Abstract: Latest trends in political discursive analysis have shown the importance of the study done from the textual and contextual perspective alike. An oral speech has the property of having been prepared in advance but expressed with some improvisation so characteristics from written and oral texts must be taken into consideration. As a consequence, the structure of the discourse and the personal contribution of the author are seemingly relevant. The oral speech analyzed here was expressed in Ghana, Africa by President Barack Obama in 2009. Given the circumstances of Obama’s African origin and of visiting the country as the President of the United States, the elements that reveal the personal attitude comprise an extra interest. The aim of this paper is to analyze the speech from a discursive viewpoint in order to describe the textual and intertextual functions.
The Marian Illustrations of the Legend of Montserrat

Abstract: The books telling the story of Montserrat used to contain illustrative engravings. Montserrat presents us with a special case, because this monastery holds an uninterrupted record of illustrated texts or of texts with engraved images from the miniatures of the Llibre Vermell and the first fruits of the printing press to the 19th century. Admittedly, in the 16th, 17th and 18th centuries the books exhibit the engravings on the cover or within the first sheets; it is only in the 19th century that illustrations appear inserted in the text so as to make the story more agreeable. The images intensify the value of texts, since illustrations can help us understand the written word and may suggest other readings of the corresponding passages, clearer and more precise. Illustrations in the books on the history of a monastery are but a part of a larger set of ideas, projects and self-service which the Church aims at the reader. Furthermore, the iconographic models of the Virgin Mary and the Mountain are of great interest for the history of engraving and of the Catalan and Spanish devotions. Los libros en que se contaba la historia de Montserrat contenían estampas que los ilustraban. Montserrat es un caso inusual, porque posee un registro ininterrumpido de textos ilustrados o acompa ados con imágenes grabadas, desde el Llibre Vermell y los primeros momentos de la imprenta hasta el siglo XIX. Bien es cierto que en los libros de los siglos XVI, XVII y XVIII sólo se colocan los grabados en la portada o en las primeras hojas; Hasta el siglo XIX no encontramos un tipo de ilustración insertada en el interior del texto amenizando la historia. Las imágenes intensifican el valor de los textos, porque nos ayudan a comprenderlos y a la vez sugieren nuevas lecturas, más precisas y claras. Las ilustraciones de los libros de historia de un santuario forman parte de todo un enjambre de ideas, proyectos y propaganda que la Iglesia quiere transmitir al lector. Por otro lado los modelos iconográficos de María y la monta a son de gran interés para la historia del grabado y de la devoción catalana y espa ola.

Abstract: This paper presents T-Cube and MetroMap, two new graphical representation models for controlling and managing the processes of software project development. They both use metaphors and visual representation techniques to address typical project management tasks. T-Cube uses a metaphor with the Rubik-Cube whereas MetroMap uses a metaphor with a metro map. The tools have been tested on real project data and a qualitative assessment shows the results of testing the visualizations with users attempting several information retrieval tasks. The utility of the tools has been positively evaluated and the article demonstrates the possibilities of visual approaches in project management.
Measurement of Transcutaneous Bilirubin with Bilicheck as a Jaundice Screening Method in Neonates in Pediatric Emergency Departments  [PDF]

Abstract: Objectives: To study the reliability of a transcutaneous bilirubinometer (Bilicheck) to determine bilirubin levels in neonates consulting for jaundice in a Paediatric Emergency Department (ED), and to evaluate its usefulness as a screening method. Methods: Prospective observational study realized between June of 2005 and December of2005 inneonates consulting at a paediatric emergency department for jaundice, in whom we realized both transcutaneous and total serum bilirubin measurements (TcB and TSB). We collected demographic variables, analytical variables (serum and transcutaneous bilirubin levels), length of stay in the ED, and need for treatment. Results: 66 children were included aged 2 to 31 days (81% of the sample were 2 to 7 days old). There was a close and statistically significant correlation between TcB and TSB (r = 0.81, p < 0.001). The area under the ROC curve was of 0.90, allowing detecting newborns with jaundice susceptible of treatment with TcB levels ≥ 13 mg/dL (sensitivity 92%, specificity 63, 5%, a positive predictive value 39% and a negative predictive value 97%). The number of venous punctures could be reduced in 50%. The medium stay in the ED was of 2 hours when performing serum measurements. Conclusions: A linear correlation exists between TcB-TSB. TcB measurement cannot replace that of TsB, however it could be used as a screening method in an ED to determine which neonates need confirmation by TsB measurement. The use of transcutaneous bilirubinometer would reduce both the number of painful interventions in neonates and the medium length of stay in ED, consequently reducing iatrogenesis.
Clinical Outcomes with a New Model of Extended Depth of Focus Intraocular Lens  [PDF]

Abstract: Purpose: To evaluate the early clinical outcomes of a new diffractive extended depth-of-focus (EDOF) intraocular lens (IOL). Methods: Prospective case series enrolling patients undergoing cataract surgery with implantation of the EDOF IOL AT LARA (Carl Zeiss Meditec, Jena, Germany). Distance, intermediate and near visual acuity were evaluated during a follow-up of 1 month. Likewise, defocus curve, binocular mesopic contrast sensitivity, perception of photic phenomena, patient satisfaction and spectacle independence were assessed. Results: The study enrolled 38 eyes of 19 patients with a mean age of 69.5 years. A significant reduction of manifest refraction and improvement in corrected distance visual acuity (CDVA) was observed with surgery (p < 0.001). All eyes achieved a 1-month postoperative binocular CDVA of 20/20 or better. Likewise, all eyes achieved a DCIVA of 20/30 or better at 1 month postoperatively, and a total of 92.1% achieved DCNVA of 20/40 or better. Concerning refractive predictability, 97.4% of eyes had a 1-month postoperative spherical equivalent within ±0.50 D. The level of patient satisfaction with distance, intermediate and near vision was high, with 95% of patients reporting that they would have the same surgical procedure with the same IOL again. Difficulties in night driving or associated to halos were low. A total of 89% of patients reported to be spectacle independent postoperatively. Conclusions: The EDOF IOL AT LARA is able to provide a functional restoration of the visual function across distances after cataract surgery, with very high levels of patient satisfaction and minimal incidence of photic phenomena.

Abstract: sudden death is a serious complication in patients with heart failure. it is mostly lethal and poorly predictable by conventional diagnosis methods. implantable automatic defibrillators' use prevents significantly this complication in patients with demonstrated malignant arrhythmias. some clinical trials have shown that the implant of a defibrillator diminishes the mortality in patient with severe systolic left ventricular dysfunction, but without evidence of serious arrhythmias. this has generated a debate in relation to if all patients with severe systolic ventricular dysfunction should be subjected to the implant of these devices. the present is a revision and discussion of the main works published recently in this respect, and also a critical point of view of the different fields that have been evaluated in controlled clinical and observational trials.

Abstract: at universities in the united states, first-year spanish instructors often wonder why their students perform below expectations in vocabulary tests. this paper addresses this question by presenting an analysis of five leading first-year college spanish textbooks published in the united states. this analysis consists of two parts: 1) mapping the textbooks' vocabulary lists as well as the vocabulary that appears in their reading input against the vocabulary-frequency information provided in a frequency dictionary of spanish: core vocabulary for learners written by mark davies and published by routledge in 2006, and 2) using estimations available in the current literature to infer logistical aspects that need to be considered in first-year college spanish such as amount of contact-time with the material to be learned, depth of vocabulary knowledge, and the relationship between the receptive and productive dimensions of vocabulary acquisition. the results show that some pedagogical fine-tuning may be needed to align the amount of vocabulary presented in textbooks as well as their vocabulary selections with research findings on vocabulary acquisition and spanish vocabulary frequency.
Consideraciones clínico epidemiológicas sobre la otitis media aguda antes del primer a?o de vida

Abstract: introduction: acute otitis media is very frequent in children and they suffer it once or more during the first year of life. objectives:to describe some of the main epidemiological characteristics of acute otitis media in children under one year old. methods: it was carried out a transversal and descriptive study in 91 children at the children's university hospital in centro habana municipality in the period september 2009 - july 2011. a survey concerning risk factors of this disease was made and statistical data such as causes and frequency of this disease were applied in software excel. results: there was a prevalence of this disease in male children. greatest morbidity was found in group of children over six months old prevailing fever and irritability. the associated risk factors more frequent were: previous acute breathing infection, bad nasal hygiene and being a passive smoker. conclusions: children's acute otitis media is a disease of multifactor occurrence.

En el ámbito de las universidades estadounidenses, a menudo los profesores de primer a o de espa ol constatan que los resultados de los exámenes de vocabulario están por debajo de sus expectativas. Este trabajo aborda esta cuestión por medio del análisis de cinco libros de texto publicados en los Estados Unidos. Este análisis consiste en dos partes: 1) una comparación tanto de las listas de vocabulario como del vocabulario de las lecturas con la información sobre vocabulario y frecuencia que aparece en A frequency dictionary of Spanish: Core vocabulary for learners, escrito por Mark Davies y publicado por Routledge en 2006, y 2) una exposición de los aspectos logísticos que deben considerarse, tales como cantidad de tiempo en contacto con la materia y profundidad del conocimiento léxico, y de cómo se perfilan estos aspectos a través de las dimensiones receptivas y productivas del proceso de adquisición de vocabulario. Los resultados muestran que los libros de texto pueden perfeccionarse en lo que se refiere a la cantidad de vocabulario y la selección del mismo para conseguir un mejor alineamiento con los conocimientos que se tienen en este momento sobre frecuencia del léxico espa ol.
