Acceso a los servicios preventivos en los regímenes contributivo y subsidiado de salud en un barrio estrato dos de la ciudad de Cali

Abstract: component of access to preventive services of the study that aimed to identify differences in access to health services among people affiliated to the contributive and subsidized system in a slums area of cali was presented. cross-sectional prevalence study, probabilistic design, complex samples, stratified, multistage sampling. 2165 people were surveyed. this population was studied according to variables in some measuring to access to and use of services models. there are significant differences in respect to the need of consulting preventive services, being in the contributory scheme the highest prevalence. less than a fifth of the population feels the need, express and uses preventive services. analysis of age, sex and type of programs shows very low frequency of use. this finding, dramatic in the case of public health, warrants the review of the social importance of preventive programs and the current state of its supply.
Aplicación clínica de marcadores periféricos de respuesta a la terapia antidepresiva: neurotrofinas y citocinas

Abstract: introduction: explanatory theories of depression have advanced in recent decades from the monoaminergic hypothesis to neurogenesis alterations to the neurohormonal hypothesis that includes the dysfunction of the inflammatory response. currently there is a growing interest in the development of biomarkers that can contribute to diagnosis and proper treatment. objectives: to describe the role of neurotrophins such as brain-derived neurotrophic factor (bdnf) and cytokines in the pathophysiology of depressive disorder in addition to reviewing and analyzing evidence about their clinical application as biomarkers of antidepressant therapy. method: relevant data research in several databases. results and conclusions: in recent years evidence of alterations in neurogenesis mediated by the expression of bdnf in the hippocampus in the pathophysiology of depression has increased and there is ample evidence that bdnf is a marker of the diagnosis of depressive disorder and also of treatment effectiveness. there is little information about other neurotrophins. there has also been increased interest in relation to depression as an "inflammatory disease" and the link with cytokines in its pathogenesis. evidence has been found for the usefulness of some cytokines especially il-1 (interleukin 1), il-6 (interleukin 6), and tnf (tumor necrosis factor) as biomarkers of antidepressant drug response in humans.

Abstract: Introducción: Las teorías explicativas de la depresión han avanzado en las últimas décadas, desde la monoaminérgica hasta la alteración de la neurogénesis, pasando por la hipótesis neurohormonal, que incluye la disfunción de la respuesta inflamatoria. Actualmente existe un creciente interés por el desarrollo de biomarcadores que contribuyan diagnóstico y tratamiento adecuado. Objetivos: Describir el rol de las neurotrofinas especialmente del factor neurotrópico derivado del cerebro (BDNF) y de las citocinas en la fisiopatología del trastorno depresivo; además, revisar y analizar la evidencia respecto de su aplicación clínica como biomarcadores de la terapia antidepresiva. Método: Búsqueda de información relevante en diferentes bases de datos. Resultados y conclusiones: En los últimos a os ha crecido la evidencia de la alteración en la neurogénesis mediada por la expresión del BDNF en el hipocampo, en la fisiopatología de la depresión y el respaldo en cuanto a su uso como biomarcador del diagnóstico y la efectividad del tratamiento antidepresivo. Existe menor información para otras neurotrofinas. También se observa un mayor interés respecto a la depresión como una "enfermedad inflamatoria", relacionando también la "cascada" de citocinas con su patogénesis. Se ha reportado evidencia amplia acerca de la utilidad de algunas citosinas, especialmente de la IL-1 (interleucina 1), IL-6 (interleucina 6) y FNT (factor de necrosis tumoral) como biomarcadores de la respuesta farmacológica antidepresiva en humanos. Abstract: Introducción: La interacción entre el campo de salud mental y las ciencias sociales puede ser muy enriquecedora si se quiere lograr un abordaje más integral de los conceptos de salud y de enfermedad mental. Objetivos: Revisar y discutir aspectos generales de la sociología y su vinculación con la salud mental y la psiquiatría. Método: Revisión de fuentes y corrientes teóricas relevantes. Conclusiones: En el ámbito histórico se han percibido momentos de encuentro y distanciamiento de ambas ciencias. En la actualidad debe generarse una actitud de interdisciplinariedad derivada del reconocimiento de una multicausalidad de los trastornos psiquiátricos, de la necesidad de investigación epidemiológica en poblaciones y del reto generado por modelos psicoterapéuticos integradores de la comunidad. Manifestaciones neuropsiquiátricas del déficit de vitamina B12

Abstract: objective: to review and illustrate the psychiatric findings related to vitamin b12 deficiency. methods: case report. results: this is a case of a patient who had the first episode of psychosis when she was 40 years old. the patient was hospitalized in the mental health hospital nuestra se?ora del perpetuo socorro, in pasto, nari?o, in 2005. the patient had also depression that got worse, and recently started alteration of the cognitive functions. she also had other neurological problems like hypoesthesia in the distal third of the four extremities and headache. a macrocitic anemia was found in the lab tests. for this reason, the levels of vitamin b12 and folic acid were analyzed, and subnormal levels of the first were discovered. during the hospitalization, the patient received symptomatic treatment with psychiatric medications, and vitamin b12 reposition was initiated. she got better with the treatment. during two years of ambulatory follow up, the patient remains without symptoms in relation to her mental health. she assists to the hematology department. this specialty has found her stable and prescribed her vitamin b12 supplement for indefinite time. conclusions: the case shows us the diversity of the neuropsychiatric symptoms that patients with vitamin b12 deficiency can have and how the symptoms can reverse when the diagnostic and the treatment are made correctly and soon.
La psicosis en epilepsia

Abstract: introduction: psychotic syndromes are frequently seen in the course of epilepsy, either during the seizures or during the period following them. this is especially associated with poor symptomatic control, refractivity, and change or discontinuation of the anticonvulsive treatment. it has also been described after temporal lobectomy prescribed to control it. objective: to describe general aspects, classification, clinical features, and treatment of psychosis associated with epilepsy. method: relevant data research in several data bases. results and conclusions: epileptic psychosis produces greater morbidity in epileptic patients that might lead to psychiatric hospitalizations, lengthening the time of neurological internment and a more complex use of the pharmacological treatment. there are ictal, interictal and postictal psychosis, along with forced normalization and post-surgery psychosis, the latter two with a lower prevalence. epileptic psychosis might share some aspects of psychiatric disorders such as mania and schizophrenia. in this case, the clinician must make a better assessment of the findings in relation to the seizures to ensure an adequate diagnosis and treatment that generally requires the intervention of both the fields of psychiatry and neurology.

Abstract: Objetivo: Revisar e ilustrar los hallazgos psiquiátricos asociados con el déficit de vitamina B12. Métodos: Reporte de caso. Resultados: Se trata de una mujer de 40 a os, quien sufrió un primer episodio de sintomatología psicótica. Por ello fue hospitalizada en el Hospital Mental Nuestra Se ora del Perpetuo Socorro, de Pasto, Nari o, en el 2005; además, presentaba cefalea, agudización de sus síntomas depresivos, alteración en sus funciones cognoscitivas de inicio reciente y quejas neurológicas concomitantes dadas por hipoestesia en tercio distal de las cuatro extremidades. En los resultados de los paraclínicos solicitados se encontró una anemia macrocítica, por lo que se analizaron las concentraciones de vitamina B12 y ácido fólico, que mostraron valores subnormales de la primera. Durante su hospitalización se manejaron los síntomas con psicofármacos y se inició la reposición de la vitamina B12, con lo que presentó mejoría de su cuadro. Dos a os después, la paciente continúa asintomática desde el punto de vista mental, asiste a controles por hematología, especialidad que la ha encontrado estable manteniendo el uso de suplencia vitamínica indefinida. Conclusiones: Este caso ilustra el compendio neuropsiquiátrico diverso que pueden presentar las personas con déficit de vitamina B12; así mismo, el carácter reversible de esta sintomatología cuando el diagnóstico y el tratamiento se hacen tempranamente y de manera adecuada. Abstract: Introducción: Los síndromes psicóticos pueden presentarse de forma frecuente durante el curso de la epilepsia, tanto en las crisis como en el período inmediatamente posterior o libre de estas; asociado con su pobre control sintomático, por refractariedad, cambio o abandono del tratamiento anticonvulsivo. También se han descrito luego del tratamiento quirúrgico de epilepsia especialmente en lobectomía temporal. Objetivo: Describir los aspectos generales, clasificación, clínica y tratamiento de las psicosis asociadas a la epilepsia. Método: Búsqueda de información relevante en diferentes bases de datos. Resultados y conclusiones: Las psicosis epilépticas ocasionan una mayor morbilidad en los pacientes epilépticos, pueden generar hospitalizaciones psiquiátricas, prolongar el tiempo de internación neurológica y hacer complejo el manejo farmacológico. Dentro de estas se encuentran la psicosis ictal, interictal y postictal, además de la normalización forzada y posquirúrgica; las últimas menos prevalentes. En su clínica pueden compartir aspectos con otros trastornos psiquiátricos como la manía y la misma esquizofrenia, situación que debe conducir al clínico a entender mejor sus hallazgos semiológicos y lo que representan en relación con las convulsiones, para asegurar un diagnóstico y manejo adecuados, que deberían de requerir el trabajo interdisciplinario entre psiquiatría y neurología. Alteraciones neuropsiquiátricas en demencia vascular
