Gesta??o Trigemelar Espontanea: Complica??es Maternas e Resultados Perinatais

Abstract: purpose: to analyze maternal complications and perinatal results of triplet pregnancies. method: retrospective study of maternal and perinatal data on all triplets weighing >500 g delivered in a period of 8 years at maternidade escola de vila nova cachoeirinha. results: between 1990-1998, 18 women gave birth to triplets, representing 1 in every 2,060 deliveries. the main complications were preterm delivery (94.4%) and preeclampsia (44.4%) and 83.3% of these patients needed hospitalization before delivery, for 1-50 days, most in order to inhibit preterm labor. cesarean section was performed in 88.9%, the mean gestational age at birth was 34.2 weeks (+ 1.8), mean weight 1,827 g (+ 421), 20.4% weighed <1,500 g and 75.9% weighed 1,500-2,499 g. birth weight discrepancy (> 25%) occurred in 38.9% of these pregnancies and 35.2% of the 54 fetuses were small for gestational age. eighty-six percent of live-born infants had neonatal morbidity and 3.7% had evident congenital anomalies. perinatal mortality was 16.7%, 7.4% due to intrauterine demise and 9.3% due to neonatal death. the mean duration of hospitalization in the neonatal ward was 18.5 days; late neonatal sepsis was the main cause of death. conclusion: triplet pregnancies had high a incidence of obstetric complications, demanded prolonged maternal hospitalization and ended almost always in surgical delivery. intrauterine and neonatal death rates were high, neonatal morbidity was detected in almost all live-born infants and their hospitalization was long, exposing these prematures to infection, their main cause of death. triplet pregnancies carry high maternal and fetal risks and should be managed at tertiary facilities.

Abstract: Objetivo: avaliar as complica es maternas e os resultados perinatais de gesta es trigemelares. Métodos: estudo retrospectivo analisando variáveis maternas e perinatais de todas gesta es trigemelares com peso fetal > 500 g ocorridas em 8 anos na Maternidade Escola de Vila Nova Cachoeirinha. Resultados: entre 1990-1998, 18 gestantes deram à luz trigemelares, representando 1:2.060 partos. As principais complica es foram a prematuridade (94,4%) e pré-eclampsia (44,4%) e 83,3% das pacientes precisaram ser internadas antes do parto por 1-50 dias, a maioria para inibi o de trabalho de parto prematuro. O parto foi cesáreo em 88,9%, a IG média ao nascer foi 34,2 semanas (+ 1,8), o peso médio foi 1.827 g (+ 421), com 20,4% <1.500 g e 75,9% pesando entre 1.500-2.499 g. Houve discordancia de peso (>25%) em 38,9% das gesta es e 35,2% dos 54 conceptos eram pequenos para idade gestacional. A morbidade neonatal acometeu 86% dos nativivos e 3,7% tiveram malforma es evidentes. A mortalidade perinatal foi de 16,7%: 7,4% natimortos e 9,3% neomortos. A permanência média no ber ário foi 18,5 dias e a principal causa de neomortalidade foi a sepse tardia. Conclus o: as gesta es trigemelares tiveram elevada incidência de complica es obstétricas, exigiram interna es maternas prolongadas e terminaram quase todas em parto operatório. A nati e neomortalidade foram elevadas, a morbidade neonatal acometeu praticamente todos os neonatos e a permanência no ber ário foi prolongada, expondo esses prematuros à infec o, principal causa de mortalidade neonatal. A gesta o trigemelar é de alto risco materno e fetal e exige atendimento em centros terciários.

Abstract: objective: to understand hiv/aids treatment regimens adherence experience in women. methods: an ethnographic study, based on the interpretative anthropology framework, was conducted with women who were participating in an "adherence group" at a public hospital. data were collected through semi-structured interviews and participant observations from january to december 2007. results: four categories emerged: (1) confronting the values regarding the experience in participating in treatment regimens adherence, (2) the singular experience to the treatment regimens adherence, (3) customs and practices regarding the experience in participating in the treatment regimens adherence, and (4) caring in the interpersonal relationships. conclusion: the hiv/aids regimens adherence experience was unique to each participant; however, women speeches suggest directions for effective hiv/aids treatment regimens adherence. each women unique experience may show others ways to come out of the shadows of a stigmatizing diagnosis.

Abstract: studies on the brazilian green tea (camellia sinensis var assamica) are still scarce when compared with the great number of investigations conducted on the green tea produced in other countries. the purposes of the present study were to evaluate the effects of infusion time, mode of packaging (tea in bulk or in tea bags) and preparation on the extraction of bioactive compounds from the brazilian green tea and on the stability of the beverage. the evaluated parameters were: the amounts of soluble solutes and phenolic compounds that were extracted as well as the antioxidant properties of the beverage using the dpph method (2,2-diphenyl-1-picrylhydrazyl radicals). the results revealed that the use of the herb in bulk with 5 minutes stirring was the most adequate condition for the extraction of bioactive compounds from the green tea. increases in the volume of the preparation without changes in the herb/water ratio increased the efficiency of the bioactive compounds extraction due to the fact that cooling a large volume of water is delayed in comparison to cooling smaller volumes. it can be said that the obtained beverages were stable for 24 hour at room temperature and in the refrigerator since no decrease in the antioxidant properties and significant changes in the contents of the major bioactive coumpounds, namely epigallocatechin gallate, epicatechin, catechin, and caffeine were detected.

Abstract: the essential oil from the leaves of rollinia leptopetala was obtained by hydrodistillation in clevenger's apparatus and its chemical composition was analyzed by gc-ms. with this technique could be identified 22 constituents in a complex mixture of monoterpenes (54.5%) and sesquiterpenes (45.5%). the main component found in the leaves was the bicyclogermacrene (22.47%). the essential oil was assayed against a strain of staphylococcus aureus possessing efflux mechanism of resistance to norfloxacin. although the essential oil did not display relevant antibacterial activity in vitro, it modulated the activity of the norfloxacin, i.e. in combination with the antibiotic it was observed a fourfold reduction in the minimum inhibitory concentration for norfloxacin, indicating inhibition of efflux pump.
Alzheimer's disease in Brazilian elderly has a relation with homocysteine but not with MTHFR polymorphisms

Abstract: objective: to investigate the association between total plasma homocysteine concentration, c677t and a1298c polymorphisms in mthfr gene and alzheimer's disease (ad) development. method: forty-three patients with probable (63%) and possible (37%) ad and 50 non-demented controls were evaluated. groups did not differ as to gender, age, scholar years, diabetes, alcohol and coffee intake and physical activity. total plasma homocysteine (hcy) levels were determined by hplc and genotyping for mthfr by pcr/rflp. mann-whitney "u" test was used to compare quantitative variable, fisher-freeman-halton test to compare genotypes and allele proportions and chi-square test to other qualitative variables. results: ad patients presented higher total plasma hcy levels than controls and the difference was statistically significant. no differences in the c677t and a1298c mthfr polymorphisms distributions were found between patients and controls. plasma homocysteine concentration did not change with mthfr genotypes. conclusion: our data confirms the association between increased plasma hcy concentration and ad and suggests that neither c677t nor a1298c mthfr polymorphisms contributed to genetic susceptibility for ad in elderly individuals in the northeast of brazil.

Abstract: This work aimed to evaluate the effect of inbreeding on the reproductive characteristics such as age at first foaling, gestation length, foaling interval in a “Mangalarga Marchador” herd in the North of Minas Gerais State, Brazil. The relationship matrix involved 2186 animals from the general register of the Catuni Farm. The inbreeding coefficient (F) was calculated using the subroutine MTDFNRM present in the MTDFREML program and its effect on the reproductive indexes was evaluated through simple linear regression. Average F of the population was of 1.45% and, of the total, 27.59% of the animals were inbred. Among the inbred animals, average F reached 5.28% (minimum of 0.1 and maximum of 28.13%). The average age of the stallions, when their foals were born, was 9.06 years while for mares it was of 8.90 years giving a medium interval of 8.98 years. The age at first foaling, gestation length and foaling interval were on average 4.71 ± 1.832 years, 327.94 ± 15.58 days and 547.78 ± 316.62 days, respectively while the herd effective size was of 185.25 animals. Effect of inbreeding on the reproductive characteristics was not significant. The inbred matings did not affect the reproductive characteristics of the Mangalarga Marchador herd. Controlled mating keep inbreeding at low values, what avoids inbreeding depression. Objetivou-se com este trabalho avaliar o efeito da endogamia sobre as características reprodutivas idade ao primeiro parto, dura o da gesta o e intervalo de partos em equinos da ra a Mangalarga Marchador criados em um rebanho na regi o Norte de Minas Gerais. A matriz de parentesco envolveu 2186 animais do arquivo zootécnico da Fazenda Catuni. O coeficiente de endogamia (F) foi calculado mediante utiliza o da subrotina MTDFNRM presente no programa MTDFREML e seu efeito sobre os índices reprodutivos por meio de regress o linear simples. O F médio da popula o foi de 1,45% e, do total de animais, 27,59% mostraram-se endogamicos. Entre os animais endogamicos, o F médio foi de 5,28% (mínimo de 0,1 e máximo de 28,13%). A média de idade dos garanh es, quando nasceram seus filhos, foi 9,06 anos enquanto a das m es foi de 8,90 anos, proporcionando um intervalo médio de gera es de 8,98 anos. A idade ao primeiro parto, a dura o da gesta o e o intervalo de partos tiveram médias de 4,71 ± 1,832 anos, 327,94 ± 15,58 dias e 547,78 ± 316,62 dias, respectivamente enquanto o tamanho efetivo para o rebanho foi de 185,25 animais. Abstract: people with prolonged dependence on mechanical ventilation require permanent care and the use of equipment that can result in longer term hospital internment. this can lead to difficulty of access for patients with acute injuries, as well as personal difficulties and stress with reduced quality of life for their families or caregivers due to such longer hospital internment. this critical review of publications dealing with dependence on mechanical ventilation among children and adolescents aimed at making information organized in a systematic manner available in order to support discussion on the subject. it should be borne in mind that changes in epidemiological profile and growing technological access determine needs such as intensive therapy hospital beds and complex home care for chronic patients, which still have limits of supply and regulatory restrictions in the brazilian public health system
Dental pulp vascular permeability changes induced by dental bleaching

Abstract: aiming to compare the effect of different light sources for dental bleaching on vascular permeability of dental pulps, forty-eight incisors were used. the bleaching agent (35 % hydrogen peroxide) was activated by halogen light; led (light emitting diode) or led, followed by laser phototherapy (lpt) (λ = 780 nm; 3 j/cm2). after the bleaching procedures, the animals received an intra-arterial dye injection and one hour later were sacrificed. the teeth were diaphanized and photographed. the amount of blue stain content of each dental pulp was quantified using a computer imaging program. the data was statistically compared (p < 0.05). the results showed a significant higher (p < 0.01) dye content in the groups bleached with halogen light, compared with the control, led and led plus lpt groups. thus, tooth bleaching activated by led or led plus lpt induces lesser resulted in increased vascular permeability than halogen light.
Dental pulp stem cells express proteins involved in the local invasiveness of odontogenic myxoma

Abstract: little is known about the histogenesis of the odontogenic myxoma (om). dental pulp stem cells could be candidate precursors of om because both om and the dental pulp share the same embryological origin: the dental papilla. for the purpose of comparing om and stem cells, this study analyzed the expression of two proteins related to om invasiveness (mmp-2 and hyaluronic acid) in human immature dental pulp stem cells (hidpscs). three lineages of hidpscs from deciduous and permanent teeth were used in this study. immunofluorescence revealed positive reactions for mmp-2 and hyaluronic acid (ha) in all hidpscs. mmp-2 appeared as dots throughout the cytoplasm, whereas ha appeared either as diffuse and irregular dots or as short fibrils throughout the cytoplasm and outside the cell bodies. the gene expression profile of each cell lineage was evaluated using rt-pcr analysis, and ha was expressed more intensively than mmp-2. ha expression was similar among the three hidpscs lineages, whereas mmp-2 expression was higher in dl-1 than in the other cell lines. the expression of proteins related to om invasiveness in hidpscs could indicate that om originates from dental pulp stem cells.
