in mice fed a HF diet. Treatment with enalapril, but not aliskiren or losartan, significantly attenuated body mass (BM) gain, glucose intolerance and insulin resistance, improved the alpha and beta cell mass and prevented the reduction of plasma adiponectin. Furthermore, enalapril treatment improved the protein expression of the pancreatic islet Pdx1, GLUT2, ACE2 and Mas receptors. Losartan treatment showed the greatest AT2R expression. Conclusion Our findings indicate that ACE inhibition with enalapril attenuated several of the deleterious effects of the HF diet. In summary, enalapril appears to be responsible for the normalization of islet morphology and function, of alpha and beta cell mass and of Pdx1 and GLUT2 expression. These protective effects of enalapril were attributed, primarily, to the reduction in body mass gain and food intake and the enhancement of the ACE2/Ang (1-7) /Mas receptor axis and adiponectin levels.
